    Use and Integration of iPads with Students with Low Incidence Disabilities in Elementary Schools

    I focused this dissertation on what is happening within schools regarding iPad usage and students with disabilities. Technology usage is thought to help provide students with another modality for learning the curriculum. iPads, specifically, are leading the technological revolution in classroom environments. One benefit of iPads in instruction is that they assist all students, especially those with disabilities by helping them complete a task with increased ease. The focus of this research was to study the usage of iPads in K-6th educational settings with students with low incidence disabilities. Technology usage among students with disabilities continues to rise, yet teachers are still unsure as to whether or not their technology integration is meaningful. I found that teachers had strong intentions to integrate iPads with students with disabilities, but oftentimes their teaching practices did not promote the usage of the devices. This study utilized a qualitative approach including semi-structured interviews, observations, and document analysis. I used these approaches to understand the following questions: In what ways are iPads used in inclusive classrooms to support the needs of students with low incidence disabilities? What are the iPad integration practices of teachers with students with low incidence disabilities? I conducted semi-structured interviews with teacher teams to see how teachers and other school personnel integrated iPads into their inclusive general education classrooms. I interviewed student users and one parent to gather their perspectives and perceptions on the usage of the iPads. I used observations as a method to gather data on the usage of iPads to meet the needs of the user and to understand how the teachers integrated iPads into the curriculum. I completed document analyses of student IEPs (Individualized Education Programs) to understand the goals and needs of the student in comparison to their instruction

    Prevalence of Low Energy Availability in Collegiate Female Runners and Implementation of Nutrition Education Intervention

    Objective: This study examined the prevalence of low energy availability in a sample of female collegiate athletes (N=25) then delivered nutrition education related to the female athlete triad and assessed change in knowledge and dietary behaviors. Methods: Average energy intake was assessed pre- and post-education using Automated Self-Administered 24- Hour Dietary Recalls. We assessed body composition with multiple-site skinfold measures. Energy expenditure was assessed with accelerometers and a physical activity diary over a 3-day period. A 73-item questionnaire was used to assess knowledge and behavior changes. Results: At baseline, 92% had an index of energy availability /kg of fat free mass/day. 40% of participants were amenorrheic, and 32% had a history of stress fractures. There was an increase in summed nutrition knowledge, post-nutrition education (p=0.001), but no increase in caloric intake (p=0.979). Conclusion: Low energy availability was common in this sample of female collegiate track athletes, but did not improve with a targeted intervention

    Fostering Communication Through Physical Activity

    Many individuals with autism experience difficulties with reliable, meaningful communication often impacting their academic and social engagement. As the first and most frequent communication partners, parents of children with autism may struggle initiating or maintaining meaningful communicative interactions, and thus require training, tools, and support. This article describes an approach to coaching parents in fostering meaningful, reciprocal communication through recreational activities as part of a larger physical activity program for parents and families of children with autism. It describes a Cycle of Communication framework as a tool for parents to recognize opportunities to structure and support their children’s communicative attempts and are discussed herein for educators, families and practitioners to adapt and use within their local contexts

    A COM-B and Theoretical Domains Framework mapping of the barriers and facilitators to effective communication and help-seeking among people with, or seeking a diagnosis of, endometriosis

    Endometriosis is a chronic condition in which tissue resembling the endometrium grows outside the womb, causing severe chronic pain. People with endometriosis report difficulty in help-seeking and communicating with healthcare professionals, contributing to diagnosis delays and ineffective management. The present study aimed to identify barriers and facilitators to effective communication using the Theoretical Domains Framework (TDF) and COM-B model to inform behavior change intervention development. This study was a qualitative semi-structured interview and open-ended survey design. Thematic Analysis was utilized to identify barriers and facilitators to effective communication which were mapped to the TDF and COM-B model. Four women aged 25 to 44 with a formal diagnosis of endometriosis participated in interviews. Thirty-three participants, aged 20–48 years, participated in the online survey, 21 of whom had a diagnosis of endometriosis (12 were currently seeking diagnosis). Five COM-B domains were identified: reflective motivation, social and physical opportunity, physical and psychological capability. Ten TDF domains were reflected in concerns surrounding dismissal, disempowerment, social norms, beliefs about consequences, cognitive resources, reinforcement, and environmental context and resources, among others. This is the first study to identify barriers and facilitators of effective communication and help-seeking in light of established behavioral change theory and frameworks for comprehensive intervention design. This provides a comprehensive explanation of challenges in help-seeking for endometriosis and represents the first step in the development of complex interventions to improve help-seeking and communication for people with endometriosis. Interventions targeting salient barriers will have greater potential to change behavior and improve outcomes

