    Health Measurement Scales: Methodological Issues

    Health scales or indices are composite tools aiming to measure a variety of clinical conditions, behaviors, attitudes and beliefs that are difficult to be measured quantitatively. During the past years, these tools have been extensively used in cardiovascular disease prevention. The already proposed scales have shown good ability in assessing individual characteristics, but had moderate predictive ability in relation to the development of chronic diseases and various other health outcomes. In this review, methodological issues for the development of health scales are discussed. Specifically, the selection of the appropriate number of components, the selection of classes for each component, the use of weights of scale components and the role of intra- or inter-correlation between components are discussed. Based on the current literature the use of components with large number of classes, as well as the use of specific weights for each scale component and the low-to-moderate inter-correlation rate between the components, is suggested in order to increase the diagnostic accuracy of the tool

    Computing the common zeros of two bivariate functions via Bézout resultants

    The common zeros of two bivariate functions can be computed by finding the common zeros of their polynomial interpolants expressed in a tensor Chebyshev basis. From here we develop a bivariate rootfinding algorithm based on the hidden variable resultant method and Bézout matrices with polynomial entries. Using techniques including domain subdivision, Bézoutian regularization, and local refinement we are able to reliably and accurately compute the simple common zeros of two smooth functions with polynomial interpolants of very high degree (≥ 1000). We analyze the resultant method and its conditioning by noting that the Bézout matrices are matrix polynomials. Two implementations are available: one on the Matlab Central File Exchange and another in the roots command in Chebfun2 that is adapted to suit Chebfun’s methodology

    Menu labelling and food choice in obese adults: a feasibility study.

    BACKGROUND: To date research examining the benefits of menu labelling in the UK is sparse. The aim of the present study was to examine the impact of menu labelling in a UK obese population. METHODS: Using a repeated measures design, 61 patients at a tier 3 weight management service completed four questionnaires to assess their food choice (control) and behaviour change when presented with 3 menu labelling formats (calorie content; nutrient content; and energy expenditure). RESULTS: All three forms of labelling increased participants weight control concerns compared to the control condition. There was a significant difference in content of food ordered in the three menu labelling formats compared to the control condition. The calorie condition had the largest percentage decrease in calories selected followed by energy expenditure and nutrient content. However, no difference was observed between the three conditions in the desire for menu labelling in restaurants to be introduced in the UK. CONCLUSIONS: The findings suggest that menu labelling should be enforced in the UK as it is both beneficial to promoting healthy eating and in demand. This study is the first to examine menu labelling in a UK obese population using energy expenditure equivalents to provide nutritional information

    Sugar-added beverages consumption among kindergarten children of Crete: effects on nutritional status and risk of obesity

    <p>Abstract</p> <p>Objective</p> <p>To assess the intake of sugar-added beverages such as soft drinks and commercially available fruit juices in kindergarten children, and to examine its association with obesity indices, physical activity levels and dietary habits.</p> <p>Methods</p> <p>A total of 856 children aged 4–7 years living in Crete, Greece in 2004–5 were included in this cross-sectional study. Nutrient and food intake was assessed with the use of 3-day weighed food records. Body measurements were used in order to assess BMI and waist circumference, and moderate-to-vigorous physical activity was calculated with the use of a questionnaire.</p> <p>Results</p> <p>Approximately 59.8% of all children consumed sugar-added beverages on a daily basis. High intake of sugar-added beverages (> 250 g/day) was associated with low intakes of calcium (p < 0.001), vitamin A and E (p < 0.010), fruits and vegetables (p = 0.007), and milk and yogurt (p = 0.048). Compared to non or low consumers, high consumers of sugar-added beverages (> 250 g/day) had higher BMI levels and two times greater risk of being overweight and/or obese (OR:2.35, p = 0.023).</p> <p>Conclusion</p> <p>High intake of sugar-added beverages in kindergarten children is associated with poor eating habits and inadequate nutrient intake, as well as increased risk for developing childhood obesity.</p

    Underestimation of overweight weight status in children and adolescents aged 0-19 years: A systematic review and meta-analysis

    Background: Perceptions of children's weight status may be important in obesity prevention and treatment. Aims: This review identifies the prevalence of the underestimation of overweight status in children by parents/main carers, children, and healthcare professionals (HCP). The review critically synthesized both quantitative and qualitative evidence to explore the factors associated with this underestimation. The diverse methods used to assess this phenomenon are reported. Methods: Pooled effect sizes were calculated using random-effects model. Published studies, up to 2020, were accessed using the following search engines: CINAHL, EMBASE, PUBMED, and Psych-Info and including the “Cited by” and “Related Articles” functions. Hand-searching was used to retrieve further articles. Publication language and location had no bearing on the nature of the included studies. Results: A total of 91 articles were included. In the quantitative studies, 55% (95% CI 49%–61%) of caregivers underestimated their child's level of overweight and obesity using a verbal scale and 47% (95% CI 36%–55%) using visual scales. Of the children studied, 34% (95% CI 25%–43%) underestimated their own level of overweight and obesity using both scales. In (n = 3) articles, HCPs reflected this misperception, but limited studies prevented meta-analysis. Underestimation was associated with the child's age, gender, BMI and parental weight status, ethnicity and education. In the qualitative studies, parents/main carers of children with overweight and obesity described their child's weight in terms other than overweight, for example, “big boned,” “thick,” and “solid.”. Conclusion: The results confirm the prevalence of underestimation of child overweight status across international studies. Understanding the factors which lead to this inaccuracy may help to improve communication within the therapeutic triad and facilitate the recognition and management of children's overweight status

