    Determination of oxygen tension in the subcutaneous tissue of cosmonauts during the Salyut-6 mission

    A polarographic technique was used to measure the oxygen tension in subcutaneous tissue of the forearm of a cosmonaut prior to, after, and on the fourth day of a space mission performed by Salut-6. A drop in the oxygen exchange rate in the peripheral tissues during weightlessness was observed. The mechanisms of this change are studied, taking into consideration the blood distribution in the organism and microcirculation disorders reflected by a decreased blood flow rate in arterial-venous junctions

    Investigation of cooling properties of the gaseous medium of a space station

    An investigation of cooling properties of the gaseous medium was performed in the biosatellite Kosmos-936 as well as in the orbital complexes Soyuz-28/Salyut-6 and Soyuz-30/Salyut-6 with the aid of an especially constructed electric dynamic catathermometer. In this instrument current was measured which was necessary to keep a steady settled temperature of the sensing device. The investigation was performed because of the disturbed heat exhange of the human body caused by lack of natural convection in weightlessness. The instrument also enabled objective estimation of the temperature of the cosmonaut's ody in six optionally selected regions. The results obtained by means of the catathermometer will also enable defining the appropriate hygienic conditions of the gaseous medium of space stations

    A case of granulomatous lung disease in a patient with Good's syndrome

    Good's syndrome is extremely rare. This adult-onset condition is characterized by a thymoma with immunodeficiency, low B- and T-cell counts, and hypo-gammaglobulinemia. The initial clinical presentation is either a mass-lesion thymoma or a recurrent infection. Patients with Good's syndrome are very susceptible to infections; common respiratory and opportunistic infections can be life-threatening. There are no reports of granulomatous lung disease in patients with Good's syndrome, although it has been observed in patients with common variable immunodeficiency, of which Good's syndrome is a subset. We describe a 53-year-old male thymoma patient who presented with respiratory symptoms caused by granulomatous lung disease and an opportunistic infection. He died of uncontrolled fungal infection despite repeated intravenous immunoglobulin and supportive care. Clinicians should look for evidence of immunologic dysfunction in thymoma patients presenting with severe recurrent infections, especially opportunistic infections

    Cerebral toxoplasmosis in a patient with myasthenia gravis and thymoma with immunodeficiency/Good’s syndrome: a case report

    Background Patients with thymoma with immunodeficiency (TWI)/Good’s syndrome characteristically have evidence of combined immunodeficiency including low or absent B-cells, hypogammaglobulinemia and defects in T-cell mediated immunity. These patients can present with common or opportunistic infections. Case presentation A 54-year-old female was diagnosed with cerebral toxoplasmosis. This occurred on a background of metastatic thymoma previously treated with chemotherapy and myasthenia gravis (MG) treated with mycophenolate mofetil, monthly intravenous immunoglobulin (IVIG) and pyridostigmine. She reported recurrent herpes zoster infection. The patient had clinical and radiological progression of cerebral infection despite completing standard induction and maintenance therapy with sulfadiazine and pyrimethamine. Investigations found a complete absence of B-cells and evidence for hypogammaglobulinemia which, together with evidence of defects in T-cell mediated immunity and thymoma, lead to a diagnosis of TWI/Good’s Syndrome. The patient has undergone prolonged high-dose therapy for toxoplasmosis and a reduction in immunosuppression with no evidence of recurrent toxoplasmosis or flare of MG. Conclusions TWI/Good’s Syndrome should be suspected in patients with thymoma and recurrent, persistent or unusual infections. If suspected serum immunoglobulins and lymphocyte subsets should be measured. These patients may need closer monitoring, higher dose and prolonged treatment of infections, and weaning of concurrent immunosuppression may be considered.</p

    Primary immunodeficiency disease (PID) has traditionally been viewed as a group of illnesses seen in the paediatric age group. New advances in diagnosis and treatment have led to an increase in the number of elderly PID patients. However, there is lack of research evidence on which to base clinical management in this group of patients. Management decisions often have to be based therefore on extrapolations from other patient cohorts or from younger patients. Data from the European Society for Immunodeficiencies demonstrates that the vast majority of elderly patients suffer from predominantly antibody deficiency syndromes. We review the management of PID disease in the elderly, with a focus on antibody deficiency disease

    Analysis of the efficiency management of local government against the challenges of public administration Modern management of a local government implies the use the various instruments supporting the decision making process. Long-term financial planning is becoming a necessity in the face of constant government deficit of local government. However, system solutions requiring the government to draw up various planning documents do not always produce the expected result. Perhaps the same definition of indicators, the corresponding limits allowable debt (in modified form in relation to the previous limits of debt) would have been sufficient to local government to draw more attention to effectiveness public spending, rather than decreasing the efficiency of their actions

    Article refers to the constitutional principle of subsidiarity. The author assumes that this principle should be applied already at the lowest level of civic activity, in the municipality and its auxiliary units. Sołectwo it is the basic case of the auxiliary unit in Poland. The article is a case study of the Sława municipality, located in the western part of Poland. The sołectwo fund as an important form of implementation of the principle of subsidiarity has been developed for a few years in the community. The author focuses on the socio – economic aspects of the initiative. The official data that illustrate the conditions, course and consequences of the development budget of the sołectwo fund are analyzed and presented in the text. The presented data indicate that allocating a certain amount of financial resources to fulfill the tasks entrusted to sołectwo can increase their social activity and trigger civil initiatives as well. It can also give a chance that the inhabitants of a village can be interested in influencing the councils and in shaping directions of spending a commune’s money. In this way they can contribute to an increase in economic efficiency and rationality of these expenditures. The process of liberating pro-growth measures can help to improve the quality of life of inhabitants of the village.Artykuł odnosi się do konstytucyjnej zasady pomocniczości.
