    No visible optical variability from a relativistic blast wave encountering a wind-termination shock

    Gamma-ray burst afterglow flares and rebrightenings of the optical and X-ray light curve have been attributed to both late time inner engine activity and density changes in the medium surrounding the burster. To test the latter, we study the encounter between the relativistic blast wave from a gamma-ray burster and a stellar wind termination shock. The blast wave is simulated using a high performance adaptive mesh relativistic hydrodynamics code, AMRVAC, and the synchrotron emission is analyzed in detail with a separate radiation code. We find no bump in the resulting light curve, not even for very high density jumps. Furthermore, by analyzing the contributions from the different shock wave regions we are able to establish that it is essential to resolve the blast wave structure in order to make qualitatively correct predictions on the observed output and that the contribution from the reverse shock region will not stand out, even when the magnetic field is increased in this region by repeated shocks.     Genetic variability in a population of Letelle sheep in South Africa

    The purpose of the study was to gain insight into the genetic variability of the Letelle sheep breed, a breed that has been managed as a closed population for 90 years, with no new genetic material being permitted into the breed. The Letelle is a South African developed dual-purpose sheep breed and is classified as a Merino type with a Spanish Merino origin. The breed exhibits good fine wool characteristics and yields high-quality mutton. Line-breeding, family-breeding, and inbreeding are applied, and multiple sire matings are practised to prevent a sire from having a large influence on the national flock. Ear samples were collected from 10 animals each from 10 commercial and 11 seed-stock flocks and genotyped using 17 microsatellite markers. Unbiased heterozygosity ranged from 0.58 to 0.68 and the observed heterozygosity from 0.52 to 0.65. The estimated effective population size (Ne) was 228.2 - 321.9. Results from analysis of molecular variance (AMOVA), a Bayesian assignment test, and a neighbour-joining (NJ) tree suggested that no genetic sub-structure existed within this population and that the seed-stock and commercial flocks could be regarded as one genetic population. The average within flock (FIS) and within breed (FIT) inbreeding coefficients were 10.1% and 14.5%, respectively. Despite the level of inbreeding, levels of genetic diversity were moderate and potentially provide opportunities for future selection and adaptation. Further testing could identify flocks in which conservation management is required as well as those with high genetic variability, which would provide the best reservoir for selection to adapt to future climatic challenges.Keywords: genetic distance, inbreeding, microsatellite markers, population structur

    Employment in Personality Disorders and the Effectiveness of Individual Placement and Support:Outcomes from a Secondary Data Analysis

    Purpose Personality disorders (PDs) are associated with severe functional impairment and subsequent high societal costs, increasing the need to improve occupational functioning in PD. Individual placement and support (IPS) is an effective, evidence-based method of supported employment, which so far has been tested in various mixed patient populations with severe mental illness (SMI, including PDs). However, the effectiveness of IPS for PDs per se remains uninvestigated. Methods Data from the SCION trial were used, including 31 SMI patients with PDs and 115 SMI patients with other primary diagnoses (primarily psychotic disorders). First, the interaction effect of diagnosis (PD vs other SMI) and intervention (IPS vs traditional vocational rehabilitation) was studied. Second, in the IPS condition, difference between diagnostic groups in time to first job was studied. Results We did not find evidence of a moderating effect of PD diagnosis on the primary effect of IPS (proportion who started in regular employment) (OR = 0.592, 95% CI 0.80–4.350, p = 0.606) after 30 months. Also, PD diagnosis did not moderate the effect of time until first job in IPS. Conclusions From the present explorative analysis we did not find evidence for a moderating effect of PD diagnosis on the effectiveness of IPS among PD participants. This indicates that IPS could be as effective in gaining employment in participants with PD as it is in participants with other SMI. Future studies, implementing larger numbers, should confirm whether IPS is equally effective in PDs and study whether augmentations or alterations to the standard IPS model might be beneficiary for PD

    Cost-effectiveness of stereotactic large-core needle biopsy for nonpalpable breast lesions compared to open-breast biopsy

    This paper demonstrates that the introduction of large-core needle biopsy (LCNB) replacing needle-localised breast biopsy (NLBB) for nonpalpable (screen-detected) breast lesions could result in substantial cost savings at the expense of a possible slight increase in breast cancer mortality. The cost-effectiveness of LCNB and NLBB was estimated using a microsimulation model. The sensitivity of LCNB (0.97) and resource use and costs of LCNB and NLBB were derived from a multicentre consecutive cohort study among 973 women who consented in getting LCNB and NLBB, if LCNB was negative. Sensitivity analyses were performed. Replacing NLBB with LCNB would result in approximately six more breast cancer deaths per year (in a target population of 2.1 million women), or in 1000 extra life-years lost from breast cancer (effect over 100 years). The total costs of management of breast cancer (3% discounted) are estimated at £4676 million with NLBB; introducing LCNB would save £13 million. The incremental cost-effectiveness ratio of continued NLBB vs LCNB would be £12 482 per additional life-year gained (3% discounted); incremental costs range from £-21 687 (low threshold for breast biopsy) to £74 378 (high sensitivity of LCNB)

    Quantitative trait loci associated with pre-weaning growth in South African Angora goats

