    Cytomegalovirus pneumonitis complicated by a central peribronchial pattern of organising pneumonia

    We present five cases of cytomegalovirus (CMV) pneumonitis occurring in patients after recent T cell deplete allogeneic stem cell transplantation (AlloHSCT). These cases were complicated by an organising pneumonia (during the recovery period) with a predominantly central peribronchial pattern. All patients presented with evidence of active CMV pneumonitis which was treated successfully with anti-viral therapy but was followed by persistent severe dyspnoea, cough and hypoxia. High resolution computed tomography (HRCT) imaging showed widespread central peribronchial consolidation with traction bronchiectasis. There was a marked clinical and physiological improvement after treatment with systemic corticosteroids. However, in all patients the lung function remained abnormal and in some cases imaging revealed a fibrosing lung disease. These cases represent a previously undescribed central peribronchial pattern of organising pneumonia complicating CMV pneumonitis that can result in chronic lung damage

    Qualitative study exploring the phenomenon of multiple electronic prescribing systems within single hospital organisations

    BACKGROUND: A previous census of electronic prescribing (EP) systems in England showed that more than half of hospitals with EP reported more than one EP system within the same hospital. Our objectives were to describe the rationale for having multiple EP systems within a single hospital, and to explore perceptions of stakeholders about the advantages and disadvantages of multiple systems including any impact on patient safety. METHODS: Hospitals were selected from previous census respondents. A decision matrix was developed to achieve a maximum variation sample, and snowball sampling used to recruit stakeholders of different professional backgrounds. We then used an a priori framework to guide and analyse semi-structured interviews. RESULTS: Ten participants, comprising pharmacists and doctors and a nurse, were interviewed from four hospitals. The findings suggest that use of multiple EP systems was not strategically planned. Three co-existing models of EP systems adoption in hospitals were identified: organisation-led, clinician-led and clinical network-led, which may have contributed to multiple systems use. Although there were some perceived benefits of multiple EP systems, particularly in niche specialities, many disadvantages were described. These included issues related to access, staff training, workflow, work duplication, and system interfacing. Fragmentation of documentation of the patient's journey was a major safety concern. DISCUSSION: The complexity of EP systems' adoption and deficiencies in IT strategic planning may have contributed to multiple EP systems use in the NHS. In the near to mid-term, multiple EP systems may remain in place in many English hospitals, which may create challenges to quality and patient safety.Peer reviewe

    ATR based infrared spectroscopy for the diagnosis of neonatal respiratory distress syndrome

    Optical spectroscopy offers a potential non-invasive, label free and rapid method to assist clinicians to diagnose diseases for which biomarkers are known. Neonatal respiratory distress syndrome (nRDS) diagnosis in preterm infants is known to be correlated with the lecithin/sphingomyelin ratio (LS ratio) in gastric aspirates, with a ratio less than 2.2 indicating that surfactant replacement therapy is needed. Currently no widespread method exists that can give clinically relevant answers in less than 2 hours from the point of sample collection as it is difficult to identify those who could benefit from prompt surfactant treatment. Various LS ratios were generated using pure dipalmitoylphosphatidylcholine (DPPC) and sphingomyelin (SM) dissolved in dichloromethane and infrared spectra generated using Attenuated Total Reflection (ATR) assisted Fourier Transform InfraRed spectrometry (FTIR). Subsequent analysis obtained the LS ratio using the spectra alone. Further, we demonstrate the application of principal component regression (PCR) and partial least squares (PLS) fits to measured spectra to assist in the determination of the LS ratio using a model trained with multiple runs of the different batches of the same concentration

    Prediction of Neonatal Respiratory Distress Biomarker Concentration by Application of Machine Learning to Mid-Infrared Spectra

    The authors of this study developed the use of attenuated total reflectance Fourier transform infrared spectroscopy (ATR–FTIR) combined with machine learning as a point-of-care (POC) diagnostic platform, considering neonatal respiratory distress syndrome (nRDS), for which no POC currently exists, as an example. nRDS can be diagnosed by a ratio of less than 2.2 of two nRDS biomarkers, lecithin and sphingomyelin (L/S ratio), and in this study, ATR–FTIR spectra were recorded from L/S ratios of between 1.0 and 3.4, which were generated using purified reagents. The calibration of principal component (PCR) and partial least squares (PLSR) regression models was performed using 155 raw baselined and second derivative spectra prior to predicting the concentration of a further 104 spectra. A three-factor PLSR model of second derivative spectra best predicted L/S ratios across the full range (R2: 0.967; MSE: 0.014). The L/S ratios from 1.0 to 3.4 were predicted with a prediction interval of +0.29, −0.37 when using a second derivative spectra PLSR model and had a mean prediction interval of +0.26, −0.34 around the L/S 2.2 region. These results support the validity of combining ATR–FTIR with machine learning to develop a point-of-care device for detecting and quantifying any biomarker with an interpretable mid-infrared spectrum

