    The archaeology of rank is in many respects the basis for the study of social organisation in ancient societies. Here burial data are considered as closely related to the social position in life. Rank-indicating features marking social position can then be correlated with biological features such as sex, age, kinship, pathological traits, and dietary data, which are possible to study with scientific methods. This study concerns seven boatgraves at Tuna, Alsike parish, Uppland, Sweden, dating to the Vendel and Viking Periods. Molecular sex identifications, stable carbon and nitrogen isotope analyses, and trace element analysis were performed on human bones to gain information on the social structure of the population. No dietary hierarchy, and thus no gender differences, could be detected. However, since we now only have studied the upper stratum of the society, next study should concern the rest of the population. This is already underway since we believe this approach is fruitful for the study of archaeological rank and its implications

    Combined life satisfaction of persons with stroke and their caregivers: associations with caregiver burden and the impact of stroke

    <p>Abstract</p> <p>Background</p> <p>Little is known about the life satisfaction of the person with stroke combined with their caregiver, i.e. the dyad, despite the fact that life satisfaction is an important rehabilitation outcome. The aim of this study was to describe the dyads combined life satisfaction and to understand this in relationship to the perceived impact of stroke in everyday life and caregiver burden.</p> <p>Methods</p> <p>In this cross-sectional study, the life satisfaction of persons and their informal caregivers was measured in 81 dyads one year post stroke. Their global life satisfaction, measured with LiSat-11, was combined to a dyad score and the dyads were then categorized as satisfied, dissatisfied or discordant. The groups were compared and analyzed regarding levels of caregiver burden, measured with the Caregiver Burden scale, and the perceived impact of stroke in everyday life, measured with the Stroke Impact Scale (SIS).</p> <p>Results</p> <p>The satisfied dyads comprised 40%, dissatisfied 26% and those that were discordant 34%. The satisfied dyads reported a significantly lower impact of the stroke in everyday life compared with the dyads that were not satisfied. As expected, dyads that were not satisfied reported a significantly greater caregiver burden compared with the satisfied dyads. The discordant group was further broken down into a group of dissatisfied and satisfied caregivers. The caregivers that were not satisfied in the discordant group perceived a significantly greater level of caregiver burden compared with the satisfied group. Even caregivers who were satisfied with life but whose care recipients were not satisfied reported caregiver burden.</p> <p>Conclusions</p> <p>Measuring combined life satisfaction provides a unique focus and appears to be a feasible way of attaining the dyads' perspective. The findings suggest that those dyads with a discordant life satisfaction could be vulnerable because of the caregivers' reported caregiver burden. These findings support the importance of a dyadic perspective and add to the understanding of the reciprocal influences between the caregiver and recipient. This knowledge has clinical implications and contributes to the identification of possible vulnerable dyads in need of tailored support.</p

    Food, Mobility, and Health in a 17th and 18th Century Arctic Mining Population in Silbojokk, Swedish SĂĄpmi

    Established in 1635, the silver mine of NasafjĂ€ll and the smeltery site in Silbojokk in Swedish SĂĄpmi were used during several phases until the late 19th century. Excavations in Silbojokk, c. 40 km from NasafjĂ€ll, have revealed buildings such as a smeltery, living houses, a bakery, and a church with a churchyard. From the beginning, both local and non-local individuals worked at the mine and the smeltery. Non-locals were recruited to work in the mine and at the smeltery, and the local SĂĄmi population was recruited to transport the silver down to the Swedish coast. Females, males, and children of different ages were represented among the individuals buried at the churchyard in Silbojokk, which was used between c. 1635 and 1770. Here we study diet, mobility, and exposure to lead (Pb) in the smeltery workers, the miners, and the local population. By employing isotopic analysis, ÎŽ13C, ÎŽ15N, ÎŽ34S, 87Sr/86Sr and elemental analysis, we demonstrate that individuals in Silbojokk had a homogenous diet, except for two individuals. In addition, both local and non-local individuals were all exposed to Pb, which in some cases could have been harmful to their health.&nbsp;La mine d’argent de NasafjĂ€ll et la fonderie de Silbojokk dans le territoire SĂĄpmi en SuĂšde ont Ă©tĂ© Ă©tablies en 1635. Elles ont fait l’objet de plusieurs phases d’utilisation jusque vers la fin du 19e siĂšcle. Des fouilles effectuĂ©es Ă  Silbojokk, Ă  une quarantaine de kilomĂštres de NasafjĂ€ll, ont permis de dĂ©couvrir des structures comme une fonderie, des maisons d’habitation, une boulangerie ainsi qu’une Ă©glise et un cimetiĂšre. DĂšs le dĂ©but, des gens de la rĂ©gion ou d’ailleurs ont travaillĂ© Ă  la mine et Ă  la fonderie. Des gens d’ailleurs ont Ă©tĂ© recrutĂ©s pour travailler Ă  la mine et Ă  la fonderie, tandis que la population locale de SĂĄmis a Ă©tĂ© engagĂ©e pour transporter l’argent sur la cĂŽte suĂ©doise. Femmes, hommes et enfants d’ñges diffĂ©rents figurent parmi les personnes enterrĂ©es au cimetiĂšre de Silbojokk, utilisĂ© entre les annĂ©es 1635 et 1770 environ. Ici, nous Ă©tudions le rĂ©gime alimentaire, la mobilitĂ© et l’exposition au plomb (Pb) des ouvriers de la fonderie, des mineurs et de la population locale.     The physical environment and multi-professional teamwork in three newly built stroke units

