    Is distortion of the bioprosthesis ring a risk factor for early calcification ?

    <p>Abstract</p> <p>Background</p> <p>As the population ages, bioprosthesis are increasingly being used in cardiac valve replacement. Pericardial bioprosthesis combine an excellent hemodynamic performance with low thrombogenicity, but valve failure associated with calcification remains a concern with these valves. We describe distortion of the bioprosthesis ring as a risk factor for early calcification.</p> <p>Methods</p> <p>A total of 510 patients over the age of 70 years underwent isolated aortic valve replacement with the Mitroflow (A12) pericardial bioprosthesis. Thirty two patients (6,2%) have undergone a second aortic valve replacement due to structural valve dysfunction resulting from valve calcification. In all patients a chest radiography and coronary angiography was performed before reoperation. A 64 Multidetector Computed Tomography (MDCT) with retrospective ECG gating study was performed in four patients to evaluate the aortic bioprosthesis.</p> <p>Results</p> <p>Chest radiography showed in all patients an irregular bioprosthesis ring. At preoperative coronary angiography a distorted bioprosthesis ring was detected in all patients. Macroscopic findings of the explanted bioprostheses included extensive calcification in all specimens.</p> <p>Conclusion</p> <p>There was a possible relationship between early bioprosthetic calcification and radiologic distortion of the bioprosthesis ring.</p

    The development of anemia is associated to poor prognosis in NKF/KDOQI stage 3 chronic kidney disease

    Background: Anemia is a common condition in CKD that has been identified as a cardiovascular (CV) risk factor in end-stage renal disease, constituting a predictor of low survival. The aim of this study was to define the onset of anemia of renal origin and its association with the evolution of kidney disease and clinical outcomes in stage 3 CKD (CKD-3). Methods: This epidemiological, prospective, multicenter, 3-year study included 439 CKD-3 patients. The origin of nephropathy and comorbidity (Charlson score: 3.2) were recorded. The clinical characteristics of patients that developed anemia according to EBPG guidelines were compared with those that did not, followed by multivariate logistic regression, Kaplan-Meier curves and ROC curves to investigate factors associated with the development of renal anemia. Results: During the 36-month follow-up period, 50% reached CKD-4 or 5, and approximately 35% were diagnosed with anemia (85% of renal origin). The probability of developing renal anemia was 0.12, 0.20 and 0.25 at 1, 2 and 3 years, respectively. Patients that developed anemia were mainly men (72% anemic vs. 69% non-anemic). The mean age was 68 vs. 65.5 years and baseline proteinuria was 0.94 vs. 0.62 g/24h (anemic vs. non anemic, respectively). Baseline MDRD values were 36 vs. 40 mL/min and albumin 4.1 vs. 4.3 g/dL; reduction in MDRD was greater in those that developed anemia (6.8 vs. 1.6 mL/min/1.73 m2/3 years). These patients progressed earlier to CKD-4 or 5 (18 vs. 28 months), with a higher proportion of hospitalizations (31 vs. 16%), major CV events (16 vs. 7%), and higher mortality (10 vs. 6.6%) than those without anemia. Multivariate logistic regression indicated a significant association between baseline hemoglobin (OR=0.35; 95% CI: 0.24-0.28), glomerular filtration rate (OR=0.96; 95% CI: 0.93-0.99), female (OR=0.19; 95% CI: 0.10-0.40) and the development of renal anemia. Conclusions: Renal anemia is associated with a more rapid evolution to CKD-4, and a higher risk of CV events and hospitalization in non-dialysis-dependent CKD patients.     Modelo hidrodinámico de alta resolución del puerto de Algeciras – proyecto SAMPA2

    El proyecto SAMPA, financiado por la Autoridad Portuaria de la Bahía de Algeciras (APBA) y Puertos del Estado entre los años 2010 y 2013, fue proyecto piloto para la integración de un modelo numérico de alta resolución en un sistema operacional en el cual el Grupo de Oceanografía Física de la Universidad de Málaga (GOFIMA) desarrolló el propio modelo hidrodinámico [Sammartino et al., 2014; Sánchez Garrido et al., 2014]. Los productos operacionales servidos por Puertos del Estado (portal PORTUS) y la APBA (sistema CMA), alimentados entre otros por las predicciones derivadas de SAMPA, representaron el primer ejemplo de difusión de un forecast océano-meteorológico accesible tanto al público generalizado como al personal técnico marítimo empleado en el puerto. El modelo numérico que estaba detrás de SAMPA, sin embargo, no tenía suficiente resolución para llegar a resolver la dinámica portuaria, y la APBA, en el 2015, financió la segunda generación del proyecto (SAMPA2), con el objetivo de cubrir ese hueco. Entre la segunda mitad del 2015 y durante todo el 2016, después de haber realizado un atento análisis de los aspectos mejorables del actual SAMPA, GOFIMA desarrolló un sistema completamente nuevo: un modelo anidado en tres dominios acoplados, que proporciona un aumento progresivo de resolución desde la escala regional hasta la portuaria. A eso se añade el valor añadido de unas herramientas de análisis de calidad de agua del Puerto de Algeciras de acuerdo con las indicaciones de la ROM5.1-13.Universidad de Málaga.     Stress compensation by gap monolayers for stacked InAs/GaAs quantum dots solar cells

    In this work we report the stacking of 10 and 50 InAs quantum dots layers using 2 monolayers of GaP for stress compensation and a stack period of 18 nm on GaAs (001) substrates. Very good structural and optical quality is found in both samples. Vertical alignment of the dots is observed by transmission electron microscopy suggesting the existence of residual stress around them. Photocurrent measurements show light absorption up to 1.2 μm in the nanostructures together with a reduction in the blue response of the device. As a result of the phosphorus incorporation in the barriers, a very high thermal activation energy (431 meV) has also been obtained for the quantum dot emission

    The ideal surgical approach is unclear in adult patients with coarctation of the aorta that is associated with other cardiovascular pathologies that require intervention. Standard median sternotomy allows simultaneous, coronary revascularization surgery, valve replacement and repair of aortic coarctation. However the collateral circulation and the anatomy of the mammary arteries must be determined, to avoid possible complications. We report a case of a 69 year-old man with aortic coarctation, aortic stenosis, coronary artery disease and internal mammary artery dilatation who underwent concomitant surgical procedures through a median sternotomy

    Desde el curso académico 2008/2009 el Proyecto Mentor de la Escuela Universitaria de Ingeniería Técnica Industrial (EUITI) de la UPM [1] participa en distintas acciones de acogida de los alumnos de nuevo ingreso propio del centro. Estas actividades son el primer contacto de los mentores con los alumnos de nuevo ingreso y además son reforzadas por el apoyo y experiencia de los tutores y mentores del curso anterior, favoreciendo la interrelación de alumnos de distintos cursos y especialidades. Para la correcta realización de estas actividades, los alumnos mentores reciben un intensivo programa de formación que incluye aspectos psicosociales propios de la mentoría [2].
