    Measuring the mode volume of plasmonic nanocavities using coupled optical emitters

    Metallic optical systems can confine light to deep sub-wavelength dimensions, but verifying the level of confinement at these length scales typically requires specialized techniques and equipment for probing the near-field of the structure. We experimentally measured the confinement of a metal-based optical cavity by using the cavity modes themselves as a sensitive probe of the cavity characteristics. By perturbing the cavity modes with conformal dielectric layers of sub-nm thickness using atomic layer deposition, we find the exponential decay length of the modes to be less than 5% of the free-space wavelength (\lambda) and the mode volume to be of order \lambda^3/1000. These results provide experimental confirmation of the deep sub-wavelength confinement capabilities of metal-based optical cavities.Comment: 11 pages, 4 figure

    Better arthritis care: what training do community-based health professionals need to improve their care of people with arthritis? a Delphi study

    Objective The aim of the present study was to identify the competencies that non-specialist community-based nurses and allied health professionals (AHPs) need to enable them to assess, care for and manage arthritis appropriately. Methods A Delphi survey with an expert panel of 43 rheumatology specialists and expert patients was used to identify the competencies needed by community-based nurses and AHPs to enable them to improve their care of people with arthritis. The process was informed by feedback from focus groups with arthritis patients, community-based nurses and AHPs. Results The core competencies in arthritis care needed by non-specialist community-based nurses and AHPs were identified. The key goals identified were to increase the understanding of arthritis and its impact on patients’ lives, and to increase the ability to help patients to self-manage their condition and access support. Competencies included an understanding of the pathology underlying inflammatory and non-inflammatory arthritis, the ability to distinguish between the two and the ability to recognize early warning signs, with an emphasis on osteoarthritis (OA), rheumatoid arthritis, gout and septic arthritis. Essential competencies included the ability to engage in shared decision making, goal setting and signposting, to provide patients with education and information and to make appropriate referrals. Conclusions Health professionals working in the community commonly encounter arthritis as a presenting problem or as a co-morbidity. The quality of care provided to people with inflammatory arthritis and OA in the community is currently variable. The present study identified the core competencies that all community-based nurses and AHPs should have in relation to OA and inflammatory arthritis

    Comparisons of Attitudes Toward Computer Use and Computer Technology Based on Gender and Race/Ethnicity Among Eighth Graders

    The purpose of this study is to determine whether or not having a laptop computer for individual use 24-hours-a-day, seven-days-a-week lessens differences between how eighth grade girls and eighth grade boys in 10 middle schools in HenricoCounty, Virginia perceive computer use and computer technology. Having a laptop computer to use at any time of the day or night eliminates the issue of competition between boys and girls for computer use. It also eliminates the need for either girls or boys to perform in front of other students while learning, thus lessening to some degree, feelings of computer anxiety. Competition for computer access and computer anxiety are two theories that have been offered to explain why many young women in previous studies have chosen not to use computers to the same extent that young men do, as well as why many young women generally do not consider computer-related careers as career options in their futures. A modified Computer Attitude Questionnaire, based upon the CAQ, Version 5.14 was used to survey eighth grade girls and boys in 10 Henrico County middle schools. Data collected from the surveys were analyzed using the statistical package, SPSS 14 for Windows. Comparisons were based on gender and on the self-reported race/ethnicity of the participants. A comparison of attitudes related to the number of computers already in the home in addition to the school-issued laptop was also conducted. A variety of statistical analyses were used in order to determine differences in attitudes between the boys and girls surveyed and the interactions between the attitudes of the respondents and their race/ethnicity.     The 2007 ERA-EDTA Registry Annual Report—a PrĂ©cis

    Introduction: This paper provides a summary of the 2006 European Renal Association-European Dialysis and Transplant Association (ERA-EDTA) Registry report. Methods: Data on renal replacement therapy (RRT) were available from 50 national and regional registries in 28 countries in Europe and bordering the Mediterranean Sea. Data sets with individual patient data were received from 35 registries, whereas 17 registries contributed data in aggregated form. For both types of registries we presented incidence, prevalence and transplant rates. Survival analysis and the calculation of expected remaining lifetimes were solely based on individual patient records.Results: In 2006, among all registries reporting to the ERA-EDTA Registry, the overall annual incidence rate of RRT was 118 per million population (pmp), and the prevalence was 630 pmp. Incidence rates varied from 213 pmp in Germany to 18 pmp in Ukraine. The overall incidence rate of RRT for end-stage renal disease (ESRD) started to decrease from 2004. The highest prevalence of RRT for ESRD was reported by Cantabria, Spain (1,234 pmp) and the lowest by Ukraine (73 pmp). Overall transplant rates were highest in Spain (61 pmp), whereas the highest transplant rates with living donor kidneys were reported from Iceland (26 pmp). The unadjusted 1-, 2- and 5-year survival of patients on RRT was 82.3 (95% confidence interval [95% CI], 82.0-82.5), 70.9 (95% CI, 70.7-71.2) and 47.5 (95% CI, 47.3-47.6) for the cohort 1997-2001, respectivel

