    How genomic information is accessed in clinical practice: an electronic survey of UK general practitioners.

    Genomic technologies are having an increasing impact across medicine, including primary care. To enable their wider adoption and realize their potential, education of primary health-care practitioners will be required. To enable the development of such resources, understanding where GPs currently access genomic information is needed. One-hundred fifty-nine UK GPs completed the survey in response to an open invitation, between September 2017 and September 2018. Questions were in response to 4 clinical genomic scenarios, with further questions exploring resources used for rare disease patients, direct-to-consumer genetic testing and collecting a family history. Respondents were most commonly GP principals (independent GPs who own their clinic) (64.8%), aged 35-49 years (54%), worked as a GP for more than 15 years (44%) and practiced within suburban locations (typically wealthier) (50.3%). The most popular 'just in time' education source for all clinical genomic scenarios were online primary care focussed resources with general Internet search engines also popular. For genomic continuous medical education, over 70% of respondents preferred online learning. Considering specific scenarios, local guidelines were a popular resource for the familial breast cancer scenario. A large proportion (41%) had not heard of Genomics England's 100,000 genome project. Few respondents (4%) would access rare disease specific Internet resources (Orphanet, OMIM). Twenty-five percent of respondents were unsure how to respond to a direct-to-consumer commercial genetic test query, with 41% forwarding such queries to local genetic services. GPs require concise, relevant, primary care focussed resources in trusted and familiar online repositories of information. Inadequate genetic education of GPs could increase burden on local genetic services

    Morbidly Obese Woman Unaware of Pregnancy until Full-Term and Complicated by Intraamniotic Sepsis with Pseudomonas

    A 32-year-old Caucasian woman of body mass index (BMI) 46 presented with urinary symptoms to accident and emergency (A&E). Acute pyelonephritis was the diagnosis. Transabdominal scan revealed a live term fetus. Both the partners were unaware of the ongoing pregnancy until diagnosed. She underwent emergency cesarean under general anaesthesia (GA) for nonreassuring CTG, severe chorioamnionitis, and moderate preecclampsia. A live male baby weighing 4400 grams delivered in poor condition. Placental tissue on culture exhibited scanty growth of pseudomonas aeruginosa. Chorioamnionitis due to pseudomonas is rare, with high neonatal morbidity and mortality. It is mostly reported among preterm prelabor rupture of membranes (PPROM). Educating the community especially morbidly obese women if they put on excessive weight or with irregular periods should seek doctor's advice and exclude pregnancy. For the primary care provider, it is of great importance to exclude pregnancy in any reproductive woman presenting with abdominal complaints. This case also brings to clinicians notice that pseudomonas can be community-acquired and can affect term pregnancies with intact or prolonged rupture of membranes

    Analisis Stabilitas dan Kekuatan Pengait Bak Angkut Kendaraan Multiguna Pedesaan

    Penggunaan material yang lebih ringan tentunya akan membuat berat kendaraan menjadi berkurang yang akan menyebabkan efisiensi energi yang digunakan akan meningkat. Demikian juga pada penelitian pada kendaraan multiguna pedesaan, untuk melanjutkan penelitian terdahulu maka akan diaplikasikan penggunaan material body carbon fiber. Namun dalam perkembangan teknologi otomotif tentunya tidak boleh melupakan keselamatan dari pengemudi dan penumpang dari kendaraan. Oleh sebab itu, perlu dilakukan analisis stabilitas arah belok dan kekuatan bak aagnkut kendaraan multiguna pedesaan dengan penggunaan material body carbon fiber. Analisis stabilitas dilakukan untuk mengetahui perilaku arah belok kendaraan yang terbagi menjadi tiga, yaitu understeer, oversteer, dan netral. Ketiga perilaku kendaraan tersebut dapat diketahui dengan melakukan beberapa analisis seperti analisis slip, skid, dan guling. Hasil dari penelitian ini adalah bahwa kendaraan multiguna pedesaan bermuatan penumpang paling stabil pada layout CG 1 (Lf=1,46 dan Lr=1,13) atau 56,37% wheelbase dari depan. Berdasarkan analisa slip, nilai Koefisien understeer menunjukkan bahwa kendaraan lebih stabil saat melintasi sudut kemiringan melintang jalan 5° dan 10°. Lalu pada kondisi percepatan lurus, pengereman lurus, dan percepatan belok pengait bak angkut masih dalam keadaan aman sehingga mampu menahan beban yang diterima dari bak angkut

