    Transmission of medical messages of patient using control signal of cellular network

    The healthcare system needs to track and monitor patients’ status and information. One of the most important requirements is that the patient is able to access the service anywhere at any time. Systems are being developed using the Internet to monitor patients’ status, and in some areas, especially rural areas and motorways, the Internet may not be available, even though the mobile network is available. In some cases, the network might be overloaded, so the patient information cannot be delivered to the hospital or medical centre. We propose a new method that uses the spare extension of the random access channel (RACH), which is carried by physical random access channel (PRACH) to send the patient information to the medical centre. We present the mathematical model of the channel and compare the results with another system from 3GPP to evaluate the results. The results show that the proposed method needs less time to transmit the patient’s information

    Lafora Disease Masquerading as Hepatic Dysfunction

    Lafora disease is fatal intractable progressive myoclonic epilepsy. It is frequently characterized by epileptic seizures, difficulty walking, muscle spasms, and dementia in late childhood or adolescence. We chronicle here an unusual case of an asymptomatic young male soccer player who presented with elevated liver enzymes. Neurological examination was unremarkable. The diagnostic workup for hepatitis, infectious etiologies, autoimmune disorders, hemochromatosis, Wilson\u27s disease, alpha-1 antitrypsin deficiency, and other related diseases was inconclusive. He subsequently underwent an uneventful percutaneous liver biopsy. Based on the pathognomonic histopathological findings, Lafora disease was considered the likely etiology. The present study is a unique illustration of this rare disorder initially manifesting with abnormal liver enzymes. It underscores the importance of clinical suspicion of Lafora disease in cases with unexplained hepatic dysfunction. Prompt liver biopsy and genetic testing should be performed to antedate the onset of symptoms in these patients

    In December 2019, a new coronavirus, now labeled as severe acute respiratory syndrome coronavirus 2, induced an episode of acute atypical respiratory illness started in Wuhan, Province of Hubei, China. The illness triggered by this virus was called coronavirus disease-19 (COVID-19). The infection is spread within humans and has triggered a global pandemic. The amount of death tolls continues to increase and a growing number of countries have been driven to create social barriers and lock-ups. The shortage of tailored counseling remains an issue. Epidemiological researches have shown that elderly patients are more vulnerable to serious diseases, while children tend to have milder symptoms. Here, we checked the latest understanding of this disease and found a possible explanation of the potential sequel and the expectations for the future

    Enhancing the Mechanical Properties of Concrete and Self-Healing Phenomena by adding Bacteria, Silica fume and Fibres

    Concrete which is the most useable material in the world after the water has flaws, it is susceptible to cracking over time. These cracks occur in the form of shear cracks, flexural cracks, tension cracks, shrinkage cracks etc. With these cracks, some hair-like cracks also occur in concrete which are not visible during the visual inspection. The propagation of these cracks in concrete allows the water and many other chemicals to seep inside the concrete and leads to a decrease in its properties. Such properties include decreasing durability, erosion of rebars, and progressive failure in the concrete strength. Therefore, the repair of hair-like cracks is also essential for the long-term safety of structures. In the present study the Silica fume, and Polypropylene fibres are added to a rich concrete along with the bacteria named Bacillus Subtilis and Calcium Lactate for enhancement of its mechanical properties and self-healing phenomena. The effect of bacteria in the healing phenomenon and other properties is compared to normal concrete by casting the cylinders and beams. The slump, compressive strength, tensile strength, and self-healing phenomena are tested and found the increase in mechanical properties of concrete. The self-healing phenomena of cracks is observed by the Scanning Electron Microscope (SEM)

    A robust control design approach for altitude control and trajectory tracking of a quadrotor

