    Mechanisms behind the success of exercise as an adjunct quit smoking aid

    It is well documented that temporarily abstinent smokers who undergo an acute bout of moderate intensity exercise experience a reduction in nicotine craving and withdrawal. Conversely, available research in chronic exercise and smoking cessation does not reliably demonstrate that combining exercise with well established treatments increases smoking abstinence rates. The overall aim of this dissertation was to investigate mechanisms underlying the beneficial effects of exercise in a group of female smokers taking part in a 14 week exercise plus nicotine replacement therapy (NRT) patch programme. Determining how smokers may benefit from exercise has important implications for designing effective interventions. Three independent yet related studies were conducted. The specific aims of study one (Chapter two) were to determine whether an acute bout of exercise would further reduce craving and withdrawal symptoms in a group of female quitters using NRT, and to explore whether the magnitude of this relief would relate to quit success. It was demonstrated that exercise provided additional craving and withdrawal benefit for these quitters. Magnitude of relief was not found to be related to end of programme quit status. Study two (Chapter three) sought to examine what bearing exercise expectancy (EXP) and credibility (CRED) beliefs of recently quit smokers had on self-reported craving and withdrawal following an acute bout of exercise, and to compare participants\u27 beliefs regarding NRT and exercise as quit smoking aids. Results showed that high exercise EXP and/or exercise CRED were found to be related to craving relief. Participants believed both NRT and exercise to be effective cessation aids. Study three (Chapter four) examined whether participants reported using exercise as a relapse prevention aid, and how readily these two behaviours can be concurrently changed. It was demonstrated that participants were able to simultaneously increase their exercise behaviour and decrease their smoking behaviour. Exercise was not reportedly used as a relapse prevention strategy. Overall, the results of these studies provide evidence that exercise is a worthwhile adjunct smoking cessation treatment, and elucidate some of the reasons as to how it may help individuals quit smoking.     The effects of acute exercise on tobacco cravings and withdrawal symptoms in temporary abstinent pregnant smokers

    Introduction: Smoking during pregnancy is common, and quitting at any point during pregnancy can yield benefits to both the fetus and mother. Smoking cessation is typically followed by withdrawal symptoms and a strong desire to smoke, both of which are likely to contribute to relapse. Research has shown that a bout of exercise minimizes cravings and tobacco withdrawal symptoms (TWS) after temporary abstinence in smokers, but these findings have not been replicated in pregnant smokers. This study examined the effect of 20. min of exercise on cravings (primary outcome) and TWS (secondary outcomes) among temporary abstinent, inactive pregnant smokers. Methods: Thirty female smokers (Mean(M) age = 25.7. years, Standard Deviation(SD) = 5.5; M weeks pregnant = 18.2, SD = 5.3; Fagerstrom Test for Cigarette Dependence = 3.3, SD = 2.2; M 9.3 cigarettes/day, SD = 4.7; M hours abstained = 17.2, SD = 2.8) were randomized to 20. min of mild-to-moderate intensity exercise (EC; n= 14) or passive (PC; n= 16) condition. Cravings and TWS were assessed immediately before, during (at 10. min), immediately post, and at 10, 20, and 30. min post-condition. Results: A 2 (condition)×6 (time) repeated measures ANOVA revealed that the EC significantly (p\u3c0.05) reduced cravings (ή2=0.46) compared with the PC, across time. Non-significant, but nevertheless, large effects were evident favouring the EC over time for TWS restlessness (ή2=0.34), stress (ή2=0.24), irritability (ή2=0.21), tension (ή2=0.15), and depression (ή2=0.14). Conclusions: Consistent with previous research, this study reveals that in pregnant smokers, a bout of exercise is associated with a reduction in cravings and similar patterns exist for TWS. Therefore, exercise may have the potential to assist in the initial stages of smoking cessation attempts during pregnancy. © 2013

    The Importance of Brain Banks for Molecular Neuropathological Research: The New South Wales Tissue Resource Centre Experience

    New developments in molecular neuropathology have evoked increased demands for postmortem human brain tissue. The New South Wales Tissue Resource Centre (TRC) at The University of Sydney has grown from a small tissue collection into one of the leading international brain banking facilities, which operates with best practice and quality control protocols. The focus of this tissue collection is on schizophrenia and allied disorders, alcohol use disorders and controls. This review highlights changes in TRC operational procedures dictated by modern neuroscience, and provides examples of applications of modern molecular techniques to study the neuropathogenesis of many different brain disorders

