    Los medicamentos inhalados son aquellos que se indican en pacientes con patologías respiratorias, introduciéndose en el organismo a través de la inspiración y pasando inevitablemente por la cavidad oral. Por ello, es importante desde el punto de vista odontológico conocer sus implicaciones en el aparato estomatognático, pudiendo así establecer un abordaje clínico que permita rehabilitar al paciente adulto devolvíendole la funcionalidad, la estética y la armonía oral.En el presente trabajo se exponen dos casos de pacientes adultos tratados con inhaladores que acuden al Servicio de Prácticas Odontológicas de la Universidad de Zaragoza. Para cada uno de ellos, a partir de los datos obtenidos en la anamnesis, la exploración extraoral e intraoral y las pruebas complementarias (radiografías, fotografías extraorales e intraorales, modelos y montaje en articulador), se establece un diagnóstico, un pronóstico y un plan de tratamiento individualizado, basándonos siempre en la evidencia científica más actualizada.<br /

    Primary Repair of Moderate Severity Rhegmatogenous Retinal Detachment: A Critical Decision-Making Algorithm

    We reviewed all the available data regarding the current management of non-complex rhegmatogenous retinal detachment and aimed to propose a new decision-making algorithm aimed to improve the single surgery success rate for mid-severity rhegmatogenous retinal detachment. An online review of the Pubmed database was performed. We searched for all available manuscripts about the anatomical and functional outcomes after the surgical management, by either scleral buckle or primary pars plana vitrectomy, of retinal detachment. The search was limited to articles published from January 1995 to December 2015. All articles obtained from the search were carefully screened and their references were manually reviewed for additional relevant data. Our search specifically focused on preoperative clinical data that were associated with the surgical outcomes. After categorizing the available data according to their level of evidence, with randomized-controlled clinical trials as the highest possible level of evidence, followed by retrospective studies, and retrospective case series as the lowest level of evidence, we proceeded to design a logical decision-making algorithm, enhanced by our experiences as retinal surgeons. A total of 7 randomized-controlled clinical trials, 19 retrospective studies, and 9 case series were considered. Additional articles were also included in order to support the observations further. Rhegmatogenous retinal detachment is a potentially blinding disorder. Its surgical management seems to depend more on a surgeon´s preference than solid scientific data or is based on a good clinical history and examination. The algorithms proposed herein strive to offer a more rational approach to improve both anatomical and functional outcomes after the first surgery

    Shannon entropy as a reliable score to diagnose human fibroelastic degenerative mitral chords: a micro-ct ex-vivo study

    This paper is aimed at identifying by means of micro-CT the microstructural differences between normal and degenerative mitral marginal chordae tendineae. The control group is composed of 21 normal chords excised from 14 normal mitral valves from heart transplant recipients. The experimental group comprises 22 degenerative fibroelastic chords obtained at surgery from 11 pathological valves after mitral repair or replacement. In the control group the superficial endothelial cells and spongiosa layer remained intact, covering the wavy core collagen. In contrast, in the experimental group the collagen fibers were arranged as straightened thick bundles in a parallel configuration. 100 cross-sections were examined by micro-CT from each chord. Each image was randomized through the K-means machine learning algorithm and then, the global and local Shannon entropies were obtained. The optimum number of clusters, K, was estimated to maximize the differences between normal and degenerative chords in global and local Shannon entropy; the p-value after a nested ANOVA test was chosen as the parameter to be minimized. Optimum results were obtained with global Shannon entropy and 2≤K≤7, providing p < 0.01; for K=3, p = 2.86⋅10-³. Fourteen LTPs from the most frequent plant food-induced allergies in the geographical area studied were printed into a microarray specifically designed for this research. 212 patients with fruit allergy and 117 food-tolerant pollen allergic subjects were recruited from seven regions of Spain with different pollen profiles, and their sera were tested with allergen microarray. This approach has proven itself to be a good tool to study cross-reactivity between members of LTP family, and could become a useful strategy to analyze other families of allergens

    The involvement of thaumatin-like proteins in plant food cross-reactivity: a multicenter study using a specific protein microarray.

