Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial
Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients. Antiulcerogenic Activity of the Essential Oil of Baccharis dracunculifolia on Different Experimental Models in Rats
Baccharis dracunculifolia DC (Asteraceae), a native plant from Brazil, commonly known as `Alecrim-do-campo` is widely used in folk medicine to treat inflammation, hepatic disorders and stomach ulcers, and it is the most important botanical source of Southeastern Brazilian propolis, known as green propolis. Its essential oil is composed of non-oxygenated and oxygenated terpenes. In this work, the effects of the essential oil obtained from the aerial parts of R dracunculifolia on gastric ulcers were evaluated. The antiulcer assays were undertaken using the following protocols in rats: nonsteroidal antiinflammatory drug (NSAID)-induced ulcer, ethanol-induced ulcer, stress-induced ulcer, and determination of gastric secretion using ligated pylorus. The treatment in the doses of 50, 250 and 500 mg/kg of R dracunculifolia essential oil significantly diminished the lesion index, the total lesion area and the percentage of lesions in comparison with both positive and negative control groups. With regard to the model of gastric secretion a reduction of gastric juice volume and total acidity was observed, as well as an increase in the gastric pH. No sign of toxicity was observed in the acute toxicity study. Considering the results, it is suggested that the essential oil of B. dracunculifolia could probably be a good therapeutic agent for the development of new phytotherapeutic medicine for the treatment of gastric ulcer. CARACTERĂSTICAS CLĂNICO-EPIDEMIOLĂGICAS DE ADULTOS COM AIDS INTERNADOS EM HOSPITAL DE REFERĂNCIA NO MUNICĂPIO DE SALVADOR â BAHIA
Introdução: A epidemia de AIDS Ă© de importĂąncia global e tem sofrido transformaçÔes em seus aspectos epidemiolĂłgicos que merecem ser investigados para o seu enfrentamento. Objetivo: Caracterizar o perfil clĂnico-epidemiolĂłgico de pacientes na faixa etĂĄria de 18 a 59 anos com AIDS internados em hospital de referĂȘncia em infectologia no municĂpio de Salvador-BA. MĂ©todos: Trata-se de um estudo descritivo, revisados os prontuĂĄrios eletrĂŽnicos de pacientes com idade entre 18 e 59 anos, internados em um hospital de referĂȘncia de Salvador-BA entre julho de 2022 a janeiro de 2023. Resultados: A amostra (nâŻ=âŻ111) foi composta de maioria pacientes cisgĂȘnero, 71,17% sexo masculino, 25,23% feminino e 3,6% se identificavam como mulheres transexuais. A maioria se classifica como heterossexuais (63,03%), pardos (70,27%), solteiros (79,28%), cuja idade Ă© contemplada pela mediana 38 e chama atenção que 56,25% encontram-se abaixo de 40 anos. 32,43% dos pacientes contavam com menos de um salĂĄrio mĂnimo mensal, 43,24% possuĂam ensino fundamental incompleto. A principal causa de internamento foi tuberculose (30,51%), seguido de neurotoxoplasmose (15,25%) e pneumocistose (9,32%). Para 54,24% dos pacientes o diagnĂłstico de SIDA ocorreu nesse primeiro internamento. Em relação aos hĂĄbitos de vida, 23,72% dos pacientes eram usuĂĄrios simultĂąneos de ĂĄlcool, tabaco e substĂąncias psicoativas, enquanto 51,69% nĂŁo fazia uso de nenhum dos 3. Observou-se resultados positivos para as sorologias descritas: VDRL 27,96%, AgHBs 5,93%, HCV 1,69%, HTLV 0%. Quanto aos desfechos, 81,36% alta, 11,02% Ăłbito, 2,54% transferidos para outras instituiçÔes, e 5,08% evadiram do serviço. Metade dos que evadiram foram reinternados no perĂodo analisado. Entre os pacientes que vieram a Ăłbito a contagem de linfĂłcitos TCD4 < 200 cel/mm3 (variou de 2 a 165 cel/mm3), e mĂ©dia de 40,69 cel/mm3, enquanto essa mesma contagem para a totalidade de pacientes internados variou entre 2 e 1269 cel/mm3, com uma mĂ©dia de 202,45 cel/mm3. Apenas 17,8% da amostra apresentava carga viral indetectĂĄvel, e 27,12% nĂŁo aderiram de forma satisfatĂłria ao tratamento. ConclusĂŁo: O estudo aponta para um perfil de pacientes em sua maioria do sexo masculino, heterossexuais, jovens, com diagnĂłstico recente de HIV ou mĂĄ adesĂŁo terapĂȘutica, incorrendo em internamentos por doenças infecciosas. Aumentar campanhas de prevenção, trabalhar diagnĂłstico precoce e adesĂŁo ao tratamento sĂŁo medidas urgentes para se conseguir as metas da OMS
