Disentangling the effects of phonation and articulation: Hemispheric asymmetries in the auditory N1m response of the human brain
BACKGROUND: The cortical activity underlying the perception of vowel identity has typically been addressed by manipulating the first and second formant frequency (F1 & F2) of the speech stimuli. These two values, originating from articulation, are already sufficient for the phonetic characterization of vowel category. In the present study, we investigated how the spectral cues caused by articulation are reflected in cortical speech processing when combined with phonation, the other major part of speech production manifested as the fundamental frequency (F0) and its harmonic integer multiples. To study the combined effects of articulation and phonation we presented vowels with either high (/a/) or low (/u/) formant frequencies which were driven by three different types of excitation: a natural periodic pulseform reflecting the vibration of the vocal folds, an aperiodic noise excitation, or a tonal waveform. The auditory N1m response was recorded with whole-head magnetoencephalography (MEG) from ten human subjects in order to resolve whether brain events reflecting articulation and phonation are specific to the left or right hemisphere of the human brain. RESULTS: The N1m responses for the six stimulus types displayed a considerable dynamic range of 115–135 ms, and were elicited faster (~10 ms) by the high-formant /a/ than by the low-formant /u/, indicating an effect of articulation. While excitation type had no effect on the latency of the right-hemispheric N1m, the left-hemispheric N1m elicited by the tonally excited /a/ was some 10 ms earlier than that elicited by the periodic and the aperiodic excitation. The amplitude of the N1m in both hemispheres was systematically stronger to stimulation with natural periodic excitation. Also, stimulus type had a marked (up to 7 mm) effect on the source location of the N1m, with periodic excitation resulting in more anterior sources than aperiodic and tonal excitation. CONCLUSION: The auditory brain areas of the two hemispheres exhibit differential tuning to natural speech signals, observable already in the passive recording condition. The variations in the latency and strength of the auditory N1m response can be traced back to the spectral structure of the stimuli. More specifically, the combined effects of the harmonic comb structure originating from the natural voice excitation caused by the fluctuating vocal folds and the location of the formant frequencies originating from the vocal tract leads to asymmetric behaviour of the left and right hemisphere
Impact of exercise-nutritional state interactions in patients with type 2 diabetes
Introduction
This study examines the role of nutritional status during exercise training in patients with type 2 diabetes mellitus by investigating the effect of endurance-type exercise training in the fasted versus the fed state on clinical outcome measures, glycemic control, and skeletal muscle characteristics in male type 2 diabetes patients.
Methods
Twenty-five male patients (glycated hemoglobin (HbA1c), 57 ± 3 mmol·mol−1 (7.4% ± 0.3%)) participated in a randomized 12-wk supervised endurance-type exercise intervention, with exercise being performed in an overnight-fasted state (n = 13) or after consuming breakfast (n = 12). Patients were evaluated for glycemic control, blood lipid profiles, body composition and physical fitness, and skeletal muscle gene expression.
Results
Exercise training was well tolerated without any incident of hypoglycemia. Exercise training significantly decreased whole-body fat mass (−1.6 kg) and increased high-density lipoprotein concentrations (+2 mg·dL−1), physical fitness (+1.7 mL·min−1·kg−1), and fat oxidation during exercise in both groups (PTIME 0.05). HbA1c concentrations significantly decreased after exercise training (PTIME < 0.001), with a significant greater reduction after consuming breakfast (−0.30% ± 0.06%) compared with fasted state (−0.08% ± 0.06%; mean difference, 0.21%; PTIME × GROUP = 0.016). No interaction effects were observed for skeletal muscle genes related to lipid metabolism or oxidative capacity.
