    Stubble rice (Oryza sativa L.) in direct sowing systems : handling alternatives

    Los objetivos principales del presente trabajo fueron: a) Estudiar cuál es el uso adecuado de las distintas herramientas para mejorar la mineralización del rastrojo de la cosecha de arroz (Oryza sativa L.) bajo siembra directa y b) Determinar, para períodos cortos, la cantidad de rastrojo depositado superficialmente y su tiempo de mineralización al utilizar las distintas técnicas de manejo del mismo. Los tratamientos fueron: cosechadora con desparramador deflector (T1DD), cosechadora sin desparramador triturador (T2SDT), cosechadora con desparramador centrífugo de caucho (T3DC) y cosechadora con desparramador metálico (T4DM). Alternativas implementadas: a) desmalezado b) rolo cuchilla y c) quemado de rastrojo. En las primeras dos alternativas, se aplicaron tres dosis de urea. Los resultados principales fueron: a) La T2SDT produjo, una mayor deposición de material en el centro de la máquina (21680 kg ha-1), b) la distribución de rastrojo de la (T3DC) fue más uniforme que para los otros tratamientos. Las principales conclusiones fueron: 1) Los desparramadores centrífugos de caucho y metálico produjeron mejor distribución del material, 2) El uso de desmalezadora o rolo cuchilla sobre rastrojo de los tratamientos T3DC y T4DM aceleró la mineralización de los mismos. 3) Las dosis de urea causó poco incremento en la mineralización del rastrojo.The main purposes of this study were to: a) Find out how tools (deflectors, spreaders and choppers) can be used best for improving mineralization of stubble from harvesting rice (Oryza sativa L.) grown under direct sowing and b) Determine, for short periods, the amount of crop residues deposited on the surface and its mineralization time with various management techniques.     Inactivation of the DNA-repair gene MGMT and the clinical response of gliomas to alkylating agents

    Background: the DNA-repair enzyme O6-methylguanine-DNA methyltransferase (MGMT) inhibits the killing of tumor cells by alkylating agents. MGMT activity is controlled by a promoter; methylation of the promoter silences the gene in cancer, and the cells no longer produce MGMT. We examined gliomas to determine whether methylation of the MGMT promoter is related to the responsiveness of the tumor to alkylating agents. Methods: we analyzed the MGMT promoter in tumor DNA by a methylation-specific polymerase-chain-reaction assay. The gliomas were obtained from patients who had been treated with carmustine (1,3-bis(2-chloroethyl)-1-nitrosourea, or BCNU). The molecular data were correlated with the clinical outcome. Results: the MGMT promoter was methylated in gliomas from 19 of 47 patients (40 percent). This finding was associated with regression of the tumor and prolonged overall and disease-free survival. It was an independent and stronger prognostic factor than age, stage, tumor grade, or performance status. Conclusions: methylation of the MGMT promoter in gliomas is a useful predictor of the responsiveness of the tumors to alkylating agents

    Hybrid strategy for unstable patients with severe carotid and cardiac disease requiring surgery

    Background: Concurrent severe carotid and cardiac disease is a challenging situation where staged surgery is probably the most common strategy. However, in patients with an unstable clinical presentation, the best approach is still a matter of debate. The aim of the study was to report in-hospital and midterm outcome in patients who received carotid artery stenting and synchronous cardiac surgery. Methods: From June 1998 to July 2012, 54 consecutive patients who were treated at a high-volume university medical center with this hybrid approach were included in the study. All of the patients received carotid angioplasty while being administered aspirin and regular unfractionated heparin. Then, all of the patients were immediately transferred to the operating room for coronary and/or cardiac valve surgery. All of the patients were administered aspirin and clopidogrel once bleeding was ruled out, after surgery. Results: There were 5 in-hospital surgical related deaths, and no patient suffered a stroke or required carotid urgent re-intervention. At follow-up (55 ± 28 months; range 1–144 months), there were no new neurological deficits, while one additional death occurred. Conclusions: In this series, synchronous carotid stenting and cardiac surgery were feasible with an acceptable complication rate in a high-surgical-risk population, which could not undergo staged procedures.

    Association between COVID-19 prognosis and disease presentation, comorbidities and chronic treatment of hospitalized patients

    mportance The rapid pandemic expansion of the disease caused by the new SARS-CoV-2 virus has compromised health systems worldwide. Knowledge of prognostic factors in affected patients can help optimize care. Objective The objective of this study was to analyze the relationship between the prognosis of COVID-19 and the form of presentation of the disease, the previous pathologies of patients and their chronic treatments. Design, participants and locations This was an observational study on a cohort of 418 patients admitted to three regional hospitals in Catalonia (Spain). As primary outcomes, severe disease (need for oxygen therapy via nonrebreather mask or mechanical ventilation) and death were studied. Multivariate binary logistic regression models were performed to study the association between the different factors and the results. Results Advanced age, male sex and obesity were independent markers of poor prognosis. The most frequent presenting symptom was fever, while dyspnea was associated with severe disease and the presence of cough with greater survival. Low oxygen saturation in the emergency room, elevated CRP in the emergency room and initial radiological involvement were all related to worse prognosis. The presence of eosinophilia (% of eosinophils) was an independent marker of less severe disease. Conclusions This study identified the most robust markers of poor prognosis for COVID-19. These results can help to correctly stratify patients at the beginning of hospitalization based on the risk of developing severe disease

