    The influence of selected factors on the distribution of epilithic diatoms in a torrential river the Kamniška Bistrica (Slovenia)

    Physical and chemical characteristics of habitats and species diversity in streams and rivers are strongly influenced by the catchment area. We analysed the influence of selected environmental and spatial variables on the diversity and species composition of epilithic diatom communities in periphyton. Samples were collected along the river course in a torrential river the Kamniška Bistrica. Sampling sites were selected in reaches distributed from the source to the outlet of the river and were under different influences from the catchment area and with different physical and chemical characteristics. The most common and dominant diatom species in the periphyton community were Achnanthes biasolettiana and A. minutissima. Achnanthes species often inhabit rivers and springs with moderate organic pollution. Another common diatom taxon was Gomphonema pumilum – a key species indicating oligosaprobic conditions. The results of the canonical correspondence analyses revealed that variance of the periphytic diatom community was explained by water temperature and conductivity as well as altitude. Diatom species richness was positively correlated with saprobic index values and abundance of filamentous algae in the river bed indicating a relatively low organic matter and nutrient input into the river system

    In evolutionary dynamics, well-mixed populations are almost always associated with all-to-all interactions; mathematical models are based on complete graphs. In most cases, these models do not predict fixation probabilities in groups of individuals mixed by flows. We propose an analytical description in the fast-flow limit. This approach is valid for processes with global and local selection, and accurately predicts the suppression of selection as competition becomes more local.     Recommendations for the treatment of epilepsy in adult patients in general practice in Belgium: an update

    In 2008, a group of Belgian epilepsy experts published recommendations for antiepileptic drug (AED) treatment of epilepsies in adults and children. Selection of compounds was based on the registration and reimbursement status in Belgium, the level of evidence for efficacy, common daily practice and the personal views and experiences of the authors. In November 2011 the validity of these recommendations was reviewed by the same group of Belgian epilepsy experts who contributed to the preparation of the original paper. The recommendations made in 2008 for initial monotherapy in paediatric patients were still considered to be valid, except for the first choice treatment for childhood absence epilepsy. This update therefore focuses on the treatment recommendations for initial monotherapy and add-on treatment in adult patients. Several other relevant aspects of treatment with AEDs are addressed, including considerations for optimal combination of AEDs (rational polytherapy), pharmacokinetic properties, pharmacodynamic and pharmacokinetic interaction profile, adverse effects, comorbidity, treatment of elderly patients, AED treatment during pregnancy, and generic substitution of AEDs

    Analysis of shared common genetic risk between amyotrophic lateral sclerosis and epilepsy

    Because hyper-excitability has been shown to be a shared pathophysiological mechanism, we used the latest and largest genome-wide studies in amyotrophic lateral sclerosis (n = 36,052) and epilepsy (n = 38,349) to determine genetic overlap between these conditions. First, we showed no significant genetic correlation, also when binned on minor allele frequency. Second, we confirmed the absence of polygenic overlap using genomic risk score analysis. Finally, we did not identify pleiotropic variants in meta-analyses of the 2 diseases. Our findings indicate that amyotrophic lateral sclerosis and epilepsy do not share common genetic risk, showing that hyper-excitability in both disorders has distinct origins

    Reducing the Clinical and Public Health Burden of Familial Hypercholesterolemia A Global Call to Action

