Factors shaping the implementation and use of Clinical Cancer Decision Tools by GPs in primary care: A qualitative framework synthesis
\ua9 BMJ Publishing Group Limited 2020.Objective Clinical Cancer Decision Tools (CCDTs) aim to alert general practitioners (GPs) to signs and symptoms of cancer, supporting prompt investigation and onward referral. CCDTs are available in primary care in the UK but are not widely utilised. Qualitative research has highlighted the complexities and mechanisms surrounding their implementation and use; this has focused on specific cancer types, formats, systems or settings. This study aims to synthesise qualitative data of GPs\u27 attitudes to and experience with a range of CCDTs to gain better understanding of the factors shaping their implementation and use. Design A systematic search of the published (MEDLINE, CINAHL, Web of Science and EMBASE) and grey literature (July 2020). Following screening, selection and assessment of suitability, the data were analysed and synthesised using normalisation process theory. Results Six studies (2011 to 2019), exploring the views of GPs were included for analysis. Studies focused on the use of several different types of CCDTs (Risk Assessment Tools (RAT) or electronic version of RAT (eRAT), QCancer and the 7-point checklist). GPs agreed CCDTs were useful to increase awareness of signs and symptoms of undiagnosed cancer. They had concerns about the impact on trust in their own clinical acumen, whether secondary care clinicians would consider referrals generated by CCDT as valid and whether integration of the CCDTs within existing systems was achievable. Conclusions CCDTs might be a helpful adjunct to clinical work in primary care, but without careful development to legitimise their use GPs are likely to give precedence to clinical acumen and gut instinct. Stakeholder consultation with secondary care clinicians and consideration of how the CCDTs fit into a GP consultation are crucial to successful uptake. The role and responsibilities of a GP as a clinician, gatekeeper, health promoter and resource manager affect the interaction with and implementation of innovations such as CCDTs
The Effect of an Informational Training Video on Parental Changes in Scoring of the ASQ-3

Objective This study investigated the effect of an informational training video (ITV) on changing parental scoring of the Ages and Stages Questionnaire, 3rd edition (ASQ-3)
Outcome Measure: ASQ-3 Standardized developmental screening tool used to test children aged 1-66 months via 21 questionnaires. Test items are organized into five developmental domains Selected for extensive psychometric properties3 Utilized the 36-month, 42-month, 48-month, 54-month and 60-month ASQ-3 Questionnaires. Converted questionnaires into Google Forms for easy administration
Participants 13 parents of preschool-aged children recruited from Lasting Impressions Child care Center in St.Paul, MN and through social media posts on Facebook and Instagram Children ranged from age 34 months, 16 days to 66 months
Methods After parents indicated interest in participation, an email allowed them to access the first Google form comprised of consent, demographic information, and the age appropriate ASQ-3 Two weeks after first submission, participants received a second email with instructions to watch the six-minute ITV and fill out the second ASQ-3 The ITV included a brief overview of domains and instruction of item administration and scoring
Results Data was analyzed using Wilcoxon Signed-Rank Test to determine a change in scoring after viewing the ITV Results demonstrated significance across four of five domains with Problem Solving the only nonsignificant domain, possibly due to collection error resulting in smaller N or parental influence
Conclusion Evidence suggests watching a training video on ASQ-3 administration changes parental scoring of their child’s performance on tested items Post-video training change in scores may demonstrate a change in parental knowledge of precision scoring
Clinical Relevance Using the ASQ-3 tool may augment parental contributions to the developmental surveillance and screening process This could impact not only the accuracy of scoring the ASQ – 3, but also on the physician’s willingness to incorporate parent feedback in decisions on referral to early interventio
Evaluating the effects of bilingual traffic signs on driver performance and safety
Variable Message Signs (VMS) can provide immediate and relevant information to road users and bilingual VMS can provide great flexibility in countries where a significant proportion of the population speak an alternative language to the majority. The study reported here evaluates the effect of various bilingual VMS configurations on driver behaviour and safety. The aim of the study was to determine whether or not the visual distraction associated with bilingual VMS signs of different configurations (length, complexity) impacted on driving performance. A driving simulator was used to allow full control over the scenarios, road environment and sign configuration and both longitudinal and lateral driver performance was assessed. Drivers were able to read one and two-line monolingual signs and two-line bilingual signs without disruption to their driving behaviour. However, drivers significantly reduced their speed in order to read four-line monolingual and four-line bilingual signs, accompanied by an increase in headway to the vehicle in front. This implies that drivers are possibly reading the irrelevant text on the bilingual sign and various methods for reducing this effect are discussed
The experience of living with knee osteoarthritis: exploring illness and treatment beliefs through thematic analysis
Purpose: Knee osteoarthritis (OA) is a major cause of disability in older adults. However, there is limited research on the daily experience of living with knee OA. We aimed to offer insight into the beliefs of patients with knee OA about their illness and treatment.
Method: Twenty-four semi-structured interviews were conducted with 17 women and 7 men with physician-diagnosed knee OA, aged between 48 and 84 years (mean 1/4 62, SD 1/4 7). The audio-taped interviews lasted from 30 min to 1 h, and were transcribed verbatim. The data were subjected to thematic analysis. The transcripts were independently coded by two researchers to increase reliability of coding.
