    Cough reflex testing with inhaled capsaicin in the study of chronic cough

    AbstractObjectives: To assess the utility of capsaicin test in the differential diagnosis of non-productive causes of chronic cough and to examine the effects of treatment on this reflex. Participants: 86 healthy volunteers and 101 patients with chronic cough: asthma (n: 54), gastroesophageal reflux (n: 35) and post-nasal drip syndrome (n: 12). Design: Prospective intervention trial. Spirometry, bronchoprovocation test with histamine (PC20), and cough challenge with ascending concentrations of capsaicin (0.49–500 μM) were initially performed in all subjects. Patients were treated for 3 months according to the origin of the cough. Concentrations that elicited two (C2) and five or more coughs (C5) were determined before and after treatment.Results: In healthy subjects, cough sensitivity to capsaicin was not influenced by gender or smoking status; however, women with chronic cough were more sensitive to cough challenge than men. C2 and C5 were significantly lower in patients with asthma or gastroesophageal reflux than in post-nasal drip syndrome. No significant correlation was observed between the capsaicin cough threshold and PC20. Cough sensitivity did not improve significantly in most patients with asthma or gastroesophageal reflux despite adequate medical treatment during 3 months. Discriminative value of capsaicin test to differentiate healthy subjects from patients with asthma or reflux was poor. Conclusions: Cough sensitivity to inhaled capsaicin is a safe and reproducible tool in the study of chronic cough. However, its usefulness for the management and differential diagnosis is limited

    Trastorno Dismórfico Corporal: revisión sistemática de un trastorno propio de la adolescencia

    Introducción: El Trastorno Dismórfico Corporal es un trastorno propio de la adolescencia, poco conocido y poco tenido en cuenta en la clínica diaria con niños y adolescentes, lo que demora el diagnóstico y empeora el pronóstico. Métodos: Con el objetivo de realizar una revisión práctica para el clínico infanto-juvenil, se realizó una búsqueda electrónica en las principales bases de datos junto a una búsqueda manual en revistas y tratados especializados. Resultados: El trastorno es muy común, con prevalencias del 1,7 al 2,5%. Dos terceras partes de los casos se inician en la adolescencia y una edad de inicio más temprana se asocia a mayor probabilidad de comorbilidad a lo largo de la evolución y a un mayor riesgo suicida. Se trata de un trastorno del espectro obsesivo-compulsivo que implica preocupaciones persistentes por uno o más defectos físicos percibidos y que no son observables por otras personas, así como comportamientos compulsivos en respuesta a dichas preocupaciones. La vergüenza y el miedo al rechazo promueven que el adolescente no explique sus síntomas y es muy habitual la demora de años en el diagnóstico, que si llega a establecerse, lo hace por lo general en la edad adulta. El curso es crónico y en caso de no diagnosticarse, el pronóstico será pobre con frecuente aislamiento social y deterioro funcional. Un diagnóstico correcto y a tiempo mejorará mucho la evolución, tratándose de un trastorno que responde bien a los tratamientos tanto farmacológicos como psicológicos. Conclusión: Conocer el trastorno dismórfico corporal es muy importante para diagnosticarlo a tiempo en el adolescente, lo cual mejorará el pronóstico de forma clara

    Outer Surface Lipoproteins from the Lyme Sisease Spirochete Exploit the Molecular Switch Mechanism of the Complement Protease C1s

    Proteolytic cascades comprise several important physiological systems, including a primary arm of innate immunity called the complement cascade. To safeguard against complement-mediated attack, the etiologic agent of Lyme disease, Borreliella burgdorferi, produces numerous outer surface–localized lipoproteins that contribute to successful complement evasion. Recently, we discovered a pair of B. burgdorferi surface lipoproteins of the OspEF-related protein family—termed ElpB and ElpQ—that inhibit antibody-mediated complement activation. In this study, we investigate the molecular mechanism of ElpB and ElpQ complement inhibition using an array of biochemical and biophysical approaches. In vitro assays of complement activation show that an independently folded homologous C-terminal domain of each Elp protein maintains full complement inhibitory activity and selectively inhibits the classical pathway. Using binding assays and complement component C1s enzyme assays, we show that binding of Elp proteins to activated C1s blocks complement component C4 cleavage by competing with C1s–C4 binding without occluding the active site. C1s-mediated C4 cleavage is dependent on activation-induced binding sites, termed exosites. To test whether these exosites are involved in Elp–C1s binding, we performed site-directed mutagenesis, which showed that ElpB and ElpQ binding require C1s residues in the anion-binding exosite located on the serine protease domain of C1s. Based on these results, we propose a model whereby ElpB and ElpQ exploit activation-induced conformational changes that are normally important for C1s-mediated C4 cleavage. Our study expands the known complement evasion mechanisms of microbial pathogens and reveals a novel molecular mechanism for selective C1s inhibition by Lyme disease spirochetes

    Submillimetric GPS distance measurement over short baselines: case study in inner consistency

    Distance determination in the open air with submillimetric accuracy is a challenging task usually carried out with the use of submillimetric distancemeters and costly observation campaigns. The present paper represents a first step in the research of the potential use of GPS for submillimetric distance determination for distances up to a few hundred metres consisting in the evaluation of GPS distance determination reproducibility. As will be concluded, reliable submillimetric precision is attainable after some hours of observation if the same equipment in both baseline ends is used, even considering that there still remain some long-term systematic effects of a few tenths of a millimetre. The need for precise absolute antenna calibration values is also shown to be critical for submillimetric distance reproducibility.This research is funded by the Spanish Ministry of Science and Innovation (AYA2011-23232).     Imaging Oxygen Distribution in Marine Sediments. The Importance of Bioturbation and Sediment Heterogeneity

    The influence of sediment oxygen heterogeneity, due to bioturbation, on diffusive oxygen flux was investigated. Laboratory experiments were carried out with 3 macrobenthic species presenting different bioturbation behaviour patterns:the polychaetes Nereis diversicolor and Nereis virens, both constructing ventilated galleries in the sediment column, and the gastropod Cyclope neritea, a burrowing species which does not build any structure. Oxygen two-dimensional distribution in sediments was quantified by means of the optical planar optode technique. Diffusive oxygen fluxes (mean and integrated) and a variability index were calculated on the captured oxygen images. All species increased sediment oxygen heterogeneity compared to the controls without animals. This was particularly noticeable with the polychaetes because of the construction of more or less complex burrows. Integrated diffusive oxygen flux increased with oxygen heterogeneity due to the production of interface available for solute exchanges between overlying water and sediments. This work shows that sediment heterogeneity is an important feature of the control of oxygen exchanges at the sediment–water interface

    This article draws upon developments in UK research on political rhetoric and political performance in order to examine the incident in 2013 when French President François Hollande committed French forces to a US-led punitive strike against Syria, after the use of chemical weapons in a Damascus suburb on 21 August. The US-led retaliation did not take place. This article analyses Hollande's declaration on 27 July and his TV appearance on 15 September. His rhetoric and style are best understood as generic to the nature of the presidential office of the Fifth Republic. The article concludes by appraising how analysis of the French case contributes to the developing literature on rhetoric, celebrity and performance