Here we investigated whether transcription factors of Nuclear Factor-kappaB (NF-κB) family, controlling neuronal plasticity and neurodegeneration, are involved in these adaptations in human chronic alcoholics. Methods and Findings -- Analysis of DNA-binding of NF-κB (p65/p50 heterodimer) and the p50 homodimer as well as NF-κB proteins and mRNAs was performed in postmortem human brain samples from 15 chronic alcoholics and 15 control subjects. The prefrontal cortex involved in alcohol dependence and cognition was analyzed and the motor cortex was studied for comparison. The p50 homodimer was identified as dominant κB binding factor in analyzed tissues. NF-κB and p50 homodimer DNA-binding was downregulated, levels of p65 (RELA) mRNA were attenuated, and the stoichiometry of p65/p50 proteins and respective mRNAs was altered in the prefrontal cortex of alcoholics. Comparison of a number of p50 homodimer/NF-κB target DNA sites, κB elements in 479 genes, down- or upregulated in alcoholics demonstrated that genes with κB elements were generally upregulated in alcoholics. No significant differences between alcoholics and controls were observed in the motor cortex. Conclusions -- We suggest that cycles of alcohol intoxication/withdrawal, which may initially activate NF-κB, when repeated over years downregulate RELA expression and NF-κB and p50 homodimer DNA-binding. Downregulation of the dominant p50 homodimer, a potent inhibitor of gene transcription apparently resulted in derepression of κB regulated genes.     As the knowledge base and importance of mitochondrial physiology to evolution, health and disease expands, the necessity for harmonizing the terminology concerning mitochondrial respiratory states and rates has become increasingly apparent. The chemiosmotic theory establishes the mechanism of energy transformation and coupling in oxidative phosphorylation. The unifying concept of the protonmotive force provides the framework for developing a consistent theoretical foundation of mitochondrial physiology and bioenergetics. We follow the latest SI guidelines and those of the International Union of Pure and Applied Chemistry (IUPAC) on terminology in physical chemistry, extended by considerations of open systems and thermodynamics of irreversible processes. The concept-driven constructive terminology incorporates the meaning of each quantity and aligns concepts and symbols with the nomenclature of classical bioenergetics. We endeavour to provide a balanced view of mitochondrial respiratory control and a critical discussion on reporting data of mitochondrial respiration in terms of metabolic flows and fluxes. Uniform standards for evaluation of respiratory states and rates will ultimately contribute to reproducibility between laboratories and thus support the development of data repositories of mitochondrial respiratory function in species, tissues, and cells. Clarity of concept and consistency of nomenclature facilitate effective transdisciplinary communication, education, and ultimately further discovery

    Effectiveness of a national quality improvement programme to improve survival after emergency abdominal surgery (EPOCH): a stepped-wedge cluster-randomised trial

    Background: Emergency abdominal surgery is associated with poor patient outcomes. We studied the effectiveness of a national quality improvement (QI) programme to implement a care pathway to improve survival for these patients. Methods: We did a stepped-wedge cluster-randomised trial of patients aged 40 years or older undergoing emergency open major abdominal surgery. Eligible UK National Health Service (NHS) hospitals (those that had an emergency general surgical service, a substantial volume of emergency abdominal surgery cases, and contributed data to the National Emergency Laparotomy Audit) were organised into 15 geographical clusters and commenced the QI programme in a random order, based on a computer-generated random sequence, over an 85-week period with one geographical cluster commencing the intervention every 5 weeks from the second to the 16th time period. Patients were masked to the study group, but it was not possible to mask hospital staff or investigators. The primary outcome measure was mortality within 90 days of surgery. Analyses were done on an intention-to-treat basis. This study is registered with the ISRCTN registry, number ISRCTN80682973. Findings: Treatment took place between March 3, 2014, and Oct 19, 2015. 22 754 patients were assessed for elegibility. Of 15 873 eligible patients from 93 NHS hospitals, primary outcome data were analysed for 8482 patients in the usual care group and 7374 in the QI group. Eight patients in the usual care group and nine patients in the QI group were not included in the analysis because of missing primary outcome data. The primary outcome of 90-day mortality occurred in 1210 (16%) patients in the QI group compared with 1393 (16%) patients in the usual care group (HR 1·11, 0·96–1·28). Interpretation: No survival benefit was observed from this QI programme to implement a care pathway for patients undergoing emergency abdominal surgery. Future QI programmes should ensure that teams have both the time and resources needed to improve patient care.     Primary Care Survival Skills in an Age of Reform

    The Triple Aim - improved outcomes/quality of care, improved patient experience, and reduced cost - is driving health care reform in Minnesota and nationally. Minnesota is among those states moving quickly to new payment models that reward practices for meeting these three goals and is gradually reducing payments based only on increased volume of services. Primary care is growing into a new leadership role to meet these new demands while managing the reality that full funding for population based care is developing more slowly than the rhetoric. There are many encouraging signs, including an expanding interest in primary care medical careers, increased examples of highly functional interdisciplinary teams and increased attention to the social determinants of health that interfere with improved patient outcomes. This plenary presentation will review key, basic steps regarding how we engage our providers and staff to move forward on this agenda. We must reframe the future and engage our teams in new ways to create new standard work flows to help patients avoid the emergency room and hospital. Who do we need as teammates to create these more effective teams? How can we more fully integrate behavioral health and medical services? How can we fund these teams?
