Systems Modeling to Implement Integrated System Health Management Capability
ISHM capability includes: detection of anomalies, diagnosis of causes of anomalies, prediction of future anomalies, and user interfaces that enable integrated awareness (past, present, and future) by users. This is achieved by focused management of data, information and knowledge (DIaK) that will likely be distributed across networks. Management of DIaK implies storage, sharing (timely availability), maintaining, evolving, and processing. Processing of DIaK encapsulates strategies, methodologies, algorithms, etc. focused on achieving high ISHM Functional Capability Level (FCL). High FCL means a high degree of success in detecting anomalies, diagnosing causes, predicting future anomalies, and enabling health integrated awareness by the user. A model that enables ISHM capability, and hence, DIaK management, is denominated the ISHM Model of the System (IMS). We describe aspects of the IMS that focus on processing of DIaK. Strategies, methodologies, and algorithms require proper context. We describe an approach to define and use contexts, implementation in an object-oriented software environment (G2), and validation using actual test data from a methane thruster test program at NASA SSC. Context is linked to existence of relationships among elements of a system. For example, the context to use a strategy to detect leak is to identify closed subsystems (e.g. bounded by closed valves and by tanks) that include pressure sensors, and check if the pressure is changing. We call these subsystems Pressurizable Subsystems. If pressure changes are detected, then all members of the closed subsystem become suspect of leakage. In this case, the context is defined by identifying a subsystem that is suitable for applying a strategy. Contexts are defined in many ways. Often, a context is defined by relationships of function (e.g. liquid flow, maintaining pressure, etc.), form (e.g. part of the same component, connected to other components, etc.), or space (e.g. physically close, touching the same common element, etc.). The context might be defined dynamically (if conditions for the context appear and disappear dynamically) or statically. Although this approach is akin to case-based reasoning, we are implementing it using a software environment that embodies tools to define and manage relationships (of any nature) among objects in a very intuitive manner. Context for higher level inferences (that use detected anomalies or events), primarily for diagnosis and prognosis, are related to causal relationships. This is useful to develop root-cause analysis trees showing an event linked to its possible causes and effects. The innovation pertaining to RCA trees encompasses use of previously defined subsystems as well as individual elements in the tree. This approach allows more powerful implementations of RCA capability in object-oriented environments. For example, if a pressurizable subsystem is leaking, its root-cause representation within an RCA tree will show that the cause is that all elements of that subsystem are suspect of leak. Such a tree would apply to all instances of leak-events detected and all elements in all pressurizable subsystems in the system. Example subsystems in our environment to build IMS include: Pressurizable Subsystem, Fluid-Fill Subsystem, Flow-Thru-Valve Subsystem, and Fluid Supply Subsystem. The software environment for IMS is designed to potentially allow definition of any relationship suitable to create a context to achieve ISHM capability
Use of Only Oral Rehydration Salt Solution for Successful Management of a Young Infant with Serum Sodium of 201 mmol/L in an Urban Diarrhoeal Diseases Hospital, Bangladesh
A boy aged 4 months 7 days was admitted to the Intensive Care Unit
(ICU) of the Dhaka Hospital of icddr,b, Dhaka, Bangladesh, with the
problems of acute watery diarrhoea with some dehydration, pneumonia,
lethargy, and hypernatraemia (serum sodium of 201 mmol/L). Correction
for hypernatraemia was tried by using only oral rehydration salt (ORS)
solution. Seizures occurred during correction of the hypernatraemia.
