    Comparison of spatial rainfall data calculated with a meteorological model and from interpolation of measurements - implications for FRAME modelled wet deposition

    This work quantifies the differences between the two sources of spatial information of precipitation on FRAME modelled wet deposition of oxidised sulphur (SOx), nitrogen (NOy) and reduced nitrogen (NHx) for two years in an area of Poland. The model was run twice, first with precipitation dataset calculated with the WRF model (pWRF). The second run was based on interpolation of measured precipitation with kriging (pOK). SOx, NOy and NHx wet deposition calculated with pWRF precipitation gives lower country mean values if compared with pOK. The maximum values are higher for pWRF precipitation. Grid to grid correlation between pOK and pWRF modelled wet deposition is similar for both years, with the lowest values for NHx and the highest for SOx. The model-measurement agreement is better for the pOK FRAME run. The results show large uncertainties related with wet deposition modelling due to uncertainties in rainfall data

    Application of WRF-Chem to forecasting PM10 concentration over Poland

    The meteorological and chemical transport model WRF-Chem was implemented to forecast PM10 concentrations over Poland. WRF-Chem version 3.5 was configured with three one-way nested domains using the GFS meteorological data and the TNO MACC II emissions. The 48 hour forecasts were run for each day of the winter and summer period of 2014 and there is only a small decrease in model performance for winter with respect to forecast lead time. The model in general captures the variability in observed PM10 concentrations for most of the stations. However, for some locations and specific episodes, the model performance is poor and the results cannot yet be used by official authorities. We argue that a higher resolution sector-based emission data will be helpful for this analysis in connection with a focus on planetary boundary layer processes in WRF-Chem and their impact on the initial distribution of emissions on both time and space

    Lower Extremity Kinetics and Muscle Activation During Gait Are Significantly Different During and After Pregnancy Compared to Nulliparous Females

    Background Low back, pelvic, and lower extremity pain are common during and after pregnancy. Understanding differences in mechanics between pregnant and non-pregnant females is a first step toward identifying potential pathological mechanisms. The primary purpose of this study was to compare joint kinetics and muscle activation during gait between females during and after pregnancy to nulliparous females. Methods Twenty pregnant females completed testing on three occasions (second trimester, third trimester, and post-partum), while 20 matched, nulliparous controls were tested once. Motion capture, force data, and surface electromyography were averaged across seven trials during gait. Lower extremity kinematics, lower extremity moments and work normalized to pre-pregnancy body mass, work distribution, and peak and average muscle activation amplitude were calculated. Independent t-tests were conducted between pregnant and nulliparous females at each time point. Results Compared to controls, peak hip abductor moments were greater throughout and after pregnancy. Females in second trimester also demonstrated greater sagittal negative ankle work and greater percent contribution of the ankle and smaller percent contribution of the hip to negative work. Compared to controls, during third trimester there were greater knee abductor, ankle plantarflexor, and ankle dorsiflexor moments and greater work at the ankle and total work. Several moment and work variables continued to be elevated post-partum compared to controls. Gluteus maximus muscle activation amplitude was smaller in second trimester and post-partum compared to controls. Significance While overall joint demands were greater during and after pregnancy, there was a smaller relative sagittal utilization of the hip early in pregnancy and smaller gluteus maximus muscle amplitude during second trimester and post-partum. Because the gluteus maximus muscle contributes to force closure and dynamic stability of the low back and pelvis, relative gluteus maximus disuse, concurrent with increased joint loads, could potentially contribute to pain during and after pregnancy

    Colloque avec actes et comité de lecture. internationale.International audienceStormwater constructed wetlands (SCW) in Strasbourg (Alsace, France), Moulin-lès-Metz (Lorraine, France), and Leuville-sur-Orge (Ile-de-France, France) receive and treat exclusively runoffs from urban catchments but with different soil uses. Made up of sedimentation ponds and a vertical subsurface flow constructed wetland, the treatment facilities lead to trap several trace metal pollution. Each site belongs to different hydroclimatic areas (modified oceanic, modified continental and semi-continental) and was designed for different purposes (watercourse protection in Strasbourg and in Leuville-sur-Orge, protection of drinking water wells in Moulins-lès-Metz). The aim of this study is to explore trace metals (Cd, Cu, Pb and Zn) behaviours in these SCW. We investigated trace metal trapping by determining concentrations in the systems compartments (filters media and plants). Then trace metal speciation was determined through sequencial extractions of the sediment and filter media. This allowed to determine their forms in soils and to anticipate their behaviour during physical and chemical condition changes. This study permits to obtain first results for the metals stability, to highlight a decrease of the concentrations through the SCW depth (from surficial sediment to clean filter media) and to observe a negligible metal trap in plants (mainly in roots)

    A multinuclear 1H, 13C and 11B solid-state MAS NMR study of 16- and 18-electron organometallic ruthenium and osmium carborane complexes

