    Standardization of a new photodiagnosis method based on LEDs for patients with solar urticaria sensitive to visible light

    Standard methods for photodiagnosis of solar urticaria are based in exposure of patient skin to different polychromatic UV and visible sources where minimal urticarial doses for different spectral bands (UVB and UVA) are established. Classical photodiagnosis devices are based in solar simulation and use of UVB and UVA enhanced fluorescent lamps. In case of visible US photodiagnosis, US patient skin is exposed for 15 min to a slight projector, provided with halogen lamp, at a distance of 15 cms and presence of erythema and/or wheals is determined as positive reaction. Slights projector is from several years almost out of market due to use of new projection digital technologies and new visible light emerging technologies are good candidates for their substitution as photodiagnosis tool. The objective of the present work is to analyze photodiagnosis of visible light solar urticaria with using a LED device in comparison to normal slight projector exposure protocol. A total of twenty patients, from 7 different photodiagnosis units have participated in the study. Patients, with SU positive to visible light (with or without to UV radiation) following the standard photodiagnosis protocols were included in the study. Slight projector used in all photodiagnosis units were of similar characteristics and irradiance at 15 cm distance, as well as total dose of visible light after 15 min were calculated for each halogen lamp device. LED exposure was performed in parallel in a closed zone of the back of the patients. For LED photodiagnosis a prototype from University of Málaga (Spain) has been developed consisting in a black box provided with 4 holes of 12 mm diameter in which each hole white warm of a LED of 1 W is emitted. Thus, each LEDs dose is controlled independently and the device allows establishing, as well as for UVB and UVA normal protocols a MUD also under visible light. In that case, maximal visible light dose is reached in less than 5 min compared to 15 min under exposure to slight projector. All patients were positive to LED warm visible light with presence of erythema and / or wheals in parallel to the exposure to the slight projector. A MUD to visible light has been established with significant variations between patients which reveals different grade to visible light sensibilization.     Frecuencia de enfermedades huérfanas en Cartagena de Indias, Colombia

    Objetivo Estimar la frecuencia de enfermedades huérfanas (EH) o raras (ER) de origen genético en Cartagena de Indias, Colombia.Metodología Se realizó un estudio descriptivo con datos de los Registros Individuales de Prestación de Servicios (RIPS), del periodo 2003-2010, conservados por el Departamento Administrativo Distrital de Salud-DADIS. Fueron escogidas las EH de origen genético a partir de la clasificación del Ministerio de Salud Colombiano, y que se identifican con la referencia Q en los Códigos Internacionales de Enfermedad (CIE10).Resultados En total se registraron 522 181 personas que consultaron al sistema de salud. El 0,8 % (n=16 527) tuvieron algún diagnóstico Q. Finalmente, 391 personas (0,075 %) tuvieron impresión diagnóstica de EH de origen genético. En esta población se encontraron 75 impresiones diagnósticas distintas. Las Malformaciones y Deformidades Congénitas del Sistema Osteomuscular (Q65-Q79) fueron las más frecuentes (38,6 %), seguidas por Otras Malformaciones Congénitas (Q080-089) con 16,6 %.Conclusiones Se obtuvo el primer reporte de la frecuencia de las EH de origen genético en la segunda ciudad más poblada sobre el litoral del Caribe Colombiano. Es posible que en los RIPS haya un subregistro de las ER que dificulta conocer la prevalencia real

    QUIJOTE scientific results -- XIII. Intensity and polarization study of supernova remnants in the QUIJOTE-MFI wide survey: CTB 80, Cygnus Loop, HB 21, CTA 1, Tycho and HB 9

