    Patients´ Experiences of Pain Following Day Surgery - At 48 Hours, Seven Days and Three Months

    Recent studies indicate that patients experience pain after day surgery for a longer period than previously known. This requires verification. This was a prospective, descriptive correlational study. A convenience sample of 298 day surgery patients undergoing various surgical procedures was asked to report pain intensity and its interference with daily function 48 hours, seven days and three months after day surgery. Correlation and regression analyses were performed. On a NRS, 55% (n=230) reported pain (≥4) 48 hours after surgery, as did 43% (n=213) at seven days. Pain interfered with normal activities at ≥4 NRS at 48 hours and at seven days, after which it decreased

    Functional loss of IKBE leads to NF-KB deregulation in aggressive chronic lymphocytic leukemia

    NF-?B is constitutively activated in chronic lymphocytic leukemia (CLL); however, the implicated molecular mechanisms remain largely unknown. Thus, we performed targeted deep sequencing of 18 core complex genes within the NF-?B pathway in a discovery and validation CLL cohort totaling 315 cases. The most frequently mutated gene was NFKBIE (21/315 cases; 7%), which encodes I?B?, a negative regulator of NF-?B in normal B cells. Strikingly, 13 of these cases carried an identical 4-bp frameshift deletion, resulting in a truncated protein. Screening of an additional 377 CLL cases revealed that NFKBIE aberrations predominated in poor-prognostic patients and were associated with inferior outcome. Minor subclones and/or clonal evolution were also observed, thus potentially linking this recurrent event to disease progression. Compared with wild-type patients, NFKBIE-deleted cases showed reduced I?B? protein levels and decreased p65 inhibition, along with increased phosphorylation and nuclear translocation of p65. Considering the central role of B cell receptor (BcR) signaling in CLL pathobiology, it is notable that I?B? loss was enriched in aggressive cases with distinctive stereotyped BcR, likely contributing to their poor prognosis, and leading to an altered response to BcR inhibitors. Because NFKBIE deletions were observed in several other B cell lymphomas, our findings suggest a novel common mechanism of NF-?B deregulation during lymphomagenesis. <br/

    Photostability of commercial sunscreens upon sun exposure and irradiation by ultraviolet lamps

    BACKGROUND: Sunscreens are being widely used to reduce exposure to harmful ultraviolet (UV) radiation. The fact that some sunscreens are photounstable has been known for many years. Since the UV-absorbing ingredients of sunscreens may be photounstable, especially in the long wavelength region, it is of great interest to determine their degradation during exposure to UV radiation. Our aim was to investigate the photostability of seven commercial sunscreen products after natural UV exposure (UVnat) and artificial UV exposure (UVart). METHODS: Seven commercial sunscreens were studied with absorption spectroscopy. Sunscreen product, 0.5 mg/cm(2), was placed between plates of silica. The area under the curve (AUC) in the spectrum was calculated for UVA (320–400 nm), UVA1 (340–400 nm), UVA2 (320–340 nm) and UVB (290–320 nm) before (AUC(before)) and after (AUC(after)) UVart (980 kJ/m(2 )UVA and 12 kJ/m(2 )of UVB) and before and after UVnat. If theAUC Index (AUCI), defined as AUCI = AUC(after)/AUC(before), was > 0.80, the sunscreen was considered photostable. RESULTS: Three sunscreens were unstable after 90 min of UVnat; in the UVA range the AUCI was between 0.41 and 0.76. In the UVB range one of these sunscreens was unstable with an AUCI of 0.75 after 90 min. Three sunscreens were photostable after 120 min of UVnat; in the UVA range the AUCI was between 0.85 and 0.99 and in the UVB range between 0.92 and 1.0. One sunscreen showed in the UVA range an AUCI of 0.87 after UVnat but an AUCI of 0.72 after UVart. Five of the sunscreens were stable in the UVB region. CONCLUSION: The present study shows that several sunscreens are photounstable in the UVA range after UVnat and UVart. There is a need for a standardized method to measure photostability, and the photostability should be marked on the sunscreen product

    Patients' experiences and perceived causes of persisting discomfort following day surgery

    <p>Abstract</p> <p>Background</p> <p>The aim of this study was to describe patients' experiences and perceived causes of persisting discomfort following day surgery. Earlier research has mainly covered symptoms and signs during a recovery period of up to one month, and not dealt with patients' perceptions of what causes persisting, longer-term discomfort.</p> <p>Methods</p> <p>This study is a part from a study carried out during the period May 2006 to May 2007 with a total of 298 day surgery patients. Answers were completed by 118 patients at 48 hours, 110 at seven days and 46 at three months to one open-ended question related to discomfort after day surgery constructed as follows: <it>If you are still experiencing discomfort related to the surgery, what is the reason, in your opinion</it>? Data was processed, quantitatively and qualitatively. Descriptive, inferential, correlation and content analyses were performed.</p> <p>Results</p> <p>The results suggest that patients suffer from remaining discomfort e.g. pain and wound problem, with effects on daily life following day surgery up to three months. Among patients' perceptions of <it>factors leading to discomfort </it>may be <it>wrongful or suboptimal treatment</it>, <it>type of surgery </it>or <it>insufficient access to provider/information.</it></p> <p>Conclusions</p> <p>The results have important implications for preventing and managing discomfort at home following day surgery, and for nursing interventions to help patients handle the recovery period better.</p

