The balance between maternity and sports career: an analysis of the experiences of women athletes
openLa maternità rappresenta una fase significativa nella vita di una donna, ma per le atlete professioniste può comportare sfide uniche e tutele inadeguate nel contesto sportivo. Questo elaborato si propone di esplorare le esperienze, le difficoltà e le tutele legate alla maternità delle donne atlete, con un focus anche sul contesto italiano. Sono state analizzate diverse interviste di atlete professioniste che hanno evidenziato una serie di sfide che queste donne devono affrontare quando decidono di diventare madri, inclusa la discriminazione, la mancanza di supporto finanziario e la pressione per ritornare rapidamente al loro sport. Inoltre, sono emersi problemi legati alle tutele lavorative delle atlete durante la gravidanza e il periodo postparto. La mancanza di diritti specifici per le atlete in termini di maternità e l'assenza di un quadro giuridico adeguato hanno portato a situazioni di ingiustizia e sfruttamento.
L'analisi di queste interviste ha anche esplorato le possibili soluzioni per affrontare queste sfide. È emersa la necessità di una maggiore consapevolezza e sensibilizzazione sulle questioni legate alla maternità delle donne atlete, insieme a politiche e normative che garantiscano tutele adeguate e un supporto finanziario durante il periodo di gravidanza e postparto.
In conclusione, questa tesi mette in luce l'importanza di affrontare le sfide specifiche legate alla maternità delle donne atlete e di adottare misure concrete per garantire un ambiente equo e inclusivo nello sport. Solo attraverso l'implementazione di tutele adeguate e la promozione di una cultura che sostiene la maternità, le atlete potranno godere dei diritti e delle opportunità che meritano nella loro carriera sportiva e nella vita personale
Hydrogen-Fueled Microbial Pathways in Biogas Upgrading Systems Revealed by Genome-Centric Metagenomics
Biogas upgrading via carbon dioxide hydrogenation is an emerging technology for electrofuel production. The biomethanation efficiency is strongly dependent on a balanced microbial consortium, whose high- resolution characterization along with their functional potential and interactions are pivotal for process optimization. The present work is the first genome-centric metagenomic study on mesophilic and thermophilic biogas upgrading reactors aiming to define the metabolic profile of more than 200 uncultivated microbes involved in hydrogen assisted methanogenesis. The outcomes from predictive functional analyses were correlated with microbial abundance variations to clarify the effect of process parameters on the community. The operational temperature significantly influenced the microbial richness of the reactors, while the H2 addition distinctively alternated the abundance of the taxa. Two different Methanoculleus species (one mesophilic and one thermophilic) were identified as the main responsible ones for methane metabolism. Finally, it was demonstrated that the addition of H2 exerted a selective pressure on the concerted or syntrophic interactions of specific microbes functionally related to carbon fixation, propionate and butanoate metabolisms. Novel bacteria were identified as candidate syntrophic acetate oxidizers (e.g., Tepidanaerobacter sp. DTU063), while the addition of H2 favored the proliferation of potential homoacetogens (e.g., Clostridia sp. DTU183). Population genomes encoding genes of Wood-Ljungdahl pathway were mainly thermophilic, while propionate degraders were mostly identified at mesophilic conditions. Finally, putative syntrophic interactions were identified between microbes that have either versatile metabolic abilities or are obligate/facultative syntrophs
Factors influencing physical activity in cancer patients during oncological treatments: a qualitative study
Introduction: Although the literature supports the importance of physical activity in the oncological context, in Italy a large number of patients are not sufficiently active. Methods: The present study aimed to explore factors influencing an active lifestyle in cancer patients during oncological treatments. Semi-structured focus groups, including 18 patients with different cancer types, were conducted at the Oncology Unit in the University Hospital Trust of Verona (Italy). The interviews were audio-recorded, transcribed verbatim, and analyzed with content analysis. Results: According to the Health Belief Model, transcripts were categorized into the following themes: benefits, barriers, and cues to action. Patients reported a series of physical, physiological, and psychological benefits deriving from an active lifestyle. The main barriers hampering the physical activity participation were represented by treatment-related side effects, advanced disease, and some medical procedures, for example, ileostomy. Several strategies that can trigger patients to exercise were identified. Medical advice, social support from family and friends, features such as enjoyment, setting goals, and owning an animal can motivate patients to perform physical activity. At the same time, an individualized program based on patients' characteristics, an available physical activity specialist to consult, more detailed information regarding physical activity in the oncological setting, and having accessible structures were found important facilitators to implementing active behavior. Conclusions: Overall, patients have a positive view regarding physical activity, and a variety of obstacles and cues to action were recognized. Considering this information may help to improve adherence to a physical activity program over time, consequently increasing the expected benefits
