Official statistics: Quo vadis?
The data revolution has resulted in discussions in the statistical community on the future of official statistics. Will official statistics survive as a brand, or will such statistics drown in the flow of data and statistics from new sources and actors, including misused statistics and fake news? The COVID-19 pandemic has been an additional driver for discussion. There is a need to maintain the quality of official statistics and highlight the value of such statistics for the users as a basis for â and supplement to â other statistics and information. It is at the same time important to implement new developments to improve and keep up the relevance of official statistics. Key pillars today are statistical legislation, quality frameworks and core values defining requirements for official statistics. Possibilities are linked to new statistics, use of new data sources and possible extended roles of the statistical institutes within coordination, collaboration, and data stewardship. The paper addresses these issues in the light of trends in official statistics since the UN Fundamental Principles of Official Statistics were formulated about 30 years ago. Quality challenges for statistics and dilemmas in defining the roles of statistical institutes are considered. The paper includes examples from Statistics Norway. Keywords: Official statistics, data revolution, coordination, cooperation, data stewardshipacceptedVersio
Low Z-4OHtam concentrations are associated with adverse clinical outcome among early stage premenopausal breast cancer patients treated with adjuvant tamoxifen
Low steadyâstate levels of active tamoxifen metabolites have been associated with inferior treatment outcomes. In this retrospective analysis of 406 estrogen receptorâpositive breast cancer (BC) patients receiving adjuvant tamoxifen as initial treatment, we have associated our previously reported thresholds for the two active metabolites, Zâendoxifen and Zâ4âhydroxyâtamoxifen (Zâ4OHtam), with treatment outcomes in an independent cohort of BC patients. Among all patients, metabolite levels did not affect survival. However, in the premenopausal subgroup receiving tamoxifen alone (n = 191) we confirmed an inferior BC âspecific survival in patients with the previously described serum concentration threshold of Zâ4OHtam †3.26 nm (HR = 2.37, 95% CI = 1.02â5.48, P = 0.039). The âdoseâresponseâ survival trend in patients categorized to ordinal concentration cutâpoints of Zâ4OHtamoxifen (†3.26, 3.27â8.13, > 8.13 nm) was also replicated (Pâtrend logârank = 0.048). Zâendoxifen was not associated with outcome. This is the first study to confirm the association between a published active tamoxifen metabolite threshold and BC outcome in an independent patient cohort. Premenopausal patients receiving 5âyear of tamoxifen alone may benefit from therapeutic drug monitoring to ensure tamoxifen effectiveness.publishedVersio

Reaching adulthood with Hirschsprung's disease : Patient experiences and recommendations for transitional care
Background/Purpose: The need for transitional care has gained increased focus in the treatment of patients with congenital colorectal disorders. We aimed to acquire in-depth knowledge about the experiences of adult patients with Hirschsprung's disease (HD) and their suggestions for transitional care. Methods: Binational study applying gender equal focus group interviews (FGI). Results: Seventeen (9 men) of 52 invited patients with median age 29 (19â43) years participated. Three themes evolved from the FGI. âScarred body and soulâ describes the somatic and psychosocial challenges the patients experienced and âlimited health literacy on HDâ refers to the patients' lack of HD knowledge. âAbsent transitionâ depicts missing transitional care and the patients' inability to find adult HD specialists. The adult HD patients strongly recommended transitional care from early teens with focus on information about HD and establishment of a peer-to-peer program. They also emphasized the possibility of being referred to a pelvic floor center. Conclusions: HD negatively influences patients' somatic and psychosocial health in childhood, adolescence and adulthood. Adult HD patients strongly recommend transitional care from early teens and the possibility for referral to a center working with pelvic floor dysfunctions. Phytochemical characterization and anti-inflammatory activity of a water extract of Gentiana purpurea roots
Ethnopharmacological relevance
Gentiana purpurea was one of the most important medicinal plants in Norway during the 18th and 19th centuries, and the roots were used against different types of gastrointestinal and airway diseases.
Aim of the study
To explore the content of bioactive compounds in a water extract from the roots, a preparation commonly used in traditional medicine in Norway, to assess the anti-inflammatory potential, and furthermore to quantify the major bitter compounds in both roots and leaves.
Materials and methods
G. purpurea roots were boiled in water, the water extract applied on a Diaion HP20 column and further fractionated with Sephadex LH20, reverse phase C18 and normal phase silica gel to obtain the low molecular compounds. 1D NMR, 2D NMR, and ESI-MS were used for structure elucidation. HPLC-DAD analysis was used for quantification. The inhibition of TNF-α secretion in ConA stimulated peripheral blood mononuclear cells (PBMCs) was investigated.
Results
Eleven compounds were isolated and identified from the hot water extract of G. purpurea roots. Gentiopicrin, amarogentin, erythrocentaurin and gentiogenal showed dose-dependent inhibition of TNF-α secretion. Gentiopicrin is the major secondary metabolite in the roots, while sweroside dominates in the leaves.
Conclusions
The present work gives a comprehensive overview of the major low-molecular weight compounds in the water extracts of G. purpurea, including metabolites produced during the decoction process, and show new anti-inflammatory activities for the native bitter compounds as well as the metabolites produced during preparation of the crude drug