    This study aimed to identify chromosomal regions associated with genetic variation in preweaning growth traits in Angora goats. A genome-wide scan was performed by genotyping 1042 offspring from 12 half-sib families using 88 microsatellite caprine markers covering 1368cM. Phenotypes were recorded at birth (BW) and weaning (WW) and analysed using GridQTL software. A total of six putative QTL were detected on six different chromosomes, all at chromosome-wide significance level. Four QTL were identified for BW on CHI 4, 8, 17 and 27 and two QTL for WW on CHI 16 and 19. QTL effects ranged from −0.32 to 0.25 in units of residual standard deviation in different families. Some of these QTL correspond to chromosomes where QTL associated with growth have been identified in other species.     Preferences and beliefs of Dutch orthopaedic surgeons and patients reduce the implementation of "Choosing Wisely" recommendations in degenerative knee disease

    Purpose: The purpose of this study was to assess which factors were associated with the implementation of “Choosing Wisely” recommendations to refrain from routine MRI and arthroscopy use in degenerative knee disease. Methods: Cross-sectional surveys were sent to 123 patients (response rate 95%) and 413 orthopaedic surgeons (response rate 62%) fulfilling the inclusion criteria. Univariate and multivariate logistic regression analyses were used to identify factors associated with implementation of “Choosing Wisely” recommendations. Results: Factors reducing implementation of the MRI recommendation among patients included explanation of added value by an orthopaedic surgeon [OR 0.18 (95% CI 0.07–0.47)] and patient preference for MRI [OR 0.27 (95% CI 0.08–0.92)]. Factors reducing implementation among orthopaedic surgeons were higher valuation of own MRI experience than existing evidence [OR 0.41 (95% CI 0.19–0.88)] and higher estimated patients’ knowledge to participate in shared decision-making [OR 0.38 (95% CI 0.17–0.88)]. Factors reducing implementation of the arthroscopy recommendation among patients were orthopaedic surgeons’ preferences for an arthroscopy [OR 0.03 (95% CI 0.00–0.22)] and positive experiences with arthroscopy of friends/family [OR 0.03 (95% CI 0.00–0.39)]. Factors reducing implementation among orthopaedic surgeons were higher valuation of own arthroscopy experience than existing evidence [OR 0.17 (95% CI 0.07–0.46)] and belief in the added value [OR 0.28 (95% CI 0.10–0.81)]. Conclusions: Implementation of “Choosing Wisely” recommendations in degenerative knee disease can be improved by strategies to change clinician beliefs about the added value of MRIs and arthroscopies, and by patient-directed strategies addressing patient preferences and underlying beliefs for added value of MRI and arthroscopies resulting from experiences of people in their environment.     Oxytocin moderates the association between testosterone-cortisol ratio and trustworthiness:A randomized placebo-controlled study

    Oxytocin has been proposed to enhance feelings of trust, however, these findings have been difficult to replicate. Environmental or hormonal factors might influence this association. We studied whether oxytocin moderates the association between the testosterone-cortisol ratio, which is associated with risk taking behavior and aggression, and trustworthiness, while controlling for the general level of trust. A randomized double-blind placebo-controlled study with 53 healthy males was performed in which 32IU oxytocin (n = 27) or placebo (n = 26) was administered intranasally. Participants subsequently played the Trust Game in which they were allocated to the role of trustee. In the third phase of the Trust Game, we found a positive association between the testosterone-cortisol-ratio and the proportion of the amount that is returned to the investor (P=<0.01). However, administration of oxytocin reduced reciprocity in those with a high testosterone-cortisol ratio after reciprocity restoration (a significant interaction effect between administration of oxytocin and the testosterone-cortisol ratio in the third phase of the Trust Game, P = 0.015). The third phase of the Trust Game represents the restoration of reciprocity and trustworthiness, after this is violated in the second phase. Therefore, our data suggest that oxytocin might hinder the restoration of trustworthiness and diminish risk-taking behavior when trust is violated, especially in those who are hormonally prone to risk-taking behavior by a high testosterone-cortisol ratio

    The enigmatic nature of the circumstellar envelope and bow shock surrounding Betelgeuse as revealed by Herschel. I. Evidence of clumps, multiple arcs, and a linear bar-like structure

    Context. The interaction between stellar winds and the interstellar medium (ISM) can create complex bow shocks. The photometers on board the Herschel Space Observatory are ideally suited to studying the morphologies of these bow shocks. Aims. We aim to study the circumstellar environment and wind-ISM interaction of the nearest red supergiant, Betelgeuse. Methods. Herschel PACS images at 70, 100, and 160 micron and SPIRE images at 250, 350, and 500 micron were obtained by scanning the region around Betelgeuse. These data were complemented with ultraviolet GALEX data, near-infrared WISE data, and radio 21 cm GALFA-HI data. The observational properties of the bow shock structure were deduced from the data and compared with hydrodynamical simulations. Results. The infrared Herschel images of the environment around Betelgeuse are spectacular, showing the occurrence of multiple arcs at 6-7 arcmin from the central target and the presence of a linear bar at 9 arcmin. Remarkably, no large-scale instabilities are seen in the outer arcs and linear bar. The dust temperature in the outer arcs varies between 40 and 140 K, with the linear bar having the same colour temperature as the arcs. The inner envelope shows clear evidence of a non-homogeneous clumpy structure (beyond 15 arcsec), probably related to the giant convection cells of the outer atmosphere. The non-homogeneous distribution of the material even persists until the collision with the ISM. A strong variation in brightness of the inner clumps at a radius of 2 arcmin suggests a drastic change in mean gas and dust density some 32 000 yr ago. Using hydrodynamical simulations, we try to explain the observed morphology of the bow shock around Betelgeuse. Conclusions: [abbreviated]Comment: 26 page
