    Drug used pattern by self-medication among the RT-PCR positive health workers in Dhaka city

    Background: To date there is no effective treatment against COVID-19. Self-medication played one of the major modes of treatment among general population as well as the health workers during this pandemic. Studying the pattern of self-medication among the health care workers (HCWs) may indicate their knowledge and skills towards rational use of medicines. The aim of the study was to assess the pattern self-medication among the COVID-19 affected HCWs.Methods: This cross-sectional survey assessed the pattern of drug used by self-medication among the health workers who were RT-PCR positive in context to Bangladesh.Results: A total of 267 HCWs data were collected. Most of the HCWs were in middle age group between 31-40 years with the mean age of 32.2±5.2. Doctors (83.9%) enrolled five times more than the nurses (16.1%). Most of the HCWs (60.3%) commenced medications just after appearance of symptoms and 27.0% went for the RT-PCR testing. Only 3.7% went to health care facilities for treatment. More than sixty percent of the respondents took medication by themselves just after start the symptoms, 19 (33.7%) took advised from the specialists and attended at health facilities 2.6%. Most of the patients (42.7%) used azithromycin as antimicrobial agent. Paracetamol was the most used drug among the participants (78.65%) followed by antihistamines (67.79%). Average number of drugs used by patients 3.1% and percentage of antibiotics per patients 42.6%.Conclusions: The prescribing practice of antibiotics shows deviation from the standard recommendation. Awareness regarding essentials drug list should be needed.

    Coupled spin-lattice fluctuations in a compound with orbital degrees of freedom: the Cr based dimer system Sr3Cr2O8

    We report on an extended fluctuation regime in the spin dimer system Sr3Cr2O8 based on anomalies in Raman active phonons and magnetic scattering. The compound has two characteristic temperatures, TS = 275 K, related to a Jahn-Teller transition with structural distortions and orbital ordering and a second, T*= 150 K, which is due to further changes in the orbital sector. Below TS quasielastic scattering marks strong fluctuations and in addition phonon anomalies are observed. For temperatures below T* we observe an exponential decrease of one phonon linewidth and determine a gap of the orbital excitations. At low temperatures the observation of two- and three-magnon scattering allows the determination of the spin excitation gap.Comment: 7 pages, 4 figures, 1 tabl

    Notochordal cells protect nucleus pulposus cells from degradation and apoptosis: implications for the mechanisms of intervertebral disc degeneration

    Abstract Introduction The relative resistance of non-chondrodystrophic (NCD) canines to degenerative disc disease (DDD) may be due to a combination of anabolic and anti-catabolic factors secreted by notochordal cells within the intervertebral disc (IVD) nucleus pulposus (NP). Factors known to induce DDD include interleukin-1 beta (IL-1ß) and/or Fas-Ligand (Fas-L). Therefore we evaluated the ability of notochordal cell conditioned medium (NCCM) to protect NP cells from IL-1ß and IL-1ß +FasL-mediated cell death and degeneration. Methods We cultured bovine NP cells with IL-1ß or IL-1ß+FasL under hypoxic serum-free conditions (3.5% O2) and treated the cells with either serum-free NCCM or basal medium (Advanced DMEM/F-12). We used flow cytometry to evaluate cell death and real-time (RT-)PCR to determine the gene expression of aggrecan, collagen 2, and link protein, mediators of matrix degradation ADAMTS-4 and MMP3, the matrix protection molecule TIMP1, the cluster of differentiation (CD)44 receptor, the inflammatory cytokine IL-6 and Ank. We then determined the expression of specific apoptotic pathways in bovine NP cells by characterizing the expression of activated caspases-3, -8 and -9 in the presence of IL-1ß+FasL when cultured with NCCM, conditioned medium obtained using bovine NP cells (BCCM), and basal medium all supplemented with 2% FBS. Results NCCM inhibits bovine NP cell death and apoptosis via suppression of activated caspase-9 and caspase-3/7. Furthermore, NCCM protects NP cells from the degradative effects of IL-1ß and IL-1ß+Fas-L by up-regulating the expression of anabolic/matrix protective genes (aggrecan, collagen type 2, CD44, link protein and TIMP-1) and down-regulating matrix degrading genes such as MMP-3. Expression of ADAMTS-4, which encodes a protein for aggrecan remodeling, is increased. NCCM also protects against IL-1+FasL-mediated down-regulation of Ank expression. Furthermore, NP cells treated with NCCM in the presence of IL-1ß+Fas-L down-regulate the expression of IL-6 by almost 50%. BCCM does not mediate cell death/apoptosis in target bovine NP cells. Conclusions Notochordal cell-secreted factors suppress NP cell death by inhibition of activated caspase-9 and -3/7 activity and by up-regulating genes contributing anabolic activity and matrix protection of the IVD NP. Harnessing the restorative powers of the notochordal cell could lead to novel cellular and molecular strategies in the treatment of DDD