Results: Six themes were developed and two of these are examined in further detail: (i) Illness representation and (ii) Beliefs about the medical and surgical control of pain. Illness representation comprised beliefs about people’s understanding of OA and their pain experience, as well as expectations about the course of illness. The second theme presented experiences of limited pain relief and concerns about the use of drugs and surgery.
Conclusion: Exploring illness representations and beliefs about medical and surgical control of pain may provide the basis for initiating psychological interventions for people with knee OA
Reliability of dried blood spot (DBS) cards in antibody measurement: A systematic review
Background
Increasingly, vaccine efficacy studies are being recommended in low-and-middle-income countries (LMIC), yet often facilities are unavailable to take and store infant blood samples correctly. Dried blood spots (DBS), are useful for collecting blood from infants for diagnostic purposes, especially in low-income settings, as the amount of blood required is miniscule and no refrigeration is required. Little is known about their utility for antibody studies in children. This systematic review aims to investigate the correlation of antibody concentrations against infectious diseases in DBS in comparison to serum or plasma samples that might inform their use in vaccine clinical trials.
Methods and findings
We searched MEDLINE, Embase and the Cochrane library for relevant studies between January 1990 to October 2020 with no language restriction, using PRISMA guidelines, investigating the correlation between antibody concentrations in DBS and serum or plasma samples, and the effect of storage temperature on DBS diagnostic performance.
We included 40 studies in this systematic review. The antibody concentration in DBS and serum/plasma samples reported a good pooled correlation, (r2 = 0.86 (ranged 0.43 to 1.00)). Ten studies described a decline of antibody after 28 days at room temperature compared to optimal storage at -20°C, where antibodies were stable for up to 200 days. There were only five studies of anti-bacterial antibodies.
Conclusions
There is a good correlation between antibody concentrations in DBS and serum/plasma samples, supporting the wider use of DBS in vaccine and sero-epidemiological studies, but there is limited data on anti-bacterial antibodies. The correct storage of DBS is critical and may be a consideration for longer term storage
P-rex1 cooperates with PDGFRβ to drive cellular migration in 3D microenvironments
Expression of the Rac-guanine nucleotide exchange factor (RacGEF), P-Rex1 is a key determinant of progression to metastasis in a number of human cancers. In accordance with this proposed role in cancer cell invasion and metastasis, we find that ectopic expression of P-Rex1 in an immortalised human fibroblast cell line is sufficient to drive multiple migratory and invasive phenotypes. The invasive phenotype is greatly enhanced by the presence of a gradient of serum or platelet-derived growth factor, and is dependent upon the expression of functional PDGF receptor β. Consistently, the invasiveness of WM852 melanoma cells, which endogenously express P-Rex1 and PDGFRβ, is opposed by siRNA of either of these proteins. Furthermore, the current model of P-Rex1 activation is advanced through demonstration of P-Rex1 and PDGFRβ as components of the same macromolecular complex. These data suggest that P-Rex1 has an influence on physiological migratory processes, such as invasion of cancer cells, both through effects upon classical Rac1-driven motility and a novel association with RTK signalling complexes
Molecular characterisation of protist parasites in human-habituated mountain gorillas (Gorilla beringei beringei), humans and livestock, from Bwindi impenetrable National Park, Uganda
Over 60 % of human emerging infectious diseases are zoonotic, and there is growing evidence of the zooanthroponotic transmission of diseases from humans to livestock and wildlife species, with major implications for public health, economics, and conservation. Zooanthroponoses are of relevance to critically endangered species; amongst these is the mountain gorilla (Gorilla beringei beringei) of Uganda. Here, we assess the occurrence of Cryptosporidium, Cyclospora, Giardia, and Entamoeba infecting mountain gorillas in the Bwindi Impenetrable National Park (BINP), Uganda, using molecular methods. We also assess the occurrence of these parasites in humans and livestock species living in overlapping/adjacent geographical regions

Verticalization of bacterial biofilms
Biofilms are communities of bacteria adhered to surfaces. Recently, biofilms
of rod-shaped bacteria were observed at single-cell resolution and shown to
develop from a disordered, two-dimensional layer of founder cells into a
three-dimensional structure with a vertically-aligned core. Here, we elucidate
the physical mechanism underpinning this transition using a combination of
agent-based and continuum modeling. We find that verticalization proceeds
through a series of localized mechanical instabilities on the cellular scale.
For short cells, these instabilities are primarily triggered by cell division,
whereas long cells are more likely to be peeled off the surface by nearby
vertical cells, creating an "inverse domino effect". The interplay between cell
growth and cell verticalization gives rise to an exotic mechanical state in
which the effective surface pressure becomes constant throughout the growing
core of the biofilm surface layer. This dynamical isobaricity determines the
expansion speed of a biofilm cluster and thereby governs how cells access the
third dimension. In particular, theory predicts that a longer average cell
length yields more rapidly expanding, flatter biofilms. We experimentally show
that such changes in biofilm development occur by exploiting chemicals that