    Rye kernel breakfast increases satiety in the afternoon - an effect of food structure

    <p>Abstract</p> <p>Background</p> <p>The structure of whole grain cereals is maintained to varying degrees during processing and preparation of foods. Food structure can influence metabolism, including perceived hunger and satiety. A diet that enhances satiety per calorie may help to prevent excessive calorie intake. The objective of this work was to compare subjective appetite ratings after consumption of intact and milled rye kernels.</p> <p>Methods</p> <p>Two studies were performed using a randomized, cross-over design. Ratings for appetite (hunger, satiety and desire to eat) were registered during an 8-h period after consumption of whole and milled rye kernels prepared as breads (study 1, n = 24) and porridges (study 2, n = 20). Sifted wheat bread was used as reference in both study parts and the products were eaten in iso-caloric portions with standardized additional breakfast foods. Breads and porridges were analyzed to determine whether structure (whole vs. milled kernels) effected dietary fibre content and composition after preparation of the products. Statistical evaluation of the appetite ratings after intake of the different breakfasts was done by paired t-tests for morning and afternoon ratings separately, with subjects as random effect and type of breakfast and time points as fixed effects.</p> <p>Results</p> <p>All rye breakfasts resulted in higher satiety ratings in the morning and afternoon compared with the iso-caloric reference breakfast with sifted wheat bread. Rye bread with milled or whole kernels affected appetite equally, so no effect of structure was observed. In contrast, after consumption of the rye kernel breakfast, satiety was increased and hunger suppressed in the afternoon compared with the milled rye kernel porridge breakfast. This effect could be related to structural differences alone, because the products were equal in nutritional content including dietary fibre content and composition.</p> <p>Conclusions</p> <p>The study demonstrates that small changes in diet composition such as cereal grain structure have the potential to effect feelings of hunger and satiety.</p> <p>Trial registration</p> <p>This trial was registered at clinicaltrials.gov as <a href="http://www.clinicaltrials.gov/ct2/show/NCT01042418">NCT01042418</a>.</p

    Living with tinnitus and Menieres disease - a patient and nurse perspective

    Tinnitus och Menieres sjukdom är båda dolda handikapp, som påverkar hörseln. Tinnitus är ett vanligt symtom, inte bara i Sverige utan även internationellt. Tinnitus definieras som en upplevelse av ljud i frånvaro av yttre stimulering. Menieres sjukdom är ett kroniskt tillstånd som drabbar innerörat och ger symtom i form av anfall med yrsel, hörselnedsättning, tinnitus samt lock för örat. Syftet med detta arbete var att beskriva hur tinnitus och Menieres sjukdom kan påverka patienten i det dagliga livet, samt hur sjuksköterskan kan agera för att dessa patienter ska känna trygghet trots sina symtom. Trygghet är en känsla som alla människor är i behov av och vid kronisk sjukdom kan det vara viktigt att känna trygghet i sin sjukdom. Inom omvårdnad är trygghet ett centralt begrepp och därför är det av stor vikt att förstå vad begreppet trygghet innebär. Känslan av trygghet är något som byggs upp av livets erfarenheter och möten med människor, man känner igen den speciella känslan man får i kroppen. Att känna trygghet är betydelsefullt för patienten i mötet med sjuksköterskan för att bekräftelse, förståelse, värdighet och integritet skall kunna förverkligas. Resultatet baserades på 17 vetenskapliga artiklar samt en avhandling och är sammanställt utifrån patient och sjuksköterskeperspektiv. Det dagliga livet påverkades fysiskt, psykiskt samt socialt vilket för många av dessa patienter resulterade i social isolering, koncentrationssvårigheter, trötthet, sömnproblem samt depression. Information, undervisning, stöd och bekräftelse var faktorer som hade stor betydelse i mötet mellan sjuksköterskan och dessa patienter för de ska känna trygghet trots sina symtom. Det behövs mer forskning inom området för att bemöta dessa personer på ett professionellt sätt

    Elastic properties of Heusler alloys by ab initio theory and lattice dynamics of graphene

    This study investigates elastic properties and lattice dynamics of promising compounds such as Heusler alloys and 2D graphene using Density Functional Theory (DFT). Simulations have been done in the software Quantum Espresso (Q.E) to produce a good model for the different solids. The lattice dynamics and elastic properties have been compared to experimental data where possible. In conclusion, the systems are stable and provide good candidates for future applications
