    A Service-Oriented Approach for Sensing in the Internet of Things: Intelligent Transportation Systems and Privacy Use Cases

    This paper presents a Sensing-as-a-Service run-time Service Oriented Architecture (SOA), called 3SOA, for the development of Internet of Things (IoT) applications. 3SOA aims to allow interoperability among various IoT platforms and support service-oriented modelling at high levels of abstraction where fundamental SOA theories and techniques are fully integrated into a practical software engineering approach. 3SOA abstracts the dependencies of the middleware programming model from the application logic. This abstraction allows the development efforts to focus on writing the application logic independently from hardware platforms, middleware, and languages in which applications are programmed. To achieve this result, IoT objects are treated as independent entities that may interact with each other using a well-defined message exchange sequence. Each object is defined by the services it provides and the coordination protocol it supports. Objects are then able to coordinate their resources to address the global objectives of the system. To practically validate our proposals, we demonstrate an intelligent transportation system and data privacy functional prototypes as proof of concepts. The use cases show that 3SOA and the presented abstraction language allow the amalgamation of macroprogramming and node-centric programming to develop real-time and efficient applications over IoT

    Culturally informed resilience in conflict settings: A literature review of Sumud in the occupied Palestinian territories

    Investigating culturally specific views and experiences of trauma and resilience can offer new insights that can aid distress management, meaning making, coping and resilience in adverse conditions, and inform emergency and disaster responses. Sumud is a Palestinian cultural construct and component of resilience in the occupied Palestinian territories (oPt). Sumud in Arabic refers to steadfastness or perseverance. This literature review focuses on research studies on Sumud in the oPt, with particular attention on the meaning and manifestations of Sumud, the role of non-violent resistance, and how Sumud and non-violent resistance informs resilience and coping in the context of a military occupation, protracted political conflict, and chronic adversity. The peer-reviewed literature was surveyed using the PubMed and PsycINFO databases. The findings indicate how Sumud is a central component of resilience and provides a meta-cognitive framework which Palestinians use to interpret, cope and respond to ongoing injustice and traumatic experiences, engendering a sense of purpose and meaning. It is both a value and an action that manifests via individual and collective action to protect family and community survival, wellbeing, dignity, Palestinian identity and culture, and a determination to remain on the land. The implications of this study and the relevance of the findings to mental health and disaster relief are considered

    Strategies and interventions for healthy adolescent growth, nutrition, and development

    Adolescence is a pivotal point in the life course, characterised by transformative physical, cognitive, and emotional growth, an openness to change, and a drive to reshape the social environment. It offers unique opportunities to adopt changes in diet and physical activity that can persist into later life. Yet pre-existing nutritional problems, including micronutrient deficiencies, food insecurity, and poor-quality diets, persist at the same time as adolescents face the rapid emergence of an obesity epidemic. Adolescent growth and nutrition has been largely overlooked in intervention and policy research. Most intervention studies have emphasised micronutrient supplementation, with few taking into account the multiple drivers of adolescent diets. This Series paper highlights that effective interventions and policies will need to cut across sectors; be supported by multifaceted and multilevel policy; and extend across education, health, food systems, social protection, and digital media. Better data standardisation and systems will be essential in coordinating and monitoring these responses. In a context of shifts in planetary ecosystems and commercial drivers, resilient food systems will need to both ensure access to healthy and affordable foods and provide the infrastructure and incentives for continuing physical activity. Intergenerational partnerships with young people will be essential in bringing about transformative change and ensuring that food policies reflect their needs and aspirations

    Microbiological culture versus 16S/18S rRNA gene PCR-sanger sequencing for infectious keratitis:a three-arm, diagnostic cross-sectional study

