    Relatório de estágio - Mestrado em Ensino da MúsicaRésumé-Section I - En septembre 2012, l’auteur a été engagé comme professeur au Conservatoire à Rayonnement Départemental du Choletais en France et a pris la classe de cor constituée de 25 élèves en charge. Dans un premier temps, l’auteur présente le projet pédagogique établi pour cette classe. Il répond aux exigences liées à son cadre d’emploi et tient compte des con naissances académiques acquises lors de sa formation à l’École Supérieure de Musique de Lisbonne et à l’École Supérieure d’Éducation. Dans un second temps, lors d’une description et d’une analyse de la mise en place de ce projet pédagogique pour l’ensemble de la classe, il aborde les sujets de l’enseignement adapté aux élèves déficients, l’enseignement aux adultes et de la transversalité des enseignements. Finalement est analysé, sur la base de ce projet pédagogique, le cas d'un élève de chaque cycle d'enseignement classique.Résumé-Section II - Depuis l’abaissement de l’âge d’entrée en 1er cycle dans l’enseignement spécialisé de la musique décrété par le ministère de la Culture dans les schémas d’orientation pédagogique des 20 dernières années, les cors standards doubles utilisés jusqu’à présent représentent de par leur poids un obstacle à l’apprentissage. En 1981, suite à une formation de 3 ans comme thérapeute de technique Alexander, le corniste anglais Pip Eastop conçoit un pied-support qui lui permet de jouer du cor tout en respectant "le bon usage de soi". Cette étude se concentre sur ce pied-support et cherche à définir s’il est possible de le considérer comme un outil ergonomiquement valide pour l’acquisition des compétences fondamentales présentées en 1ère section. Sont présentés un historique de l'évolution du poids du cor, les solutions envisageables pour l'enseignement du cor, une étude empirique basée sur des questionnaires effectuée au sein de la classe de cor du Conservatoire à Rayonnement départemental du Choletais et pour conclure est abordée en discussion finale les conséquences de l'ergonomie sur l'enseignement spécialisé de la musique.Abstract-Section I - In September 2012, the author was taken on as a French horn teacher at the "Conservatoire à Rayonnement Départemental du Choletais" in France, taking over the corresponding class of 25 students. Firstly, the author presents the educational project designed specifically for this class. It meets the requirements of his job description, and reflects the academic knowledge acquired during his training at the "Escola Superior de Musica de Lisboa," as well as at the "Escola Superior da Educacao." Secondly, by describing and analysing the implementation of this educational project for the whole class, the author examines issues regarding the education of "special-needs" students, adult education and the transversatily of musical teaching syllabuses. Finally, based on the educational project, the author analyses the individual case of a student from each of the classical cycle.Abstract Section II - Since the lowering of the age of entry to the first cycle in special, musical education in a decree enacted over the past twenty years by the Ministry of Culture, the double- standard French horns used so far have been a barrier to learning, simply through their sheer weight. In 1981, after three years training as a therapist using the "Alexander Method," the English French horn player, Pip Eastop, designed a hornstick which enabled him to play while maintaining "the proper use of the self." This study focuses on the hornstick, and attempts to ascertain whether it is possible for the hornstick to be considered as an ergonomically-valid tool for the acquisition of the basic skills presented in the first section. The author presents the history of the evolution of the weight of the French horn, some possible solutions for teaching the French horn, as well as an empirical study based on questionnaires completed by members of the French horn classes at the "Conservatoire à Rayonnement Département du Choletais," before concluding by examining the ergonomic consequences on teaching methods.N/

    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    Antibody-Mediated Rejection: A Review

    Chronic antibody injury is a serious threat to allograft outcomes and is therefore the center of active research. In the continuum of allograft rejection, the development of antibodies plays a critical role. In recent years, an increased recognition of molecular and histologic changes has provided a better understanding of antibody-mediated rejection (AMR), as well as potential therapeutic interventions. However, several pathways are still unknown, which accounts for the lack of efficacy of some of the currently available agents that are used to treat rejection.We review the current diagnostic criteria for AMR; AMR paradigms; and desensitization, treatment, and prevention strategies.Chronic antibody-mediated endothelial injury results in transplant glomerulopathy, manifested as glomerular basement membrane duplication, double contouring, or splitting. Clinical manifestations of AMR include proteinuria and a rise in serum creatinine. Current strategies for the treatment of AMR include antibody depletion with plasmapheresis (PLEX), immunoadsorption (IA), immunomodulation with intravenous immunoglobulin (IVIG), and T cell- or B cell-depleting agents. Some treatment benefits have been found in using PLEX and IA, and some small nonrandomized trials have identified some benefits in using rituximab and the proteasome inhibitor-based therapy bortezomib. More recent histologic follow-ups of patients treated with bortezomib have not shown significant benefits in terms of allograft outcomes. Furthermore, no specific treatment approaches have been approved by the US Food and Drug Administration. Other agents used for more difficult rejections include bortezomib and eculizumab (an anti-C5 monoclonal antibody).AMR is a fascinating field with ample opportunities for research and progress in the future. Despite the use of advanced techniques for the detection of human leukocyte antigen (HLA) or non-HLA donor-specific antibodies, alloimmune response remains an important barrier for successful long-term allograft function. Treatment of AMR with currently available therapies has produced a variety of results, some of them suboptimal, precluding the development of standardized protocols. New therapies are promising, but randomized controlled trials are needed to find surrogate markers and improve the efficacy of therapy

