Avaluació del Programa de Col·laboració entre Atenció Primària i Salut Mental en La Ribera
És patent que els serveis d’AP s’han convertit no solament en el punt d’entrada del sistema sanitari, sinó també en el principal medi des d’on les necessitats i demandes en problemes mentals són rebudes i en molts casos ateses.
Més investigacions sobre l’eficàcia dels diversos elements dels models de col•laboració entre AP i SM centrats en els pacients psiquiàtrics i intervencions són necessàries, existint una mancança general d’aquestos estudis i de la seua aplicació local en Europa. A més a més, cal investigar la relació de psiquiatria amb atenció primària en depressió però també en altres malalties mentals molt prevalents en AP com els trastorns ansiosos, poc estudiats en la literatura.
En l’àrea sanitària de La Ribera es duu a terme una col•laboració entre MAP i SM d’anys d’evolució, però fins recentment no s’han intentat igualar els mètodes i docència en els diversos sectors inclosos. L’avaluació de la col•laboració és necessària (Younès et al., 2008).
En aquesta tesi es descriu la posada en marxa de mètodes d’avaluació una vegada es porte a terme l’homogenització de tota l’àrea del model d’enllaç emprat. Aquesta avaluació pot ser considerada per l’impacte en els patrons de derivacions d’AP a SM, per la capacitat del MAP i l’habilitat per tal de detectar i tractar malalties mentals que no són derivades a SM i per la qualitat dels resultats d’aquestes intervencions. Tanmateix, fins ara no s’han establert mecanismes de mesura tàcits i perdurables que aconseguisquen donar llum sobre els suposats beneficis de l’aplicació del model d’enllaç en l’Àrea de La Ribera.
Donada la importància descrita de la relació entre AP i SM i la necessitat d’una adient comunicació entre aquestes dos vessants, es considera que primer cal estudiar la derivació de MAP a SM i com incideix el model emprat en l’adequada derivació. En aquest sentit s’estudiaran les derivacions fetes a l’àrea, es farà una intervenció reglada homogènia sobre l’adequació de les derivacions i finalment es tornarà a valorar la qualitat de les derivacions.
Seguidament i en relació a estudiar les abans descrites com malalties psiquiàtriques més freqüents en AP, s’analitzaran diversos ítems referents a l’atenció a síndromes depressius i ansiosos i com incideix al respecte el model d’enllaç i les intervencions proposades. A més a més, l’habilitat dels MAP alhora de valorar pacients psiquiàtrics pot variar molt (Bakker et al., 2009) i és per això l’èmfasi amb que s’ha proposat la forma metòdica d’abordament dels processos depressius i ansiosos.
La tesi respondrà a la necessitat d’estudis que puguen ser traslladats al “mon real” ja demandats en previs estudis (Younès et al., 2008). És per això que no busca respondre a la pregunta de si és efectiu el mode de relació entre MAP i SM (ja estudiat abans com s’exposa en esta introducció), sinó mostrar l’aplicació i resultats en eixe “món real” i analitzar tant la forma més freqüent de relació entre AP i SM (derivació) com les patologies més freqüents que necessiten coordinació (ansietat i depressió).It is known that Primary Health Care has become not only the main entrance of the health system but also the place where more often mental health problemes are assessed and treated. More studies about the efficacy of the different models of collaboration between Primary Health Care and Psichiatric Services are needed, specially in Europe. Furthermore, it is needed to know how is assessed and treated depression and anxiety depending on the relationship between Primary Care and Psychiatric Services. This study aims to analyze the ongoing relationship between those services in La Ribera (Spain), where all over it is used a link model. It has been analyzed de referrals from Primary Health Care to Psychiatric Services and also how Genenal Practitioners assess depression and anxiety. It has been also proposed several scales in order to assess better those patologies and as a method of screening
The economic burden of localized prostate cancer and insights derived from cost-effectiveness studies of the different treatments
Prostate cancer is one of the most frequent and impacting malignant neoplasms for men. In particular, localized prostate cancer has a notably high incidence and prevalence, despite which a solid consensus on treatment and procedure of care has not yet been reached. This article aims to shed light on this challenge by characterizing the economic burden and cost-effectiveness of different treatment strategies for localized prostate cancer after analyzing published comparable data from studies conducted in OECD countries
Effectiveness of a transdiagnostic internet-based protocol for the treatment of emotional disorders versus treatment as usual in specialized care: study protocol for a randomized controlled trial
Background
Emotional disorders (depression and anxiety disorders) are highly prevalent mental health problems. Although evidence showing the effectiveness of disorder-specific treatments exists, high comorbidity rates among emotional disorders limit the utility of these protocols. This has led some researchers to focus their interest on transdiagnostic interventions, a treatment perspective that might be more widely effective across these disorders. Also, the current way of delivering treatments makes it difficult provide assistance to all of the population in need. The use of the Internet in the delivery of evidence-based treatments may help to disseminate treatments among the population. In this study, we aim to test the effectiveness of EmotionRegulation, a new transdiagnostic Internet-based protocol for unipolar mood disorders, five anxiety disorders (panic disorder, agoraphobia, social anxiety disorder, generalized anxiety disorder and anxiety disorder not otherwise specified), and obsessive-compulsive disorder in comparison to treatment as usual as provided in Spanish public specialized mental health care. We will also study its potential impact on basic temperament dimensions (neuroticism/behavioral inhibition and extraversion/behavioral activation). Expectations and opinions of patients about this protocol will also be studied.