CARACTERĂSTICAS CLĂNICO-EPIDEMIOLĂGICAS DE PACIENTES IDOSOS COM AIDS INTERNADOS EM HOSPITAL DE REFERĂNCIA NO MUNICĂPIO DE SALVADOR, BAHIA
Introdução/Objetivo: O vĂrus da imunodeficiĂȘncia humana (HIV), causador da AIDS, ainda afeta milhĂ”es de pessoas no mundo, o que impacta no nĂșmero de idosos infectados. O aumento da expectativa de vida da população geral e a invisibilidade com que Ă© tratada a exposição ao risco nessa faixa etĂĄria leva ao diagnĂłstico tardio. Tem-se como objetivo investigar as caracterĂsticas epidemiolĂłgicas de pacientes idosos com AIDS internados em hospital de referĂȘncia no municĂpio de Salvador, estado da Bahia. MĂ©todos: Foram revisados os prontuĂĄrios eletrĂŽnicos de pacientes idosos internados com AIDS em hospital de referĂȘncia em infectologia em Salvador - BA durante o ano de 2022. Resultados: Entre os 123 pacientes internados cujos prontuĂĄrios foram avaliados, 12 eram idosos, procedentes de Salvador, de idade entre 60 e 78 anos (mediana 64,5), do sexo masculino (66,6%) e feminino (33,3%), de cor autodeclarada parda (91,6%) e amarelo (8,3%) e a maioria com ensino fundamental incompleto (58,3%); A mediana da contagem de cĂ©lulas CD4 foi 198,5 (variando 37 â 1400), apenas 33,3% da amostra possui CV indetectĂĄvel (âŒindetectĂĄvel a > 1 milhĂŁo). 16% (2 pacientes) descobriram o HIV nessa internação e os demais tinham conhecimento entre 2 e 30 anos. As manifestaçÔes clĂnicas mais frequentes observadas foram febre e perda ponderal, um paciente foi internado por neurotoxoplasmose e os demais por condiçÔes clinicas (ex. pielonefrite, celulite, colecistite, infecção urinĂĄria). A mediana do tempo de permanĂȘncia foi de 14 dias, 83,3% dos idosos internados receberam alta para acompanhamento ambulatorial e 8,3% evoluĂram a Ăłbito. ConclusĂŁo: Os resultados apontam a AIDS entre idosos pardos associados com baixa escolaridade, reflexo da falta de conhecimento sobre os meios de prevenção contra o HIV pela ausĂȘncia de campanhas e testagem nessa população. Ademais, esses resultados destacam a importĂąncia do acompanhamento mĂ©dico contĂnuo para os idosos vivendo com HIV/AIDS, visando controlar a progressĂŁo da doença, prevenir complicaçÔes e melhorar os desfechos clĂnicos. O tratamento antirretroviral e o suporte adequado sĂŁo essenciais para garantir a qualidade de vida e reduzir a morbidade e a mortalidade nessa população
PERFIL EPIDEMIOLĂGICO E CLĂNICO DOS PACIENTES INTERNADOS COINFECTADOS COM SĂFILIS E HIV EM UM HOSPITAL REFERĂNCIA, EM SALVADOR, BAHIA
Introdução: O HIV e a sĂfilis sĂŁo doenças sexualmente transmissĂveis, sendo frequente quadros de coinfecção. A sĂfilis aumenta o risco de transmissĂŁo do HIV por causar Ășlceras genitais e o HIV piora o desfecho da sĂfilis. Assim, Ă© de suma importĂąncia reconhecer a interação entre ambas as doenças. Objetivo: Descrever o perfil epidemiolĂłgico e clĂnico dos pacientes coinfectados com sĂfilis e HIV internados em um hospital especializado em doenças infectocontagiosas, no municĂpio de Salvador, Bahia, de 2022 a 2023. MĂ©todo: Trata-se de um estudo transversal descritivo, informaçÔes obtidas no prontuĂĄrio mĂ©dico eletrĂŽnico, entre 2022 e 2023, com amostra de conveniĂȘncia. Resultados: Foram analisados 131 perfis de indivĂduos com HIV/AIDS, sendo que 33 possuĂam VDRL sĂ©rico positivo (25.19%). A maioria dos pacientes que apresentavam coinfecção eram do sexo masculino (75.76%), de orientação heterossexual (48.48%), solteiro (84.85%), sem filhos (63.64%), de cor autodeclarada parda (63.64%), com ensino fundamental incompleto (45.45%), renda inferior a um salĂĄrio mĂnimo (30.30%) e sem religiĂŁo (39.39%). 