    Musculoskeletal symptoms among energy distribution network linemen

    BACKGROUND: Linemen should be evaluated regarding the presence of musculoskeletal symptoms to guide the identification of risk factors for development of work-related musculoskeletal disorders (WMSD) and to allow the implementation of preventive measures. OBJECTIVE: To assess the occurrence of WMSD symptoms among linemen working at a regional branch of an electricity distribution company, to investigate whether there were differences in the proportions of symptomatic workers among the functions performed, and to perform a preliminary survey of the main risk factors present. METHODS: Thirty male linemen (mean age 38.1±5.5 years) were evaluated, divided into three teams according to their job function (Live Line Linemen, LLL; Maintenance/Emergency Linemen, MEL; Commercial Linemen, CL). Musculoskeletal symptoms were identified on a body map, qualified using the McGill questionnaire and quantified using a numerical scale. The DASH questionnaire was also applied to evaluate the impact of the shoulder symptoms on the workers' performance. RESULTS: Seventy percent of the linemen presented at least one musculoskeletal symptom in the shoulders, back or knees. All of the LLL team presented musculoskeletal symptoms and these workers had the highest scores in the DASH questionnaire (28±15). Sixty-seven percent of the MEL team presented symptoms, and their DASH score was 8±11. Fifty percent of the CL team presented symptoms, but none of them had shoulder symptoms. The proportion of workers with shoulder symptoms was related to their job function (p=0.02). CONCLUSIONS: A high proportion of the linemen presented symptoms which varied according to the occupational activity. Interventions are needed to reduce the risk of WMSD among the linemen evaluated.CONTEXTUALIZAÇÃO: A presença de sintomas musculoesqueléticos em eletricistas deve ser caracterizada para auxiliar na identificação de fatores de riscos para os distúrbios osteomusculares relacionados ao trabalho (DORT) e para possibilitar implementação de medidas preventivas. OBJETIVO: Avaliar a ocorrência de sintomas de DORT em eletricistas de uma regional de uma empresa de distribuição de energia elétrica, verificar se existiam diferenças entre as funções exercidas quanto à proporção de trabalhadores sintomáticos, e realizar um levantamento preliminar dos principais fatores de riscos presentes. MÉTODOS: Foram avaliados 30 eletricistas do sexo masculino (38,1±5,5 anos), divididos em três equipes conforme a função exercida (ELV=Eletricistas Linha Viva; EEM=Eletricistas Emergência/Manutenção; EC=Eletricistas Comerciais). Sintomas musculoesqueléticos foram identificados em mapa corporal, qualificados pelo Questionário McGill e quantificados por escala numérica. O questionário DASH também foi aplicado para avaliar impacto dos sintomas nos ombros na performance dos trabalhadores. RESULTADOS: 70% dos eletricistas apresentaram ao menos um sintoma musculoesquelético nos ombros, coluna ou joelhos. A equipe ELV apresentou sintomas musculoesqueléticos em 100% dos trabalhadores e maior pontuação no questionário DASH (28±15). A equipe EEM apresentou sintomas em 67% dos trabalhadores e pontuação DASH de 8±11, e a equipe EC apresentou sintomas em 50%, mas sem sintomas nos ombros. A proporção de trabalhadores sintomáticos nos ombros estava associada à função exercida (p=0.02). CONCLUSÕES: Eletricistas avaliados apresentaram alta proporção de trabalhadores sintomáticos, que variou conforme a atividade ocupacional. Intervenções são necessárias para reduzir os riscos de DORT dentre os eletricistas avaliados.12312

    Representações sociais sobre hipertensão arterial e o cuidado: o discurso do sujeito coletivo