    Q1Q1Artículo completoE1-E13IMPORTANCE Familial hypercholesterolemia (FH) is an underdiagnosed and undertreated genetic disorder that leads to premature morbidity and mortality due to atherosclerotic cardiovascular disease. Familial hypercholesterolemia affects 1 in 200 to 250 people around the world of every race and ethnicity. The lack of general awareness of FH among the public and medical community has resulted in only 10% of the FH population being diagnosed and adequately treated. The World Health Organization recognized FH as a public health priority in 1998 during a consultation meeting in Geneva, Switzerland. The World Health Organization report highlighted 11 recommendations to address FH worldwide, from diagnosis and treatment to family screening and education. Research since the 1998 report has increased understanding and awareness of FH, particularly in specialty areas, such as cardiology and lipidology. However, in the past 20 years, there has been little progress in implementing the 11 recommendations to prevent premature atherosclerotic cardiovascular disease in an entire generation of families with FH. OBSERVATIONS In 2018, the Familial Hypercholesterolemia Foundation and the World Heart Federation convened the international FH community to update the 11 recommendations. Two meetings were held: one at the 2018 FH Foundation Global Summit and the other during the 2018 World Congress of Cardiology and Cardiovascular Health. Each meeting served as a platform for the FH community to examine the original recommendations, assess the gaps, and provide commentary on the revised recommendations. The Global Call to Action on Familial Hypercholesterolemia thus represents individuals with FH, advocacy leaders, scientific experts, policy makers, and the original authors of the 1998 World Health Organization report. Attendees from 40 countries brought perspectives on FH from low-, middle-, and high-income regions. Tables listing country-specific government support for FH care, existing country-specific and international FH scientific statements and guidelines, country-specific and international FH registries, and known FH advocacy organizations around the world were created. CONCLUSIONS AND RELEVANCE By adopting the 9 updated public policy recommendations created for this document, covering awareness; advocacy; screening, testing, and diagnosis; treatment; family-based care; registries; research; and cost and value, individual countries have the opportunity to prevent atherosclerotic heart disease in their citizens carrying a gene associated with FH and, likely, all those with severe hypercholesterolemia as well

    Measuring internet skills in a general population

    This study assessed the construct and criterion validity of the short version of the Internet Skills Scale and examined whether its four dimensions - Operational, Information Navigation, Social, and Creative skills - are influenced by a higher-order dimension of general internet skills as one second-order factor. In 2018, a face-to-face survey comprising ofthe 20-item Internet Skills Scale and 22 other items related to digital inclusion was conducted in a sample of 814 internet users in Slovenia. The results of exploratory and confirmatory factor analyses, as well as other multivariate methods, showed that the Internet Skills Scale is characterized by high to adequate convergent and divergent validity.Acceptable criterion validity was observed for Operational and Information Navigation skills. In terms of measurement invariance, the data supported configural and metric invariance, whereas the scalar invariance was not fully confirmed, suggesting that older adults\u27 lower scores on the Creative skills items were not related to lower levels of internet skills in the same way as they were among younger individuals. Last, the results provided original evidence of the Internet Skills Scale as a second-order construct, meaning that a single summative Internet Skills Scale score could be created as an adequate measure of an individual\u27s internet skills

    Modelling sediment fluxes in the Danube River Basin with SWAT

    Sediment management is of prior concern in the Danube Basin for provision of economic and environmental services. This study aimed at assessing current (1995–2009) sediment fluxes of the Danube Basin with SWAT model and identifying sediment budget knowledge gaps. After hydrologic calibration, hillslope gross erosion and sediment yields were broadly calibrated using ancillary data (measurements in plots and small catchments, and national and European erosion maps). Mean annual sediment concentrations (SSC) from 269 gauging stations (2968 station-year entries; median 19 mg/L, interquartile range IQR 10–36 mg/L) were used for calibrating in-stream sediments. SSC residuals (simulations-observations) median was 2 mg/L (IQR − 14; + 22 mg/L). In the validation dataset (172 gauging stations; 1457 data-entries, median 17 mg/L, IQR 10–28), median residual was 9 mg/L (IQR − 9; + 39 mg/L). Percent bias in an independent dataset of annual sediment yields (SSY; 689 data-entries in 95 stations; median 52 t/km2/y, IQR 20–151 t/km2/y) was − 21.5%. Overall, basin-wide model performance was considered satisfactory. Sediment fluxes appeared overestimated in some regions (Sava and Velika Morava), and underestimated in others (Siret-Prut and Romanian Danube), but unbiased elsewhere. According to the model, most sediments were generated by hillslope erosion. Streambank degradation contributed about 5% of sediments, and appeared important in high stream power Alpine reaches. Sediment trapping in reservoirs and floodplain deposition was probably underestimated and counterbalanced by high stream deposition. Factor analysis showed that model underestimations were correlated to Alpine and karst areas, whereas underestimations occurred in high seismicity areas of the Lower Danube. Contemporary sediment fluxes were about one third of values reported for the 1980s for several tributaries of the Middle and Lower Danube. Knowledge gaps affecting the sediment budget were identified in the contributions of some erosion processes (glacier erosion, gully erosion and mass movements), and in-stream sediment dynamics. © 2017 The Author
