Effect of ketamine on intravenous patient-controlled analgesia using hydromorphone and ketorolac after the Nuss surgery in pediatric patients
Effect of ketamine pretreatment for anaesthesia in patients undergoing percutaneous transluminal balloon angioplasty with continuous remifentanil infusion
BACKGROUND: An appropriate level of sedation and pharmacological assist are essential during percutaneous transluminal balloon angioplasty (PTA). Ketamine provides good analgesia while preserving airway patency, ventilation, and cardiovascular stability with an opioid sparing effect suggesting that it would be ideal in combination with remifentanil and midazolam in spontaneously breathing patients. We evaluated the effect of a small dose of ketamine added to midazolam and remifentanil on analgesia/sedation for PTA procedures.
METHODS: Sixty-four patients receiving PTA were enrolled. The Control group received midazolam 1.0 mg i.v. and continuous infusion of remifentanil 0.05 Āµg/kg/min. The Ketamine group received, in addition, an intravenous bolus of 0.5 mg/kg ketamine. Patients' haemodynamic data were monitored before remifentanil infusion, 5 min after remifentanil infusion, at 1, 3, 5, 30 min after incision, and at admission to the recovery room. Verbal numerical rating scales (VNRS) and sedation [OAA/S (Observer's Assessment of Alertness/Sedation)] scores were also recorded.
RESULTS: The VNRS values at 1, 3, and 5 min after incision and OAA/S scores at 5 min after remifentanil infusion, and 1, 3, and 5 min after incision were lower in the Ketamine group than in the Control group. In the Control group, the VNRS value at 1 min after incision significantly increased and OAA/S values at 3, 5, and 30 min after incision significantly decreased compared to baseline values, while there were no significant changes in the ketamine group.
CONCLUSIONS: A small dose of ketamine as an adjunct sedative to the combination of midazolam and remifentanil produced a better quality of sedation and analgesia than without ketamine and provided stable respiration without cardiopulmonary deterioration.ope
Latent adrenal Ewing sarcoma family of tumors: A case report
Ewing sarcoma family of tumors (ESFT) is derived from the neural crest, which originates from basal embryo cells in the primitive neural tube. ESFT often arises at the bone, chest wall, and soft tissues of the thoracic region. However, ESFT that arises from the adrenal gland is much rarer and it is usually revealed by clinical symptoms. We report an autopsy case of suicidal hanging, in which adrenal ESFT was incidentally revealed. To our knowledge, this is the first case of latent ESFT arising from the adrenal gland. Autopsy can sometimes reveal latent disease. Some of these latent diseases are very rare and we would not be able to detect them without a complete autopsy. As forensic pathologists, we should attempt to perform a complete autopsy and report new discoveries for the development of medicine

Finances, largely explained by financial socialization, have important associations with relationship quality. The primary purpose of this is to critically review research in the field of finances and romantic relationships, specifically how individuals are financially socialized, perceptions of their own financial situation, and factors such as debt. From the critical review, I discuss several themes including the importance of financial mutuality between individuals in romantic relationships. I also note that most of the research reviewed is specific to samples that have college student connections. I end the review discussing connections and implications for relationship and financial counseling
Ultrafast Carrier Dynamics in MXene Thin Films
Terahertz (THz) spectroscopy makes it possible to study the interaction between electromagnetic radiation and matter to understand the intrinsic properties of materials and nanostructures. For MXenes, a family of 2D nanomaterials, radiation in the THz range of the electromagnetic spectrum is ideal for the non- invasive characterization of ultrafast charge carrier dynamics. In this paper, time-resolved THz spectroscopy techniques were used to study the carrier dynamics and photoexcitation of two such MXenes, Mo2Ti2C3 and Nb2C. Drude and Drude-Smith conductivity models were used to analyze complex conductivity components (Ļ1 and Ļ2) for both materials and were compared to titanium carbide (Ti3C2), a well-studied MXene. We find that while Mo2Ti2C3 has considerable intrinsic conductivity, albeit lower than Ti3C2, the conductivity of Nb2C MXene is negligible. We also establish that effects of photoexcitation on these two MXenes diverge from those of Ti3C2, as they exhibit long-lived positive photoconductivity in contrast with conductivity suppression in Ti3C2. These results demonstrate the wealth of different optoelectronic properties of MXenes and possibility of tailoring these properties with chemical composition and structure
