Education Regarding Advance Directives Improves the End-of-Life Choices Documentation in Heart Failure Patients

Background: Heart failure affects the lives of more than 6 million people in the United States and outpatient heart failure clinics offer an opportunity to educate patients while providing evidence-based care. A needs analysis revealed that many heart failure patients do not have an advance directive (AD) on file. Purpose: The DNP project aims to implement an educational program that provides the tools and knowledge to heart failure patients to assist patients in determining their end-of-life care goals and allow them to document these goals in the Five Wishes document. Methods: This quality improvement project consisted of a didactic educational program offered to newly enrolled heart failure patients in a rural outpatient heart failure clinic that focused on general information about the diagnosis of heart failure, the prognosis, and what an advance directive is and its benefit. All 12 patients were provided with free copies of the Five Wishes document. Results: Key results included an increase of 25% of patients who understand this disease better and the benefits of having an AD to express their end-of-life choices. Four patients (33.3%) returned a completed AD to the clinic to file their medical records. Conclusion: This quality improvement project was multifaceted and stressed the importance of increasing patient knowledge through an educational session about heart failure and advance directives in patients diagnosed with heart failure, which would increase the completion rates of advance directives in these patients

Drive, filter, and stick: A protein sorting conspiracy in photoreceptors.

The sorting of proteins into different functional compartments is a fundamental cellular task. In this issue, Maza et al. (2019. J. Cell Biol https://doi.org/10.1083/jcb.201906024) demonstrate that distinct protein populations are dynamically generated in specialized regions of photoreceptors via an interplay of protein-membrane affinity, impeded diffusion, and driven transport

Objectives. To systematically review and meta-analyze the effectiveness of yoga for reducing blood pressure in adults with hypertension and to assess the modifying influences of type and length of yoga intervention and type of comparison group. Methods. Academic Search Premier, AltHealthWatch, BIOSIS/Biological Abstracts, CINAHL, Cochrane Library, Embase, MEDLINE, PsycINFO, PsycARTICLES, Natural Standard, and Web of Science databases were screened for controlled studies from 1966 to March 2013. Two authors independently assessed risk of bias using the Cochrane Risk of Bias Tool. Results. All 17 studies included in the review had unclear or high risk of bias. Yoga had a modest but significant effect on systolic blood pressure (SBP) (4.17 [6.35, 1.99], ) and diastolic blood pressure (DBP) (3.62 [4.92, 1.60], ). Subgroup analyses demonstrated significant reductions in blood pressure for (1) interventions incorporating 3 basic elements of yoga practice (postures, meditation, and breathing) (SBP: 8.17 mmHg [12.45, 3.89]; DBP: 6.14 mmHg [9.39, 2.89]) but not for more limited yoga interventions; (2) yoga compared to no treatment (SBP: 7.96 mmHg [10.65, 5.27]) but not for exercise. Conclusion. Yoga can be preliminarily recommended as an effective intervention for reducing blood pressure. Additional rigorous controlled trials are warranted to further investigate the potential benefits of yoga

Perceived Benefits of Yoga among Urban School Students: A Qualitative Analysis

    Get PDF
This study reports on the findings of a qualitative evaluation of a yoga intervention program for urban middle and high school youth in New York City public and charter schools. Six focus groups were conducted with students who participated in a year-long yoga program to determine their perceptions of mental and physical benefits as well as barriers and challenges. Results show that students perceived the benefits of yoga as increased self-regulation, mindfulness, self-esteem, physical conditioning, academic performance, and stress reduction. Barriers and challenges for a yoga practice include lack of time and space. The extent to which the benefits experienced are interrelated to one another is discussed. Suggestions for future research and school-based programming are also offered

In 2019, the Supreme Court of the Republic of Indonesia issued Supreme Court Regulation Number 1 of 2019 concerning Case Administration and Trial in Court Electronically or can be referred to as e-Court jo Supreme Court Regulation Number 7 of 2022. Sleman District Court Class IA is one of the executors of judicial power at the first level of justice which has a role to conduct justice, one of which is e-Court services. The implementation of public services must be based on speed, convenience, and affordability. Based on this, the thesis writing conducted is how "Implementation of Public Service Principles in e-Court Service at Sleman District Court Class IA". In this research, empirical legal research method is used which research is conducted focusing on social facts. The data obtained came from primary data in the form of interviews, observations, and questionnaires, and secondary data in the form of laws, books, journals, and the internet. This research concluded that the principles of speed and convenience have been implemented in e-Court services at Sleman Class IA District Court with services provided by eCourt, namely e-Filing, e-Payment, e-Summons, and e-Litigation. The principle of affordability is seen in access to transport and fees because the summons is made electronically via e-mail address and the advocate does not need to come to court except in the process of proof. The most common internal obstacles of Sleman Class IA District Court are slow internet network. The most common obstacle originating from the external of Sleman Class IA District Court is the size of documents is too large. The effort of Sleman District Court Class IA is done by improving internet network and officers, while for external parties to pay more attention to the provisions in e-Court

A Randomized Controlled Trial on the Effects of Yoga on Stress Reactivity in 6th Grade Students

There is an increasing interest in developing school programs that improve the ability of children to cope with psychosocial stress. Yoga may be an appropriate intervention as it has demonstrated improvements in the ability of children to manage psychosocial stress. Yoga is thought to improve the control of reactivity to stress via the regulation of the autonomic nervous system. The current study examined the effects of yoga compared to a physical education class on physiological response (blood pressure (BP) and heart rate (HR)) to behavioral stressor tasks (mental arithmetic and mirror tracing tasks). Data analysis of BP and HR was performed using a 2 × 2 × 4 repeated measures ANOVA (time × group × stressor time points). 30 (17 male) 6th graders participated in the study. Yoga did not provide significant differences in stress reactivity compared to a physical education class (group × time: systolic (F(1,28)=.538, P=.470); diastolic (F(1,28)=.1.061, P=.312); HR (F(1,28)=.401, P=.532)). The lack of significant differences may be due to the yoga intervention failing to focus on stress management and/or the stressor tasks not adequately capturing attenuation of stressor response

Yoga and Emotion Regulation in High School Students: A Randomized Controlled Trial

Middle adolescents (15–17 years old) are prone to increased risk taking and emotional instability. Emotion dysregulation contributes to a variety of psychosocial difficulties in this population. A discipline such as yoga offered during school may increase emotion regulation, but research in this area is lacking. This study was designed to evaluate the impact of a yoga intervention on the emotion regulation of high school students as compared to physical education (PE). In addition, the potential mediating effects of mindful attention, self-compassion, and body awareness on the relationship between yoga and emotion regulation were examined. High school students were randomized to participate in a 16-week yoga intervention (n=19) or regular PE (n=18). Pre-post data analyses revealed that emotion regulation increased significantly in the yoga group as compared to the PE group (F (1,32) = 7.50, p=.01, and eta2 = .19). No significant relationship was discovered between the changes in emotion regulation and the proposed mediating variables. Preliminary results suggest that yoga increases emotion regulation capacities of middle adolescents and provides benefits beyond that of PE alone
