    The Experience of Pregnancy for Women with Bipolar Disorder: An Exploratory Study

    Background: Bipolar disorder often emerges in an individual’s late teens and early twenties, thus women with bipolar disorder are impacted for the majority of their childbearing years. Pregnancy brings a unique set of challenges to this population, including risk of relapse, teratogenicity of medications, and increased risk of postpartum psychosis, yet no research exists on this population’s experience of pregnancy. Design: This study utilized semi-structured telephone interviews to gather preliminary qualitative data on the experience of pregnancy for women with bipolar disorder with the long-term goal of developing patient education materials and improving nursing-led interventions to help manage illness during pregnancy and postpartum. Method: A purposive sample of four English-speaking women, age 29-39, with a confirmed diagnosis of bipolar disorder I or II and who had given birth within the last eighteen months, was recruited nationally via the electronic and mass media, and postings on bulletin boards in a range of educational and related settings. One open-ended focused telephone interview was held with each client, and conducted by the first author. She was also responsible for doing the audiotaping, transcription, and coding of the data using a descriptive qualitative design.  Results: Women with bipolar disorder described a unique pregnancy experience, influenced by the intersection of mental illness, pregnancy, and pharmacotherapy. Recurrent themes included hopefulness, patient and healthcare provider as advocates, the importance of interdisciplinary care, education, community support and the mixed impact of medication. Discussion & Conclusion: This research contributes to initiatives designed to improve healthcare for pregnant women with bipolar disorder, augment patient education materials, and inform the design of nurse-led interventions. Empirical research findings become even more valuable tools for improving patient care when paired with the voices of patients themselves. As pregnant women are a historically under-studied population, this study also contributes to the larger body of scientific research on this population.&nbsp

    During the last 30 years, economic and social historians have collected and analysed large amounts of anthropometric data in order to explore key aspects of the human past. Attention has also been devoted to the examination of factors that can exert an influence on stature. This article outlines the different ways in which climate might influence stature, either directly or indirectly. It then uses Geographical Information System (GIS) software to explore the relationship between variations in temperature and precipitation and the average heights of men in France, India, Mexico, Spain and the United States (US) over the last two centuries. It is possible to observe an influence of climate on stature in some countries, especially during the nineteenth century, but the relationship weakens across time and largely disappears in recent decades.     Optimizing the Stark-decelerator beamline for the trapping of cold molecules using evolutionary strategies

    We demonstrate feedback control optimization for the Stark deceleration and trapping of neutral polar molecules using evolutionary strategies. In a Stark-decelerator beamline pulsed electric fields are used to decelerate OH radicals and subsequently store them in an electrostatic trap. The efficiency of the deceleration and trapping process is determined by the exact timings of the applied electric field pulses.     Renormalization Group Theory for a Perturbed KdV Equation

    We show that renormalization group(RG) theory can be used to give an analytic description of the evolution of a perturbed KdV equation. The equations describing the deformation of its shape as the effect of perturbation are RG equations. The RG approach may be simpler than inverse scattering theory(IST) and another approaches, because it dose not rely on any knowledge of IST and it is very concise and easy to understand.     Annular pancreas: endoscopic and pancreatographic findings from a tertiary referral ERCP center

    Background and Aims Annular pancreas is a congenital anomaly whereby pancreatic tissue encircles the duodenum. Current knowledge of endoscopic findings of annular pancreas is limited to small case series. The aim of this study was to describe the endoscopic and pancreatographic findings of patients with annular pancreas at a large tertiary care ERCP center. Methods This is a retrospective observational study. Our Institutional Review Board–approved, prospectively collected ERCP database was queried for cases of annular pancreas. The electronic medical records were searched for patient and procedure-related data. Results From January 1, 1994, to December 31, 2016, 46 patients with annular pancreas underwent ERCP at our institution. Index ERCP was technically successful in 42 patients (91.3%), and technical success was achieved in all 46 patients (100%) after 2 attempts, when required. A duodenal narrowing or ring was found in most patients (n = 39, 84.8%), yet only 2 (4.3%) had retained gastric contents. Pancreas divisum was found in 21 patients (45.7%), 18 of which were complete divisum. Pancreatobiliary neoplasia was the indication for ERCP in 7 patients (15.2%). Pancreatographic findings consistent with chronic pancreatitis were noted in 15 patients (32.6%) at the index ERCP. Conclusion This is the largest series describing the endoscopic and pancreatographic findings of patients with annular pancreas. We found that 45.7% of patients had concurrent pancreas divisum. Endoscopic therapy was successful in most patients at our institution after 1 ERCP, and in all patients after a second ERCP. Nearly one-third of patients had findings consistent with chronic pancreatitis at the time of index ERCP. It is unclear whether this may be a feature of the natural history of annular pancreas