    Purpose: To explore multi-professional teamwork in relation to the physical environment in three newly built or renovated stroke units. Materials and methods: An observational study was undertaken. The participants were all staff members of a multi-professional team working in the reviewed stroke units. The data were collected using behavioural mapping and semi-structured observations, and they were analysed by content analysis and descriptive statistics. Results: Out of all the observations in the behavioural mapping, very few were of two or more members from the team together with a patient. None of the included stroke units had a co-location for all the members of the multi-professional team. Three main categories emerged from the analysis of the interviews: (i) the hub of the unit; (ii) the division of places; and (iii) power imbalance. All the categories reflected the teamwork in relation to parts of the physical environment. Conclusion: The design of the physical environment is important for multi-professional teamwork. Emphasis must be placed on better understanding the impact of the physical environment and on incorporating the evidence related to multi-professional teamwork during the design of stroke units.IMPLICATIONS FOR REHABILITATION Understanding the link between the physical environment and effective teamwork can lead to more tailored and supportive design solutions. The design of the physical environment should be considered as a vital part of effective teamwork in stroke units. The physical environment should include shared workstations, allowing team members to meet and communicate face to face

    Endotoxin neutralization and anti-inflammatory effects of tobramycin and ceftazidime in porcine endotoxin shock

    INTRODUCTION: Antibiotics used for treatment of severe bacterial infections have been shown to exert effects on the inflammatory response in addition to their antibacterial effects. The aim of the present study was to investigate whether the biological effects of endotoxin in a porcine model could be neutralized by tobramycin, and whether tobramycin or ceftazidime was able to modulate the inflammatory response. METHOD: Thirteen piglets were subjected to endotoxin infusion at an initial rate of 4 Όg/kg per hour, which was reduced to 1 Όg/kg per hour after 30 min. Before endotoxin infusion, the animals received saline (n = 4), ceftazidime (n = 5), or tobramycin (n = 4) at clinically relevant doses. Physiological parameters were measured and blood samples were taken hourly for 6 hours for analysis of tumour necrosis factor-α, IL-6 and endotoxin concentrations. RESULTS: All of the animals exhibited physiological signs of severe sepsis without major differences between the groups. Plasma endotoxin concentration was stable after 1 hour. There were no differences in endotoxin concentration or initial tumour necrosis factor-α and IL-6 concentrations between the groups. At 6 hours the IL-6 concentration was significantly lower in the ceftazidime group than in the saline group (P < 0.05), and in both the ceftazidime and the tobramycin groups there were significantly greater reductions from peak values (P < 0.05). CONCLUSION: There was no neutralization of the biological effects of endotoxin in this porcine model. However, our data indicate a possible anti-inflammatory effect exerted by both ceftazidime and tobramycin, which manifested as a significantly greater reduction in IL-6 in comparison with the untreated group

    Serum concentrations of Thymidine kinase 1 measured using a novel antibody-based assay in patients with Hodgkin Lymphoma

    Background: Thymidine kinase 1 (TK1) is an intracellular protein associated with DNA synthesis, expressed during the G1 phase and remained elevated through the M phase, with a potential as a biomarker for cell proliferation. In this study, we explore the possible use of TK1 in Hodgkin lymphoma (HL).Methods: Serum concentrations of TK1 (S-TK1) were measured in 46 newly diagnosed HL patients using prospectively collected biobanked serum samples. The samples were analyzed using a novel antibody-based TK1 immunosorbent assay (ELISA).Results: The concentrations of S-TK1 were elevated in HL patients compared with healthy controls (median 0.32 mu g/L vs. 0.24 mu g/L, P = 0.003). A further increase in S-TK1 was observed during the treatment. The S-TK1 concentrations were higher in patients with advanced stage disease, low B-Hb, elevated P-LD and in those with B-symptoms. A high ESR correlated with low S-TK1.Conclusions: The study results suggest that S-TK1, measured using a novel antibody-based assay, has the potential to be a biomarker in HL. However, while S-TK1 levels are elevated at baseline compared with healthy controls, a limited number of patients and comparatively short follow-up time render reliable -conclusions difficult