    Willingness to participate in a randomized trial comparing catheters to fistulas for vascular access in incident hemodialysis patients: an international survey of nephrologists

    BACKGROUND: Current guidelines favor fistulas over catheters as vascular access. Yet, the observational literature comparing fistulas to catheters has important limitations and biases that may be difficult to overcome in the absence of randomization. However, it is not clear if physicians would be willing to participate in a clinical trial comparing fistulas to catheters. OBJECTIVES: We also sought to elicit participants' opinions on willingness to participate in a future trial regarding catheters and fistulas. DESIGN: We created a three-part survey consisting of 19 questions. We collected demographic information, respondents' knowledge of the vascular access literature, appropriateness of current guideline recommendations, and their willingness to participate in a future trial. SETTING: Participants were recruited from Canada, Europe, Australia, and New Zealand. PARTICIPANTS: Participants include physicians and trainees who are involved in the care of end-stage renal disease patients requiring vascular access. MEASUREMENTS: Descriptive statistics were used to describe baseline characteristics of respondents according to geographic location. We used logistic regression to model willingness to participate in a future trial. METHODS: We surveyed nephrologists from Canada, Europe, Australia, and New Zealand to assess their willingness to participate in a randomized trial comparing fistulas to catheters in incident hemodialysis patients. RESULTS: Our results show that in Canada, 86 % of respondents were willing to participate in a trial (32 % in all patients; 54 % only in patients at high risk of primary failure). In Europe and Australia/New Zealand, the willingness to participate in a trial that included all patients was lower (28 % in Europe; 25 % in Australia/New Zealand), as was a trial that included patients at high risk of primary failure (38 % in Europe; 39 % in Australia/New Zealand). Nephrologists who have been in practice for a few years, saw a larger volume of patients, or self-identified as experts in vascular access literature were more likely to participate in a trial. LIMITATIONS: Survey distribution was limited to vascular access experts in participating European countries and ultimately led to a discrepancy in numbers of European to non-European respondents overall. Canadian views are likely over-represented in the overall outcomes. CONCLUSIONS: Our survey results suggest that nephrologists believe there is equipoise surrounding the optimal vascular access strategy and that a randomized controlled study should be undertaken, but restricted to those individuals with a high risk of primary fistula failure

    Genetic fine-mapping of DIPLOSPOROUS in Taraxacum (dandelion; Asteraceae) indicates a duplicated DIP-gene

    Background DIPLOSPOROUS (DIP) is the locus for diplospory in Taraxacum, associated to unreduced female gamete formation in apomicts. Apomicts reproduce clonally through seeds, including apomeiosis, parthenogenesis, and autonomous or pseudogamous endosperm formation. In Taraxacum, diplospory results in first division restitution (FDR) nuclei, and inherits as a dominant, monogenic trait, independent from the other apomixis elements. A preliminary genetic linkage map indicated that the DIP-locus lacks suppression of recombination, which is unique among all other map-based cloning efforts of apomeiosis to date. FDR as well as apomixis as a whole are of interest in plant breeding, allowing for polyploidization and fixation of hybrid vigor, respectively. No dominant FDR or apomixis genes have yet been isolated. Here, we zoom-in to the DIP-locus by largely extending our initial mapping population, and by analyzing (local) suppression of recombination and allele sequence divergence (ASD). Results We identified 24 recombinants between two most closely linked molecular markers to DIP in an F1-population of 2227 plants that segregates for diplospory and lacks parthenogenesis. Both markers segregated c. 1:1 in the entire population, indicating a 1:1 segregation rate of diplospory. Fine-mapping showed three amplified fragment length polymorphisms (AFLPs) closest to DIP at 0.2 cM at one flank and a single AFLP at 0.4 cM at the other flank. Our data lacked strong evidence for ASD at marker regions close to DIP. An unexpected bias towards diplosporous plants among the recombinants (20 out of 24) was found. One third of these diplosporous recombinants showed incomplete penetrance of 50-85% diplospory. Conclusions Our data give interesting new insights into the structure of the diplospory locus in Taraxacum. We postulate a locus with a minimum of two DIP-genes and possibly including one or two enhancers or cis-regulatory elements on the basis of the bias towards diplosporous recombinants and incomplete penetrance of diplospory in some of them. We define the DIP-locus to 0.6 cM, which is estimated to cover ~200-300 Kb, with the closest marker at 0.2 cM. Our results confirm the minor role of suppression of recombination and ASD around DIP, making it an excellent candidate to isolate via a chromosome-walking approach.