    Introduction. Unmanned aerial vehicles as quadcopters, twin rotors, fixed-wing crafts, and helicopters are being used in many applications these days. Control approaches applied on the quadrotor after decoupling the model or separate altitude control and trajectory tracking have been reported in the literature. A robust linear H∞ controller has been designed for both altitude control and circular trajectory tracking at the desired altitude. Problem. The ability of the quadrotor system to hover at a certain height and track any desired trajectory makes their use in many industrial applications in both military and civil applications. Once a controller has been designed, it may not be able to maintain the desired performance in practical scenarios, i.e. in presence of wind gusts. Originality. This work presents the control strategy to ensure both altitude control and trajectory tracking using a single controller. Purpose. However, there is a need for a single controller that ensures both altitude control and trajectory tracking. Novelty. This paper presents a robust H∞ control for altitude control and trajectory tracking for a six degree of freedom of unmanned aerial vehicles quadrotor. Methodology. Multi input multi output robust H∞ controller has been proposed for the quadrotor for altitude control and tracking the desired reference. For the controller validation, a simulation environment is developed in which a 3D trajectory is tracked by the proposed control methodology. Results. Simulation results depict that the controller is efficient enough to achieve the desired objective at minimal control efforts. Practical value. To verify that the proposed approach is able to ensure stability, altitude control, and trajectory tracking under practical situations, the performance of the proposed control is tested in presence of wind gusts. The ability of the controller to cater to the disturbances within fractions of seconds and maintaining both transient and steady-state performance proves the effectiveness of the controller.Вступ. Безпілотні літальні апарати, такі як квадрокоптери, двороторні апарати, апарати з нерухомими крилами та гелікоптери сьогодні використовуються у багатьох сферах застосування. У літературі повідомляється про підходи до керування, застосовані на квадрокоптері після від’єднання моделі або окремого контролю висоти та відстеження траєкторії. Надійний лінійний регулятор H∞ був розроблений як для контролю висоти, так і для відстеження кругової траєкторії на потрібній висоті. Проблема. Здатність квадрокоптерної системи зависати на певній висоті та відстежувати будь-яку бажану траєкторію робить їх застосування можливим у багатьох сферах як у військових, так і в цивільних цілях. Розроблений контролер може не підтримувати бажані характеристики у реальних умовах, тобто за наявності поривів вітру. Оригінальність. У цій роботі представлена стратегія керування, яка забезпечує як контроль висоти, так і відстеження траєкторії за допомогою одного контролера. Мета. Однак існує потреба в єдиному контролері, який забезпечує як контроль висоти, так і відстеження траєкторії. Новизна. У цій статті представлено надійний регулятор H∞ для контролю висоти та відстеження траєкторії для шести ступенів свободи безпілотних літальних апаратів. Методологія. Для квадрокоптера запропоновано багатовхідний багатовихідний надійний контролер H∞ для контролю висоти та відстеження бажаного курсу. Для перевірки контролера розробляється середовище моделювання, в якому тривимірна траєкторія відстежується за запропонованою методологією керування. Результати. Результати моделювання показують, що контролер є досить ефективним для досягнення бажаної мети при мінімальних зусиллях контролю. Практична цінність. Щоб переконатися, що запропонований підхід здатний забезпечити стабільність, контроль висоти та відстеження траєкторії в реальних ситуаціях, параметри запропонованого контролю перевіряються за наявності поривів вітру. Здатність контролера усувати порушення протягом кількох секунд і підтримувати як перехідні, так і стабільні показники доводить ефективність контролера

    Effect of exogenous application of nicotinic acid on morpho-physiological characteristics of Hordeum vulgare L. under water stress

    Abiotic stresses, such as high temperature and drought conditions, greatly influence the development of plants and the quality and quantity of products. Barley (Hordeum vulgare L.) crop production is largely impacted by drought, affecting growth, yield, and ultimately the productivity of the crop in hot arid/semi-arid conditions. The current pot experiment was directed to observe the outcome of nicotinic acid (NA) treatments on barley’s physiological, biochemical, and production attributes at two capacity levels, i.e., 100% normal range and withholding water stress. Randomized complete block design (RCBD) was used during the experimentation with the two-factor factorial arrangement. NA was applied exogenously by two different methods, i.e., foliar and soil application (fertigation). NA solution contained various application levels, such as T1 = control, foliar applications (T2 = 0.7368 gL−1, T3 = 1.477 gL−1, T4 = 2.2159 gL−1), and soil applications (T5 = 0.4924 gL−1, T6 = 0.9848 gL−1, and T7 = 1.4773 gL−1). Results depicted that, overall, foliar treatments showed better effects than control and soil treatments. Plant growth was preeminent under T4 treatment, such as plant height (71.07 cm), relative water content (84.0%), leaf water potential (39.73-MPa), leaf area index (36.53 cm2), biological yield (15.10 kgha−1), grain yield (14.40 kgha−1), harvest index (57.70%), catalase (1.54 mmolg−1FW−1), peroxidase (1.90 g−1FWmin−1), and superoxide dismutase (52.60 µgFW−1) were superior under T4 treatment. Soil plant analysis development (54.13 µgcm−2) value was also higher under T4 treatment and lowest under T7 treatment. In conclusion, NA-treated plants were more successful in maintaining growth attributes than non-treated plants; therefore, the NA foliar treatment at the rate of 2.2159 gL−1 is suggested to find economical crop yield under drought conditions. The present study would contribute significantly to improving the drought tolerance potential of barley through exogenous NA supply in water deficit areas

