    The proximal ulnar perforator flap (PUPF) for hand digital reconstruction: an anatomical study

    Degloving injuries of the fingers represent a reconstructive challenge. Even if poorly described in literature, the proximal ulnar perforator flap (PUPF), based on perforator of the anterior ulnar recurrent artery or directly on a perforator branch of the ulnar artery, meets the requested criteria for the ideal coverage. We performed a cadaveric study in order to clarify the anatomical basis and vascularization of the PUPF flap. Eight injected upper limb specimens were dissected for this study: perforators were followed down to their origin and classified in terms of number, length, diameters and distances between their emergence and specific pre-determined landmarks as the medial humeral epicondyle. At least one ulnar perforator in the proximal third of the forearm was identified in all the specimens. In 50% of the upper limbs, the perforator branch came directly from the ulnar artery, while in the 87.5% a perforator branch came from the anterior recurrent ulnar artery; in 3 out of 8 cases both perforator branches were described. Mean lengths of the perforator branch were 57.9 mm and 44.3 mm, respectively and the mean diameters measured at their origin were 0.99 mm and 1.17 mm respectively. Our data illustrate the consistency of at least one perforator branch from the proximal third of the ulnar artery, most commonly coming from the anterior recurrent ulnar artery. Considering our results, the PUPF could be a good alternative to the classical free flaps for the resurfacing of the finger defects

    Moisture-Dependent Engineering Properties of Chia (Salvia hispanica L.) Seeds

    Salvia hispanica L., whose common name is chia, is an annual herbaceous plant belonging to the Lamiaceae or Labiatae family. This botanical species, native to southern Mexico and north‐ ern Guatemala, was an important crop in pre-Columbian Mesoamerica in conjunction with corn, beans and amaranth. Chia seeds were valuated not only for food, but also for medi‐ cines and paints [1]. Its cultivation was banned by Spanish conquerors and replaced by exot‐ ic crops (wheat and barley) [2]. Nowadays, chia seeds are being reintroduced to western diets in order to improve human health. These seeds have been investigated and recommended due to their oil content with the highest proportion of α-linolenic acid (omega-3) compared to other natural source known to date [3, 4], and also because of their high levels of protein, antioxidant, dietary fiber, vita‐ mins and minerals [5, 6]. Chia seeds from Argentina exhibited 30.0 - 38.6 g oil/100 g, with 60.7 - 67.8 g/100 g of α-linolenic acid [7, 8]. Figure 1 shows the chemical composition of chia seed [9]. Chia seed is traditionally consumed in Mexico, the southwestern U.S., and South America, but it is not widely known in Europe. However, in 2009, the European Union approved chia seeds as a novel food, allowing them to comprise up to 5% of a bread product´s total matter [10]. Today, chia is mostly grown in Mexico, Bolivia, Argentina, Ecuador, Australia, and Guatemala, and it has been demonstrated that the species has great potential as a future crop plant [7, 11].Salvia hispanica fruit consist in four nutlets, similar to an indehiscent achene, which contains a single seed. These nutlets, are commercially named as seeds, and in the text, we will use this last term. The plant produces small white and dark seeds. Most of chia population that is commercially grown today contains a low percentage of white seeds. Their shapes are oval and in general, the white seeds are somewhat larger than the black ones. Ixtaina et al. [12], reported length, width and thickness value of 2.11, 1.32 and 0.81 mm for dark seeds and 2.15, 1.40 and 0.83 mm for white seeds, respectively. Chia seeds are shown in Figure 2. Photographs of dark and white chia seeds (13x) The knowledge of engineering properties constitutes important and essential data for the design of machines, storage structures, and processes. The value of this basic information is not only important to engineers but also to food scientists, processors, and other scientists who may exploit these properties and find new uses.Engineering seed properties and their dependence on moisture content are necessary in the design of equipment for transporting, storage and/or processing. The knowledge of the mor‐ phology and size distribution of chia seeds is essential for the adequate design of the equip‐ ment for cleaning, grading and separation. Gravimetric properties are useful for the design of equipment related to aeration, drying, storage and transport. Bulk density determines the capacity of storage and transport systems while true density is useful for separation equip‐ ment; porosity of the mass of seeds determines the resistance to airflow during the aeration and drying of seeds. The frictional properties, such as the angle of repose and the static coef‐ ficient of friction, are important for the design of grain bins and other storage structures whose operation is influenced by the compressibility and flow behaviour of materials. Sev‐ eral researchers investigated the moisture dependence of engineering properties of seed or grain and reported different behaviour of these properties as a function moisture content. The aim of this work was to evaluate the engineering properties of dark chia seed as a func‐ tion of the moisture content and to compare their behavior with that of other grains.Fil: Guiotto, Estefania Nancy.     Indications, outcomes, and complications of neoumbilical reconstruction: A systematic review.