    Functional Big-step Semantics

    When doing an interactive proof about a piece of software, it is important that the underlying programming language’s semantics does not make the proof unnecessarily difficult or unwieldy. Both smallstep and big-step semantics are commonly used, and the latter is typically given by an inductively defined relation. In this paper, we consider an alternative: using a recursive function akin to an interpreter for the language. The advantages include a better induction theorem, less duplication, accessibility to ordinary functional programmers, and the ease of doing symbolic simulation in proofs via rewriting. We believe that this style of semantics is well suited for compiler verification, including proofs of divergence preservation. We do not claim the invention of this style of semantics: our contribution here is to clarify its value, and to explain how it supports several language features that might appear to require a relational or small-step approach. We illustrate the technique on a simple imperative language with C-like for-loops and a break statement, and compare it to a variety of other approaches. We also provide ML and lambda-calculus based examples to illustrate its generality

    The projection and measurement of cyberpower

    Cyberspace and cyberpower are terms that are increasingly used in common parlance, but are notoriously difficult to define and measure. This article builds on previous work defining the properties of cyberspace in terms of vertical layers, which when combined with a representation of distance presents a three-dimensional model. The unique attributes of cyberspace can be harnessed for power projection, the aim of which is ultimately to alter the behaviour of individuals. Although cyberspace has yet to be used as a medium to demonstrate conventional hard power of coercion and threats supported by physical force, it does present a suitable medium for the projection of soft power of attraction and imitation. These are defined within the context of the online environment and by drawing on the techniques used to optimise Web-based commerce, potential methods of implementing and measuring the success of a campaign of cyberpower projection are proposed

    Strain hardening behavior of lightweight hybrid polyvinyl alcohol (PVA) fiber reinforced cement composites

    Experimental results on the strain hardening and multiple cracking behaviors of polyvinyl alcohol (PVA) fiber reinforced cementitious composites under bending are reported in this paper. Different hybrid combinations of PVA fibers with different lengths and volume fractions are considered to reinforce the mortar matrix. Among different hybrid combinations, the composite containing 2% thicker PVA fibers of 12 mm length and 1% thinner PVA fibers of 6 mm length and the composite containing 2% thicker PVA fibers of 24 mm length and 1% thinner PVA fibers of 6 mm length showed the best performance in terms of highest ultimate load, largest CMOD (crack mouth opening displacement) at peak load and multiple cracking behavior. The effects of four types of light weight sands on the strain hardening and multiple cracking behavior of hybrid fiber composites are also evaluated in this study. It has been observed that the ultimate load and CMOD at peak load for all light weight hybrid fiber composites are almost the same irrespective of volume fractions of light weight sand. The composites containing finer light weight sands exhibited higher ultimate load than those containing coarser light weight sands. It is also observed that the hybrid fiber composite containing normal silica sand exhibited higher ultimate load than the composites with light weight sands

    Absence of inspiratory laryngeal constrictor muscle activity during nasal neurally adjusted ventilatory assist in newborn lambs

    It has been demonstrated that a progressive increase in nasal pressure support ventilation (nPSV) leads to an active inspiratory glottal closure in non-sedated newborn lambs, which limits lung ventilation (24, 33). Unlike nPSV, the pressure delivered during nasal Neurally Adjusted Ventilatory Assist (nNAVA) is synchronized to the diaphragm electrical activity on inspiration (36). Given the tight neural integration of the glottal dilators and constrictors with diaphragm activity on inspiration and expiration respectively, the aim of the present study was to test the hypothesis that inspiratory glottal closure does not develop during nNAVA. Polysomnographic recordings were performed in eight non-sedated, chronically instrumented lambs, which were ventilated with progressively increasing levels of nPSV and nNAVA, in random order. States of alertness, diaphragm and glottal muscle electrical activity, tracheal pressure, SpO2, tracheal PETCO2 and respiratory inductive plethysmography were continuously recorded. While phasic inspiratory glottal constrictor electrical activity appeared with increasing levels of nPSV in 5 out of 8 lambs, it was never observed at any nNAVA level in any lamb, even at maximal achievable nNAVA levels. In addition, a decrease in arterial PCO2 was neither necessary nor sufficient for the development of phasic inspiratory glottal constrictor activity. In conclusion, nNAVA does not induce active glottal closure in non-sedated newborn lambs at high-pressure levels, in contrast to nPSV

    Investigation of Natural Effective Gamma Dose Rates case study: Ardabil Province in Iran

    Gamma rays pose enough energy to form charged particles and adversely affect human health. Since, the external exposure of human beings to natural environmental gamma radiation normally exceeds that from all man-made sources combined, natural background gamma dose rates and corresponding annual effective doses were determined for selected cities of Ardabil province from 2009 to 2010. Outdoor gamma dose rates were measured using an Ion Chamber Survey Meter in 105 locations in selected districts. Average absorbed dose for Ardabil, Sar-Ein, Germy, Neer, Shourabil Recreational Lake, and Kosar were determined as 265, 219, 344, 233, 352, and 358 nSvh-1, respectively. Although dose rates recorded for Germi and Kosar are comparable with some areas with high natural radiation background, however, the dose rates in other districts are well below the levels reported for such locations. Average annual effective dose due to indoor and outdoor gamma radiation for Ardabil province was estimated to be 1.73 (1.35-2.39) mSv, which is on average 2 times higher than the world population weighted average