    Background: To compare the diagnostic performance of microbiological culture and 16S/18S rRNA gene polymerase chain reaction (PCR)-Sanger sequencing for infectious keratitis (IK) and to analyse the effect of clinical disease severity on test performance and inter-test concordance. Methods: This was a three-arm, diagnostic cross-sectional study. We included all eligible patients who presented with presumed bacterial/fungal keratitis to the Queen's Medical Centre, Nottingham, UK, between June 2021 and September 2022. All patients underwent simultaneous culture (either direct or indirect culture, or both) and 16S (pan-bacterial)/18S (pan-fungal) ribosomal RNA (rRNA) PCR-Sanger sequencing. The bacterial/fungal genus and species identified on culture were confirmed using matrix-assisted laser desorption/ionization-time-of-flight mass spectrometry. Relevant clinical data were also collected to analyze for any potential clinico-microbiological correlation. Main outcome measures included the diagnostic yield, test accuracy (including sensitivity and specificity), and inter-test agreement [including percent agreement and Cohen's kappa (k)]. Results: A total of 81 patients (86 episodes of IK) were included in this study. All organisms identified were of bacterial origin. Diagnostic yields were similar among direct culture (52.3%), indirect culture (50.8%), and PCR (43.1%; p = 0.13). The addition of PCR enabled a positive diagnostic yield in 3 (9.7%) direct culture-negative cases. Based on composite reference standard, direct culture had the highest sensitivity (87.5%; 95% CI, 72.4–95.3%), followed by indirect culture (85.4%; 95% CI, 71.6–93.5%) and PCR (73.5%; 95% CI, 59.0–84.6%), with 100% specificity noted in all tests. Pairwise comparisons showed substantial agreement among the three tests (percent agreement = 81.8–86.2%, Cohen's k = 0.67–0.72). Clinico-microbiological correlation demonstrated higher culture-PCR concordance in cases with greater infection severity. Conclusions: This study highlights a similar diagnostic performance of direct culture, indirect culture and 16S rRNA PCR for bacterial keratitis, with substantial inter-test concordance. PCR serves as a useful diagnostic adjuvant to culture, particularly in culture-negative cases or those with lesser disease severity (where culture-PCR concordance is lower)

Modern microelectronics and communication technologies offer the type of smart living that looked practically inconceivable just a few decades ago. The Internet of Things (IoT) is one of the main drivers of the future smart spaces. It enables new operational technologies and offers vital financial and environmental benefits. With IoT, spaces are evolving from being just 'smart' to become intelligent and connected. This survey paper focuses on how to leverage IoT technologies to build a modular approach to smart campuses. The paper identifies the key benefits and motivation behind the development of IoT-enabled campus. Then, it provides a comprehensive view of general types of smart campus applications. Finally, we consider the vital design challenges that should be met to realise a smart campus

    2010 update of the ASAS/EULAR recommendations for the management of ankylosing spondylitis

    This first update of the ASAS/EULAR recommendations on the management of ankylosing spondylitis (AS) is based on the original paper, a systematic review of existing recommendations and the literature since 2005 and the discussion and agreement among 21 international experts, 2 patients and 2 physiotherapists in a meeting in February 2010. Each original bullet point was discussed in detail and reworded if necessary. Decisions on new recommendations were made — if necessary after voting. The strength of the recommendations (SOR) was scored on an 11-point numerical rating scale after the meeting by email. These recommendations apply to patients of all ages that fulfill the modified NY criteria for AS, independent of extra-articular manifestations, and they take into account all drug and non-drug interventions related to AS. Four overarching principles were introduced, implying that one bullet has been moved to this section. There are now 11 bullet points including 2 new ones, one related to extra-articular manifestations and one to changes in the disease course. With a mean score of 9.1 (range 8-10) the SOR was generally very good

    An Obscure Case of Hepatic Subcapsular Hematoma

    Spontaneous liver bleeding is often reported in preeclampsia. It is otherwise rare and has been linked to gross anatomical lesions and coagulopathy. We report a case of subcapsular hematoma of the liver without any apparent lesion and in the absence of coagulopathy. A 41-year-old male, paraplegic for 16 years, presented to the emergency department 3 days after sudden onset of right upper quadrant and shoulder pain. He had been on vitamins and 5,000 units subcutaneous heparin 12-hourly at the nursing home for the last month. He was in no distress, afebrile, with stable vitals. Physical examination showed a diverting colostomy, tender hepatomegaly and sacral decubiti. A fecal occult blood test was negative. There was spastic paraplegia below the level of T12. Two days after admission, the patient was afebrile and hemodynamically stable. PTT, PT, liver profile, BUN and creatinine were all normal, however his hemoglobin had dropped from 11.3 to 7.6 g/dl. An abdominal CT scan revealed an isolated 9.0 × 1.8 cm subcapsular hematoma. The patient received blood transfusion in the intensive care unit and was discharged 7 days later. In conclusion, spontaneous liver hemorrhage occurs in the nonobstetrical population in the setting of gross anatomical lesions or coagulopathy.
