    Expansion of methods employed in the kinematic analysis of human movement for diagnosing of the physical and mental health of subjects can be traced back to the 1990`s when new information technologies and electronic recording systems started their development boom. Evaluation methods of body movement for the diagnostics of physical and mental health expanded significantly in clinical practice. This study presents an overview of these methods with the focus on how applicable the analysis of human movement can be in military practice, where they are currently marginally used. The aim of this study is to offer some recommendations on how particular methods could be utilized in an army context. This article also suggests the most appropriate methods of quantitative evaluation for posture and motion control in the course of standing, gait and other activities carried out in military training and active duty

    Reclaimed Carbon and Flax Fibre Composites:Manufacturing and Mechanical Properties

    The feasibility of using the HiPerDiF (high performance discontinuous fibre) method to manufacture highly aligned discontinuous fibres intermingled hybrid composites with flax and reclaimed carbon fibres (rCF), and the potential benefits of so doing, are investigated in this paper. It is demonstrated that, despite their hydrophilic nature, flax fibres are not affected by this water-based process. Intermingled flax/rCF hybrid composites are characterised in terms of their tensile and vibrational response. It is concluded that natural/rCF fibre hybrid composites can be a viable solution for those applications where a reduction in primary mechanical properties, e.g., stiffness and strength, is an acceptable trade-off for the enhancement of secondary properties, e.g., noise, vibration, and harshness (NVH) mitigation, and the reduction of monetary costs

    Multivariable Prediction Model for Biochemical Response to First-Generation Somatostatin Receptor Ligands in Acromegaly

    CONTEXT: First-generation somatostatin receptor ligands (fg-SRLs) represent the mainstay of medical therapy for acromegaly, but they provide biochemical control of disease in only a subset of patients. Various pretreatment biomarkers might affect biochemical response to fg-SRLs. OBJECTIVE: To identify clinical predictors of the biochemical response to fg-SRLs monotherapy defined as biochemical response (insulin-like growth factor (IGF)-1 ≤ 1.3 × ULN (upper limit of normal)), partial response (>20% relative IGF-1 reduction without normalization), and nonresponse (≤20% relative IGF-1 reduction), and IGF-1 reduction. DESIGN: Retrospective multicenter study. SETTING: Eight participating European centers. METHODS: We performed a meta-analysis of participant data from 2 cohorts (Rotterdam and Liège acromegaly survey, 622 out of 3520 patients). Multivariable regression models were used to identify predictors of biochemical response to fg-SRL monotherapy. RESULTS: Lower IGF-1 concentration at baseline (odds ratio (OR) = 0.82, 95% confidence interval (CI) 0.72-0.95 IGF-1 ULN, P = .0073) and lower bodyweight (OR = 0.99, 95% CI 0.98-0.99 kg, P = .038) were associated with biochemical response. Higher IGF-1 concentration at baseline (OR = 1.40, (1.19-1.65) IGF-1 ULN, P ≤ .0001), the presence of type 2 diabetes (oral medication OR = 2.48, (1.43-4.29), P = .0013; insulin therapy OR = 2.65, (1.02-6.70), P = .045), and higher bodyweight (OR = 1.02, (1.01-1.04) kg, P = .0023) were associated with achieving partial response.     Acromegaly is a rare disorder caused by chronic growth hormone (GH) hypersecretion. While diagnostic and therapeutic methods have advanced, little information exists on trends in acromegaly characteristics over time. The Liège Acromegaly Survey (LAS) Database, a relational database, is designed to assess the profile of acromegaly patients at diagnosis and during long-term follow-up at multiple treatment centers. The following results were obtained at diagnosis. The study population consisted of 3173 acromegaly patients from ten countries; 54.5% were female. Males were significantly younger at diagnosis than females (43.5 vs 46.4 years; P 3100 patients is the largest international acromegaly database and shows clinically relevant trends in the characteristics of acromegaly at diagnosis

    Significant benefits of AIP testing and clinical screening in familial isolated and young-onset pituitary tumors

