    La Marina asturlucense: experiencia de gobernanza en un teritorio muy complejo

    Este trabajo analiza el efecto de distintas formas de gobernanza territorial en dos territorios similares, funcionalmente integrados, pero en comunidades autónomas distintas. Los resultados muestran la inexistencia del dilema protección del paisaje-desarrollo económico. Muestra también la conveniencia de ajustar las regulaciones y el marco institucional de gobernanza a la realidad funcional del territorio. En este caso, creando mecanismos de cooperación multinivel que se ajusten a la polimórfica realidad micropolitana. Por último, se detecta la conveniencia de crear liderazgos para la gobernanza territorial, así como poner en marcha mecanismos de participación que contribuyan a canalizar flujos de información entre ciudadanía y administraciones, rompiendo con algunos posos culturales que suponen una rémora para el desarrollo territorial.This paper analyzes the effect of different forms of territorial governance in two similar and integrated territories, ruled under different regional autonomous administrations. The results show the absence of the protection of the economic landscape-economic performance dilemma. Moreover, shows the appropriateness of adjusting the rules and the institutional framework of governance to the functional reality of the territory, creating multilevel cooperation mechanisms adapted to the polimorphic micropolitan reality. Finally, detected the desirability of leadership, as well as participation mechanisms implementation, able of channelling information flows between citizens and administrations, breaking with some cultural grounds posed a setback for territorial development

    Linoleic acid-derived oxylipins and isoprostanes plasma levels are influenced by 1,25-Dihydroxyvitamin D levels in middle-aged sedentary adults: The FIT-AGEING study

    Introduction: Vitamin D – concretely its active form 1,25-dihydroxyvitamin D (1,25(OH)2D) – maintains several physiological processes. Oxylipins are oxidized lipids derived from ω-6 and ω-3 polyunsaturated fatty acids involved in inflammation. Little is known about the association of 1,25(OH)2D with inflammatory parameters in middle-aged populations – who could be at risk of vitamin D deficiency –. The aim of this study was to investigate the relationship between 1,25(OH)2D plasma levels with circulating white blood cells, platelets counts and oxylipins levels. Materials and methods: A total of 74 (53 % women) middle-aged (40–65 years old) adults were recruited for this cross-sectional study. 1,25(OH)2D plasma levels were measured using an immunochemiluminometric assay. White blood cells and platelets were analyzed by hemocytometry. ω-6 and ω-3 oxylipins plasma levels were measured using liquid chromatography - tandem mass spectrometry. Simple and multiple linear regression models, and Pearson correlation analyses, were performed to study the association of 1,25(OH)2D levels with WBC and platelets counts, and oxylipins, respectively. Results: 1,25(OH)2D plasma levels were positively related with linoleic acid-derived oxylipins and isoprostanes plasma levels, whereas an inverse relationship with dihomo-γ-linolenic acid/linoleic acid and arachidonic acid/ linoleic acid ratios was unveiled. No significant associations were observed for circulating ω-3 oxylipins, white blood cells levels or platelets count. Conclusions: Linoleic acid-derived oxylipins and isoprostanes plasma levels may be influenced by 1,25(OH)2D plasma levels. Further investigations are needed to elucidate the impact of other vitamin D forms upon circulating oxylipins levels.Spanish GovernmentChina Scholarship Council 201607060017 20170706001

    Developments and results in the context of the JEM-EUSO program obtained with the ESAF simulation and analysis framework

    JEM-EUSO is an international program for the development of space-based Ultra-High Energy Cosmic Ray observatories. The program consists of a series of missions which are either under development or in the data analysis phase. All instruments are based on a wide-field-of-view telescope, which operates in the near-UV range, designed to detect the fluorescence light emitted by extensive air showers in the atmosphere. We describe the simulation software ESAF in the framework of the JEM-EUSO program and explain the physical assumptions used. We present here the implementation of the JEM-EUSO, POEMMA, K-EUSO, TUS, Mini-EUSO, EUSO-SPB1 and EUSO-TA configurations in ESAF. For the first time ESAF simulation outputs are compared with experimental data.Comunidad de Madri

    Generalización fractal de células preneoplásicas y cancerígenas del epitelio escamoso cervical. Una nueva metodología de aplicación clínica

    A partir de la evaluación de las dimensiones fractales y aplicando el concepto de variabilidad y armonía matemática intrínseca celular (AMI), se desarrolló un método matemático de aplicación clínica para el diagnóstico de células preneoplásicas y neoplásicas del epitelio escamoso cervical, el cual supera el diagnóstico de ASCUS (células escamosas atípicas de significado no determinado). A partir de un método desarrollado previamente para el diagnóstico de células normales, ASCUS y L-SIL, se realizaron permutaciones estructurales fractales con los valores generales de la AMI y de variabilidad fractal para normalidad y enfermedad, buscando los prototipos generales de células normales, preneoplásicas y neoplásicas. Se midieron cinco células ASCUS y cinco cancerígenas del epitelio escamoso del cuello uterino y se encontró que para la normalidad hay 18 prototipos, mientras que para la anormalidad 44, incluyendo todos los estados de evolución hasta carcinoma.Estos resultados fueron confirmados al comparar los resultados obtenidos por las técnicas convencionales con los obtenidos por esta metodología; lo que hace pensar que se desarrolló una nueva metodología que permite diferenciar los distintos tipos de células del epitelio escamoso del cuello uterino (normales o cancerígenas)