These were difficult to control and required three doses of injection
lorazepam, a loading dose of injection phenobarbitone, followed by
injection phenytoin and finally two doses of injection mannitol (even
though there was no clinical or imaging evidence by ultrasonography or
computed tomography of cerebral oedema). The correction was continued
with ORS, and all the anticonvulsants were successfully weaned without
any further seizures, and the patient recovered without any overt
neurological sequelae. We present a case report of extreme
hypernatraemia, which was successfully managed using only ORS
Firm finances, weather derivatives and geography
This paper considers some intellectual, practical and political dimensions of collaboration between human and physical geographers exploring how firms are using relatively new financial products – weather derivatives – to displace any costs of weather-related uncertainty and risk. The paper defines weather derivatives and indicates how they differ from weather insurance products before considering the geo-political, cultural and economic context for their creation. The paper concludes by reflecting on the challenges of research collaboration across the human–physical geography divide and suggests that while such initiatives may be undermined by a range of institutional and intellectual factors, conversations between physical and human geographers remain and are likely to become increasingly pertinent. The creation of a market in weather derivatives raises a host of urgent political and regulatory questions and the confluence of natural and social knowledges, co-existing within and through the geography academy, provides a constructive and creative basis from which to engage with this new market and wider discourses of uneven economic development and climate change
Cam morphology but neither acetabular dysplasia nor pincer morphology is associated with osteophytosis throughout the hip: findings from a cross-sectional study in UK Biobank
Objectives: to examine whether acetabular dysplasia (AD), cam and/or pincer morphology are associated with radiographic hip osteoarthritis (rHOA) and hip pain in UK Biobank (UKB) and, if so, what distribution of osteophytes is observed.Design: participants from UKB with a left hip dual-energy X-ray absorptiometry (DXA) scan had alpha angle (AA), lateral centre-edge angle (LCEA) and joint space narrowing (JSN) derived automatically. Cam and pincer morphology, and AD were defined using AA and LCEA. Osteophytes were measured manually and rHOA grades were calculated from JSN and osteophyte measures. Logistic regression was used to examine the relationships between these hip morphologies and rHOA, osteophytes, JSN, and hip pain.Results: 6,807 individuals were selected (mean age: 62.7; 3382/3425 males/females). Cam morphology was more prevalent in males than females (15.4% and 1.8% respectively). In males, cam morphology was associated with rHOA [OR 3.20 (95% CI 2.41–4.25)], JSN [1.53 (1.24–1.88)], and acetabular [1.87 (1.48–2.36)], superior [1.94 (1.45–2.57)] and inferior [4.75 (3.44–6.57)] femoral osteophytes, and hip pain [1.48 (1.05–2.09)]. Broadly similar associations were seen in females, but with weaker statistical evidence. Neither pincer morphology nor AD showed any associations with rHOA or hip pain.Conclusions: cam morphology was predominantly seen in males in whom it was associated with rHOA and hip pain. In males and females, cam morphology was associated with inferior femoral head osteophytes more strongly than those at the superior femoral head and acetabulum. Further studies are justified to characterise the biomechanical disturbances associated with cam morphology, underlying the observed osteophyte distribution
Gender and Acute Myocardial Infarction: Is There a Different Response to Thrombolysis?
AbstractObjectives. This study sought to 1) determine the effect of gender on early and late infarct-related artery patency and reocclusion after thrombolytic therapy for acute myocardial infarction; 2) examine the effect of gender on left ventricular function in response to injury/reperfusion; and 3) assess the independent contribution of gender to early (30-day) mortality after acute myocardial infarction.Background. Women have a higher mortality rate than men after myocardial infarction. However, the effect of gender on infarct-related coronary artery patency and left ventricular response to injury/reperfusion have not been fully defined in the thrombolytic era.Methods. Patency rates and global and regional left ventricular function were determined in patients at 90 min and 5 to 7 days after thrombolytic therapy for acute myocardial infarction. The effect of gender on infarct-related artery patency and left ventricular function was determined. Thirty-day mortality differences between women and men were compared.Results. Women were significantly older and had more hypertension, diabetes, hypercholesterolemia, heart failure and shock. They were less likely to have had a previous myocardial infarction, history of smoking or previous bypass surgery. Ninety-minute patency rates (Thrombolysis in Myocardial Infarction [TIMI] flow grade 3) in women and men were 39% and 38%, respectively (p = 0.5). Reocclusion rates were 8.7% in women versus 5.1% in men (p = 0.14). Women had more recurrent ischemia than men (21.4% vs. 17.0%, respectively, p = 0.01). Ninety-minute ejection fraction and regional ventricular function were clinically similar in women and men with TIMI 2 or 3 flow (ejection fraction [mean ± SD]: 63.4 ± 6% vs. 59.4 ± 0.7%, p = 0.02; number of chords: 21.4 ± 0.9 vs. 21.0 ± 1.9, p = 0.7; SD/chord: −2.4 ± 08 vs. −2.4 ± 0.2, p = 0.9, respectively). No clinically significant differences in left ventricular function were noted at 5- to 7-day follow-up. Women had a greater hyperkinetic response than men in the noninfarct zone (SD/chord: 2.4 ± 0.2 vs. 1.7 ± 0.1, p = 0.005). The 30-day mortality rate was 13.1% in women versus 4.8% in men (p ≤ 0.0001). After adjustment for other clinical and angiographic variables, gender remained an independent determinant of 30-day mortality.Conclusions. Women do not differ significantly from men with regard to either early infarct-related artery patency rates or reocclusion after thrombolytic therapy or ventricular functional response to injury/reperfusion. Gender was an independent determinant of 30-day mortality after acute myocardial infarction.(J Am Coll Cardiol 1997;29:35–42)
Hypophosphataemia among Severely-malnourished Children: Case Series
Phosphorus is an essential substance in our body, and hypophosphataemia
(HP) is well-described in rickets, refeeding syndrome, diabetic
ketoacidosis (DKA), and in chronic alcohol-abuse. However, to our
knowledge, HP among severely-malnourished children has not been studied
in detail, and information on prevalence, severity, and treatment is
scarce. Currently, there are only a few published case reports of HP.