    Context Germline mutations in the aryl hydrocarbon receptor-interacting protein (AIP) gene are responsible for a subset of familial isolated pituitary adenoma (FIPA) cases and sporadic pituitary neuroendocrine tumors (PitNETs). Objective To compare prospectively diagnosed AIP mutation-positive (AIPmut) PitNET patients with clinically presenting patients and to compare the clinical characteristics of AIPmut and AIPneg PitNET patients. Design 12-year prospective, observational study. Participants & Setting We studied probands and family members of FIPA kindreds and sporadic patients with disease onset ≤18 years or macroadenomas with onset ≤30 years (n = 1477). This was a collaborative study conducted at referral centers for pituitary diseases. Interventions & Outcome AIP testing and clinical screening for pituitary disease. Comparison of characteristics of prospectively diagnosed (n = 22) vs clinically presenting AIPmut PitNET patients (n = 145), and AIPmut (n = 167) vs AIPneg PitNET patients (n = 1310). Results Prospectively diagnosed AIPmut PitNET patients had smaller lesions with less suprasellar extension or cavernous sinus invasion and required fewer treatments with fewer operations and no radiotherapy compared with clinically presenting cases; there were fewer cases with active disease and hypopituitarism at last follow-up. When comparing AIPmut and AIPneg cases, AIPmut patients were more often males, younger, more often had GH excess, pituitary apoplexy, suprasellar extension, and more patients required multimodal therapy, including radiotherapy. AIPmut patients (n = 136) with GH excess were taller than AIPneg counterparts (n = 650). Conclusions Prospectively diagnosed AIPmut patients show better outcomes than clinically presenting cases, demonstrating the benefits of genetic and clinical screening.     An increased cardiovascular risk and mortality in hypopituitary patients receiving conventional hormonal treatment without GH replacement have been shown in several studies. Various atherogenic risk factors including endothelial dysfunction - an early event in the atherogenesis - are more expressed in adult GH deficient patients. Changes in microcirculation and vascular reactivity could represent an early maker of developing vascular changes.     An insight into long-term effects of biochar application on forest soils

    Protection and appropriate management of forests is one of the key instruments for climate change adaptation. Soil amendments with biochar have shown to be promising in achieving this goal; however, the evaluation of its long-term effects on forest soils has largely been neglected. To assess the advantages and drawbacks of biochar in forest soils, data from relict charcoal hearths (RCH) can be a potent tool as they show changes in soil properties after up to several hundred years. RCHs can be found in places of former metallurgical hot spots and their presence leaves characteristic formations identifiable on a large scale using laser detection technologies. Forest soils with biochar amendment show an increase in base cations, shift towards more alkaline pH, smaller bulk density and seem to be especially beneficial to hostile environments. Sites with favourable conditions may show little to no improvement or may even be adversely affected. Still, with proper investigation, areas with affordable feedstock materials and poor forest soils—such as spruce monocultures of Central Europe—may benefit from biochar amendments and continue to do so in the long term. © 2022, The Author(s), under exclusive licence to Springer-Verlag GmbH Germany, part of Springer Nature.Technology Agency of the Czech Republic, TACR: TL0200016

    Background: Introducing anthracycline antibiotics into the treatment of children's malignancies contributed enormously to the excellent outcome in children and adolescents. Nevertheless, the cardiotoxicity of such was discovered as early as in the 1970's. Only recently stress tests have been used to detect late cardiotoxic effects of anti-neoplastic drugs.Aim: To determine the function of the left ventricle (LV) in asymptomatic patients who were treated with chemotherapy containing anthracyclines in childhood or adolescence.Methods: We examined 137 patients (81 men, 56 women), aged 16.6±4.8 years, in whom a malignant disease was diagnosed at the age of 9.1±4.8 years and who were treated with a cumulative dose of anthracyclines of 242±110 mg/m2. The control group comprised 30 subjects (14 men, 16 women, mean age 19.5±5.2 years).