    An fMRI investigation of the effects of attempted naming on word retrieval in aphasia

    In healthy controls, picture naming performance can be facilitated by a single prior exposure to the same picture ("priming"). This priming phenomenon is utilized in the treatment of aphasia, which often includes repeated picture naming as part of a therapeutic task. The current study sought to determine whether single and/or multiple exposures facilitate subsequent naming in aphasia and whether such facilitatory effects act through normal priming mechanisms. A functional magnetic resonance imaging paradigm was employed to explore the beneficial effects of attempted naming in two individuals with aphasia and a control group. The timing and number of prior exposures was manipulated, with investigation of both short-term effects (single prior exposure over a period of minutes) and long-term effects (multiple presentations over a period of days). Following attempted naming, both short-term and long-term facilitated items showed improvement for controls, while only the long-term condition showed benefits at a behavioral level for the participants with aphasia. At a neural level, effects of long-term facilitation were noted in the left precuneus for one participant with aphasia, a result also identified for the equivalent contrast in controls. It appears that multiple attempts are required to improve naming performance in the presence of anomia and that for some individuals with aphasia the source of facilitation may be similar to unimpaired mechanisms engaged outside the language network

    Diagnostic accuracy of optical coherence tomography for diagnosing glaucoma: secondary analyses of the GATE study

    Background/Aims: To assess the diagnostic performance of retinal nerve fibre layer (RNFL) data of optical coherence tomography (OCT) for detecting glaucoma. Methods: Secondary analyses of a prospective, multicentre diagnostic study (Glaucoma Automated Tests Evaluation (GATE)) referred to hospital eye services in the UK were conducted. We included data from 899 of 966 participants referred to hospital eye services with suspected glaucoma or ocular hypertension. We used both eyes’ data and logistic regression-based receiver operator characteristics analysis to build a set of models to measure the sensitivity and specificity of the average and inferior quadrant RNFL thickness data of OCT. The reference standard was expert clinician examination including automated perimetry. The main outcome measures were sensitivity at 0.95 specificity and specificity at 0.95 sensitivity and the corresponding RNFL thickness thresholds. We explored the possibility of accuracy improvement by adding measures of within-eye and between-eye variation, scan quality, intraocular pressure (IOP) and age. Results: Glaucoma was diagnosed in at least one eye in 17% of participants. Areas under the curve were between 0.83 and 0.88. When specificity was fixed at 0.95, the sensitivity was between 0.38 and 0.55, and the highest values were reached with models including the inferior quadrant rather than the average RNFL thickness. Fixing specificity at 0.95, the sensitivity was between 0.36 and 0.58. The addition of age, refractive error, IOP or within-subject variation did not improve the accuracy. Conclusion: RNFL thickness data of OCT can be used as a diagnostic test, but accuracy estimates remain moderate even in exploratory multivariable modelling of aiming to improve accuracy

    Embodied spatial practices and everyday organization: the work of tour guides and their audiences

    This article introduces an interactional perspective to the analysis of organizational space. The study is based on the analysis of over 100 hours of video recordings of guided tours undertaken within two sites (an historic house and a world-famous museum), coupled with interviews and field observations. The analysis is informed by ethnomethodology and conversation analysis in order to focus on the everyday organization of these tours, and the lived experience of inhabiting museum spaces. We use an interactional lens to unpack the ‘embodied spatial practices’ critical to the work of tour guides and their audiences, which reveals how the sense and significance of the workspace emerges moment to moment, and in relation to the ongoing work at hand. As a result, for those with an interest in organizational space, the article introduces a novel perspective, and methods, to highlight the dynamic and interactional production of workspaces. Additionally, for those with an interest in practice, the article demonstrates the fundamental import of taking spatial arrangements seriously when analysing the organization of work

    Can Automated Imaging for Optic Disc and Retinal Nerve Fiber Layer Analysis Aid Glaucoma Detection?

