    Seeking for a fingerprint: analysis of point processes in actigraphy recording

    Motor activity of humans displays complex temporal fluctuations which can be characterized by scale-invariant statistics, thus documenting that structure and fluctuations of such kinetics remain similar over a broad range of time scales. Former studies on humans regularly deprived of sleep or suffering from sleep disorders predicted change in the invariant scale parameters with respect to those representative for healthy subjects. In this study we investigate the signal patterns from actigraphy recordings by means of characteristic measures of fractional point processes. We analyse spontaneous locomotor activity of healthy individuals recorded during a week of regular sleep and a week of chronic partial sleep deprivation. Behavioural symptoms of lack of sleep can be evaluated by analysing statistics of duration times during active and resting states, and alteration of behavioural organization can be assessed by analysis of power laws detected in the event count distribution, distribution of waiting times between consecutive movements and detrended fluctuation analysis of recorded time series. We claim that among different measures characterizing complexity of the actigraphy recordings and their variations implied by chronic sleep distress, the exponents characterizing slopes of survival functions in resting states are the most effective biomarkers distinguishing between healthy and sleep-deprived groups.Comment: Communicated at UPON2015, 14-17 July 2015, Barcelona. 21 pages, 11 figures; updated: figures 4-7, text revised, expanded Sec. 1,3,

    The Vigilance Decrement in Executive Function Is Attenuated When Individual Chronotypes Perform at Their Optimal Time of Day

    Time of day modulates our cognitive functions, especially those related to executive control, such as the ability to inhibit inappropriate responses. However, the impact of individual differences in time of day preferences (i.e. morning vs. evening chronotype) had not been considered by most studies. It was also unclear whether the vigilance decrement (impaired performance with time on task) depends on both time of day and chronotype. In this study, morning-type and evening-type participants performed a task measuring vigilance and response inhibition (the Sustained Attention to Response Task, SART) in morning and evening sessions. The results showed that the vigilance decrement in inhibitory performance was accentuated at non-optimal as compared to optimal times of day. In the morning-type group, inhibition performance decreased linearly with time on task only in the evening session, whereas in the morning session it remained more accurate and stable over time. In contrast, inhibition performance in the evening-type group showed a linear vigilance decrement in the morning session, whereas in the evening session the vigilance decrement was attenuated, following a quadratic trend. Our findings imply that the negative effects of time on task in executive control can be prevented by scheduling cognitive tasks at the optimal time of day according to specific circadian profiles of individuals.     Sleep disturbances in an arctic population: The Tromsø Study

    <p>Abstract</p> <p>Background</p> <p>Prevalence estimates for insomnia range from 10 to 50% in the adult general population. Sleep disturbances cause great impairment in quality of life, which might even rival or exceed the impairment in other chronic medical disorders. The economic implications and use of health-care services related to chronic insomnia represent a clinical concern as well as a pronounced public health problem. Hypnotics are frequently prescribed for insomnia, but alcohol and over-the-counter sleep aids seem to be more widely used by insomniacs than prescription medications. Despite the complex relationship between insomnia and physical and mental health factors, the condition appears to be underrecognized and undertreated by health care providers, probably due to the generally limited knowledge of the causes and natural development of insomnia.</p> <p>Methods/Design</p> <p>The Tromsø Study is an ongoing population-based cohort study with five previous health studies undertaken between 1974 and 2001. This protocol outlines a planned study within the sixth Tromsø Study (Tromsø VI), aiming at; 1) describing sleep patterns in a community-based sample representative of the general population of northern Norway, and 2) examining outcome variables of sleep disturbances against possible explanatory and confounding variables, both within a cross-sectional approach, as well as retrospectively in a longitudinal study – exploring sleep patterns in subjects who have attended two or more of the previous Tromsø studies between 1974 and 2009. First, we plan to perform a simple screening in order to identify those participants with probable sleep disturbances, and secondly to investigate these sleep disturbances further, using an extensive sleep-questionnaire. We will also collect biological explanatory variables, i.e. blood samples, weight, height and blood pressure. We plan to merge data on an individual level from the Tromsø VI Study with data from the Norwegian Prescription Database (NorPD), which is a national registry including data for all prescription drugs issued at Norwegian pharmacies. Participants with sleep disturbances will be compared with pair-matched controls without sleep disturbances.</p> <p>Discussion</p> <p>Despite ongoing research, many challenges remain in the characterization of sleep disturbances and its correlates. Future mapping of the biological dimensions, natural history, as well as the behavioral and drug-related aspects of sleep disturbances in a representative population samples is clearly needed.</p

