    Desecuritising cybersecurity: towards a societal approach

    Cybersecurity is often treated as a national security issue with responses to attacks implemented by military and intelligence agencies. This has created path dependencies in which tensions between the private sector and government have continued, where over-classification of cyberthreats has occurred, and where the broader societal impacts of malicious use of the internet have been underestimated. Drawing on the societal security concept established by the Copenhagen School of International Relations, we seek to reframe cybersecurity theory and policy. In the first section of the article we establish a theoretical approach to cybersecurity that emphasises the impact of cyberattacks on society, including on the health, energy and transport sectors. The second section draws on the history of cyberconflict to assess the ways the internet has been used to exacerbate societal tensions between identity groups and to create incohesion and societal security dilemmas. This section reinterprets the way the Kosovo War, Millennium (Y2 K) Bug, 9/11 and the WannaCry incident shaped and reflected cyber policy. The final section explores how a process of cyber desecuritisation might be achieved, including through discursive change and an enhanced role for the societal sector in the event of major cyberattacks

    The state of web accessibility for people with cognitive disabilities: a rapid evidence assessment

    Increased digitisation of day-to-day activities was occurring prior to the COVID-19 pandemic. The pandemic only accelerated the virtual shift, making web accessibility an urgent issue, especially for marginalised populations. Despite decades of work to develop, refine, and implement web accessibility standards, people with cognitive disabilities regularly experience many barriers to web accessibility. To inform ongoing work to improve web accessibility for people with cognitive disabilities, a systematic review was conducted. The main question guiding this review is: what are the state-of-the-art of interventions that support web accessibility for citizens, 9 years of age and up, living with cognitive impairment? A set of 50 search strings were entered into three academic databases: SCOPUS, ProQuest, and Web of Science. Systematic screening procedures narrowed the search returns to a total of 45 included papers. A data analysis revealed themes associated with the lived experiences of people with cognitive disabilities, tools for improving web accessibility, and methodological best practices for involving people with cognitive disabilities in research. These findings have immediate implications for ongoing research and the development of meaningful solutions to the problem of web accessibility for people with cognitive disabilities.info:eu-repo/semantics/publishedVersio

    Recursive quantum repeater networks

    Internet-scale quantum repeater networks will be heterogeneous in physical technology, repeater functionality, and management. The classical control necessary to use the network will therefore face similar issues as Internet data transmission. Many scalability and management problems that arose during the development of the Internet might have been solved in a more uniform fashion, improving flexibility and reducing redundant engineering effort. Quantum repeater network development is currently at the stage where we risk similar duplication when separate systems are combined. We propose a unifying framework that can be used with all existing repeater designs. We introduce the notion of a Quantum Recursive Network Architecture, developed from the emerging classical concept of 'recursive networks', extending recursive mechanisms from a focus on data forwarding to a more general distributed computing request framework. Recursion abstracts independent transit networks as single relay nodes, unifies software layering, and virtualizes the addresses of resources to improve information hiding and resource management. Our architecture is useful for building arbitrary distributed states, including fundamental distributed states such as Bell pairs and GHZ, W, and cluster states.Comment: 14 page

    Association between benzodiazepine coprescription and mortality in people on opioid replacement therapy: a population-based cohort study

    Objective To investigate the association between opioid replacement therapy (ORT) and benzodiazepine (BZD) coprescription and all-cause mortality compared with the prescription of ORT alone. Design Population-based cohort study. Setting Scotland, UK. Participants Participants were people prescribed ORT between January 2010 and end of December 2020 aged 18 years or above. Main outcome measures All-cause mortality, drug-related deaths and non-drug related deaths. Secondary outcome ORT continuous treatment duration. Analysis Cox regression with time-varying covariates. Results During follow-up, 5776 of 46 899 participants died: 1398 while on coprescription and 4378 while on ORT only. The mortality per 100 person years was 3.11 during coprescription and 2.34 on ORT only. The adjusted HR for all-cause mortality was 1.17 (1.10 to 1.24). The adjusted HR for drug-related death was 1.14 (95% CI, 1.04 to 1.24) and the hazard for death not classified as drug-related was 1.19 (95% CI, 1.09 to 1.30). Conclusion Coprescription of BZDs in ORT was associated with an increased risk of all-cause mortality, although with a small effect size than the international literature. Coprescribing was also associated with longer retention in treatment. Risk from BZD coprescription needs to be balanced against the risk from illicit BZDs and unplanned treatment discontinuation. A randomised controlled trial is urgently needed to provide a clear clinical direction. Trial registration number NCT0462299