    Neoumbilicoplasty aims to reconstruct an aesthetically pleasing new umbilicus following agenesis, malignancy, anatomical distortion, or umbilicus loss. Despite the wide variety of surgical techniques described, literature is scarce when it comes to standardized categorization of these as well as the clear definition of patients' selections, specific indications, final outcomes, and possible complications. According to available literature, this work aims to evaluate different surgical approaches, and correlate them to specific surgical needs, to simplify the surgical choice and patient management. A systematic review was performed in December 2020 in PubMed, Web of Science, and MedLine Ovid databases according to the PRISMA guidelines. A total of 41 studies and 588 patients were finally included. On the basis of the evidence of the literature collected, we divided the studies into four groups according to the neoumbilicoplasty techniques: single suture or purse-string suture, single flap, multiple flap, and skin graft. Patients' surgical comorbidities, neoumbilicoplasty indications, and aesthetic and surgical outcomes were investigated. Direct suture and single and multiple flap techniques assured overall, satisfactory cosmetic outcomes with a low rate of surgical complications. Whereas suture-only techniques were chosen mostly by general surgeons/urologists in laparoscopic surgery, the single flap was the preferred method to reconstruct the umbilicus in open abdominal surgery or combined abdominoplasty with herniorrhaphy. Multiple flap and skin grafts were adopted in abdominoplasty-related umbilicus reconstruction, although the latter option showed impactful aesthetic and surgical complications. Umbilicoplasty can assure generally pleasant aesthetic outcomes with relatively low complication rates.     Human platelet lysate as a potential clinical-translatable supplement to support the neurotrophic properties of human adipose-derived stem cells

    Background: The autologous nerve graft, despite its donor site morbidity and unpredictable functional recovery, continues to be the gold standard in peripheral nerve repair. Rodent research studies have shown promising results with cell transplantation of human adipose-derived stem cells (hADSC) in a bioengineered conduit, as an alternative strategy for nerve regeneration. To achieve meaningful clinical translation, cell therapy must comply with biosafety. Cell extraction and expansion methods that use animal-derived products, including enzymatic adipose tissue dissociation and the use of fetal bovine serum (FBS) as a culture medium supplement, have the potential for transmission of zoonotic infectious and immunogenicity. Human-platelet-lysate (hPL) serum has been used in recent years in human cell expansion, showing reliability in clinical applications. Methods: We investigated whether hADSC can be routinely isolated and cultured in a completely xenogeneic-free way (using hPL culture medium supplement and avoiding collagenase digestion) without altering their physiology and stem properties. Outcomes in terms of stem marker expression (CD105, CD90, CD73) and the osteocyte/adipocyte differentiation capacity were compared with classical collagenase digestion and FBS-supplemented hADSC expansion. Results: We found no significant differences between the two examined extraction and culture protocols in terms of cluster differentiation (CD) marker expression and stem cell plasticity, while hADSC in hPL showed a significantly higher proliferation rate when compared with the usual FBS-Added medium. Considering the important key growth factors (particularly brain-derived growth factor (BDNF)) present in hPL, we investigated a possible neurogenic commitment of hADSC when cultured with hPL. Interestingly, hADSC cultured in hPL showed a statistically higher secretion of neurotrophic factors BDNF, glial cell-derived growth factor (GDNF), and nerve-derived growth factor (NFG) than FBS-cultured cells. When cocultured in the presence of primary neurons, hADSC which had been grown under hPL supplementation, showed significantly enhanced neurotrophic properties. Conclusions: The hPL-supplement medium could improve cell proliferation and neurotropism while maintaining stable cell properties, showing effectiveness in clinical translation and significant potential in peripheral nerve research