Conclusions
Endurance-type exercise training in the fasted or fed state do not differ in their efficacy to reduce fat mass, increase fat oxidation capacity, and increase cardiorespiratory fitness and high-density lipoprotein concentrations or their risk of hypoglycemia in male patients with type 2 diabetes. HbA1c seems to be improved more with exercise performed in the postprandial compared with the postabsorptive state
Radio-metal cross-linking of alginate hydrogels for non-invasive in vivo imaging
Alginate hydrogels are cross-linked polymers with high water content, tuneable chemical and material properties, and a range of biomedical applications including drug delivery, tissue engineering, and cell therapy. However, their similarity to soft tissue often renders them undetectable within the body using conventional bio-medical imaging techniques. This leaves much unknown about their behaviour in vivo, posing a challenge to therapy development and validation. To address this, we report a novel, fast, and simple method of incorporating the nuclear imaging radio-metal 111In into the structure of alginate hydrogels by utilising its previously-undescribed capacity as an ionic cross-linking agent. This enabled non-invasive in vivo nuclear imaging of hydrogel delivery and retention across the whole body, over time, and across a range of model therapies including: nasal and oral drug delivery, stem cell transplantation, and cardiac tissue engineering. This information will facilitate the development of novel therapeutic hydrogel formulations, encompassing alginate, across disease categories
Selective scattering between Floquet-Bloch and Volkov states in a topological insulator
The coherent optical manipulation of solids is emerging as a promising way to
engineer novel quantum states of matter. The strong time periodic potential of
intense laser light can be used to generate hybrid photon-electron states.
Interaction of light with Bloch states leads to Floquet-Bloch states which are
essential in realizing new photo-induced quantum phases. Similarly, dressing of
free electron states near the surface of a solid generates Volkov states which
are used to study non-linear optics in atoms and semiconductors. The
interaction of these two dynamic states with each other remains an open
experimental problem. Here we use Time and Angle Resolved Photoemission
Spectroscopy (Tr-ARPES) to selectively study the transition between these two
states on the surface of the topological insulator Bi2Se3. We find that the
coupling between the two strongly depends on the electron momentum, providing a
route to enhance or inhibit it. Moreover, by controlling the light polarization
we can negate Volkov states in order to generate pure Floquet-Bloch states.
This work establishes a systematic path for the coherent manipulation of solids

Combining estimates of interest in prognostic modelling studies after multiple imputation: current practice and guidelines
Background: Multiple imputation (MI) provides an effective approach to handle missing covariate
data within prognostic modelling studies, as it can properly account for the missing data
uncertainty. The multiply imputed datasets are each analysed using standard prognostic modelling
techniques to obtain the estimates of interest. The estimates from each imputed dataset are then
combined into one overall estimate and variance, incorporating both the within and between
imputation variability. Rubin's rules for combining these multiply imputed estimates are based on
asymptotic theory. The resulting combined estimates may be more accurate if the posterior
distribution of the population parameter of interest is better approximated by the normal
distribution. However, the normality assumption may not be appropriate for all the parameters of
interest when analysing prognostic modelling studies, such as predicted survival probabilities and
model performance measures.
Methods: Guidelines for combining the estimates of interest when analysing prognostic modelling
studies are provided. A literature review is performed to identify current practice for combining
such estimates in prognostic modelling studies.
Results: Methods for combining all reported estimates after MI were not well reported in the
current literature. Rubin's rules without applying any transformations were the standard approach
used, when any method was stated.
Conclusion: The proposed simple guidelines for combining estimates after MI may lead to a wider
and more appropriate use of MI in future prognostic modelling studies
A Prospective Longitudinal Study of the Clinical Outcomes from Cryptococcal Meningitis following Treatment Induction with 800 mg Oral Fluconazole in Blantyre, Malawi
Introduction: Cryptococcal meningitis is the most common neurological infection in HIV infected patients in Sub Saharan Africa, where gold standard treatment with intravenous amphotericin B and 5 flucytosine is often unavailable or difficult to administer. Fluconazole monotherapy is frequently recommended in national guidelines but is a fungistatic drug compromised by uncertainty over optimal dosing and a paucity of clinical end-point outcome data.
