    We investigate the power of a ripping head in the process of concrete cutting. Using nonlinear embedding methods we study the corresponding time series obtained during the cutting process. The calculated maximal Lyapunov exponent indicates the exponential divergence typical for chaotic or stochastic systems. The recurrence plots technique has been used to get nonlinear process statistics for identification and description of nonlinear dynamics, lying behind the cutting process

    Gastroesophageal reflux disease - unit description, diagnosis and treatment

    Many GPs are increasingly dealing with patients complaining of ailments likely to suggest gastro-esophageal reflux disease (GERD). These symptoms include heartburn, abdominal pain, and a feeling of esophageal reflux (regurgitation). GERD is one of the most common gastrointestinal diseases that gastroenterologists meet in their practice (1, 2). In North America the problem is affected from 18.1% to even 27.8% of the population. The situation is similar in Europe, where the proportion of people with reflux symptoms is in the range of 8.8% - 25.9%. Among European countries, the prevalence of GERD symptoms is higher in the north of the continent than in the south. The growing problem of overweight and obesity that makes GERD more and more recognized in the population of children and adolescents (3) is a worrying fact. Interestingly, reflux-related complaints are much less frequent in eastern Asia, affecting only 2.5% -7.8% of the population (4)

A patient with an acute traumatic subdural hematoma chronically treated with rivaroxaban – a challenge for neurosurgeons Wojciech Czyżewski1, Bartłomiej Tyzo2, Jakub Litak2, Arkadiusz Podkowiński2 1 Medical University of Lublin, Department of Didactics and Medical Simulation 2 Department of Neurosurgery and Pediatric Neurosurgery SPSK-4 in Lublin Abstract Rivaroxaban (Xareltoâ), selective direct inhibitor of activated clotting factor Xa belongs to the new oral anticoagulatns (NOACs). Rivaroxaban works to inhibit thrombin formation, and the interruption of the intrinsic and extrinsic pathway of blood coagulation and is strongly dose-dependent. It prolongs Prothrombin Time (PT) and Activated Partial Thromboplastin Time (APTT) without influence on bleeding time and platelet aggregation. It is indicated as a prophylactic to cerebral infarction and peripheral thrombosis in adults with nonvalvular atrial fibrillation as well as the treatment and secondary prevention of deep vein thrombosis and pulmonary thromboembolism. We are presenting a case of an elderly female patient chronically treated with rivaroxaban, who was qualified for neurosurgical treatment in a life threatening state due to an acute subdural hematoma. Despite the lack of unequivocal guidelines for pharmacological and surgical management in cases of intracranial haemorrhage in adults whom are chronically treated with direct factor Xa inhibitors, we managed to obtain a favourable outcome. Działanie leku polega na hamowaniu wytwarzania trombiny, zahamowanie zewnątrz- i wewnątrzpochodnego szlaku krzepnięcia jest silnie zależne od przyjętej dawki. Dochodzi do wydłużenia czasu protrombinowego (PT) oraz czasu częściowej tromboplastyny po aktywacji (APTT), bez wpływu na czas krwawienia i agregację płytek. Preparat znajduje zastosowanie w profilaktyce udaru i zatorowości obwodowej u dorosłych pacjentów z niezastawkowym migotaniem przedsionków oraz leczeniu i profilaktyce wtórnej zakrzepicy żył głębokich i zatorowości płucnej. Prezentujemy przypadek pacjentki w podeszłym wieku przewlekle leczonej rywaroksabanem, która została zakwalifikowana w stanie zagrożenia życia do leczenia neurochirurgicznego z powodu ostrego krwiaka podtwardówkowego. Pomimo braku jednoznacznych wytycznych postępowania farmakologicznego oraz operacyjnego w sytuacji krwotoku wewnątrzczaszkowego u chorych leczonych bezpośrednimi inhibitorami aktywnego czynnika X, u prezentowanej pacjentki uzyskano pomyślny przebieg leczenia. Są najczęstszą przyczyną zgłaszania się do lekarza i mają znaczący wymiar ekonomiczny. W związku z rozwojem cywilizacyjnym, stresem , siedzącym trybem życia i ograniczeniem aktywności procesy  degeneracyjno - wytwórcze w kręgosłupie szyjnym stają się bardziej powszechne. Radikulopatia szyjna jest neurologicznym stanem wywołanym kompresją lub uszkodzeniem korzeni nerwowych szyjnego odcinka kręgosłupa. Stan ten generuje ból w obrębie szyi  nie rzadko promieniuje do obręczy barkowej i kończyn górnych. Postępowanie diagnostyczne jest procesem wieloetapowym. Wywiad i badanie fizykalne pełnia znaczącą rolę a badania obrazowe uzupełniają pełną diagnozę. Farmakoterapia i ograniczenie obciążeń redukują ból w ostrej fazie.W przypadku nieskuteczności leczenia zachowawczego , przewlekłego bólu czy tez postępujących deficytów neurologicznych przeprowadzenie zabiegu operacyjnego obarczającego uciśnięte struktury nerwowe staje się jedynym słusznym postępowaniem . Słowa kluczowe: szyjna dyskopatia, radikulopatia, ból kręgosłupa szyjnego  AbstractThe cervical spine pain represents serious diagnostic medical problem .It is the most common reason for visiting the doctor and has economic dimension . According to Development of Civilization , stress , sitting lifestyle and limitation of physical  activity  Degenerative and Osteoarthritic  syndromes become more common. Cervical radiculopathy  is a neurological condition caused by compression or damage of the nerve roots ofthe cervical spine.  This leads to the occurrence of the neck pain  radiating  to the shoulders and upper extremities. Diagnostic path is a multistage process. Medical interview and physical examination perform essential role . Imaging tools complete proper diagnosis . Pharmacotherapy and short term immobilization reduce pain in a acute phase of disease .     NUMERICAL ANALYSIS OF SPINAL LOADS IN SPONDYLOLISTHESIS TREATMENT USING PEDICLE SCREWS – PRELIMINARY RESEARCH

    The aim of this experimental study was to analyse the influence of lumbar stabilisation used in the treatment of spondylolisthesis on the biome-chanical properties of the human lumbar spine. FEM models were built on the basis of pre-surgical CT scans, routinely used in medical practice. MIMICS software was used to process the results of the neuroimaging study and to create 3D models. Two models were built: with and without a stabiliser. A static load analysis was performed for a normal upper-body load. The simulations allowed to determine the stresses in the individual discs for both models, with and without the transpedicular stabiliser