    Introduction: A new fractal diagnostic method was applied for assessing cervix cells using the Intrinsic Mathematical Harmony (IMH) concept and cellular variability, which mathematically differentiates normal, L-SIL and H-SIL cells making unnecessary the diagnostic of ASCUS cells.Objective: To confirm the diagnostic capability of developed methodology by a blind study of comparison with Gold Standard.Methods: Digital pictures were taken of 50 cells from cytology of 20 to 55 year-old women; 5 with normal diagnosis and 45 with several lesion degrees until carcinoma, including 5 with ASCUS diagnosis. Fractal dimension of three mathematical objects was calculated by superposing five grids: nucleus, cytoplasm and totality. Moreover, their fractal dimension was assessed through the Intrinsic Mathematical Harmony concept and cellular variability. The obtained diagnosis was compared with conventional cytopathologic diagnosis determining sensibility, specificity and Kappa coefficient.Results: 100% Sensibility and specificity, and a Kappa Coefficient of 1 were obtained.Conclusions: The results, in a population different to the initial one, are an evidence of the ability of this methodolo­gy for objectively and quantitatively diagnosing the normal, L-SIL and H-SIL cells, as well as for clarifying the ASCUS cells diagnosis, based on fractal dimension and IMH concept and variability.Antecedentes: Se desarrolló un método diagnóstico fractal para evaluar células del cuello uterino utilizando el concepto de Armonía Matemática Intrínseca (AMI) y variabilidad celular, el cual diferencia matemáticamente células normales de células L-SIL y H-SIL, haciendo innecesario el diagnóstico de células ASCUS.Objetivo: confirmar la capacidad diagnóstica de la metodología desarrollada mediante un estudio ciego de comparación con el Gold StandardMétodos: se tomaron fotografías digitales de 50 preparaciones citológicas de mujeres entre 20 y 55 años: 5 con diagnóstico de citología normal y 45 con diferentes grados de lesión hasta carcinoma, incluyendo 5 ASCUS. Se calculó la dimensión fractal de tres objetos matemáticos: núcleo, citoplasma y totalidad, a partir de la superposición de cinco rejillas. Además se evaluó su dimensión fractal mediante el concepto de AMI y variabilidad celular. Los resultados obtenidos se compararon con el diagnóstico citopatológico convencional determinando su sensibilidad, especificidad y coeficiente kappa.Resultados: La sensibilidad y especificidad fue del 100%, y el coeficiente Kappa de 1.Conclusiones: Los resultados en una población diferente a la inicial son una evidencia de la capacidad de esta metodología para diagnosticar objetiva y cuantitativamente células normales, L-SIL y H-SIL, así como aclarar el diagnóstico de las células ASCUS con base en la dimensión fractal y el concepto de AMI y variabilidad. Introdução: Um novo método de diagnóstico fractal foi aplicado para avaliar as células do colo do útero usando con­ceito intrínseco harmonia matemática (IMH) e variabilidade celular, que normalmente diferencia matematicamente, LSIL e HSIL células que fazem o diagnóstico de células desnecessários ASCUS.Objetivo: Para confirmar a capacidade de diagnóstico da metodologia desenvolvida por um estudo cego de compa­ração com Gold Standard.Métodos: Foram tiradas fotos digitais de 50 células de citologia de mulheres com 20 a 55 anos de idade; 5 com diagnóstico normal e 45 com vários graus de lesão até carcinoma, incluindo 5 com diagnóstico de ASCUS. A di­mensão fractal de três objetos matemáticos foi calculada através da superposição de cinco grades: núcleo, citoplasma e totalidade. Além disso, sua dimensão fractal foi avaliada através do conceito de harmonia matemática Intrínseca e da variabilidade celular. O diagnóstico obtido foi comparado com o diagnóstico citopatológico convencional, deter­minando a sensibilidade, especificidade e coeficiente Kappa.Resultados: Foram obtidos 100% de sensibilidade e especificidade e um Coeficiente Kappa de 1.Conclusões: Os resultados, em uma população diferente da inicial, são uma evidência da habilidade desta meto­dologia para o diagnóstico objetivo e quantitativo das células normais, L-SIL e H-SIL, bem como para esclarecer o diagnóstico das células ASCUS, Baseado na dimensão fractal e no conceito e variabilidade de IMH

    Pancreatic metastases from renal cell carcinoma. Postoperative outcome after surgical treatment in a Spanish multicenter study (PANMEKID)

    Background: Renal Cell Carcinoma (RCC) occasionally spreads to the pancreas. The purpose of our study is to evaluate the short and long-term results of a multicenter series in order to determine the effect of surgical treatment on the prognosis of these patients. Methods: Multicenter retrospective study of patients undergoing surgery for RCC pancreatic metastases, from January 2010 to May 2020. Variables related to the primary tumor, demographics, clinical characteristics of metastasis, location in the pancreas, type of pancreatic resection performed and data on short and long-term evolution after pancreatic resection were collected. Results: The study included 116 patients. The mean time between nephrectomy and pancreatic metastases' resection was 87.35 months (ICR: 1.51-332.55). Distal pancreatectomy was the most performed technique employed (50 %). Postoperative morbidity was observed in 60.9 % of cases (Clavien-Dindo greater than IIIa in 14 %). The median follow-up time was 43 months (13-78). Overall survival (OS) rates at 1, 3, and 5 years were 96 %, 88 %, and 83 %, respectively. The disease-free survival (DFS) rate at 1, 3, and 5 years was 73 %, 49 %, and 35 %, respectively. Significant prognostic factors of relapse were a disease free interval of less than 10 years (2.05 [1.13-3.72], p 0.02) and a history of previous extrapancreatic metastasis (2.44 [1.22-4.86], p 0.01). Conclusions: Pancreatic resection if metastatic RCC is found in the pancreas is warranted to achieve higher overall survival and disease-free survival, even if extrapancreatic metastases were previously removed. The existence of intrapancreatic multifocal compromise does not always warrant the performance of a total pancreatectomy in order to improve survival
