    La contaminación generada por el sector automotriz es un problema de medio ambiente por el que sectores gubernamentales y privados han tomado acciones para contrarrestarla. Uno de esos esfuerzos procura de proveer una solución para la emisión de gases de producidos de diferentes sistemas de combustión. El objetivo del presente proyecto es diseñar un sistema de alta calidad y confiable, capaz de transformar una cantidad considerable de los gases emitidos en nuevo combustible antes de ser liberados de vuelta al entorno. El módulo de control fue desarrollado considerando los requerimientos demandados por la industria automotriz. El controlador fue basado sobre la arquitectura AUTOSAR, este también incluyó el protocolo de comunicación estándar CAN 2.0 desempeñado con el microcontrolador validado como grado 2 por el Consejo de Electrónica Automotriz, y el sensor SHT11 usado fue certificado contra RoHS. La arquitectura de software cumple con la complejidad inherente de las especificaciones de AUTOSAR, por consiguiente, diferentes técnicas fueron requeridas para su solución, incluyendo la definición, diagramas de límites, especificación de requerimientos, interfaces de software e interacción de módulos. Una vez que los requerimientos fueron conocidos, el código fue implementado. Como resultado, este módulo puede ser categorizado como un producto de grado automotriz que puede ser introducido en el mercado automotriz.The pollution generated by the automotive sector has been an environmental issue in the latest years, and nowadays, different governmental and private sectors have taken actions on this matter. One of these efforts tries to provide a solution for contaminating gas emissions produced from different fuel combustion systems. The aim of the present project is to design a reliable and high-quality system that senses the environmental temperature, relative humidity, and calculates the dew point to control a catalyzer capable of transforming a considerable amount of exhaust gases into a new fuel component before they are released back into the environment. The control module was developed considering the requirements demanded by the automotive industry. The controller was based on an AUTOSAR architecture, it also included the standard CAN 2.0 communication protocol performed within the microcontroller validated as grade 2 by the Automotive Electronics Council, and the SHT11 sensor used was certified against RoHS. Equally important, the software architecture complied with the complexity inherent in AUTOSAR specifications. Therefore, different techniques were required for its solution, including, boundary diagram, requirement specifications, software interface, and module interaction definitions. Once these requirements where met, the code was implemented.     The association between different domains of quality of life and symptoms in primary care patients with emotional disorders

    Despite the importance of quality of life (QoL) in primary care patients with emotional disorders, the specific influence of the symptoms of these disorders and the sociodemographic characteristics of patients on the various QoL domains has received scant attention. The aim of the present study of primary care patients with emotional disorders was to analyse the associations between four different QoL domains and the most prevalent clinical symptoms (i.e., depression, anxiety and somatization), while controlling for sociodemographic variables. A total of 1241 participants from 28 primary care centres in Spain were assessed with the following instruments: the Patient Health Questionnaire (PHQ)-9 to evaluate depression; the Generalized Anxiety Disorder Scale (GAD)-7 for anxiety; PHQ-15 for somatization; and the World Health Organization Quality of Life Instrument-Short Form (WHOQOL- Bref) to assess four broad QoL domains: physical health, psychological health, social relationships, and environment. The associations between the symptoms and QoL domains were examined using hierarchical regression analyses. Adjusted QoL mean values as a function of the number of overlapping diagnoses were calculated. The contribution of sociodemographic variables to most QoL domains was modest, explaining anywhere from 2% to 11% of the variance. However, adding the clinical variables increased the variance explained by 12% to 40% depending on the specific QoL domain. Depression was the strongest predictor for all domains. The number of overlapping diagnoses adversely affected all QoL domains, with each additional diagnosis reducing the main QoL subscales by 5 to 10 points. In primary care patients with a diagnostic impression of an emotional disorders as identified by their treating GP, clinical symptoms explained more of the variance in QoL than sociodemographic factors such as age, sex, level of education, marital status, work status, and income. Given the strong relationship between depressive symptoms and QoL, treatment of depression may constitute a key therapeutic target to improve QoL in people with emotional disorders in primary care

    Roads of the Caribbean: regional analysis from Environmental Impact Assessments in Colombia

    The road infrastructure produces several impacts on the environment. In the Colombian Caribbean region, the growing road infrastructure threatens systems equilibrium in diverse and not well-known ways. Despite the several administrative tools available in Colombian legislation to manage every project impact, the big picture of the interconnected regional ecosystems falls out of scope. To study the large-scale problems in landscape, we analyzed eight Environmental Impact Studies of road construction projects in the region. We did a scientific and institutional literature review to shed light on the possible unseen problems and future challenges. Results suggest that Environmental Impact Assessments for each project focus on construction processes, ignore accumulative and residual effects, and use typified measures to cover a broad set of impacts. We offer recommendations from an integrated analysis perspective for future projects to understand landscape and water systems alteration at the regional scale

    The Latin American experience of allografting patients with severe aplastic anaemia: real-world data on the impact of stem cell source and ATG administration in HLA-identical sibling transplants

