    Medicolegal Study of Alleged Rape Victim Cases in Mumbai Region

    Background: The sexual assault on women and female children is one of the most heinous crimes against mankind. Sexual violence is ubiquitous; it occurs in every culture, in all levels of society and in every country of the world. Sexual violence has a significant negative impact on the health of the population. The root causes of sexual and gender-based violence lie in a society’s attitudes towards and practices of gender discrimination, which place women in a subordinate position in relation to men.Methods: The prospective study was conducted in Mumbai region from 1st January 2011 to 31st December 2012 in which 152 cases of alleged rape victims which was booked under the section 376 of Indian Penal Code which was brought for medical examination. Strict confidentiality about their names was maintained.Results: The most common age group comprised ages between 16-20 years in which total 63 (41.44%) cases were observed. Most of the victims i.e. 99 (65.13%) were unmarried. Most of the incidences took place at accused house, i.e. 69 (45.39%). In 142 (93.42%) cases the assailant was known to the victim. In 113 (74.34%) cases the time interval between the alleged incidence and examination was more than seven days. In 139 (91.44%) cases the hymen showed old tears at multiple positions. None of the report out of 66 reports from forensic science laboratory was found to be positive for spermatozoa.Conclusion: Rape cases show a constantly rising pattern with a low conviction rate. There is no age safe from rape, maximum cases being in the age group 16-20 years as there are multiple physical, social and mental factors playing a role. Unmarried young women are at increased risk. Significant number of cases report late to the police/doctor. The delay results into loss of valuable biological trace material evidences

    A Low Power Asynchronous Viterbi Decoder using LEDR Encoding

    With the consumer demand for increased content and as a result, increasing high data bandwidth continuing to drive communications systems, coding for error control has become extraordinarily important. One way to improve the Bit Error Rate (BER), while maintaining high data reliability, is to use an error correction technique like the Viterbi algorithm. Originally conceived by Andrew Viterbi as an error-correction scheme for noisy digital communication, the Viterbi algorithm provides an efficient method for Forward Error Correction (FEC) that improves channel reliability. Today, it is used in many digital communications systems in applications as diverse as LTE Physical Downlink Control Channel (PDCCH), CDMA and GSM, digital cellular, dial up modems, satellite, deep-space communications, and 802.11 wireless LANs. Though it is useful for error correction it dissipates large power. A lot many researches were carried out at architectural as well as algorithmic level to optimize the ACS (Add compare and Select) unit and Survival Memory Management in Synchronous Viterbi Decoders. But still there is a problem of power dissipation which requires more technical solutions. Due to requirements of high speed, low power, low weight and long battery life a low power Viterbi decoders has a great demand in the communication field. This paper proposed the method for survivor path storage and decoding as Minimum Transition Hybrid Register Exchange Method along with handshaking protocol as Level Encoded dual rail (LEDR) encoding to make the system asynchronous.     A prospective open-label randomized comparative study in Alzheimer’s disease between two commonly used drugs in coastal Indian population

    Background: Currently, therapy for Alzheimer’s disease (AD) is only symptomatic. Only two classes of drugs are approved by the United States Food and Drug Administration. Our study aimed at comparing efficacy and safety of memantine and donepezil in moderate to severe AD patients.Methods: Totally, 22 patients with moderate to severe AD were randomized into the 2 arms of the study. The study was divided into an initial 4 weeks for determination of onset of efficacy and subsequent 28 weeks of the treatment phase. Onset of efficacy and response was defined as >20% and >50% reduction in the mean total score of functional dementia scale (FDS) and clinical global impression scale (CGIS) from baseline to the study end, respectively.Results: Onset of efficacy on FDS and CGIS was 16.7% (mean-time 61.25 days) and 80% (mean-time 36 days) with memantine and donepezil, respectively. Response was 89.3% and 40% with memantine and Donepezil, respectively. Total reduction in FDS and CGIS score of from baseline to the study end was 39.50, 40.00, and 25.60, 27.20 with memantine and donepezil, respectively. Tolerability was 86.33% and 20% with memantine and donepezil, respectively. Anorexia, muscle cramps, constipation, headache, and insomnia, were the common side-effects and self-limiting. Safety was 100% in both groups.Conclusions: Onset of efficacy was faster with donepezil seen at 2 weeks. Response, improvement in CGIS, FDS, and tolerability were better seen with memantine at 40 weeks. Thus, in similar clinical settings, memantine can be preferred

    Clinico-hematological evaluation in cases of pancytopenia at tertiary care centre: largest cross-sectional study

    Background:  Pancytopenia is characterised by a reduction in all the three cellular elements of blood (erythrocytes, leukocytes and platelets) below the normal reference range leading to anaemia, leucopenia and thrombocytopenia. It is a feature of many serious conditions. The present study was conducted to assess aetiology, clinical profile and bone marrow morphology of conditions presenting with pancytopenia.Methods: A two years cross-sectional study from July 2017 to June 2019 was conducted in the Department of Pathology. Total of 300 pancytopenia patient were studied and their clinical features, peripheral smear finding and bone marrow morphology were studied by using marrow aspiration and biopsy.Results: Among 300 cases studied, maximum patients were in the age group 11-20 years (19.66%) with male (50.66%) predominance. Most of the patients presented with weakness (91.66%) and fever (56.66%) as chief complains. The commonest physical finding was pallor (94%) followed by splenomegaly (27.33%). Macrocytic anaemia (43.66%) was commonest peripheral finding. The commonest cause of pancytopenia was megaloblastic anaemia (32.66%) followed by dimorphic anaemia (21%), aplastic anaemia (16%) and acute leukaemia (13.33%).Conclusions: The present study concludes that detailed clinical history, primary haematological investigations along with bone marrow examinations is essential to determine the cause of pancytopenia

    Recent years have witnessed tremendous growth of nanotechnology based drug delivery system which reduces drug toxicity and side effects and increases the therapeutic index of the drug. Aim of the study is to develop a biodegradable, non-toxic nanoparticle, solely from natural polymers. Zein – pectin nanoparticle comprising of a hydrophobic zein core and a hydrophilic pectin shell was developed by ultrasonication method. SEM images confirm the nanosize of the nanoparticle. UV- Visible and FT-IR spectroscopic results confirm the incorporation of zein, pectin and the encapsulation of the model drug quercetin in the nanoparticle.     In medicine, it is well known that healthy individuals have different physical and mental characteristics. Ancient Indian medicine, Ayurveda and the Persian-Arabic traditional Unani medicine has two distinct approaches for the classification of human subjects according to their temperaments. The individual temperament is an important foundation for personalized medicine, which can help in the prevention and treatment of many diseases including COVID-19. This paper attempts to explore the relationship of the utmost important concepts of these systems called individual temperament named as Prakruti in Ayurveda and Mizaj in Unani practice using mathematical modelling. The results of mathematical modelling can be adopted expediently for the development of algorithms that can be applied in medical informatics. For this, a significant literature review has been carried out. Based on the previous researchers' reviews the essential parameters have been identified for making the relationship and hypothesis were framed. The mathematical modelling was adopted to propose the existence of the relationship between the parameters of such an ancient and rich medicine systems.