    Processing Apples to Puree or Juice Speeds Gastric Emptying and Reduces Postprandial Intestinal Volumes and Satiety in Healthy Adults

    Background: Whole apples produce greater satiety than processed apples, but the underlying mechanisms remain unclear. Objective: Our aim was to assess the intragastric processing of apple preparations and the associated small and large bowel contents using MRI. Methods: An open label , 3-way crossover, randomized, controlled trial. 18 healthy adults [mean ± SD age, 25±4y; BMI(kg/m 2), 22.7±3.5] underwent serial MR scans on 3 occasions separated by 7d, after consumption of isocaloric (178 kcal) portions of either whole apples, apple puree or apple juice. Gastric emptying, small bowel water content (SBWC)(Primary endpoint), were measured at baseline and at 45 minutes intervals (0-270 min) post meal ingestion. Fullness and satiety were also assessed at each time point. Treatment effects between groups were analyzed using ANOVA. Results: Gastric emptying half-time (GE t 50) was greater (P < 0.0001) after participants consumed whole apple (mean ± SEM), 65 (3.3) min) compared with when they consumed apple puree (41(2.8) min) or apple juice (38 (2.9) min), times which did not differ. Postprandial AUC (135-270 min) SBWC was also greater for whole apples than puree (P = 0.025) and juice (P = 0.0004) but juice and puree did not differ. AUC for fullness and satiety (0-270 min) post-ingestion was also greater (P = 0.002 and 0.004 respectively) for whole apple compared to juice but juice and puree did not differ. Conclusions: Gastric emptying is slower after whole apples consumption causing greater sensation of fullness and satiety than puree or juice in healthy adults. Whole apples increased small bowel and colonic contents during the later phase of the study which may be relevant for subsequent food consumption.     About this Executive Summary This comprehensive executive summary reports on a case study designed to demonstrate the application of a number of the key concepts and methods developed in the INTEGRATE-HTA project to the assessment of complex technologies. The case study focuses on models of home based palliative care with and without an additional element of caregiver support, known as reinforced and non-reinforced home based palliative care respectively. What is already known about the topic With changing disease patterns in Europe, increasingly complex health care technologies, such as palliative care, have gained importance. Current HTA methods rarely take account of wider legal, ethical and socio-cultural issues or context and implementation and are not adequately equipped to assess highly complex technologies, despite considerable progress in recent years. As a result, HTA is rarely applied to highly complex health technologies. What this case study report adds The INTEGRATE-HTA project developed concepts and methods for the assessment of complex technologies, taking into account legal, ethical and socio-cultural issues as well as context and implementation. This case study is designed to demonstrate the application of a number of the key concepts and methods developed in the INTEGRATE-HTA project to home based palliative care, with and without an additional element of caregiver support, as an example of a complex technology; known as “reinforced” and “non-reinforced” home based palliative care respectively. The INTEGRATE-HTA model, developed to enable integration of relevant assessment aspects, is used to structure this report. The case study reports on the application of some of the concepts and methods developed within the INTEGRATE-HTA project to the assessment of effectiveness as well as economic, sociocultural, ethical, and legal aspects; patient preferences and patient-specific moderators of treatment and context and implementation issues related to reinforced and non-reinforced home based palliative care. The highly complex nature of reinforced and non-reinforced home based palliative care is illustrated through an assessment of complexity characteristics. | 4 The case study also involved extensive lay and professional stakeholder involvement, using a variety of methods, to inform the HTA process at key stages throughout the project. Key messages are highlighted for both HTA research and for palliative care. The implications for research and practice The concepts and methods developed in the INTEGRATE-HTA project have been shown to be feasible and to have the potential to offer added value, but require further development and application in the assessment of other complex technologies. Reinforced and non-reinforced models palliative care are highly complex. The case study findings offer some insights into their effectiveness as well as economic, sociocultural, ethical, and legal issues; patient preferences and patient-specific moderators of treatment as well as context and implementation issues