This case series describes three cases of HP that presented to Dhaka
Hospital of the International Centre for Diarrhoeal Disease Research,
Bangladesh (icddr,b). Our first case required mechanical ventilation
for respiratory distress associated with severe hypokalaemia (K 1.1
mmol/L) and moderate hypophosphataemia (P 2.1 mg/dL). The second case
presented with severe sepsis which was associated with symptomatic
hypocalcaemia (Ca 1.68 mmol/L), hypokalaemia (K 1.82 mmol/L), and
severe hypophosphataemia (P 0.9 mg/dL). The third case presented with
pneumonia and sepsis which were complicated by hypokalaemia (K 2.05
mmol/L) and severe hypophosphataemia (P 1.1 mg/dL). Marked lethargy and
severe hypotonia were associated with HP in all of these cases.
Manifestations of HP are diverse and can occur in association with
other electrolyte imbalances, especially among malnourished children.
Malnutrition, combined with sepsis, is one of the major killers of
children younger than 5 years of age, and both malnutrition and sepsis
can cause HP. It is concluded that the underlying causes of morbidity,
including HP, should be actively sought and treated to reduce the
mortality of children aged below five years
AMI Large Array radio continuum observations of Spitzer c2d small clouds and cores
We perform deep 1.8 cm radio continuum imaging towards thirteen protostellar
regions selected from the Spitzer c2d small clouds and cores programme at high
resolution (25") in order to detect and quantify the cm-wave emission from
deeply embedded young protostars. Within these regions we detect fifteen
compact radio sources which we identify as radio protostars including two
probable new detections. The sample is in general of low bolometric luminosity
and contains several of the newly detected VeLLO sources. We determine the 1.8
cm radio luminosity to bolometric luminosity correlation, L_rad -L_bol, for the
sample and discuss the nature of the radio emission in terms of the available
sources of ionized gas. We also investigate the L_rad-L_IR correlation and
suggest that radio flux density may be used as a proxy for the internal
luminosity of low luminosity protostars.Comment: submitted MNRA
Statistical evaluation of 571 GaAs quantum point contact transistors showing the 0.7 anomaly in quantized conductance using millikelvin cryogenic on-chip multiplexing
The mass production and the practical number of cryogenic quantum devices producible in a single chip are limited to the number of electrical contact pads and wiring of the cryostat or dilution refrigerator. It is, therefore, beneficial to contrast the measurements of hundreds of devices fabricated in a single chip in one cooldown process to promote the scalability, integrability, reliability, and reproducibility of quantum devices and to save evaluation time, cost and energy. Here, we use a cryogenic on-chip multiplexer architecture and investigate the statistics of the 0.7 anomaly observed on the first three plateaus of the quantized conductance of semiconductor quantum point contact (QPC) transistors. Our single chips contain 256 split gate field effect QPC transistors (QFET) each, with two 16-branch multiplexed source-drain and gate pads, allowing individual transistors to be selected, addressed and controlled through an electrostatic gate voltage process. A total of 1280 quantum transistors with nano-scale dimensions are patterned in 5 different chips of GaAs heterostructures. From the measurements of 571 functioning QPCs taken at temperatures T= 1.4 K and T= 40 mK, it is found that the spontaneous polarisation model and Kondo effect do not fit our results. Furthermore, some of the features in our data largely agreed with van Hove model with short-range interactions. Our approach provides further insight into the quantum mechanical properties and microscopic origin of the 0.7 anomaly in QPCs, paving the way for the development of semiconducting quantum circuits and integrated cryogenic electronics, for scalable quantum logic control, readout, synthesis, and processing applications
Canada-Africa Relations in Changing Core-Periphery Dynamics: A Chance to "Come Back" Differently
The Department of Foreign Affairs Canada sees the dynamism at play across the African continent as calling out for Canadian engagement. Africa in the twenty-first century is no longer the continent emerging from colonial rule; it seeks new forms of relationships with international partners. The African Development Bank, for instance, has identified five priorities for inclusive growth on the continent. The challenges are huge, as is the potential for transformative change. But the conditions for international collaboration in achieving these goals have changed; African leaders are seeking new forms of associations and teamwork. Canada has an opportunity to "come back" differently if it can look beyond its narrow mining interests and become an active partner working with public authorities in need of new and bold international partnerships. Unfortunately, Trudeau's "Canada is back" campaign does not look set to change the status quo. And, in a world where the political economic power is moving east, African countries do not have much reason to listen to Canada