24.24% faziam uso somente de ĂĄlcool e tabaco e 24.24% eram alcoolistas, tabagistas e usuĂĄrios de substĂąncias psicoativas. A idade mĂ©dia dos pacientes foi 37.5 anos (24-66). No geral, nĂŁo possuĂam internaçÔes prĂ©vias (54.55%), outras comorbidades (75.76%) e receberam alta melhorado (81.82%). A mĂ©dia da contagem de cĂ©lulas CD4 foi 148.3 (4-615), sendo que 9.09% tinham CV indetectĂĄvel, 57.58% possuĂa diagnĂłstico de HIV hĂĄ mais de 1 ano e 12.12% iniciaram a TARV durante o internamento. 18.8% tiveram como diagnĂłstico neurossĂfilis, observando-se como sintomas frequentes nesses pacientes: febre (50%), dĂ©ficit motor (66.67%) e perda ponderal (50%). Entre os pacientes com neurossĂfilis que realizaram estudo do lĂquido cefalorraquidiano (83.33%), notou-se uma mĂ©dia de 51.6 de celularidade (2-127), 95.2 de proteĂna (27-157), 61.2 (44-91) de glicose e VDRL positivo em 40% dos casos. 3,03% desenvolveu sĂfilis ocular. ConclusĂŁo: Houve predomĂnio de homens jovens adultos, pardos, heterossexuais, solteiros, com ensino fundamental incompleto, renda inferior a um salĂĄrio mĂnimo, sem internaçÔes prĂ©vias e outras comorbidades, que faziam uso de ĂĄlcool, tabaco e substĂąncias psicoativas. PERFIL EPIDEMIOLĂGICO DOS PACIENTES USUĂRIOS DE SUBSTĂNCIAS PSICOATIVAS COM AIDS INTERNADOS EM UM HOSPITAL DE REFERĂNCIA EM SALVADOR, BAHIA
Introdução: A infecção pelo HIV tem alta prevalĂȘncia em pacientes usuĂĄrios de substĂąncias psicoativas. Essa associação contribui para piores desfechos de saĂșde individuais e representa uma significativa barreira no combate Ă transmissĂŁo do vĂrus e aderĂȘncia ao tratamento. Objetivos: Apresentar o perfil clĂnico-epidemiolĂłgico dos pacientes usuĂĄrios de substĂąncias ilĂcitas internados com AIDS em um hospital especializado em doenças infecto-contagiosas na capital da Bahia. MĂ©todos: Neste estudo transversal, foi realizada uma anĂĄlise descritiva dos dados levantados por meio do prontuĂĄrio eletrĂŽnico de pacientes internados no hospital entre julho e outubro de 2022. Foram incluĂdos pacientes com 18 anos ou mais, portadores do vĂrus HIV e usuĂĄrios de maconha, cocaĂna ou crack. Resultados: Dos 131 prontuĂĄrios revisados, houve 47 internaçÔes de 43 pacientes usuĂĄrios de substĂąncias psicoativas com AIDS. Destes: 62,79% (27) eram homens, 79,07% eram heterossexuais, 72,09% eram autodeclarados pardos, 58,14% possuĂam ensino fundamental incompleto, 76,74% eram solteiros, 48,84% tinham renda inferior a um salĂĄrio-mĂnimo. A idade mĂ©dia dos pacientes foi de 39,27 (Min 18; Max 72) anos, o tempo mĂ©dio de internação foi de 26,17 dias, 51,16% dos pacientes tiveram pelo menos 1 internação prĂ©via no mesmo hospital, a mediana do CD4/mm3 desses pacientes foi de 132,5 (10 a 1266), e mediana da carga viral foi de 222.702 (26 a 9.868.945); 12 (28,0%) pacientes referiam boa adesĂŁo a TARV. 12 (28%) pacientes tiveram o diagnĂłstico de HIV nessa internação e a mĂ©dia de CD4 nesse grupo foi de 111 cel/mm3. Tuberculose (26,32%), Toxoplasmose (19,3%) foram as principais infecçÔes diagnosticadas durante o internamento. VDRL reagente em 13 (30,2%), HCV 2 (4,6%), Ag Hbs 1 (2,3%) dos pacientes. Desfecho foi alta com melhora em 76,7%, evasĂŁo/alta a pedido em 11,64% e 11,64% evoluĂram para Ăłbito. ConclusĂŁo: Na amostra, houve predomĂnio de homens adultos, pardos, heterossexuais, com baixo nĂvel de escolaridade e renda. Dentre as causas de internação prevaleceram as coinfecçÔes por doenças oportunistas. Esses apresentaram internaçÔes de longa duração, com altos Ăndices de mortalidade, evasĂŁo do serviço e reinternação. Neotropical freshwater fisheries : A dataset of occurrence and abundance of freshwater fishes in the Neotropics