    Blood gases and cardiovascular shunt in the South American lungfish (Lepidosiren paradoxa) during normoxia and hyperoxia

    The South American lungfish (Lepidosiren paradoxa) has an arterial P(O2), (Pa(O2)) as high as 70-100 mm Hg, corresponding to Hb-O(2) saturations from 90% to 95%, which indicates a moderate cardiovascular right to left (R-L) shunt. In hyperoxia (50% O(2)), we studied animals in: (1) aerated water combined with aerial hyperoxia, which increased Pa(O2) from 78 +/- 2 to 114 +/- 3 mm Hg and (2) and aquatic hyperoxia (50% O(2)) combined room air, which gradually increased Pa(O2) from 75 +/- 4 mm Hg to as much as 146 +/- 10 mm Hg. Further, the hyperoxia (50%) depressed pulmonary ventilation from 58 +/- 13 to 5.5 +/- 3.0 mLBTPS kg h(-1), and Pa(CO2) increased from 20 +/- 2 to 31 +/- 4 mm Hg, while pHa became reduced from 7.56 +/- 0.03 to 7.31 +/- 0.09. At the same time, venous P(O2) (Pv(O2)) rose from 40.0 +/- 2.3 to 46.4 +/- 1.2 mm Hg and, concomitantly, Pvco, increased from 23.2 +/- 1.1 to 32.2 +/- 0.5 mm Hg. R-L shunts were estimated to about 19%, which is moderate when compared to most amphibians. (C) 2010 Elsevier B.V.     Hemodynamic and ventilatory response to different levels of hypoxia and hypercapnia in carotid body-denervated rats

    OBJECTIVE: Chemoreceptors play an important role in the autonomic modulation of circulatory and ventilatory responses to changes in arterial O2 and/or CO2. However, studies evaluating hemodynamic responses to hypoxia and hypercapnia in rats have shown inconsistent results. Our aim was to evaluate hemodynamic and respiratory responses to different levels of hypoxia and hypercapnia in conscious intact or carotid body-denervated rats. METHODS: Male Wistar rats were submitted to bilateral ligature of carotid body arteries (or sham-operation) and received catheters into the left femoral artery and vein. After two days, each animal was placed into a plethysmographic chamber and, after baseline measurements of respiratory parameters and arterial pressure, each animal was subjected to three levels of hypoxia (15, 10 and 6% O2) and hypercapnia (10% CO2). RESULTS: The results indicated that 15% O2 decreased the mean arterial pressure and increased the heart rate (HR) in both intact (n = 8) and carotid body-denervated (n = 7) rats. In contrast, 10% O2did not change the mean arterial pressure but still increased the HR in intact rats, and it decreased the mean arterial pressure and increased the heart rate in carotid body-denervated rats. Furthermore, 6% O2 increased the mean arterial pressure and decreased the HR in intact rats, but it decreased the mean arterial pressure and did not change the HR in carotid body-denervated rats. The 3 levels of hypoxia increased pulmonary ventilation in both groups, with attenuated responses in carotid body-denervated rats. Hypercapnia with 10% CO2 increased the mean arterial pressure and decreased HR similarly in both groups. Hypercapnia also increased pulmonary ventilation in both groups to the same extent. CONCLUSION: This study demonstrates that the hemodynamic and ventilatory responses varied according to the level of hypoxia. Nevertheless, the hemodynamic and ventilatory responses to hypercapnia did not depend on the activation of the peripheral carotid chemoreceptors

    Aestivation in the South American lungfish, Lepidosiren paradoxa: Effects on cardiovascular function, blood gases, osmolality and leptin levels

    The African (Protopterus sp.) and South American lungfish (Lepidosiren paradoxa) inhabit shallow waters, that seasonally dry out, which induces aestivation and cocoon formation in Protopterus. Differently, L. paradoxa has no cocoon, and it aestivates in a simple burrow. In water PaCO(2) is 21.8 +/- 0.4 mmHg (mean values +/- S.E.M.; n = 5), whereas aestivation for 20 days increased PaCO(2) to as much as 37.6 +/- 2.1 mmHg, which remained the same after 40 days (35.8 +/- 3.3 mmHg). Concomitantly. the plasma [HCO(3)(-)]-values for animals in water were 22.5 +/- 0.5 mM, which after 20 days increased to 40.2 +/- 2.3 mM and after 40 days to 35.8 +/- 3.3 mM. Initially in water, PaO(2) was 87.7 +/- 2.0 mmHg, but 20 days in aestivation reduced the value to 80.5 +/- 2.2 and later (40 days) to 77.1 +/- 3.0 mmHg. Meanwhile, aestivation had no effect on pHa and hematocrit. The blood pressures were equal for animals in the water or in the burrow (P(mean) similar to 30 mmHg), and cardiac frequency (f(H)) fell from 31 beats min(-1) to 22 beats min(-1) during 40 days of aestivation. The osmolality (mOsm kg H(2)O(-1)) was elevated after 20 and 40 days of aestivation but declined upon return to water. The transition front activity to aestivation involves new set-points for the variables that determine the acid-base status and PaO(2) of the animals, along with a reduction of cardiac frequency.