    HelixComplex snail mucus as a potential technology against O3 induced skin damage

    Mucus form H. aspersa muller has been reported to have several therapeutic proprieties, such as antimicrobial activity, skin protection and wound repair. In this study, we have analyzed H. aspersa mucus (Helixcomplex) bio-adhesive efficacy and its defensive properties against the ozone (O3) (0.5 ppm for 2 hours) exposure in human keratinocytes and reconstructed human epidermis models. Cytotoxicity, tissue morphology and cytokine levels were determined. We confirmed HelixComplex regenerative and bio-adhesive properties, the latter possibly via the characteristic mucopolysaccharide composition. In addition, HelixComplex was able to protect from O3 exposure by preventing oxidative damage and the consequent pro-inflammatory response in both 2D and 3D models. Based on this study, it is possible to suggest HelixComplex as a potentially new protective technology against pollution induced skin damage

    Determinants of muscle carnosine content

    The main determinant of muscle carnosine (M-Carn) content is undoubtedly species, with, for example, aerobically trained female vegetarian athletes [with circa 13 mmol/kg dry muscle (dm)] having just 1/10th of that found in trained thoroughbred horses. Muscle fibre type is another key determinant, as type II fibres have a higher M-Carn or muscle histidine containing dipeptide (M-HCD) content than type I fibres. In vegetarians, M-Carn is limited by hepatic synthesis of β-alanine, whereas in omnivores this is augmented by the hydrolysis of dietary supplied HCD’s resulting in muscle levels two or more times higher. β-alanine supplementation will increase M-Carn. The same increase in M-Carn occurs with administration of an equal molar quantity of carnosine as an alternative source of β-alanine. Following the cessation of supplementation, M-Carn returns to pre-supplementation levels, with an estimated t1/2 of 5–9 weeks. Higher than normal M-Carn contents have been noted in some chronically weight-trained subjects, but it is unclear if this is due to the training per se, or secondary to changes in muscle fibre composition, an increase in β-alanine intake or even anabolic steroid use. There is no measureable loss of M-Carn with acute exercise, although exercise-induced muscle damage may result in raised plasma concentrations in equines. Animal studies indicate effects of gender and age, but human studies lack sufficient control of the effects of diet and changes in muscle fibre composition

    Penelitian ini bertujuan untuk mengetahui sejauh mana sistem pengendalian intern penerimaan dan pengeluaran kas yang dilakukan PT. Asuransi Bringin Sejahtera Artamamakmur Cabang Medan dalam menjalankan kegiatan usahanya guna mencapai tujuan perusahaan. Teori yang digunakan dalam penelitian ini adalah teori Mulyadi. Metode yang digunakan dalam penelitian ini adalah Penelitian Kualitatif, dengan teknik analisis data Deskriptif, yaitu dengan terlebih dahulu mengumpulkan data, mengklasifikasikan serta menafsirkan data sehingga dapat memberikan gambaran yang jelas mengenai masalah yang diteliti. Dalam pengumpulan data menggunakan teknik wawancara dan dokumentasi, dimana data yang diambil merupakan data primer yang berupa bukti catatan atau laporan historis yang telah tersusun dalam arsip yang dipublikasi maupun yang tidak dipublikasikan. Hasil penelitian ini menunjukan bahwa pengendalian intern penerimaan dan pengeluaran kas PT. Asuransi Bringin Sejahtera Artamakmur Cabang Medan belum sepenuhnya memenuhi unsur-unsur pengendalian intern, karena masih adanya beberapa hal yang tidak sesuai dengan teori yang ada. Diantaranya, masih ada perangkapan tugas yang dilakukan bagian kasir dengan bagian akuntansi. Demikian juga dalam hal pembagian tugas, dimana bagian kasir diberi tanggung jawab dalam hal penagihan premi, yang sebaiknya tugas ini bukan dipegang unit tersebut. Praktik-praktik ini ditakutkan akan mengurangi keakuratan pencatatan dan juga akan membuka peluang terjadinya penyelewengan terhadap kas yang mengakibatkan kerugian perusahaan. Hal ini juga akan mengurangi keefektifan dalam praktik kerja bagian kasir, karena terlalu banyak memegang fungsi
