    Ocular surface immune transcriptome and tear cytokines in corneal infection patients

    BackgroundMicrobial keratitis is one of the leading causes of blindness globally. An overactive immune response during an infection can exacerbate damage, causing corneal opacities and vision loss. This study aimed to identify the differentially expressed genes between corneal infection patients and healthy volunteers within the cornea and conjunctiva and elucidate the contributing pathways to these conditions’ pathogenesis. Moreover, it compared the corneal and conjunctival transcriptomes in corneal-infected patients to cytokine levels in tears.MethodsCorneal and conjunctival swabs were collected from seven corneal infection patients and three healthy controls under topical anesthesia. RNA from seven corneal infection patients and three healthy volunteers were analyzed by RNA sequencing (RNA-Seq). Tear proteins were extracted from Schirmer strips via acetone precipitation from 38 cases of corneal infection and 14 healthy controls. The cytokines and chemokines IL-1β, IL-6, CXCL8 (IL-8), CX3CL1, IL-10, IL-12 (p70), IL-17A, and IL-23 were measured using an antibody bead assay.ResultsA total of 512 genes were found to be differentially expressed in infected corneas compared to healthy corneas, with 508 being upregulated and four downregulated (fold-change (FC) <−2 or > 2 and adjusted p <0.01). For the conjunctiva, 477 were upregulated, and 3 were downregulated (FC <−3 or ≥ 3 and adjusted p <0.01). There was a significant overlap in cornea and conjunctiva gene expression in patients with corneal infections. The genes were predominantly associated with immune response, regulation of angiogenesis, and apoptotic signaling pathways. The most highly upregulated gene was CXCL8 (which codes for IL-8 protein). In patients with corneal infections, the concentration of IL-8 protein in tears was relatively higher in patients compared to healthy controls but did not show statistical significance.ConclusionsDuring corneal infection, many genes were upregulated, with most of them being associated with immune response, regulation of angiogenesis, and apoptotic signaling. The findings may facilitate the development of treatments for corneal infections that can dampen specific aspects of the immune response to reduce scarring and preserve sight

    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    Evaluation of Strategies for Reducing The Incidence of Adverse Events during Extended Wear of Silicone Hydrogel Contact Lenses

    Ocular inflammation secondary to bacterial contamination of contact lenses during wear remains a problem. Rare but sight-threatening contact lens related corneal infection can also occur. Previous studies have shown that overnight lens wear in the same lens results in increased risk of corneal infection and contamination of the lens surface by bacteria has been associated with adverse responses. Increased bacterial load on lenses is a risk factor for development of corneal infection. The objective of this thesis was to investigate the effect on adverse event incidence when a range of lens maintenance interventions were adopted, during extended wear, including daily lens replacement, daily lens cleaning or daily antibiotic drop instillation. The hypothesis tested was that a new or cleaned lens or one exposed to an antibiotic, during extended wear, would result in less bacterially-driven adverse events.A prospective extended wear clinical study (with no intervention) was conducted to determine the risks related to extended wear and adverse event incidence. This study determined the incidence rate of infiltrative and mechanical events and also showed that lens contamination increased the risk of developing inflammatory adverse eventsThis was followed by a series of extended wear studies, each investigating the effect of different lens maintenance interventions on adverse event incidence at the same study site. Nightly lens replacement during extended wear had the least reduction in adverse events. Handling lenses prior to overnight lens wear appeared to overwhelm the benefit offered by a new lens surface. Morning lens replacement during extended wear resulted in reduced incidence of mechanical (1.2% vs 5.2%, p = 0.02) and total adverse events (4.0% vs 8.9%, p = 0.04). The interventions of morning lens replacement, daily lens cleaning and daily antibiotic drop instillation, during extended wear, did not significantly reduce infiltrative adverse events. However, overall there was a consistent trend toward reduced adverse events with these interventions compared to the control groups suggesting that reduced exposure to potentially contaminated lenses might reduce complications which has implications for development of contact lenses with an antimicrobial surface