Methods/Design
The study is a randomized controlled trial. 200 participants recruited in specialized care will be allocated to one of two treatment conditions: a) EmotionRegulation or b) treatment as usual. Primary outcome measures will be the BAI and the BDI-II. Secondary outcomes will include a specific measure of the principal disorder, and measures of neuroticism/behavioral inhibition and extraversion/behavioral activation. Patients will be assessed at baseline, post-treatment, and 3- and 12-month follow-ups. Intention to treat and per protocol analyses will be performed.
Discussion
Although the effectiveness of face-to-face transdiagnostic protocols has been investigated in previous studies, the number of published transdiagnostic Internet-based programs is still quite low. To our knowledge, this is the first randomized controlled trial studying the effectiveness of a transdiagnostic Internet-based treatment for several emotional disorders in public specialized care. Combining both a transdiagnostic approach with an Internet-based therapy format may help to decrease the burden of mental disorders, reducing the difficulties associated with disorder-specific treatments and facilitating access to people in need of treatment. Diagnóstico y prevalencia de trastornos de la personalidad en atención ambulatoria. Estudio descriptivo.
Introducción: los trastornos de personalidad (TP) son difíciles de diagnosticar. La literatura científica informa de su elevada prevalencia en muestras clínicas, pero en la práctica clínica diaria su diagnóstico es muy poco frecuente. Objetivos: Este estudio pretende estimar la prevalencia de TP en una Unidad ambulatoria de Salud Mental (USM), en situación real, para primeras citas. Método: Se aplicó el Cuestionario Salamanca (CS) como instrumento de cribado y, con los puntos de corte mínimo (2/3) y máximo (5/6), se comparó con el diagnóstico clínico realizado por un psicólogo clínico o psiquiatra, sin conocer el resultado del cribado con CS. Resultados: El diagnóstico de TP realizado por un profesional fue de un 7%, frente al 100% de positivos obtenido por el CS en el punto de corte mínimo y el 53% con el máximo: 11,8% para Clúster A, 19,8% Clúster B y 21,4% en el C. Conclusiones: La elevada prevalencia de TPs en los centros de salud mental, junto con el infradiagnóstico clínico, son otros factores que ensombrecen el pronóstico de estos pacientes
Comparative efficacy of two primary care interventions to assist withdrawal from long term benzodiazepine use: A protocol for a clustered, randomized clinical trial
<p>Abstract</p> <p>Background</p> <p>Although benzodiazepines are effective, long-term use is not recommended because of potential adverse effects; the risks of tolerance and dependence; and an increased risk of hip fractures, motor vehicle accidents, and memory impairment. The estimated prevalence of long-term benzodiazepine use in the general population is about 2,2 to 2,6%, is higher in women and increases steadily with age. Interventions performed by General Practitioners may help patients to discontinue long-term benzodiazepine use. We have designed a trial to evaluate the effectiveness and safety of two brief general practitioner-provided interventions, based on gradual dose reduction, and will compare the effectiveness of these interventions with that of routine clinical practice.</p> <p>Methods/Design</p> <p>In a three-arm cluster randomized controlled trial, general practitioners will be randomly allocated to: a) a group in which the first patient visit will feature a structured interview, followed by visits every 2-3 weeks to the end of dose reduction; b) a group in which the first patient visit will feature a structured interview plus delivery of written instructions to self-reduce benzodiazepine dose, or c) routine care. Using a computerized pharmaceutical prescription database, 495 patients, aged 18-80 years, taking benzodiazepine for at least 6 months, will be recruited in primary care health districts of three regions of Spain (the Balearic Islands, Catalonia, and Valencia). The primary outcome will be benzodiazepine use at 12 months. The secondary outcomes will include measurements of anxiety and depression symptoms, benzodiazepine dependence, quality of sleep, and alcohol consumption.</p> <p>Discussion</p> <p>Although some interventions have been shown to be effective in reducing benzodiazepine consumption by long-term users, the clinical relevance of such interventions is limited by their complexity. Fraseología relacionada con psiquiatría y psicología del español coloquial y estigma en salud mental.