    Life without professional work-perceptions about one’s self, interpersonal relations and social life after retirement

    The aim of this study is to understand how healthy, older adults in Sweden perceive their life situation after retirement. The study is based on a lifeworld approach, and a phenomenographic method was used. Eighteen participants were interviewed, and data were analysed according to the phenomenographic principle of qualitatively different categories. Two categories were developed. The first category, “perceptions that draw attention inward, towards one’s self”, was further described in three subcategories: Sense of decreased status in society, the desire to keep aging at a distance, and contemplation of one’s own existence. The second category, “perceptions that draw attention outward, away from one’s self” was further described in the following four subcategories: caretaking of family members, involvement in social relationships, finding of deep meaning in animals and nature and engagement with society. In the discussion, the findings are further illuminated through comparisons with concepts such as maturity, wisdom and gerotranscendence, and reflections on the findings’ relevance to a caring context follow. The conclusion suggests this study can provide knowledge that will allow healthcare providers to bridge the gap between generations in order to provide high-quality care. However, for a more profound caring dialogue, for example, about the end of life, a deeper analysis is required

    Relationship between occupational gaps in everyday life, depressive mood and life satisfaction after acquired brain injury

    Objective: To explore the relationship between occupational gaps, depressive mood and life satisfaction in persons who have acquired a brain injury during the past 1–4 years and to test the Occupational Gaps Questionnaire. Design: A cross-sectional study. Subject: A total of 116 persons with traumatic brain injury or subarachnoid haemorrhage acquired 1-4 years previously. Methods: A postal survey with questions on occupational gaps, focusing on the domains instrumental activities of daily living, social life, leisure and work (Occupational Gaps Questionnaire), life satisfaction (LiSat-11 checklist) and depressive mood (Hospital Anxiety and Depression Scale). Rasch analyses and principal component analyses were performed to ensure that data from the LiSat-11 and Occupational Gaps Questionnaire could be used subsequently as valid unidimensional measures in regression and correlational analyses. Results: Calibration of the Occupational Gaps Questionnaire and the LiSat-11 revealed that the items and persons demonstrated acceptable goodness-of-fit to the Rasch models respectively, supporting internal scale validity and person-response validity. In addition, principal component analyses revealed that the measures could be used as valid uni-dimensional estimations of occupational gaps and life satisfaction. There was a strong relationship between the extent of occupational gaps and perceived life satisfaction, a weaker relationship with depressive mood and a non-significant relationship with the aetiological diagnoses and life satisfaction. The factors explained 32% (occupational gaps), 6% (depressive mood), and 2% (diagnosis), respectively, of the total explained variance (40%). Conclusion: There was a strong correlation between participation in desired everyday occupations and life satisfaction 1–4 years after an acquired brain injury. This indicates that individually perceived occupational gaps, as recorded by the Occupational Gaps Questionnaire, could be relevant targets for tailored interventions in order to improve life satisfaction among clients with acquired brain injuries

    Compound-specific amino acid isotopic proxies for detecting freshwater resource consumption

    AbstractOf central importance to palaeodietary reconstruction is a clear understanding of relative contributions of different terrestrial (i.e., C3 vs. C4 plants) and aquatic (i.e., freshwater vs. marine) resources to human diet. There are, however, significant limitations associated with the ability to reconstruct palaeodiet using bulk collagen stable isotope compositions in regions where diverse dietary resources are available. Recent research has determined that carbon-isotope analysis of individual amino acids has considerable potential to elucidate dietary protein source where bulk isotopic compositions cannot. Using ή13CAA values for human and faunal remains from Zvejnieki, Latvia (8th – 3rd millennia BCE), we test several isotopic proxies focused on distinguishing freshwater protein consumption from both plant-derived and marine protein consumption. We determined that the Δ13CGly-Phe and Δ13CVal-Phe proxies can effectively discriminate between terrestrial and aquatic resource consumption, and the relationship between essential ή13CAA values and the Δ13CGly-Phe and Δ13CVal-Phe proxies can differentiate among the four protein consumption groups tested here. Compound-specific amino acid carbon-isotope dietary proxies thus enable an enhanced understanding of diet and resource exploitation in the past, and can elucidate complex dietary behaviour