    Ordered Mesostructured CdS Nanowire Arrays with Rectifying Properties

    Highly ordered mesoporous CdS nanowire arrays were synthesized by using mesoporous silica as hard template and cadmium xanthate (CdR2) as a single precursor. Upon etching silica, mesoporous CdS nanowire arrays were produced with a yield as high as 93 wt%. The nanowire arrays were characterized by XRD, N2adsorption, TEM, and SEM. The results show that the CdS products replicated from the mesoporous silica SBA-15 hard template possess highly ordered hexagonal mesostructure and fiber-like morphology, analogous to the mother template. The current–voltage characteristics of CdS nanoarrays are strongly nonlinear and asymmetrical, showing rectifying diode-like behavior

    The rise of noncommunicable diseases in Latin America and the Caribbean: challenges for public health policies

    The health landscape in Latin America and the Caribbean is changing quickly. The region is undergoing a demographic and epidemiological transition in which health problems are highly concentrated on noncommunicable diseases (NCDs). In light of this, the region faces two main challenges: (1) develop cost-effective policies to prevent NCD risk factors, and (2) increase access to quality healthcare in a scenario in which a large share of the labor force is employed in the informal sector. This paper describes both alternative interventions to expand health insurance coverage and their trade-off with labor informality and moral hazard problems. The paper also focuses on obesity as a case example of an NCD, and emphasizes how lack of knowledge along with self-control problems would lead people to make suboptimal decisions related to food consumption, which may later manifest in obesity problems.Fil: Anauati, Maria Victoria.     5-a-day fruit and vegetable food product labels: reduced fruit and vegetable consumption following an exaggerated compared to a modest label.

    BACKGROUND: Food product labels based on the WHO 5-a-day fruit and vegetable (FV) message are becoming increasingly common, but these labels may impact negatively on complementary or subsequent FV consumption. This study aimed to investigate the impact of a '3 of your 5-a-day' versus a '1 of your 5-a-day' smoothie product label on subsequent FV consumption. METHODS: Using an acute experimental design, 194 participants (90 males, 104 females) were randomised to consume a smoothie labelled as either '3 of your 5-a-day' (N = 97) or '1 of your 5-a-day' (N = 97) in full, following a usual breakfast. Subsequent FV consumption was measured for the rest of the day using 24-h recall. Usual FV consumption was also assessed via 24-h recall for the day before the study. RESULTS: Regression analyses revealed a significantly lower subsequent FV consumption following smoothies displaying the '3 of your 5-a-day' label compared to the '1 of your 5-a-day' label (Beta = - 0.15, p = 0.04). Secondary analyses revealed these effects to be driven mainly by changes to consumption in usual high FV consumers, in females and in vegetable as opposed to fruit consumption. CONCLUSIONS: These findings demonstrate a role for label information in food intake, and the potential negative impacts of an exaggerated food product label on healthy food consumption and healthy dietary profiles

    Trans Fat Consumption and Aggression

    Background: Dietary trans fatty acids (dTFA) are primarily synthetic compounds that have been introduced only recently; little is known about their behavioral effects. dTFA inhibit production of omega-3 fatty acids, which experimentally have been shown to reduce aggression. Potential behavioral effects of dTFA merit investigation. We sought to determine whether dTFA are associated with aggression/irritability. Methodolgy/Prinicpal Findings: We capitalized on baseline dietary and behavioral assessments in an existing clinical trial to analyze the relationship of dTFA to aggression. Of 1,018 broadly sampled baseline subjects, the 945 adult men and women who brought a completed dietary survey to their baseline visit are the target of this analysis. Subjects (seen 1999– 2004) were not on lipid medications, and were without LDL-cholesterol extremes, diabetes, HIV, cancer or heart disease. Outcomes assessed adverse behaviors with impact on others: Overt Aggression Scale Modified-aggression subscale (primary behavioral endpoint); Life History of Aggression; Conflict Tactics Scale; and self-rated impatience and irritability. The association of dTFA to aggression was analyzed via regression and ordinal logit, unadjusted and adjusted for potential confounders (sex, age, education, alcohol, and smoking). Additional analyses stratified on sex, age, and ethnicity, and examined the prospective association.