Methods: From July 2010 until March 2011, HIV infected adults with a first episode of cryptococcal meningitis were
recruited at Queen Elizabeth Central Hospital, Blantyre, Malawi. Patients were treated with oral fluconazole monotherapy 800 mg daily, as per national guidelines. ART was started at 4 weeks. Outcomes and factors associated with treatment failure were assessed 4, 10 and 52 weeks after fluconazole initiation.
Results: Sixty patients were recruited. 26/60 (43%) died by 4 weeks. 35/60 (58.0%) and 43/56 (77%) died or failed treatment by 10 or 52 weeks respectively. Reduced consciousness (Glasgow Coma Score ,14 of 15), moderate/severe neurological disability (modified Rankin Score .3 of 5) and confusion (Abbreviated Mental Test Score ,8 of 10) were all common at baseline and associated with death or treatment failure. ART prior to recruitment was not associated with better outcomes.
Conclusions: Mortality and treatment failure from cryptococcal meningitis following initiation of treatment with 800 mg oral fluconazole is unacceptably high. To improve outcomes, there is an urgent need for better therapeutic strategies and point-of-care diagnostics, allowing earlier diagnosis before development of neurological deficit

A meta-analytic review of stand-alone interventions to improve body image
Objective
Numerous stand-alone interventions to improve body image have been developed. The
present review used meta-analysis to estimate the effectiveness of such interventions, and
to identify the specific change techniques that lead to improvement in body image.
Methods
The inclusion criteria were that (a) the intervention was stand-alone (i.e., solely focused on
improving body image), (b) a control group was used, (c) participants were randomly
assigned to conditions, and (d) at least one pretest and one posttest measure of body
image was taken. Effect sizes were meta-analysed and moderator analyses were conducted.
A taxonomy of 48 change techniques used in interventions targeted at body image
was developed; all interventions were coded using this taxonomy.
Results
The literature search identified 62 tests of interventions (N = 3,846). Interventions produced
a small-to-medium improvement in body image (d+ = 0.38), a small-to-medium reduction in
beauty ideal internalisation (d+ = -0.37), and a large reduction in social comparison tendencies
(d+ = -0.72). However, the effect size for body image was inflated by bias both within
and across studies, and was reliable but of small magnitude once corrections for bias were
applied. Effect sizes for the other outcomes were no longer reliable once corrections for
bias were applied. Several features of the sample, intervention, and methodology moderated
intervention effects. Twelve change techniques were associated with improvements in
body image, and three techniques were contra-indicated.
Conclusions
The findings show that interventions engender only small improvements in body image, and
underline the need for large-scale, high-quality trials in this area. The review identifies effective
techniques that could be deployed in future interventions
The lay health worker-patient relationship in promoting pulmonary rehabilitation (PR) in COPD: What makes it work?
Lay health workers (LHWs) can improve access to services and adherence to treatment, as well as promoting
self-care and prevention. Their effect in promoting uptake and adherence in pulmonary rehabilitation (PR) for
chronic obstructive pulmonary disease (COPD) has not been tested. PR is the most effective treatment for the
symptoms and disability of COPD, but this effectiveness is undermined by poor rates of completion. Trained
LHWs with COPD, who also have first-hand experience of PR, are well placed to help overcome the
documented barriers to its completion. The relationship between LHWs and patients may be one of the
keys to their effectiveness but it has been little explored. Semi-structured qualitative interviews were used with
the aim of examining the LHW-patient partnership in a feasibility study of trained PR-experienced LHWs used
to support COPD patients referred to PR. Twelve volunteers with COPD who completed LHW training
supported 66 patients referred for PR. All 12 of these LHWs gave end-of-study interviews, 21 COPD patients
supported by LHWs were also interviewed. Patients reported that the LHWs were keen to share their
experiences of PR, and that this had a positive impact. The enthusiasm of the LHWs for PR was striking.
The common bond between LHWs and patients of having COPD together with the LHWs positive, first-hand