    OBJETIVO: Analisar as representações sociais da equipe de saúde sobre a hipertensão arterial e a maneira como desenvolvem os cuidados específicos aos hipertensos. MÉTODOS: Utilizou-se a entrevista semiestruturada com 21 profissionais de saúde que atuavam em Unidades Básicas de Saúde, a técnica do discurso do sujeito coletivo para organização dos dados e interpretação pela teoria das representações sociais. RESULTADOS: As representações dos profissionais revelaram um distanciamento entre as ações preconizadas pelo programa de controle e as que se realizam nas unidades de saúde. CONCLUSÕES: Os profissionais reconhecem a hipertensão como uma doença grave, com grandes consequências, porém as dificuldades estruturais por parte dos serviços, aliadas à desmotivação do usuário para participação em ações de promoção em saúde, não favorecem um cuidado em saúde voltado para as necessidades dos portadores de hipertensão. Tais resultados indicam a necessidade de ações interdisciplinares na prática assistencial que impactem positivamente na oferta e qualidade dos cuidados.OBJETIVO: Analizar las representaciones sociales del equipo de salud sobre la hipertensión arterial y la manera cómo desarrollan los cuidados específicos a los hipertensos. Tales resultados indican la necesidad de acciones interdisciplinaris en la práctica asistencial que impacten positivamente en el ofrecimiento y calidad de los cuidados.OBJECTIVE: To analyze the social representations of the health team about arterial hypertension and the manner that develops specific care with hypertensive patients. METHODS: We used semi-structured interviews with 21 health professionals who worked in the Basic Health Units. The technique of collective subject discourse was used for organization and interpretation of the data, using the theory of social representation. RESULTS: The representations of health professionals showed a gap between the actions recommended by the control program and those taking place in health facilities. CONCLUSIONS: Professionals recognized hypertension as a serious disease, with great consequences, but the structural difficulties for part of the services, together with the user's motivation to participate in activities to promote health, did not favor health care focused on the needs of those suffering with hypertension. These results indicate the need for interdisciplinary actions in care practice that impact positively on the delivery and quality of care

    The magnetic field and multiple planets of the young dwarf AU~Mic

    In this paper we present an analysis of near-infrared spectropolarimetric and velocimetric data of the young M dwarf AU Mic, collected with SPIRou at the Canada-France-Hawaii telescope from 2019 to 2022, mostly within the SPIRou Legacy Survey. With these data, we study the large- and small-scale magnetic field of AU Mic, detected through the unpolarized and circularly-polarized Zeeman signatures of spectral lines. We find that both are modulated with the stellar rotation period (4.86 d), and evolve on a timescale of months under differential rotation and intrinsic variability. The small-scale field, estimated from the broadening of spectral lines, reaches 2.61±0.052.61\pm0.05 kG. The large-scale field, inferred with Zeeman-Doppler imaging from Least-Squares Deconvolved profiles of circularly-polarized and unpolarized spectral lines, is mostly poloidal and axisymmetric, with an average intensity of 550±30550\pm30 G. We also find that surface differential rotation, as derived from the large-scale field, is \simeq30% weaker than that of the Sun. We detect the radial velocity (RV) signatures of transiting planets b and c, although dwarfed by activity, and put an upper limit on that of candidate planet d, putatively causing the transit-timing variations of b and c. We also report the detection of the RV signature of a new candidate planet (e) orbiting further out with a period of 33.39±0.1033.39\pm0.10 d, i.e., near the 4:1 resonance with b. The RV signature of e is detected at 6.5σ\sigma while those of b and c show up at \simeq4σ\sigma, yielding masses of 10.22.7+3.910.2^{+3.9}_{-2.7} and 14.23.5+4.814.2^{+4.8}_{-3.5} Earth masses for b and c, and a minimum mass of 35.25.4+6.735.2^{+6.7}_{-5.4} Earth masses for e.Comment: MNRAS, in press (20 pages and 12 figures + 9 pages of supplementary material

    Allergic reaction related to ramipril use: a case report

    <p>Abstract</p> <p>Background</p> <p>Angiotensin-converting enzyme (ACE) inhibitors are widely prescribed for patients with diabetes as a nephroprotector drug or to treat hypertension. Generally they are safe for clinical practice, but the relationship between these drugs and angioedema is known. The exact mechanism for ACE inhibitors-induced angioedema is not clear and it is still a matter of discussion.</p> <p>Case Report</p> <p>We reported a case of a 23-year-old black female with an 11 year history of type 1 diabetes, regularly monitored in the department of diabetes, in use of 0,98 UI/kg/day of human insulin, which presented an allergic reaction 24 h after ramipril use. The drug had been prescribed to treat diabetic nephropathy. There was no previous history of drug induced or alimentary allergy. The patient was instructed to discontinue the use of ramipril and oral antihistaminic drug and topical corticosteroid were prescribed. Skin biopsies were performed and confirmed the clinical hypothesis of pharmacodermy. The evaluation of ACE polymorphism identified <it>DD </it>genotype. Six months after the withdrawal of ramipril the patient was prescribed the angiotensin-II receptor blocker (ARB) losartan as nephroprotector. She remained well without adverse reactions.</p> <p>Conclusions</p> <p>ACE inhibitors-induced angioedema is uncommon and the clinical presentation is variable with lips, tongue, oropharinge, and larynge as the most common locations. The presence of angioedema during treatment requires the immediate cessation of treatment due to the risk of possible severe complications. The case reported presented moderate symptoms, with the development of early onset edema in uncommon regions. ACE <it>DD </it>genotype had been associated with angioedema-ACE inhibitors induced. In patients who have experienced ACE inhibitor-related angioedema, ARB should be used cautiously used. However in the case of our patient, the prescription of losartan as nefroprotector did not result in any recurrent adverse effect.</p

