Tesi

Introduction: University social responsibility is present in the discourses of academics, but it fails to permeate the actions that lead to the transformation of the university, requiring a paradigm shift. At present there are conceptual confusions that make its promotion and transformative practice very difficult, in many universities it is confused with social projection, which reduces its transversal dimension.  Objective: To analyze the scientific evidence related to university social responsibility in Latin America.  Methods: Systematic review of the literature, between the months of August to December 2021; the search for articles was carried out in the databases: Scopus, Scielo, Doaj, Lilacs and Redalyc; the use of Descriptors in Health Sciences (DeCS) and the combination of the Boolean operators OR and AND were applied. Full texts in Spanish between the years 2015 and 2021 were considered.  Results: 508 articles were obtained, which were submitted to the PRISMA methodology, selecting 20 articles, grouped into four categories according to the Social Responsibility Manual of the Úrsula Model: management, training, cognitive, and social participation.  Conclusions: USR is an inalienable moral commitment, it is important in university management to promote sustainable development, with ethics and respect for human rights, the participation of committed actors is necessary, considering that it currently has a welfare and reductionist approach, scarce support, socialization and implementation and there is a need to unify criteria.

Experience with the use of Rituximab for the treatment of rheumatoid arthritis in a tertiary Hospital in Spain: RITAR study

There is evidence supporting that there are no relevant clinical differences between dosing rituximab 1000 mg or 2000 mg per cycle in rheumatoid arthritis (RA) patients in clinical trials, and low-dose cycles seem to have a better safety profile. Our objective was to describe the pattern of use of rituximab in real-life practice conditions. Methods: Rituximab for RA in clinical practice (RITAR) study is a retrospective cohort study from 2005 to 2015. Eligibility criteria were RA adults treated with rituximab for active articular disease. Response duration was the main outcome defined as months elapsed from the date of rituximab first infusion to the date of flare. A multivariable analysis was performed to determine the variables associated with response duration. Results: A total of 114 patients and 409 cycles were described, 93.0% seropositive and 80.7% women. Rituximab was mainly used as second-line biological therapy. On demand retreatment was used in 94.6% of cases versus fixed 6 months retreatment in 5.4%. Median response duration to on demand rituximab cycles was 10 months (interquartile range, 7–13). Multivariable analysis showed that age older than 65 years, number of rituximab cycles, seropositivity, and first- or second-line therapy were associated with longer response duration. The dose administered at each cycle was not significantly associated with response duration. Conclusions: Our experience suggests that 1000 mg rituximab single infusion on demand is a reasonable schedule for long-term treatment of those patients with good response after the first cycles, especially in seropositive patients and when it is applied as a first- or second-line biological therap

Utility of flow cytometry studies in the management of patients with multiple myeloma

Purpose of review: Although the input of multiparameter flow cytometry (MFC) into the clinical management of multiple myeloma (MM) patients has faced some reluctance, continuously growing evidence supports the utility of MFC in this disease. Recent findings: MFC immunophenotyping of bone marrow and peripheral blood plasma cells affords cost-effective assessment of clonality, and provides prognostic information on the risk of progression in smoldering MM, and the identification of active MM patients with dismal outcome (e.g.: high numbers of circulating tumor cells) or long-term survival despite sub-optimal responses through the characterization of MGUS-like phenotypes. Extensive data indicates that MRD monitoring can be used as biomarker to evaluate treatment efficacy and act as surrogate for survival. The time has come to address within clinical trials, the exact role of baseline risk factors and MRD monitoring for tailored therapy in MM, which implies systematic usage of highly sensitive cost-effective, readily available and standardized MRD techniques such as MFC. Summary: Next-generation MFC should be considered mandatory in the routine evaluation of MM patients both at diagnosis and after therapy, and represents an attractive technique to integrate with high-throughput DNA and RNA-seq methods to help understanding the mechanisms behind dissemination and chemoresistance of MM

Intermittent BRAF inhibition in advanced BRAF mutated melanoma results of a phase II randomized trial

Combination treatment with BRAF (BRAFi) plus MEK inhibitors (MEKi) has demonstrated survival benefit in patients with advanced melanoma harboring activating BRAF mutations. Previous preclinical studies suggested that an intermittent dosing of these drugs could delay the emergence of resistance. Contrary to expectations, the first published phase 2 randomized study comparing continuous versus intermittent schedule of dabrafenib (BRAFi) plus trametinib (MEKi) demonstrated a detrimental effect of the “on−off” schedule. Here we report confirmatory data from the Phase II randomized open-label clinical trial comparing the antitumoral activity of the standard schedule versus an intermittent combination of vemurafenib (BRAFi) plus cobimetinib (MEKi) in advanced BRAF mutant melanoma patients (NCT02583516). The trial did not meet its primary endpoint of progression free survival (PFS) improvement. Our results show that the antitumor activity of the experimental intermittent schedule of vemurafenib plus cobimetinib is not superior to the standard continuous schedule. Detection of BRAF mutation in cell free tumor DNA has prognostic value for survival and its dynamics has an excellent correlation with clinical response, but not with progression. NGS analysis demonstrated de novo mutations in resistant cases

The objective sought to determine the effects on emotional literacy in the elaboration of digital drawings and traditional drawings in the cooperative and individual pedagogical format. 120 schoolchildren from vulnerable contexts participated. The data was collected by a scale based on Fury's proposals. Se advierten efectos de mejora en el cambio de laalfabetización emocional, y en sus componentes de expresividad y valencia. Improvement effects are noted in the change of emotionalliteracy, and in its components of expressiveness and valence. A higherlevel was detected in the change from negative to positive emotions in thedrawings of children who used tablets collaboratively, as opposed to thosewho did so individually

Clinical implications of changing thyroglobulin and antithyroglobulin antibodies analytical methods in the follow-up of patients with differentiated thyroid carcinoma

    Get PDF
Background and aims: Patients’ response to treatment in differentiated thyroid cancer (DTC) is classified according to serum thyroglobulin concentrations (Tg), usually using the American Thyroid Association guidelines and considering potential interfering anti-thyroglobulin antibodies (Ab-Tg). We aim to evaluate the clinical implications of changing Tg and Ab-Tg quantification method. Material and methods: Tg and Ab-Tg were quantified in 82 serum samples (60 from DTC patients) by Elecsys and Access immunoassays. Results: Elecsys immunoassay rendered higher values of Tg than Access: mean bias 5.03 ng/mL (95%CI:- 14.14–24.21). In DTC patients, there was an almost perfect agreement for response classification (kappa index = 0.833). Discrepancies appeared in patients with undetermined response, with a more tendency to subclassification with Access. Ab-Tg showed a poor correlation (r = 0.5394). When Elecsys cut-off was reduced to 43 IU/ mL, agreement for positive/negative classification improved from a kappa index of 0.607 to 0.650. Prospective study with personalized follow-up showed that only 6.3% of Tg results required an analytical confirmation, being confirmed 93% of them. Conclusions: Despite the biases observed, clinical impact of an analytical change is minimal in patients’ management. However, cautious and personalized follow-up period after the change is still mandatory, especially in patients with Tg levels between 0.2 and 1 ng/mL