The Neotropical region hosts 4225 freshwater fish species, ranking first among the world's most diverse regions for freshwater fishes. Our NEOTROPICAL FRESHWATER FISHES data set is the first to produce a large-scale Neotropical freshwater fish inventory, covering the entire Neotropical region from Mexico and the Caribbean in the north to the southern limits in Argentina, Paraguay, Chile, and Uruguay. We compiled 185,787 distribution records, with unique georeferenced coordinates, for the 4225 species, represented by occurrence and abundance data. The number of species for the most numerous orders are as follows: Characiformes (1289), Siluriformes (1384), Cichliformes (354), Cyprinodontiformes (245), and Gymnotiformes (135). The most recorded species was the characid Astyanax fasciatus (4696 records). We registered 116,802 distribution records for native species, compared to 1802 distribution records for nonnative species. The main aim of the NEOTROPICAL FRESHWATER FISHES data set was to make these occurrence and abundance data accessible for international researchers to develop ecological and macroecological studies, from local to regional scales, with focal fish species, families, or orders. We anticipate that the NEOTROPICAL FRESHWATER FISHES data set will be valuable for studies on a wide range of ecological processes, such as trophic cascades, fishery pressure, the effects of habitat loss and fragmentation, and the impacts of species invasion and climate change. Enhanced infection prophylaxis reduces mortality in severely immunosuppressed HIV-infected adults and older children initiating antiretroviral therapy in Kenya, Malawi, Uganda and Zimbabwe: the REALITY trial

Introduction: Mortality from infections is high in the first 6 months of antiretroviral therapy (ART) among HIVâinfected adults and children with advanced disease in subâSaharan Africa. Whether an enhanced package of infection prophylaxis at ART initiation would reduce mortality is unknown.
Methods:
The REALITY 2Ă2Ă2 factorial openâlabel trial (ISRCTN43622374) randomized ARTânaĂŻve HIVâinfected adults and children >5 years with CD4 <100 cells/mm3. This randomization compared initiating ART with enhanced prophylaxis (continuous cotrimoxazole plus 12 weeks isoniazid/pyridoxine (antiâtuberculosis) and fluconazole (antiâcryptococcal/candida), 5 days azithromycin (antiâbacterial/protozoal) and singleâdose albendazole (antiâhelminth)), versus standardâofâcare cotrimoxazole. Isoniazid/pyridoxine/cotrimoxazole was formulated as a scored fixedâdose combination. Two other randomizations investigated 12âweek adjunctive raltegravir or supplementary food. The primary endpoint was 24âweek mortality.
Results:
1805 eligible adults (n = 1733; 96.0%) and children/adolescents (n = 72; 4.0%) (median 36 years; 53.2% male) were randomized to enhanced (n = 906) or standard prophylaxis (n = 899) and followed for 48 weeks (3.8% lossâtoâfollowâup). Median baseline CD4 was 36 cells/mm3 (IQR: 16â62) but 47.3% were WHO Stage 1/2. 80 (8.9%) enhanced versus 108(12.2%) standard prophylaxis died before 24 weeks (adjusted hazard ratio (aHR) = 0.73 (95% CI: 0.54â0.97) p = 0.03; Figure 1) and 98(11.0%) versus 127(14.4%) respectively died before 48 weeks (aHR = 0.75 (0.58â0.98) p = 0.04), with no evidence of interaction with the two other randomizations (p > 0.8). Enhanced prophylaxis significantly reduced incidence of tuberculosis (p = 0.02), cryptococcal disease (p = 0.01), oral/oesophageal candidiasis (p = 0.02), deaths of unknown cause (p = 0.02) and (marginally) hospitalisations (p = 0.06) but not presumed severe bacterial infections (p = 0.38). Serious and grade 4 adverse events were marginally less common with enhanced prophylaxis (p = 0.06). CD4 increases and VL suppression were similar between groups (p > 0.2).
Conclusions:
Enhanced infection prophylaxis at ART initiation reduces early mortality by 25% among HIVâinfected adults and children with advanced disease. The pill burden did not adversely affect VL suppression. Policy makers should consider adopting and implementing this lowâcost broad infection prevention package which could save 3.3 lives for every 100 individuals treated