    Accurate masses and radii of normal stars: modern results and applications

    This paper presents and discusses a critical compilation of accurate, fundamental determinations of stellar masses and radii. We have identified 95 detached binary systems containing 190 stars (94 eclipsing systems, and alpha Centauri) that satisfy our criterion that the mass and radius of both stars be known to 3% or better. To these we add interstellar reddening, effective temperature, metal abundance, rotational velocity and apsidal motion determinations when available, and we compute a number of other physical parameters, notably luminosity and distance. We discuss the use of this information for testing models of stellar evolution. The amount and quality of the data also allow us to analyse the tidal evolution of the systems in considerable depth, testing prescriptions of rotational synchronisation and orbital circularisation in greater detail than possible before. The new data also enable us to derive empirical calibrations of M and R for single (post-) main-sequence stars above 0.6 M(Sun). Simple, polynomial functions of T(eff), log g and [Fe/H] yield M and R with errors of 6% and 3%, respectively. Excellent agreement is found with independent determinations for host stars of transiting extrasolar planets, and good agreement with determinations of M and R from stellar models as constrained by trigonometric parallaxes and spectroscopic values of T(eff) and [Fe/H]. Finally, we list a set of 23 interferometric binaries with masses known to better than 3%, but without fundamental radius determinations (except alpha Aur).     Circumstellar disks and planets. Science cases for next-generation optical/infrared long-baseline interferometers

    We present a review of the interplay between the evolution of circumstellar disks and the formation of planets, both from the perspective of theoretical models and dedicated observations. Based on this, we identify and discuss fundamental questions concerning the formation and evolution of circumstellar disks and planets which can be addressed in the near future with optical and infrared long-baseline interferometers.     The effect of a manual instrumentation technique on five types of premolar root canal geometry assessed by microcomputed tomography and three-dimensional reconstruction

    <p>Abstract</p> <p>Background</p> <p>Together with diagnosis and treatment planning, a good knowledge of the root canal system and its frequent variations is a necessity for successful root canal therapy. The selection of instrumentation techniques for variants in internal anatomy of teeth has significant effects on the shaping ability and cleaning effectiveness. The aim of this study was to reveal the differences made by including variations in the internal anatomy of premolars into the study protocol for investigation of a single instrumentation technique (hand ProTaper instruments) assessed by microcomputed tomography and three-dimensional reconstruction.</p> <p>Methods</p> <p>Five single-root premolars, whose root canal systems were classified into one of five types, were scanned with micro-CT before and after preparation with a hand ProTaper instrument. Instrumentation characteristics were measured quantitatively in 3-D using a customized application framework based on MeVisLab. Numeric values were obtained for canal surface area, volume, volume changes, percentage of untouched surface, dentin wall thickness, and the thickness of dentin removed. Preparation errors were also evaluated using a color-coded reconstruction.</p> <p>Results</p> <p>Canal volumes and surface areas were increased after instrumentation. Prepared canals of all five types were straightened, with transportation toward the inner aspects of S-shaped or multiple curves. However, a ledge was formed at the apical third curve of the type II canal system and a wide range in the percentage of unchanged canal surfaces (27.4-83.0%) was recorded. The dentin walls were more than 0.3 mm thick except in a 1 mm zone from the apical surface and the hazardous area of the type II canal system after preparation with an F3 instrument.</p> <p>Conclusions</p> <p>The 3-D color-coded images showed different morphological changes in the five types of root canal systems shaped with the same hand instrumentation technique. Premolars are among the most complex teeth for root canal treatment and instrumentation techniques for the root canal systems of premolars should be selected individually depending on the 3-D canal configuration of each tooth. Further study is needed to demonstrate the differences made by including variations in the internal anatomy of teeth into the study protocol of clinical RCT for identifying the best preparation technique.</p
