The Silences Framework: A Method for researching sensitive themes and marginalized health perspectives (English version)
Objective: To describe the experience of applying of The Silences Framework to underpin health research investigating Tuberculosis/HIV/AIDS coinfection .
Method: The Silences Framework originally developed following a study exploring the decisions and silences surrounding black Caribbean men living in England, discussing the themes 'sexual health' and 'ethnicity'. Following this study a conceptual a theory for research on sensitive issues and health care of marginalized populations was developed called 'Screaming Silences' which forms the foundation of The Silences Framework. Screaming Silences define research areas and experiences that are poorly studied, little understood or silenced.
Results: The Silences Framework supports researchers in revealing "silences" in the subjects they study - as such results may reflect how beliefs, values, and experiences of some groups influence their health. This framework provides the application of four complementary stages: working the silences, hearing silences, voicing silences and working with the silences. The analysis occurs cyclically and can be repeated as long as the silences inherent in a study are not revealed.
Conclusion: this article presents The Silences Framework and the application of the notion of "sounds of silence", mapping an antiessentialist theoretical framework for its use in sensitive research in health and nursing areas, being a reference for other researchers in studies involving marginalized populations.

Fructooligosaccharide production by Penicillium expansum
Fructooligosaccharides (FOS) production
by Penicillium expansum was evaluated. In a first
stage, the best conditions for P. expansum growth and
sporulation were established with potato/dextrose/
agar being the most suitable medium at between 22
and 25°C, giving good growth and good sporulation.
The inocula from this medium were used for FOS
production using shake-flask cultures, and yielded
0.58 g FOS/g sucrose (3.25 g FOS/l.h), demonstrating
the potential of this strain for sucrose conversion

Peripheral leukocyte profile in people with temporal lobe epilepsy reflects the associated proinflammatory state
INTRODUCTION: Markers of low-grade peripheral inflammation have been reported amongst people with epilepsy. The mechanisms underlying this phenomenon are unknown. We attempted to characterize peripheral immune cells and their activation status in people with temporal lobe epilepsy (TLE) and healthy controls. METHODS AND RESULTS: Twenty people with TLE and 19 controls were recruited, and peripheral blood lymphocyte and monocyte subsets evaluated ex vivo by multi-color flow cytometry. People with TLE had higher expression of HLA-DR, CD69, CTLA-4, CD25, IL-23R, IFN-Îł, TNF and IL-17 in CD4(+) lymphocytes than controls. Granzyme A, CTLA-4, IL-23R and IL-17 expression was also elevated in CD8(+) T cells from people with TLE. Frequency of HLA-DR in CD19(+) B cells and regulatory T cells CD4(+)CD25(+)Foxp3(+) producing IL-10 was higher in TLE when compared with controls. A negative correlation between CD4(+) expressing co-stimulatory molecules (CD69, CD25 and CTLA-4) with age at onset of seizures was found. The frequency of CD4(+)CD25(+)Foxp3(+) cells was also positively correlated with age at onset of seizures. CONCLUSION: Immune cells of people with TLE show an activation profile, mainly in effector T cells, in line with the low-grade peripheral inflammation
Myeloid Sirtuin 2 expression does not impact long-term Mycobacterium tuberculosis control
Sirtuins (Sirts) regulate several cellular mechanisms through deacetylation of several transcription factors and enzymes. Recently, Sirt2 was shown to prevent the development of inflammatory processes and its expression favors acute Listeria monocytogenes infection. The impact of this molecule in the context of chronic infections remains unknown. We found that specific Sirt2 deletion in the myeloid lineage transiently increased Mycobacterium tuberculosis load in the lungs and liver of conditional mice. Sirt2 did not affect long-term infection since no significant differences were observed in the bacterial burden at days 60 and 120 post-infection. The initial increase in M. tuberculosis growth was not due to differences in inflammatory cell infiltrates in the lung, myeloid or CD4+ T cells. The transcription levels of IFN-?, IL-17, TNF, IL-6 and NOS2 were also not affected in the lungs by Sirt2-myeloid specific deletion. Overall, our results demonstrate that Sirt2 expression has a transitory effect in M. tuberculosis infection. In vitro activity of daptomycin, linezolid and rifampicin on Staphylococcus epidermidis biofilms
Owing to their massive use, Staphylococcus
epidermidis has recently developed significant resistance to
several antibiotics, and became one of the leading causes of
hospital-acquired infections. Current antibiotics are typically
ineffective in the eradication of bacteria in biofilmassociated
persistent infections. Accordingly, the paucity
of effective treatment against cells in this mode of growth
is a key factor that potentiates the need for new agents
active in the prevention or eradication of biofilms. Daptomycin
and linezolid belong to the novel antibiotic therapies
that are active against gram-positive cocci. On the other
hand, rifampicin has been shown to be one of the most
potent, prevalent antibiotics against S. epidermidis biofilms.
Therefore, the main aim of this study was to study
the susceptibility of S. epidermidis biofilm cells to the two
newer antimicrobial agents previously mentioned, and
compare the results obtained with the antimicrobial effect
of rifampicin, widely used in the prevention/treatment of
indwelling medical device infections. To this end the in
vitro activities of daptomycin, linezolid, and rifampicin on
S. epidermidis biofilms were accessed, using these antibiotics
at MIC and peak serum concentrations. The results
demonstrated that at MIC concentration, rifampicin was the
most effective antibiotic tested. At peak serum concentration,
both strains demonstrated similar susceptibility to
rifampicin and daptomycin, with colony-forming units
(CFUs) reductions of approximately 3â4 log10, with a
slightly lower response to linezolid, which was also more
strain dependent. However, considering all the parameters
studied, daptomycin was considered the most effective
antibiotic tested, demonstrating an excellent in vitro
activity against S. epidermidis biofilm cells. In conclusion,
this antibiotic can be strongly considered as an acceptable
therapeutic option for S. epidermidis biofilm-associated
infections and can represent a potential alternative to rifampicin
in serious infections where rifampicin resistance