    Organizational configuration of hospitals succeeding in attracting and retaining nurses

    Organizational configuration of hospitals succeeding in attracting and retaining nurses. This paper contrasts structural and managerial characteristics of low- and high-turnover hospitals, and describes the organizational configuration of attractive hospitals. In countries facing nurse shortages and turnover, some hospitals succeed in recruiting and retaining nurses. In Magnet Hospitals, managerial practices and environmental characteristics increase nurses\u2019 job satisfaction and their commitment to the organization, which in turn decreases nurse turnover. Such an approach suggests that organizations are best understood as clusters of interconnected structures and practices, i.e. organizational configurations rather than entities whose components can be understood in isolation. From a sample of 12 hospitals whose nurse turnover was studied for 1 year, structural and organizational features of hospitals in the first and fourth quartiles, i.e. attractive (turnover11\uc68%) were contrasted. A questionnaire, including perceptions of health-related factors, job demands, stressors, work schedules, organizational climate, and work adjustments antecedent to turnover, was received from 401 nurses working in attractive hospitals (response rate - 53\uc68%) and 774 nurses in conventional hospitals (response rate \ubc 54\uc65%). Structural characteristics did not differentiate attractive and conventional hospitals, but employee perceptions towards the organization differed strikingly. Differences were observed for risk exposure, emotional demands, role ambiguity and conflicts, work-family conflicts, effort-reward imbalance and the meaning of work, all in favour of attractive hospitals (P < 0.01). Relationships with nursing management, work ability and satisfaction with working time, handover shifts and schedules were also better in attractive hospitals (P < 0.001). Job satisfaction and commitment were higher in attractive hospitals, whereas burnout and intention to leave were lower (P < 0.001). Organizational characteristics are key factors in nurse attraction and retention. Nurses face difficulties in their work situations, but some hospitals are perceived as healthy organizations. The concept of attractive institutions could serve as a catalyst for improvement in nurses\u2019 work environments in Europe

    Contributive sources analysis: A measure of neural networks' contribution to brain activations

    Item does not contain fulltextGeneral linear model (GLM) is a standard and widely used fMRI analysis tool. It enables the detection of hypothesis-driven brain activations. In contrast, Independent Component Analysis (ICA) is a powerful technique, which enables the detection of data-driven spatially independent networks. Hybrid approaches that combine and take advantage of GLM and ICA have been proposed. Yet the choice of the best method is still a challenge, considering that the techniques may yield slightly different results regarding the number of brain regions involved in a task. A poor statistical power or the deviance from the predicted hemodynamic response functions is possible cause for GLM failures in extracting some activations picked by ICA. However, there might be another explanation for different results obtained with GLM and ICA approaches, such as networks cancelation. In this paper, we propose a new supplementary method that can give more insight into the functional data as well as help to clarify inconsistencies between the results of studies using GLM and ICA. We introduce a contributive sources analysis (CSA), which provides a measure of the number and the strength of the neural networks that significantly contribute to brain activation. CSA, applied to fMRI data of anti-saccades, enabled us to verify whether the brain regions involved in the task are dominated by a single network or serve as key nodes for particular networks interaction. Moreover, when applying CSA to the atlas-defined regions-of-interest, results indicated that activity of the parieto-medial temporal network was suppressed by the eye field network and the default mode network. Thus, this effect of networks cancelation explains the absence of parieto-medial temporal activation within the GLM results. Together, those findings indicate that brain activations are a result of complex network interactions. Applying CSA appears to be a useful tool to reveal additional findings outside the scope of the "fixed-model" GLM and data-driven ICA approaches