Opportunities for Mitigating Soil Compaction in Europe-Case Studies from the SoilCare Project Using Soil-Improving Cropping Systems
Soil compaction (SC) is a major threat for agriculture in Europe that affects many ecosystem functions, such as water and air circulation in soils, root growth, and crop production. Our objective was to present the results from five short-term (25 cm) compaction using subsoiling tillage treatments to depths of 35 cm (Sweden) and 60 cm (Romania). The other SSs addressed both topsoil and subsoil SC (>25 cm, Norway and United Kingdom; >30 cm, Italy) using deep-rooted bio-drilling crops and different tillage types or a combination of both. Each SS evaluated the effectiveness of the SICSs by measuring the soil physical properties, and we calculated SC indices. The SICSs showed promising results-for example, alfalfa in Norway showed good potential for alleviating SC (the subsoil density decreased from 1.69 to 1.45 g cm(-1)) and subsoiling at the Swedish SS improved root penetration into the subsoil by about 10 cm-but the effects of SICSs on yields were generally small. These case studies also reflected difficulties in implementing SICSs, some of which are under development, and we discuss methodological issues for measuring their effectiveness. There is a need for refining these SICSs and for evaluating their longer-term effect under a wider range of pedoclimatic conditions
Scenario Analyses Concerning Energy Efficiency and Climate Protection in Regional and National Residential Building Stocks. Examples from Nine European Countries. - EPISCOPE Synthesis Report No. 3
This report documents methodological aspects and selected results of the scenario analyses to assess refurbishment as well as energy saving processes and project future energy consumption.. It covers scenario calculations conducted for regional residential building stocks in Salzburg/Austria, the Comunidat Valenciana/Spain, the Piedmont Region/Italy, the national non-profit housing stock in the Netherlands as well as the national residential building stocks in Germany, England, Greece, Norway, and Slovenia.
Thereby, the objective of the scenario analysis is not a prediction of future energy demand in the respective building stock. Rather, the objective is to show the potential future impact of predefined assumptions. This may help respective key actors and policy makers to decide on strategies and policies for transforming building stocks towards carbon dioxide neutrality
Guidelines on the management of abdominal aortic aneurysms: updates from the Italian Society of Vascular and Endovascular Surgery (SICVE)
The objective of these Guidelines was to revise and update the previous 2016 Italian Guidelines on Abdominal Aortic Aneurysm Disease, in accordance with the National Guidelines System (SNLG), to guide every practitioner toward the most correct management pathway for this pathology. The methodology applied in this update was the GRADE-SIGN version methodology, following the instructions of the AGREE quality of reporting checklist as well. The first methodological step was the formulation of clinical questions structured according to the PICO (Population, Intervention, Comparison, Outcome) model according to which the Recommendations were issued. Then, systematic reviews of the Literature were carried out for each PICO question or for homogeneous groups of questions, followed by the selection of the articles and the assessment of the methodological quality for each of them using qualitative checklists. Finally, a Considered Judgment form was filled in for each clinical question, in which the features of the evidence as a whole are assessed to establish the transition from the level of evidence to the direction and strength of the recommendations. These guidelines outline the correct management of patients with abdominal aortic aneurysm in terms of screening and surveillance. Medical management and indication for surgery are discussed, as well as preoperative assessment regarding patients' background and surgical risk evaluation. Once the indication for surgery has been established, the options for traditional open and endovascular surgery are described and compared, focusing specifically on patients with ruptured abdominal aortic aneurysms as well. Finally, indications for early and late postoperative follow-up are explained. The most recent evidence in the Literature has been able to confirm and possibly modify the previous recommendations updating them, likewise to propose new recommendations on prospectively relevant topics
The Efficacy of Sodium Channel Blockers to Prevent Phencyclidine-Induced Cognitive Dysfunction in the Rat: Potential for Novel Treatments for Schizophrenia □ S
ABSTRACT Sodium channel inhibition is a well precedented mechanism used to treat epilepsy and other hyperexcitability disorders. The established sodium channel blocker and broad-spectrum anticonvulsant lamotrigine is also effective in the treatment of bipolar disorder and has been evaluated in patients with schizophrenia. Double-blind placebo-controlled clinical trials found that the drug has potential to reduce cognitive symptoms of the disorder. However, because of compound-related side-effects and the need for dose titration, a conclusive evaluation of the drug's efficacy in patients with schizophrenia has not been possible. In this series of studies in the rat, we compared the efficacy of the two new molecules to prevent a cognitive deficit induced by the N-methyl-D-aspartic acid receptor antagonist phencyclidine (PCP) in the reversal-learning paradigm in the rat. We also explored the effects of the drugs to prevent brain activation and neurochemical effects of PCP. We found that, like lamotrigine, both GSK2 and GSK3 were able to prevent the deficit in reversal learning produced by PCP, thus confirming their potential in the treatment of cognitive symptoms of schizophrenia. However, higher doses than those required for anticonvulsant efficacy of the drugs were needed for activity in the reversal-learning model, suggesting a lower therapeutic window relative to mechanism-dependent central side effects for this indication