    Extracorporeal membrane oxygenator as a bridge to successful surgical repair of bronchopleural fistula following bilateral sequential lung transplantation: a case report and review of literature

    <p>Abstract</p> <p>Background</p> <p>Lung transplantation (LTx) is widely accepted as a therapeutic option for end-stage respiratory failure in cystic fibrosis. However, airway complications remain a major cause of morbidity and mortality in these patients, serious airway complications like bronchopleural fistula (BPF) are rare, and their management is very difficult.</p> <p>Case presentation</p> <p>A 47-year-old man with end-stage respiratory failure due to cystic fibrosis underwent bilateral sequential lung transplantation. Severe post-operative bleeding occurred due to dense intrapleural adhesions of the native lungs. He was re-explored and packed leading to satisfactory haemostasis. He developed a bronchopleural fistula on the 14<sup>th </sup>post-operative day. The fistula was successfully repaired using pericardial and intercostal vascular flaps with veno-venous extracorporeal membrane oxygenator (VV-ECMO) support. Subsequently his recovery was uneventful.</p> <p>Conclusion</p> <p>The combination of pedicled intercostal and pericardial flaps provide adequate vascular tissue for sealing a large BPF following LTx. Veno-venous ECMO allows a feasible bridge to recovery.</p

    Body image disturbance and surgical decision making in egyptian post menopausal breast cancer patients

    <p>Abstract</p> <p>Background</p> <p>In most developing countries, as in Egypt; postmenopausal breast cancer cases are offered a radical form of surgery relying on their unawareness of the subsequent body image disturbance. This study aimed at evaluating the effect of breast cancer surgical choice; Breast Conservative Therapy (BCT) versus Modified Radical Mastectomy (MRM); on body image perception among Egyptian postmenopausal cases.</p> <p>Methods</p> <p>One hundred postmenopausal women with breast cancer were divided into 2 groups, one group underwent BCT and the other underwent MRM. Pre- and post-operative assessments of body image distress were done using four scales; Breast Impact of Treatment Scale (BITS), Impact of Event Scale (IES), Situational Discomfort Scale (SDS), and Body Satisfaction Scale (BSS).</p> <p>Results</p> <p>Preoperative assessment showed no statistical significant difference regarding cognitive, affective, behavioral and evaluative components of body image between both studied groups. While in postoperative assessment, women in MRM group showed higher levels of body image distress among cognitive, affective and behavioral aspects.</p> <p>Conclusion</p> <p>Body image is an important factor for postmenopausal women with breast cancer in developing countries where that concept is widely ignored. We should not deprive those cases from their right of less mutilating option of treatment as BCT.</p

    Using risk factor statistics in decision making: prospects and challenges

Statistical (numerical) data is rapidly becoming an increasing part of the repertoire of knowledge available to professionals. However, social workers can at times seem reluctant to use statistics to inform their judgement processes when considering the assessment and seriousness of risk factors. This paper explores and considers the potential use of statistical methods in understanding risk factors. Potentials of using statistics include reducing variability in human judgements, increasing accuracy and reducing human bias. When contemplating these potentials, it is important to consider ethical, legal and organizational as well as technical issues. Acknowledging that potentials and challenges need to be weighed, it is imperative that the profession engages in the development of adequate statistical methods to improve the knowledge and skill base for social work in the best interests of clients and families