    YesThe first 1H, 13C, 31P and 11B solid state MAS NMR studies of electron- deficient carborane-containing ruthenium and osmium complexes [Ru/Os(p-cym)(1,2-dicarba-closo-dodecaborane-1,2- dithiolate)] are reported. The MAS NMR data from these 16-electron complexes are compared to those of free carborane-ligand and an 18-electron triphenylphosphine ruthenium adduct, and reveal clear spectral differences between 16- and 18-electron organometallic carborane systems in the solid state.We thank the Swiss National Science Foundation (grant no.     An intervention program to reduce the number of hospitalizations of elderly patients in a primary care clinic

    <p>Abstract</p> <p>Background</p> <p>The elderly population consumes a large share of medical resources in the western world. A significant portion of the expense is related to hospitalizations.</p> <p>Objectives</p> <p>To evaluate an intervention program designed to reduce the number of hospitalization of elderly patients by a more optimal allocation of resources in primary care.</p> <p>Methods</p> <p>A multidimensional intervention program was conducted that included the re-engineering of existing work processes with a focus on the management of patient problems, improving communication with outside agencies, and the establishment of a system to monitor quality of healthcare parameters. Data on the number of hospitalizations and their cost were compared before and after implementation of the intervention program.</p> <p>Results</p> <p>As a result of the intervention the mean expenditure per elderly patient was reduced by 22.5%. The adjusted number of hospitalizations/1,000 declined from 15.1 to 10.7 (29.3%). The number of adjusted hospitalization days dropped from 132 to 82 (37.9%) and the mean hospitalization stay declined from 8.2 to 6.7 days (17.9%). The adjusted hospitalization cost (/1,000patients)droppedfrom/1,000 patients) dropped from 32,574 to $18,624 (42.8%). The overall clinic expense, for all age groups, dropped by 9.9%.</p> <p>Conclusion</p> <p>Implementation of the intervention program in a single primary care clinic led to a reduction in hospitalizations for the elderly patient population and to a more optimal allocation of healthcare resources.</p

    Antibiotic consumption and antimicrobial resistance in Poland; findings and implications

    Background: The problem of inappropriate use of antibiotics and the resulting growth in antimicrobial resistance (AMR) has implications for Poland and the world. The objective of this paper was to compare and contrast antibiotic resistance and antibiotic utilisation in Poland in recent years versus other European countries, including agreed quality indicators, alongside current AMR patterns and ongoing policies and initiatives in Poland to influence and improve antibiotic prescribing. Methods: A quantitative ten-year analysis (2007-2016) of the use of antibiotics based on European Centre for Disease Prevention and Control (ECDC) data combined with a literature review on AMR rates and antimicrobial stewardship initiatives. Results: The system of monitoring AMR and appropriate strategies to address AMR rates remain underdeveloped in Poland. The role of microbiological diagnostics and efforts to prevent infections is currently underestimated by physicians. Overall, Poland had one of the highest rates of total consumption of antibiotics in the analysed European countries. Total consumption of antibacterials for systemic use and relative consumption of beta-lactamase sensitive penicillins were characterized by small but statistically significant average annual increases between 2007 and 2016 (from 22.2DIDs to 23.9 DIDs and from 0.8% to 1.3%, respectively). Conclusions: The integrated activities around appropriate antibiotic prescribing in the pre- and post-graduate training of physicians and dentists seem to be particularly important, as well as changes in policies on prescribing antibiotics within ambulatory care. AMR and appropriate prescribing of antibiotics should be the focus of health policy actions in Poland

    Metabolic Changes in the Visual Cortex Are Linked to Retinal Nerve Fiber Layer Thinning in Multiple Sclerosis

    OBJECTIVE: To investigate the damage to the retinal nerve fiber layer as part of the anterior visual pathway as well as an impairment of the neuronal and axonal integrity in the visual cortex as part of the posterior visual pathway with complementary neuroimaging techniques, and to correlate our results to patients' clinical symptoms concerning the visual pathway. DESIGN, SUBJECTS AND METHODS: Survey of 86 patients with relapsing-remitting multiple sclerosis that were subjected to retinal nerve fiber layer thickness (RNFLT) measurement by optical coherence tomography, to a routine MRI scan including the calculation of the brain parenchymal fraction (BPF), and to magnetic resonance spectroscopy at 3 tesla, quantifying N-acetyl aspartate (NAA) concentrations in the visual cortex and normal-appearing white matter. RESULTS: RNFLT correlated significantly with BPF and visual cortex NAA, but not with normal-appearing white matter NAA. This was connected with the patients' history of a previous optic neuritis. In a combined model, both BPF and visual cortex NAA were independently associated with RNFLT. CONCLUSIONS: Our data suggest the existence of functional pathway-specific damage patterns exceeding global neurodegeneration. They suggest a strong interrelationship between damage to the anterior and the posterior visual pathway