    Prevalence of Obesity among Jordanian School-aged Adolescents in Greater Amman

    The objective of this study was to estimate the prevalence of obesity among Jordanian school female adolescents in greater Amman.  A representative random sample of 501 female adolescents school children, 10 to 17 years of age was recruited from 3 schools from greater Amman. The anthropometric data for adolescents (10-17 years old) were entered and compared to the growth reference curves/charts developed in 2006/2007 by the World Health Organization (WHO) using WHO 2009 AnthroPlus software. After sorting and cleaning of the data, descriptive analysis was conducted to obtain prevalence of obesity, overweight, and thinness. Malnourished children were defined as those with overweight, obese, or thinness. The overall average prevalences of thinness, normal weight, overweight, and obesity were 7.2% , 46.8%, 36.2% and 9.9 %, respectively. Among the thinness category (WAZ <-2 SD)  , the highest percentage (16.7%)  is for 16-17 age group followed by 14-15 age group (6.7%) while it is 5.3% in 10-11 age group. An overall prevalence of malnutrition characterized by overweight and obesity together among all age groups is 46.1%.  As a conclusion, the study showed that more than half (53.3%) of Jordanian school adolescents in greater Amman are malnourished, with a prevalence of overweight, obesity and thinness at the same time, requiring different strategies to control either type. Further research about the dietary habits and lifestyle behaviors of Jordanian adolescents is recommended. Keywords: Prevalence, Obesity, School-aged children, Adolescents, Greater Amman, Jorda

    A 79-year-old female with benign past medical history presented to the gastroenterology clinic complaining of long-standing symptoms of dyspepsia. Esophagogastroduodenoscopy showed nodular smooth mucosa in the second part of the duodenum. The morphologic and immunophenotypic findings were consistent with low-grade follicular lymphoma. The purpose of this manuscript is to educate the reader on this unusual finding that is pathognomonic for gastrointestinal lymphoma.Una mujer de 79 años, sin antecedentes patológicos de importancia, consultó al servicio de gastro-enterología por síntomas de dispepsia de larga data. It is for this reason that our team has designed a system to incorporate scCO2 into a through-tool lubrication delivery system. Our specific design focuses on drilling applications involving softer materials where chip removal can become an issue.     Approximately three-quarters of Americans agree that the country is heading in the wrong direction, but there is considerable division over whether the country needs to move backward — toward an idealized, homogeneous past — or forward, toward a more diverse future. Though most Americans favor moving forward, a sizable minority yearn for a country reminiscent of the 1950s, embrace the idea that God created America to be a new promised land for European Christians, view newcomers as a threat to American culture, and believe that society has become too soft and feminine. This minority is composed primarily of self-identified Republicans, white evangelical Protestants, and white Americans without a college degree. The majority of Americans, however, especially younger Americans, the religiously unaffiliated, and Democrats, are more likely to embrace a competing vision for the future of America that is more inclusive

    [EN] Sleep apnea has become in the sleep disorder that causes greater concern in recent years due to its morbidity and mortality, higher medical care costs and poor people quality of life. Some proposals have addressed sleep apnea disease in elderly people, but they have still some technical limitations. For these reasons, this paper presents an innovative system based on fog and cloud computing technologies which in combination with IoT and big data platforms offers new opportunities to build novel and innovative services for supporting the sleep apnea and to overcome the current limitations. Particularly, the system is built on several low-power wireless networks with heterogeneous smart devices (i.e, sensors and actuators). In the fog, an edge node (Smart IoT Gateway) provides IoT connection and interoperability and pre-processing IoT data to detect events in real-time that might endanger the elderly's health and to act accordingly. In the cloud, a Generic Enabler Context Broker manages, stores and injects data into the big data analyzer for further processing and analyzing. The system's performance and subjective applicability are evaluated using over 30 GB size datasets and a questionnaire fulfilled by medicals specialist, respectively. Results show that the system data analytics improve the health professionals' decision making to monitor and guide sleep apnea treatment, as well as improving elderly people's quality of life.