    Purpose- To compare the diagnostic performance of automated imaging for glaucoma. Design- Prospective, direct comparison study. Participants- Adults with suspected glaucoma or ocular hypertension referred to hospital eye services in the United Kingdom. Methods- We evaluated 4 automated imaging test algorithms: the Heidelberg Retinal Tomography (HRT; Heidelberg Engineering, Heidelberg, Germany) glaucoma probability score (GPS), the HRT Moorfields regression analysis (MRA), scanning laser polarimetry (GDx enhanced corneal compensation; Glaucoma Diagnostics (GDx), Carl Zeiss Meditec, Dublin, CA) nerve fiber indicator (NFI), and Spectralis optical coherence tomography (OCT; Heidelberg Engineering) retinal nerve fiber layer (RNFL) classification. We defined abnormal tests as an automated classification of outside normal limits for HRT and OCT or NFI ≥ 56 (GDx). We conducted a sensitivity analysis, using borderline abnormal image classifications. The reference standard was clinical diagnosis by a masked glaucoma expert including standardized clinical assessment and automated perimetry. We analyzed 1 eye per patient (the one with more advanced disease). We also evaluated the performance according to severity and using a combination of 2 technologies. Main Outcome Measures- Sensitivity and specificity, likelihood ratios, diagnostic, odds ratio, and proportion of indeterminate tests. Results- We recruited 955 participants, and 943 were included in the analysis. The average age was 60.5 years (standard deviation, 13.8 years); 51.1% were women. Glaucoma was diagnosed in at least 1 eye in 16.8%; 32% of participants had no glaucoma-related findings. The HRT MRA had the highest sensitivity (87.0%; 95% confidence interval [CI], 80.2%–92.1%), but lowest specificity (63.9%; 95% CI, 60.2%–67.4%); GDx had the lowest sensitivity (35.1%; 95% CI, 27.0%–43.8%), but the highest specificity (97.2%; 95% CI, 95.6%–98.3%). The HRT GPS sensitivity was 81.5% (95% CI, 73.9%–87.6%), and specificity was 67.7% (95% CI, 64.2%–71.2%); OCT sensitivity was 76.9% (95% CI, 69.2%–83.4%), and specificity was 78.5% (95% CI, 75.4%–81.4%). Including only eyes with severe glaucoma, sensitivity increased: HRT MRA, HRT GPS, and OCT would miss 5% of eyes, and GDx would miss 21% of eyes. A combination of 2 different tests did not improve the accuracy substantially. Conclusions- Automated imaging technologies can aid clinicians in diagnosing glaucoma, but may not replace current strategies because they can miss some cases of severe glaucoma

    Real-time measurement of metabolic rate during freezing and thawing of the wood frog, Rana sylvatica: Implications for overwinter energy use

    Ectotherms overwintering in temperate ecosystems must survive low temperatures while conserving energy to fuel post-winter reproduction. Freeze-tolerant wood frogs, Rana sylvatica, have an active response to the initiation of ice formation that includes mobilising glucose from glycogen and circulating it around the body to act as a cryoprotectant. We used flow-through respirometry to measure CO2 production (VCO2) in real time during cooling, freezing and thawing. CO2 production increases sharply at three points during freeze-thaw: at +1°C during cooling prior to ice formation (total of 104±17 μl CO2 frog-1 event-1), at the initiation of freezing (565±85 μl CO 2 frog-1 freezing event-1) and after the frog has thawed (564±75 μl CO2 frog-1 freezing event-1). We interpret these increases in metabolic rate to represent the energetic costs of preparation for freezing, the response to freezing and the re-establishment of homeostasis and repair of damage after thawing, respectively. We assumed that frogs metabolise lipid when unfrozen and that carbohydrate fuels metabolism during cooling, freezing and thawing, and when frozen. We then used microclimate temperature data to predict overwinter energetics of wood frogs. Based on the freezing and melting points we measured, frogs in the field were predicted to experience as many as 23 freeze-thaw cycles in the winter of our microclimate recordings. Overwinter carbohydrate consumption appears to be driven by the frequency of freeze-thaw events, and changes in overwinter climate that affect the frequency of freeze-thaw will influence carbohydrate consumption, but changes that affect mean temperatures and the frequency of winter warm spells will modify lipid consumption