Introduction: The use of language opens a window to the patient’s thinking and
affectivity. The clinical interview is the main instrument of psychiatric practice and where
elements related to stigma can sometimes be detected.
Material and methods: A qualitative-descriptive study was carried out to find out the 10
most frequent phraseological units related to psychiatry/psychology that were detected
in the medical outpatient unit of psychiatry and psychology in a mental health center
in La Ribera (Valencia, Spain). The origin and meaning of said phraseology and its
relationship with the items of the AQ-27 stigma scale were analyzed.
Results: The most used phraseological unit was: “Depresión de caballo”, “estar como una
cabra”, “tonto de capirote”, “tener un día de locos”, “estar como una regadera”, “estar
en Babia”, “irse la pinza”, “faltar un tornillo”, “cruzarse los cables” and “majadero”. The
different stigma associated with phraseological units described were dangerousness,
segregation, avoidance, fear and anger.
Conclusions: Perhaps today’s society may have more interest in moving away from the
problems related to mental illness than trying to understand them or thinking about how
to deal with them. It would be convenient to have more studies on the social concepts
related to mental illness in order to better act on the stigma of mental illness and also to
provide the population with accurate information about mental pathology.Introducción: El empleo del lenguaje abre una ventana al pensamiento y la afectividad
del paciente. La entrevista clínica es el principal instrumento de la práctica psiquiátrica y
donde a veces se pueden detectar elementos relacionados con el estigma.
Material y método: Estudio descriptivo de los 10 elementos fraseológicos relacionados
con psiquiatría/psicología más frecuentes detectados en las consultas médicas de
psiquiatría/psicología en un centro de salud mental en La Ribera (Valencia, España).
Se analizó el origen y significado de dicha fraseología y su relación con los ítems de la
escala de estigma AQ-27.
Resultados: La fraseología más utilizada fue: Depresión de caballo, estar como una
cabra, tonto de capirote, tener un día de locos, estar como una regadera, estar en Babia,
irse la pinza, faltar un tornillo, cruzarse los cables y majadero. Los estigmas asociados
a la fraseología han sido peligrosidad, segregación, miedo, evitación e ira. Los estigmas

Conclusiones: Quizá la sociedad actual pueda tener más interés en apartarse de los
problemas relacionados con la enfermedad mental que intentarlos entender o pensar
en cómo abordarlos. Sería conveniente tener más estudios sobre los conceptos sociales
relacionados con la enfermedad mental para así poder actuar mejor sobre el estigma en
la enfermedad mental y también proveer a la población de información veraz sobre la
patología mental
Revisión sobre la relación y derivaciones entre Atención Primaria y Salud Mental
Objetivo: Profundizar en las relaciones entre Atención Primaria (AP) y Salud Mental (SM). Saber cómo se deriva de AP a SM y cómo mejorar este aspecto. Diseño: Revisión sistemática en forma de estudio bibliométrico sobre la relación entre AP y SM y sobre las derivaciones de AP a SM. Fuentes de datos: Bases bibliográficas Pubmed y Cochrane. Se acotó la búsqueda a 5 ó 10 años. Se añadieron artículos más antiguos referenciados en la primera búsqueda si eran relevantes. Se empleó una combinación de términos Mesh y termas en texto libre con operador “booleanos”. Se priorizaron los estudios de nuestro entorno, los de gran tamaño muestral, aportaciones más novedosas o los que describían intervenciones. Resultados: Se encontraron 6979 artículos. Después de la selección se estudiaron 61 artículos. Conclusiones: La variabilidad y carencias metodológicas constatadas en gran parte de los estudios sobre la relación entre AP y SM son notables, destacando la evaluación de resultados a largo plazo.