    Cross-reactivity of plant foods is an important phenomenon in allergy, with geographical variations with respect to the number and prevalence of the allergens involved in this process, whose complexity requires detailed studies. We have addressed the role of thaumatin-like proteins (TLPs) in cross-reactivity between fruit and pollen allergies. A representative panel of 16 purified TLPs was printed onto an allergen microarray. The proteins selected belonged to the sources most frequently associated with peach allergy in representative regions of Spain. Sera from two groups of well characterized patients, one with allergy to Rosaceae fruit (FAG) and another against pollens but tolerant to food-plant allergens (PAG), were obtained from seven geographical areas with different environmental pollen profiles. Cross-reactivity between members of this family was demonstrated by inhibition assays. Only 6 out of 16 purified TLPs showed noticeable allergenic activity in the studied populations. Pru p 2.0201, the peach TLP (41%), chestnut TLP (24%) and plane pollen TLP (22%) proved to be allergens of probable relevance to fruit allergy, being mainly associated with pollen sensitization, and strongly linked to specific geographical areas such as Barcelona, Bilbao, the Canary Islands and Madrid. The patients exhibited mayor que50% positive response to Pru p 2.0201 and to chestnut TLP in these specific areas. Therefore, their recognition patterns were associated with the geographical area, suggesting a role for pollen in the sensitization of these allergens. Finally, the co-sensitizations of patients considering pairs of TLP allergens were analyzed by using the co-sensitization graph associated with an allergen microarray immunoassay. Our data indicate that TLPs are significant allergens in plant food allergy and should be considered when diagnosing and treating pollen-food allergy

    Aunque la población de Ibama trabaja en diferentes aspectos de forma autónoma y colaborativa, hallamos que, a pesar de lo dicho, se presenta una ausencia de propuestas culturales. Lo anterior, según hemos detectado, se genera debido a la falta de cohesión social e interés por la promoción cultural, así como el desaprovechamiento de los recursos físicos, humanos y tecnológicos con los que cuentan. Frente a esto "Al imaginarnos nos vemos", es un proyecto para la creación y exhibición de cine comunitario, el cual se realizará en Ibama, corregimiento adscrito al municipio de Yacopí, en Cundinamarca. Se planea desarrollar en dos días, espacios de formación, exhibición y, sobre todo, de creación artística, teniendo como base el cine comunitario, pues Al imaginarnos nos vemos será un espacio para representar todos los sentimientos, pensamientos, expectativas e imaginarios por medio del arte. lo que se justifica con razón de que: Consideramos que el cine comunitario es una gran herramienta para fortalecer lazos de identidad cultural, apropiación del territorio, promover la autogestión, disminuir la brecha digital y, finalmente, crear nuevas propuestas culturales nacidas desde la comunidad. Por esto, consideramos que todo el proceso que se llevará a cabo por medio del cine comunitario no solo servirá para fortalecer competencias tecnológicas y conocimientos para el desarrollo cultural, sino que servirá de base para promover futuros líderes culturales, artistas, crear una red de apoyo y, finalmente, promover la realización de nuevas propuestas desde la comunidad. en este sentido este proyecto tiene el objetivo general de Incentivar la generación y realización de actividades culturales desde la comunidad de Ibama, utilizando como herramienta el Cine comunitario. Mediente las estrategias de Realizar un festival de cine comunitario, en donde la población de Ibama sea espectadora, aprendiz y creadora. Fortalecer competencias en el uso de nuevas tecnologías. Realizar tres talleres de formación en las siguientes áreas: gestión cultural, bases creativas para contar una historia y uso de teléfonos inteligentes en la realización de cine. Beneficiando Niños, jóvenes y adultos de la población de Ibama, Cundinamarca.Although the Ibama population works autonomously and collaboratively in different aspects, we find that, despite what has been said, there is an absence of cultural proposals. In this sense, this project has the general objective of encouraging the generation and implementation of cultural activities from the Ibama community, using Community Cinema as a tool. It mediates the strategies of Carrying out a community film festival, where the Ibama population is a spectator, learner and creator. Strengthen skills in the use of new technologies.     Este trabajo abarca el estudio general de las pérdidas que se tienen en una Empresa Eléctrica Distribuidora, en el que se realiza el análisis de las pérdidas técnicas y la descripción de las causas que provocan las pérdidas comerciales. El presente trabajo considera el estudio de las perdidas técnicas y luego el análisis de uno de los procesos administrativos. Sin limitación de los recursos, se ha procedido a estudiar las pérdidas técnicas y comerciales; y de esta última el proceso de contratación. Se ha dado mayor énfasis a las Pérdidas Técnicas debido a su importante participación en las pérdidas globales de la empresa en los actuales momentos y por la considerable inversión que se debe realizar para poder contra restarlas, en comparación con las pérdidas comerciales, por ello la importancia de hacer un estudio técnico válido dado el elevado costo que con lleva su reducción. Con el análisis técnico y la evaluación económica de las alternativas de mejoras se procede a elaborar un Plan de reducción, en el cual se priorizan las acciones que debe optar la empresa. Con este plan se pretende disminuir el porcentaje de las pérdidas técnicas y comerciales del Sistema Eléctrico de EMELORO S.A