    Identification of circulating microRNA profiles associated with pulmonary function and radiologic features in survivors of SARS-CoV-2-induced ARDS

    There is a limited understanding of the pathophysiology of postacute pulmonary sequelae in severe COVID-19. The aim of current study was to define the circulating microRNA (miRNA) profiles associated with pulmonary function and radiologic features in survivors of SARS-CoV-2-induced ARDS. The study included patients who developed ARDS secondary to SARS-CoV-2 infection (n = 167) and a group of infected patients who did not develop ARDS (n = 33). Patients were evaluated 3 months after hospital discharge. The follow-up included a complete pulmonary evaluation and chest computed tomography. Plasma miRNA profiling was performed using RT-qPCR. Random forest was used to construct miRNA signatures associated with lung diffusing capacity for carbon monoxide (DLCO) and total severity score (TSS). Kyoto Encyclopedia of Genes and Genomes (KEGG) and Gene Ontology (GO) enrichment analyses were conducted. DLCO < 80% predicted was observed in 81.8% of the patients. TSS showed a median [P25;P75] of 5 [2;8]. The miRNA model associated with DLCO comprised miR-17-5p, miR-27a-3p, miR-126-3p, miR-146a-5p and miR-495-3p. Concerning radiologic features, a miRNA signature composed by miR-9-5p, miR-21-5p, miR-24-3p and miR-221-3p correlated with TSS values. These associations were not observed in the non-ARDS group. KEGG pathway and GO enrichment analyses provided evidence of molecular mechanisms related not only to profibrotic or anti-inflammatory states but also to cell death, immune response, hypoxia, vascularization, coagulation and viral infection. In conclusion, diffusing capacity and radiological features in survivors from SARS-CoV-2-induced ARDS are associated with specific miRNA profiles.     Genome-wide transcriptional profiling of pulmonary functional sequelae in ARDS- secondary to SARS-CoV-2 infection

    Background: Up to 80% of patients surviving acute respiratory distress syndrome (ARDS) secondary to SARS-CoV- 2 infection present persistent anomalies in pulmonary function after hospital discharge. There is a limited un-derstanding of the mechanistic pathways linked to post-acute pulmonary sequelae. Aim: To identify the molecular underpinnings associated with severe lung diffusion involvement in survivors of SARS-CoV-2-induced ARDS. Methods: Survivors attended to a complete pulmonary evaluation 3 months after hospital discharge. RNA sequencing (RNA-seq) was performed using Illumina technology in whole-blood samples from 50 patients with moderate to severe diffusion impairment (DLCO<60%) and age- and sex-matched individuals with mild-normal lung function (DLCO≥60%). A transcriptomic signature for optimal classification was constructed using random forest. Transcriptomic data were analyzed for biological pathway enrichment, cellular deconvolution, cell/tissue-specific gene expression and candidate drugs. Results: RNA-seq identified 1357 differentially expressed transcripts. A model composed of 14 mRNAs allowed the optimal discrimination of survivors with severe diffusion impairment (AUC=0.979). Hallmarks of lung sequelae involved cell death signaling, cytoskeleton reorganization, cell growth and differentiation and the immune response. Resting natural killer (NK) cells were the most important immune cell subtype for the pre-diction of severe diffusion impairment. Components of the signature correlated with neutrophil, lymphocyte and monocyte counts. A variable expression profile of the transcripts was observed in lung cell subtypes and bodily tissues. One upregulated gene, TUBB4A, constitutes a target for FDA-approved drugs. Conclusions: This work defines the transcriptional programme associated with post-acute pulmonary sequelae and provides novel insights for targeted interventions and biomarker development.MCGH is the recipient of a predoctoral fellowship from the University of Lleida.     EFECTO DE LOS FOSFOLÍPIDOS MARINOS SOBRE BIOMARCADORES NUTRICIONALES Y DE ESTRÉS FISIOLÓGICO DE LAS PARALARVAS DEL PULPO COMÚN (OCTOPUS VULGARIS) BAJO CONDICIONES DE CULTIVO ESTANDARIZADAS EN TRES CENTROS

    El efecto del enriquecimiento de la Artemia con fosfolípidos marinos (LC60), sobre el crecimiento, la supervivencia y los biomarcadores de condición nutricional (ARN/ADN) y de estrés fisiológico (proteínas de estrés (HSP70), enzimas antioxidantes y peroxidación lipídica) en paralarvas cultivadas de pulpo común (Octopus vulgaris) se evaluó bajo un protocolo estandarizado en tres centros. Las paralarvas recién eclosionadas mostraron diferencias significativas entre centros en peso seco y en la mayor parte de los biomarcadores analizados. Por el contario, en paralarvas cultivadas de 15 días, se encontraron diferencias significativas debidas a la dieta, presentando un incremento en la tasa de crecimiento específica (TCE) en el grupo alimentado con Artemia enriquecida con LC60 y mayores niveles de actividad GPX T y GR. Sin embargo, la dieta no tuvo efecto en la supervivencia de las paralarvas. Los biomarcadores de condición nutricional y de estrés fisiológicos presentaron diferencias entre los centros. Los resultados indican que el uso de fosfolípidos marinos como enriquecimiento de Artemia mejora el crecimiento de las paralarvas, si bien no se observa un efecto positivo de dicho tratamiento sobre la supervivencia de las mismas