The contrast-enhanced Doppler ultrasound with perfluorocarbon exposed sonicated albumin does not improve the diagnosis of renal artery stenosis compared with angiography
There are no studies investigating the effect of the contrast infusion on the sensitivity and specificity of the main Doppler criteria of renal artery stenosis (RAS). Our aim was to evaluate the accuracy of these Doppler criteria prior to and following the intravenous administration of perfluorocarbon exposed sonicated albumin (PESDA) in patients suspected of having RAS. Thirty consecutive hypertensive patients (13 males, mean age of 57 ± 10 years) suspected of having RAS by clinical clues, were submitted to ultrasonography (US) of renal arteries before and after enhancement using continuous infusion of PESDA. All patients underwent angiography, and haemodynamically significant RAS was considered when â„50%. At angiography, it was detected RAS â„50% in 18 patients, 5 with bilateral stenosis. After contrast, the examination time was slightly reduced by approximately 20%. In non-enhanced US the sensitivity was better when based on resistance index (82.9%) while the specificity was better when based on renal aortic ratio (89.2%). The predictive positive value was stable for all indexes (74.0%â88.0%) while negative predictive value was low (44%â51%). The specificity and positive predictive value based on renal aortic ratio increased after PESDA injection respectively, from 89 to 97.3% and from 88 to 95%. In hypertensives suspected to have RAS the sensitivity and specificity of Duplex US is dependent of the criterion evaluated. Enhancement with continuous infusion of PESDA improves only the specificity based on renal aortic ratio but do not modify the sensitivity of any index
Perceived difficulty and appropriateness of decision making by General Practitioners: a systematic review of scenario studies
Background: Health-care quality in primary care depends largely on the appropriateness of General Practitionersâ (GPs; Primary Care or Family Physicians) decisions, which may be influenced by how difficult they perceive decisions to be. Patient scenarios (clinical or case vignettes) are widely used to investigate GPsâ decision making. This review aimed to identify the extent to which perceived decision difficulty, decision appropriateness, and their relationship have been assessed in scenario studies of GPsâ decision making; identify possible determinants of difficulty and appropriateness; and investigate the relationship between difficulty and appropriateness.
Methods: MEDLINE, EMBASE, PsycINFO, the Cochrane Library and Web of Science were searched for scenario studies of GPsâ decision making. One author completed article screening. Ten percent of titles and abstracts were checked by an independent volunteer, resulting in 91% agreement. Data on decision difficulty and appropriateness were extracted by one author and descriptively synthesised. Chi-squared tests were used to explore associations between decision appropriateness, decision type and decision appropriateness assessment method.
Results: Of 152 included studies, 66 assessed decision appropriateness and five assessed perceived difficulty. While no studies assessed the relationship between perceived difficulty and appropriateness, one study objectively varied the difficulty of the scenarios and assessed the relationship between a measure of objective difficulty and appropriateness. Across 38 studies where calculations were possible, 62% of the decisions were appropriate as defined by the appropriateness standard used. Chi-squared tests identified statistically significant associations between decision appropriateness, decision type and decision appropriateness assessment method. Findings suggested a negative relationship between decision difficulty and appropriateness, while interventions may have the potential to reduce perceived difficulty.
Conclusions: Scenario-based research into GPsâ decisions rarely considers the relationship between perceived decision difficulty and decision appropriateness. The links between these decisional components require further investigation
cDNA cloning and functional expression of the α-d-galactose-binding lectin frutalin in escherichia coli
cDNA clones encoding frutalin, the α-d-galactose-binding lectin expressed in breadfruit seeds (Artocarpus incisa), were isolated and sequenced. The deduced amino acid sequences indicated that frutalin may be encoded by a family of genes. The NCBI database searches revealed that the frutalin sequence is highly homologous with jacalin and mornigaG sequences. Frutalin cDNA was re-amplified and cloned into the commercial expression vector pET-25b(+) for frutalin production in Escherichia coli. An experimental factorial design was employed to maximise the soluble expression of the recombinant lectin. The results indicated that temperature, time of induction, concentration of IPTG and the interaction between the concentration of IPTG and the time of induction had the most significant effects on the soluble expression level of recombinant frutalin. The optimal culture conditions were as follows: induction with 1 mM IPTG at 22°C for 20 h, yielding 16 mg/l of soluble recombinant frutalin. SDS-PAGE and Western blot analysis revealed that recombinant frutalin was successfully expressed by bacteria with the expected molecular weight (17 kDa). These analyses also showed that recombinant frutalin was mainly produced as insoluble protein. Recombinant frutalin produced by bacteria revealed agglutination properties and carbohydrate-binding specificity similar to the native breadfruit lectin.Fundação para a CiĂȘncia e a Tecnologia (FCT