    We use the new QUIJOTE-MFI wide survey (11, 13, 17 and 19 GHz) to produce spectral energy distributions (SEDs), on an angular scale of 1 deg, of the supernova remnants (SNRs) CTB 80, Cygnus Loop, HB 21, CTA 1, Tycho and HB 9. We provide new measurements of the polarized synchrotron radiation in the microwave range. For each SNR, the intensity and polarization SEDs are obtained and modelled by combining QUIJOTE-MFI maps with ancillary data. In intensity, we confirm the curved power law spectra of CTB 80 and HB 21 with a break frequency νb\nu_{\rm b} at 2.00.5+1.2^{+1.2}_{-0.5} GHz and 5.01.0+1.2^{+1.2}_{-1.0} GHz respectively; and spectral indices respectively below and above the spectral break of 0.34±0.04-0.34\pm0.04 and 0.86±0.5-0.86\pm0.5 for CTB 80, and 0.24±0.07-0.24\pm0.07 and 0.60±0.05-0.60\pm0.05 for HB 21. In addition, we provide upper limits on the Anomalous Microwave Emission (AME), suggesting that the AME contribution is negligible towards these remnants. From a simultaneous intensity and polarization fit, we recover synchrotron spectral indices as flat as 0.24-0.24, and the whole sample has a mean and scatter of 0.44±0.12-0.44\pm0.12. The polarization fractions have a mean and scatter of 6.1±1.96.1\pm1.9\%. When combining our results with the measurements from other QUIJOTE studies of SNRs, we find that radio spectral indices are flatter for mature SNRs, and particularly flatter for CTB 80 (0.240.06+0.07-0.24^{+0.07}_{-0.06}) and HB 21 (0.340.03+0.04-0.34^{+0.04}_{-0.03}). In addition, the evolution of the spectral indices against the SNRs age is modelled with a power-law function, providing an exponent 0.07±0.03-0.07\pm0.03 and amplitude 0.49±0.02-0.49\pm0.02 (normalised at 10 kyr), which are conservative with respect to previous studies of our Galaxy and the Large Magellanic Cloud.Comment: 33 pages, 15 figure, 15 tables.     Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     CIBERER : Spanish national network for research on rare diseases: A highly productive collaborative initiative

This innovative research structure comprises 11 different specific areas dedicated to the main public health priorities in the National Health System. CIBERER, the thematic area of CIBER focused on rare diseases (RDs) currently consists of 75 research groups belonging to universities, research centers, and hospitals of the entire country. CIBERER's mission is to be a center prioritizing and favoring collaboration and cooperation between biomedical and clinical research groups, with special emphasis on the aspects of genetic, molecular, biochemical, and cellular research of RDs. This research is the basis for providing new tools for the diagnosis and therapy of low-prevalence diseases, in line with the International Rare Diseases Research Consortium (IRDiRC) objectives, thus favoring translational research between the scientific environment of the laboratory and the clinical setting of health centers. In this article, we intend to review CIBERER's 15-year journey and summarize the main results obtained in terms of internationalization, scientific production, contributions toward the discovery of new therapies and novel genes associated to diseases, cooperation with patients' associations and many other topics related to RD research

    The effects of winter severity and population density on body stores in the Iberian wild goat (Capra pyrenaica) in a highly seasonal mountain environment

    International audienceWe studied the factors that determined kidney fat stores (KFs) and kidney stores (Ks)-defined as the residuals from the linear regression of kidney mass and kidney fat, respectively, on body weight-in 463 Iberian wild goats (Capra pyrenaica) from the Sierra Nevada (southern Spain). Despite the fact that body stores in both sexes were highest during the warmest months of the year and lowest during the coldest months when food resources are limited, the observed pattern was sex- and age-dependent. The KFs of male goats fell more than those of females in winter, and the yearlings of both sexes needed one season more than young or adults to restore their KFs. Goats of all age classes showed the same seasonal patterns in their Ks, although Ks were lower in females than in males throughout the yearly cycle. In addition, we found strong delayed effects of both snowfalls and population density on body stores, and in years with a lot of snow, goats' KFs reached their lowest levels in the current winter-spring, but the highest in the following summers and autumns. This pattern was less noticeable in the Ks. Population density negatively affected the body stores of wild goats, especially in winter, and the amount of snow fallen in the year of birth (cohort effect) did not seem to influence the body stores in our data set. In addition, we assessed the accuracy of the residuals from the regression between body size and body mass for monitoring body condition of live wild goats and concluded that, although it poorly indicates fat stores, it could be used as a general proxy of body condition. Finally, we discuss the expected effects of climate warming on body stores in this Mediterranean Caprinae species