    We studied 298 patients with severe aplastic anaemia (SAA) allografted in four Latin American countries. The source of cells was bone marrow (BM) in 94 patients and PBSCs in 204 patients. Engraftment failed in 8.1% of recipients with no difference between BM and PBSCs (P = 0.08). Incidence of acute GvHD (aGvHD) for BM and PBSCs was 30% vs 32% (P = 0.18), and for grades III–IV was 2.6% vs 11.6% (P = 0.01). Chronic GvHD (cGvHD) between BM and PBSCs was 37% vs 59% (P = 0.002) and extensive 5% vs 23.6% (P = 0.01). OS was 74% vs 76% for BM vs PBSCs (P = 0.95). Event-free survival was superior in patients conditioned with anti-thymocyte globulin (ATG)-based regimens compared with other regimens (79% vs 61%, P = 0.001) as excessive secondary graft failure was seen with other regimens (10% vs 26%, P = 0.005) respectively. In multivariate analysis, aGvHD II–IV (hazard ratio (HR) 2.50, confidence interval (CI) 1.1–5.6, P = 0.02) and aGvHD III–IV (HR 8.3 CI 3.4–20.2, Po0.001) proved to be independent negative predictors of survival. In conclusion, BM as a source of cells and ATG-based regimens should be standard because of higher GvHD incidence with PBSCs, although the latter combining with ATG in the conditioning regimen could be an option in selected high-risk patient

    Compromising between European and US allergen immunotherapy schools: Discussions from GUIMIT, the Mexican immunotherapy guidelines

    Background: Allergen immunotherapy (AIT) has a longstanding history and still remains the only disease-changing treatment for allergic rhinitis and asthma. Over the years 2 different schools have developed their strategies: the United States (US) and the European. Allergen extracts available in these regions are adapted to local practice. In other parts of the world, extracts from both regions and local ones are commercialized, as in Mexico. Here, local experts developed a national AIT guideline (GUIMIT 2019) searching for compromises between both schools. Methods: Using ADAPTE methodology for transculturizing guidelines and AGREE-II for evaluating guideline quality, GUIMIT selected 3 high-quality Main Reference Guidelines (MRGs): the European Academy of Allergy, Asthma and Immunology (EAACI) guideines, the S2k guideline of various German-speaking medical societies (2014), and the US Practice Parameters on Allergen Immunotherapy 2011. We formulated clinical questions and based responses on the fused evidence available in the MRGs, combined with local possibilities, patient's preference, and costs. We came across several issues on which the MRGs disagreed. These are presented here along with arguments of GUIMIT members to resolve them. GUIMIT (for a complete English version, see Supplementary data) concluded the following: Results: Related to the diagnosis of IgE-mediated respiratory allergy, apart from skin prick testing complementary tests (challenges, in vitro testing and molecular such as species-specific allergens) might be useful in selected cases to inform AIT composition. AIT is indicated in allergic rhinitis and suggested in allergic asthma (once controlled) and IgE-mediated atopic dermatitis. Concerning the correct subcutaneous AIT dose for compounding vials according to the US school: dosing tables and formula are given; up to 4 non-related allergens can be mixed, refraining from mixing high with low protease extracts. When using European extracts: the manufacturer's indications should be followed; in multi-allergic patients 2 simultaneous injections can be given (100% consensus); mixing is discouraged. In Mexico only allergoid tablets are available; based on doses used in all sublingual immunotherapy (SLIT) publications referenced in MRGs, GUIMIT suggests a probable effective dose related to subcutaneous immunotherapy (SCIT) might be: 50–200% of the monthly SCIT dose given daily, maximum mixing 4 allergens. Also, a table with practical suggestions on non-evidence-existing issues, developed with a simplified Delphi method, is added. Finally, dissemination and implementation of guidelines is briefly discussed, explaining how we used online tools for this in Mexico. Conclusions: Countries where European and American AIT extracts are available should adjust AIT according to which school is followed

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Implicaciones nutricionales de las dietas de eliminación

    Introducción: La piedra angular del tratamiento de alergia alimentaria es la eliminación de los alimentos causantes. Sin embargo, los niños que restringen el consumo de alimentos básicos tienen un mayor riesgo de desnutrición. El objetivo del estudio fue identificar el estado nutricional de pacientes con dieta de eliminación e identificar la proporción de pacientes del grupo con verdadera alergia alimentaria. Métodos: Se realizó un estudio transversal de enero a octubre de 2014 en el Hospital Infantil de México Federico Gómez. Se incluyeron pacientes de 1 a 11 años con historia de eliminación de, al menos, uno de cinco alimentos (huevo, leche, trigo, maíz, soya) por un mínimo de 6 meses. Se realizó la valoración nutricional completa y se compararon los índices antropométricos con tablas de Z score para la edad. Se analizaron los datos por medio de estadística descriptiva, y posteriormente con prueba de Kruskal-Wallis y correlación de Spearman. Resultados: Los alimentos más frecuentemente eliminados fueron leche, soya, huevo, maíz y trigo. Al comparar el número de alimentos eliminados de la dieta con los distintos índices antropométricos evaluados, se encontró que entre mayor cantidad de alimentos eliminados, el score Z de peso para la edad (PE) y talla para la edad (TE), así como peso para la talla (PT) fueron menores, y el más afectado fue la reserva grasa. Solamente en el 5% de los niños se corroboró alergia alimentaria. Conclusiones: Nuestro estudio confirma la necesidad de una correcta asesoría nutricional en aquellos pacientes que cuenten con dietas de eliminación, así como el sobrediagnóstico que existe de alergia alimentaria
