    Zkoumání přechodových stavů zdroje tlaku pro obvod se servoventilem

    Authors present results of laboratory researches carried out on electrohydraulic servodrive which is working under intensive occurring of dynamic states of operation. Carried out laboratory researches aimed to characterise mutual interaction between a servo-drive and a hydraulic power supply. In the paper authors take note of pressure time diagrams in output port of the hydraulic power unit and input port of the servodrive both connected together with hydraulic hose. Authors are studding influence of working parameters settings of the hydraulic power supply into pressure course in the feeding line. We also investigated how real courses of measured pressures are different from the pressure value established for the relief valve.Autoři představují výsledky laboratorního výzkumu uskutečněného na elektrohydraulickém servopohonu, který pracuje při intenzivním působení dynamického zatěžování. Tento laboratorní výzkum směřoval k charakterizaci vzájemných interakcí mezi servopohonem a hydraulickým napájecím zdrojem. V příspěvku představili časové průběhy tlaku výstupního kanálu hydraulického čerpadla a vstupního kanálu servopohonu propojených společnou hydraulickou hadicí. Byl rovněž studován vliv nastavených pracovních parametrů hydraulického čerpadla v závislosti na průběhu tlaku v napájecím vedení. Dále byl zkoumán, jak se liší reálný průběh měřeného tlaku od požadovaného průběhu

    Entrepreneurial University – from Ideas to Reality

    The aim of this study is to formulate and highlight the thesis on reasonability and capability of applying such attitudes and pursuing such activities in the practical operation of institutions of higher education that reflect the idea of the entrepreneurial university which has become prominent nowadays. The idea is described in brief and its present status is indicated here. The authors are seeking conditions for the practical implementation of this idea. They formulate four attributes of the entrepreneurial university, naming them economic, market, innovative and managerial orientations. They point to specific criteria of identifying each of the orientations. They present the general assessment of entrepreneurship of Polish institutions of higher education. Their conclusions highlight the most topical conditions for implementing the idea of entrepreneurial university, such as: building the economic strength and market position of the university, developing relations with the socioeconomic environment, internationalisation and innovativeness. The authors give support to the idea of the entrepreneurial university but – by examining the Polish reality – they notice the need for actions in the sphere of law, which regulates the university’s capabilities to act, and point to the need for entrepreneurship-oriented transformations of lawyers and university management staff’s awareness.Janusz Olearnik: [email protected]ława Pluta-Olearnik: [email protected]. dr hab.     The Study of Release of Chlorhexidine from Preparations with Modified Thermosensitive Poly-N-isopropylacrylamide Microspheres

    The aim of this study was to investigate and compare the release rates of chlorhexidine (CX) base entrapped in the polymeric beads of modified poly-N-isopropylacrylamides (pNIPAMs) at temperatures below and over the volume phase transition temperature (VPTT) of synthesized polymers: pNIPAM-A with terminal anionic groups resulting from potassium persulfate initiator, pNIPAM-B with cationic amidine terminal groups, and pNIPAM-C comprising anionic terminals, but with increased hydrophobicity maintained by the N-tert-butyl functional groups. The preparations, assessed in vitro below the VPTT, release an initial burst of CX at different time periods between 120 and 240 min, followed by a period of 24 h, when the rate of release remains approximately constant, approaching the zero-order kinetics; the release rates for the polymers beads are as follows: pNIPAM-C>pNIPAM-B>pNIPAM-A. The pattern of release rates at temperature over the VPTT is as follows: pNIPAM-C>pNIPAM-A>pNIPAM-B. In the presence of pNIPAM-C, the duration between the start of the release and the attained minimal inhibitory concentration (MIC) for most of the microbes, in conditions over the VPTT, increased from 60 to 90 min. The release prolongation could be ascribed to some interactions between the practically insoluble CX particle and the hydrophobic functional groups of the polymer

    Physicochemical characterization and dissolution studies of acyclovir solid dispersions with Pluronic F127 prepared by the kneading method

    The dissolution rate of anhydrous acyclovir was improved by the preparation of physical mixtures and solid dispersions with the non-ionic polymer Pluronic F127 using the kneading method at different drug-to-polymer ratios. The obtained physical mixtures and solid -dispersions were examined in terms of drug content and possible physical and chemical interactions between the drug and polymer using FTIR spectral studies, differential scanning calorimetry and powder X-ray diffraction analysis. The dissolution rate of acyclovir was determined using the rotating disk method. It was found that the minimal content of the polymer within the mixtures needed to increase the dissolution rate of the drug was 50 %

    Is mastectomy with immediate breast reconstruction an alternative to breast-conserving treatment?

    Mimo że leczenie oszczędzające pierś uznawane jest za metodę z wyboru w leczeniu chorych na wczesnego raka piersi, w niektórych badaniach zaobserwowano wzrost częstości wykonywanych mastektomii. Coraz częściej w trakcie mastektomii wykonywana jest natychmiastowa rekonstrukcja piersi, mająca na celu zmniejszenie okaleczenia związanego z utratą piersi. Wykazano, że zarówno leczenie oszczędzające pierś, jak i mastektomia z natychmiastową rekonstrukcją piersi są zabiegami bezpiecznymi onkologicznie i przynoszą porównywalny komfort życia. Jednak zabieg mastektomii z rekonstrukcją natychmiastową piersi jest obarczony większym odsetkiem powikłań pooperacyjnych, zwłaszcza w przypadku zastosowania radioterapii uzupełniającej. Dodatkową korzyścią leczenia oszczędzającego pierś jest możliwość odstąpienia od wykonania limfadenektomii pachowej w przypadku przerzutów ograniczonych do 1–2 węzłów wartowniczych, przez co chore mogą uniknąć powikłań związanych z usunięciem węzłów chłonnych pachowych. Ustalając plan leczenia chorych, należy zwrócić uwagę na potencjalną przewagę, jaką posiada leczenie oszczędzające pierś, nad mastektomią z rekonstrukcją natychmiastową pod względem ryzyka wystąpienia powikłań.Beside the fact that breast-conserving treatment is the method of choice in early breast cancer, several studies have demonstrated good results when mastectomy is involved in the treatment. Such studies are currently increasing. Immediate breast reconstruction is gaining popularity as the method that mitigates post-mastectomy deformation. Comparing breast-conserving treatment and immediate breast reconstruction after mastectomy both methods did not worsen the treatment outcome and have a positive impact to quality of life. However, the complications rate is higher after mastectomy with immediate breast reconstruction, especially in the subgroup of patients receiving postmastectomy radiotherapy. Breast-conserving therapy additionally gives the opportunity to avoid axillary lymph nodes dissection in the subgroup of patients with positive 1–2 sentinel nodes. It should be taken into consideration when a treatment schedule is established, that breast-conserving therapy has an advantage regarding risk of complications

    Thrombocytopenia is the most common haemostatic disorder in patients admitted to Intensive Care Units (ICUs). The mechanisms contributing to a decrease in the platelet count in critically ill patients are multifactorial, among which sepsis and trauma are the most frequent. A differential diagnosis of profound thrombocytopenia is crucial for effective treatment. A low platelet count is a strong independent predictor of morbidity and mortality because it is associated with life-threatening bleeding or thrombosis. This article aims to outline the definition and pathophysiology of thrombocytopenia and present a three-step algorithm of the clinical management of this haemostatic disorder