    Clinical appearance and microscopic analysis of mucin balls associated with contact lens wear

    Purpose: The structure of mucin balls collected from silicone hydrogel contact lens wearers was examined to determine their nature. Methods: Tears containing mucin balls were collected using a capillary tube. These were processed for light microscopic histochemistry, scanning electron microscopy, and electron microscopic elemental analysis. Mucin balls were also observed in vivo using confocal microscopy. Results: Histology showed that the mucin balls were PAS positive, indicating that glycoproteins form a major component. Lipids and bacteria were not detected. Scanning electron microscopy did not show the surface to be smooth but revealed a variation in density across the surface. Elemental analysis was inconclusive. Conclusions: Mucin balls are likely to be made from collapsed mucin and are unlikely to have been formed as a result of pearling around a silicon, lipid, or bacterial kernel

    Quantification of protein deposits on silicone hydrogel materials using stable-isotopic labeling and multiple reaction monitoring

    This study was designed to use multiple reaction monitoring (MRM) for accurate quantification of contact lens protein deposits. Worn lenses used with a multipurpose disinfecting solution were collected after wear. Individual contact lenses were extracted and then digested with trypsin. MRM in conjunction with stable-isotope-labeled peptide standards was used for protein quantification. The results show that lysozyme was the major protein detected from both lens types. The amount of protein extracted from contact lenses was affected by the lens material. Except for keratin-1 (0.83 ± 0.61 vs 0.77 ± 0.20, p = 0.81) or proline rich protein-4 (0.11 ± 0.04 vs 0.15 ± 0.12, p = 0.97), the amounts of lysozyme, lactoferrin, or lipocalin-1 extracted from balafilcon A lenses (12.9 ± 9.01, 0.84 ± 0.50 or 2.06 ± 1.6, respectively) were significantly higher than that extracted from senofilcon A lenses (0.88 ± 0.13, 0.50 ± 0.10 or 0.27 ± 0.23, respectively) (p < 0.05). The amount of protein extracted from contact lenses was dependent on both the individual wearer and the contact lens material. This may have implications for the development of clinical responses during lens wear for different people and with different types of contact lenses. The use of MRM-MS is a powerful analytical tool for the quantification of specific proteins from single contact lenses after wear.13 page(s

    Effect of cholesterol deposition on bacterial adhesion to contact lenses

    Purpose. To examine the effect of cholesterol on the adhesion of bacteria to silicone hydrogel contact lenses. Methods. Contact lenses, collected from subjects wearing Acuvue Oasys or PureVision lenses, were extracted in chloroform:methanol (1:1, v/v) and amount of cholesterol was estimated by thin-layer chromatography. Unworn lenses were soaked in cholesterol, and the numbers of Pseudomonas aeruginosa strains or Staphylococcus aureus strains that adhered to the lenses were measured. Cholesterol was tested for effects on bacterial growth by incubating bacteria in medium containing cholesterol. Results. From ex vivo PureVision lenses, 3.4 ± 0.3 μg/lens cholesterol was recovered, and from Acuvue Oasys lenses, 2.4 ± 0.2 to 1.0 ± 0.1 μg/lens cholesterol was extracted. Cholesterol did not alter the total or viable adhesion of any strain of P. aeruginosa or S. aureus (p > 0.05). However, worn PureVision lenses reduced the numbers of viable cells of P. aeruginosa (5.8 ± 0.4 log units) compared with unworn lenses (6.4 ± 0.2 log units, p = 0.001). Similarly, there were fewer numbers of S. aureus 031 adherent to worn PureVision (3.05 ± 0.8 log units) compared with unworn PureVision (4.6 ± 0.3 log units, p = 0.0001). Worn Acuvue Oasys lenses did not affect bacterial adhesion. Cholesterol showed no effect on the growth of any test strain. Conclusions. Although cholesterol has been shown to adsorb to contact lenses during wear, this lipid does not appear to modulate bacterial adhesion to a lens surface.9 page(s