A New Genetic Risk Score to Predict the Outcome of Locally Advanced or Metastatic Breast Cancer Patients Treated With First-Line Exemestane: Results From a Prospective Study
Currently there are no reliable biomarkers to predict outcome of exemestane treatment. We designed a prospective study to investigate whether constitutive genetic background might affect response to therapy. In a population of 302 advanced breast cancer patients treated with exemestane we showed that a 5-polymorphism-based genetic score could be used to identify patients with different risks of progression and death.Introduction: Approximately 50% of locally advanced or metastatic breast cancer (MBC) patients treated with first-line exemestane do not show objective response and currently there are no reliable biomarkers to predict the outcome of patients using this therapy. The constitutive genetic background might be responsible for differences in the outcome of exemestane-treated patients. We designed a prospective study to investigate the role of germ line polymorphisms as biomarkers of survival. Patients and Methods: Three hundred two locally advanced or MBC patients treated with first-line exemestane were genotyped for 74 germ line polymorphisms in 39 candidate genes involved in drug activity, hormone balance, DNA replication and repair, and cell signaling pathways. Associations with progression-free survival (PFS) and overall survival (OS) were tested with multivariate Cox regression. Bootstrap resampling was used as an internal assessment of results reproducibility. Results: Cytochrome P450 19A1-rs10046TC/CC, solute carrier organic anion transporter 1B1-rs4149056TT, adenosine triphosphate binding cassette subfamily G member 2-rs2046134GG, fibroblast growth factor receptor-4-rs351855TT, and X-ray repair cross complementing 3-rs861539TT were significantly associated with PFS and then combined into a risk score (0-1, 2, 3, or 4-6 risk points). Patients with the highest risk score (4-6 risk points) compared with ones with the lowest score (0-1 risk points) had a median PFS of 10 months versus 26.3 months (adjusted hazard ratio [AdjHR], 3.12 [95% confidence interval (CI), 2.18-4.48]; P < .001) and a median OS of 38.9 months versus 63.0 months (AdjHR, 2.41 [95% CI, 1.22-4.79], P = .012), respectively. Conclusion: In this study we defined a score including 5 polymorphisms to stratify patients for PFS and OS. This score, if validated, might be translated to personalize locally advanced or MBC patient treatment and management
Tracking of Energy Performance Indicators in Residential Building Stocks – Different Approaches and Common Results - EPISCOPE Synthesis Report No. 4
A central task of IEE EPISCOPE project was to carry out energy balance calculations and scenario analysis for national, regional or local residential building stocks against the background of energy saving and climate protection targets. The EPISCOPE Synthesis Report No. 4 documents the individual approaches of collecting information for the investigated residential building stocks as a foundation for building stock models and scenario calculations. Issues related to the availability of data and data quality are discussed, and concepts for a continuous monitoring (a regular data collection) are presented as a basis for a future tracking of energy performance in the observed building stocks
COVID-19 Severity in Multiple Sclerosis: Putting Data Into Context
Background and objectives: It is unclear how multiple sclerosis (MS) affects the severity of COVID-19. The aim of this study is to compare COVID-19-related outcomes collected in an Italian cohort of patients with MS with the outcomes expected in the age- and sex-matched Italian population. Methods: Hospitalization, intensive care unit (ICU) admission, and death after COVID-19 diagnosis of 1,362 patients with MS were compared with the age- and sex-matched Italian population in a retrospective observational case-cohort study with population-based control. The observed vs the expected events were compared in the whole MS cohort and in different subgroups (higher risk: Expanded Disability Status Scale [EDSS] score > 3 or at least 1 comorbidity, lower risk: EDSS score ≤ 3 and no comorbidities) by the χ2 test, and the risk excess was quantified by risk ratios (RRs). Results: The risk of severe events was about twice the risk in the age- and sex-matched Italian population: RR = 2.12 for hospitalization (p < 0.001), RR = 2.19 for ICU admission (p < 0.001), and RR = 2.43 for death (p < 0.001). The excess of risk was confined to the higher-risk group (n = 553). In lower-risk patients (n = 809), the rate of events was close to that of the Italian age- and sex-matched population (RR = 1.12 for hospitalization, RR = 1.52 for ICU admission, and RR = 1.19 for death). In the lower-risk group, an increased hospitalization risk was detected in patients on anti-CD20 (RR = 3.03, p = 0.005), whereas a decrease was detected in patients on interferon (0 observed vs 4 expected events, p = 0.04). Discussion: Overall, the MS cohort had a risk of severe events that is twice the risk than the age- and sex-matched Italian population. This excess of risk is mainly explained by the EDSS score and comorbidities, whereas a residual increase of hospitalization risk was observed in patients on anti-CD20 therapies and a decrease in people on interferon