    Weak Kaon Production off the Nucleon

    The weak kaon production off the nucleon induced by neutrinos is studied at the low and intermediate energies of interest for some ongoing and future neutrino oscillation experiments. This process is also potentially important for the analysis of proton decay experiments. We develop a microscopical model based on the SU(3) chiral Lagrangians. The basic parameters of the model are fpi, the pion decay constant, Cabibbo's angle, the proton and neutron magnetic moments and the axial vector coupling constants for the baryons octet, D and F, that are obtained from the analysis of the semileptonic decays of neutron and hyperons. The studied mechanisms are the main source of kaon production for neutrino energies up to 1.2 to 1.5 GeV for the various channels and the cross sections are large enough to be amenable to be measured by experiments such as Minerva and T2K

    The strange particle production induced by (anti)neutrino off nucleon has been studied for ∣ΔS∣=0|\Delta S|=0 and ∣ΔS∣=1|\Delta S|=1 channels. The reactions those we have considered are for the production of single kaon/antikaon, eta and associated particle production processes. We have developed a microscopical model based on the SU(3) chiral Lagrangian. The basic parameters of the model are fπf_\pi, the pion decay constant, Cabibbo angle, the proton and neutron magnetic moments and the axial vector coupling constants for the baryons octet. For antikaon production we have also included Σ∗\Sigma^*(1385) resonance and for eta production S11S_{11}(1535) and S11S_{11}(1650) resonances are included.Comment: To appear in AIP Conf.     Antineutrino induced antikaon production off the nucleon

    The charged current antikaon production off nucleons induced by antineutrinos is studied at low and intermediate energies. We extend here our previous calculation on kaon production induced by neutrinos. We have developed a microscopic model that starts from the SU(3) chiral Lagrangians and includes background terms and the resonant mechanisms associated to the lowest lying resonance in the channel, namely, the Sigma*(1385). Our results could be of interest for the background estimation of various neutrino oscillation experiments like MiniBooNE and SuperK. They can also be helpful for the planned antineutrino experiments like MINERvA, NOvA and T2K phase II and for beta-beam experiments with antineutrino energies around 1 GeV.Comment: 15 pages and 6 figures.     Karangasem regency is one of nine districts in Bali, which has tourism potential and can be developed into an attractive destination. But now the development of tourism destinations in the district is still not optimal so that requires a strategy in its development efforts. The purpose of this research is to identify the potential of tourism in Karangasem Regency by making the analysis of class / level and identify the cluster of tourist destinations based on the type of tourism, the availability of facilities and facilities and tourist visits, and make the development through SWOT cluster analysis which in the future can be a reference In developing conservation based tourism area in Karangasem regency. The method used is a mix method that combines qualitative and quantitative research analysis. The results of this research is the class / order of tourism destinations in karangasem district shows that the level of development of the destination is still in the middle to lower level, the number of indicators that have not been met make the pattern of development is still in the order of 3-6. Based on the survey results there are clusters of tourist destinations in Karangasem regency consisting of: nature tourism (nature) & beach (coastal), heritage tourism, and agro. Based on the above, it is necessary to make efforts and strategies to develop tourism destinations in Karangasem Regency such as tourism product development strategy based on tourism and conservation tourism, technology utilization in promotion strategy, tourism product strengthening and human resources quality improvement strategy for tourism management in Karangasem regency.
