    The panamanian climbing rat, mammalia, rodentia, cricetidae, Tylomys panamensis (Gray, 1873): new report in Darien

    This study reports a new record for Tylomys panamensis, increasing its range by 121 km, 66º NW from the original endemic distribution. It has been considered as Data Deficient by IUCN as no information has been published for almost 64 years. Based on our long term surveillance at the Darien’s canopy, using Orion Camera System (OCS), we have been able to obtain information about their circadian and monthly activity over the course of one year. The presence of T. panamensis remarks the importance of the largest National Park of the Mesoamerican region and claim urgency to be protected from its actual devastation, particularly the montane forest in the rest of the provinces of Darien and Panama. El estudio informa un nuevo avistamiento para Tylomys panamensis, incrementando su rango a 121 km, 66º NW de su distribución original. Esta especie ha sido considerada como Data Deficiente por UICN dado que no habría información publicada por casi 64 años. Basados en nuestra vigilancia de dosel en Darién a largo plazo utilizando el Sistema de Cámaras Orión (SCO), hemos podido obtener información de su actividad circadiana y actividad mensual por espacio de un año. La presencia de T. panamensis confirma la importancia del mayor Parque Nacional de la región Mesoamericana clamando con urgencia a ser protegido de su actual devastación, y en particular los bosques montañosos del resto de las provincias de Darién y Panamá

    BEHAVIOURAL NOTES OF Zentrygon goldmani oreas (NELSON, 1912) AND OTHER WILD DOVES AT THE CHUCANTÍ PRIVATE NATURAL RESERVE (CPNR), DARIEN, PANAMA

    The province of Darien in eastern Panama is well known for its endemic doves.  Most of them are poorly studied and threatened. The aim of this study was to recognize the wild dove’s diversity from CPNR and describe an ethological baseline. We compiled 38,592-day hours, with information on circadian activity and behavioral data from camera traps from December 2012 to May 2014 in a cloud forest. We detected four species of wild doves and obtained information on circadian activity for the most frequent species detected. An ethogram was built for Zentrygon goldmani and Leptotila cassinii in five categories: transversal walking, foraging, courtship, copulation and walking together. Courtship occurred during December, February, July and September, in both the dry and rainy seasons. We also detected Geotrygon montana and Zentrygon lawrencii for the Pacific slope of Panama and for the Chucanti Private Natural Reserve. The diversity found regarding wild doves represents a unique opportunity to study this endemic species at CPNR, and another important reason for its conservation

    La Provincia de Contumazá pertenece a una de las trece que conforman el Departamento de Cajamarca, que se localiza en el norte del País, se divide en ocho distritos, tiene poco desarrollo educativo, la infraestructura vial no favorece la comunicación interna y externa, y una mala distribución de recursos y desarrollo de su población local han impedido que la provincia se desarrolle plenamente. Ante esta situación, se desarrolla el presente Planeamiento Estratégico, el cual propone que existe un gran potencial en la región, y en especial en la provincia de Contumazá. Con el soporte del Modelo Secuencial diseñado por el Dr. Fernando D’Alessio Ipinza, se parte de un análisis de la situación actual y se plantea la visión de “ser una de las tres provincias más competitivas de Cajamarca en el 2027”. A partir de esta visión, misión, valores, objetivos de largo plazo, objetivos de corto plazo y se plantean las estrategias necesarias para aprovechar las oportunidades del entorno y disminuir sus amenazas, así como para explotar las fortalezas y combatir las debilidades. El ejecutar las estrategias planteadas implicará el éxito del Plan, al alcanzar los objetivos planteados, los cuales son viables y deberán contar con la aceptación de los involucrados. Los interesados deberán hacer propios los compromisos y metas propuestas, y responder así a los intereses de desarrollo general de la comunidad contumacinaThe Province of Contumazá is one of the thirteen provinces that make up the Region of Cajamarca. It’s located in the North of the Country and it’s divided in eight districts. It has little educational development, and the poor road infrastructure does not favor the internal and external communications. This, and a poor wealth distribution have prevented the province from developing properly. To address the current situation, this Strategic Plan has been developed focusing on the existence of great potential in the region, and particularly in the province of Contumazá. With the aid of the Sequential Model designed by Dr. Fernando D’Alessio Ipinza, this strategic plan commences with an analysis of the current situation, and formulates a vision for the Province: “To be one of the three most competitive provinces of Cajamarca by 2027”. This vision allows to define a mission, values, long-term objectives, short-term objectives and strategies considering the environmental opportunities and threats, as well as the province’s inner strengths and weaknesses. The execution of the proposed strategies will achieve the Strategic Plan’s objectives within the stipulated timeframe of ten years. These strategies are viable and should have the acceptance of all stakeholders, so that they take ownership of the commitments and goals proposed for the achievement of the strategic objectives for the development of the Province of ContumazáTesi

    SELNET clinical practice guidelines for bone sarcoma

    Bone sarcoma are infrequent diseases, representing < 0.2% of all adult neoplasms. A multidisciplinary management within reference centers for sarcoma, with discussion of the diagnostic and therapeutic strategies within an expert multidisciplinary tumour board, is essential for these patients, given its heterogeneity and low frequency. This approach leads to an improvement in patient's outcome, as demonstrated in several studies. The Sarcoma European Latin-American Network (SELNET), aims to improve clinical outcome in sarcoma care, with a special focus in Latin-American countries. These Clinical Practice Guidelines (CPG) have been developed and agreed by a multidisciplinary expert group (including medical and radiation oncologist, surgical oncologist, orthopaedic surgeons, radiologist, pathologist, molecular biologist and representatives of patients advocacy groups) of the SELNET consortium, and are conceived to provide the standard approach to diagnosis, treatment and follow-up of bone sarcoma patients in the Latin-American context

    Cardiovascular disease, chronic kidney disease, and diabetes mortality burden of cardiometabolic risk factors from 1980 to 2010: A comparative risk assessment

    Background: High blood pressure, blood glucose, serum cholesterol, and BMI are risk factors for cardiovascular diseases and some of these factors also increase the risk of chronic kidney disease and diabetes. We estimated mortality from cardiovascular diseases, chronic kidney disease, and diabetes that was attributable to these four cardiometabolic risk factors for all countries and regions from 1980 to 2010. Methods: We used data for exposure to risk factors by country, age group, and sex from pooled analyses of population-based health surveys. We obtained relative risks for the effects of risk factors on cause-specific mortality from meta-analyses of large prospective studies. We calculated the population attributable fractions for each risk factor alone, and for the combination of all risk factors, accounting for multicausality and for mediation of the effects of BMI by the other three risks. We calculated attributable deaths by multiplying the cause-specific population attributable fractions by the number of disease-specific deaths. We obtained cause-specific mortality from the Global Burden of Diseases, Injuries, and Risk Factors 2010 Study. We propagated the uncertainties of all the inputs to the final estimates. Findings: In 2010, high blood pressure was the leading risk factor for deaths due to cardiovascular diseases, chronic kidney disease, and diabetes in every region, causing more than 40% of worldwide deaths from these diseases; high BMI and glucose were each responsible for about 15% of deaths, and high cholesterol for more than 10%. After accounting for multicausality, 63% (10·8 million deaths, 95% CI 10·1-11·5) of deaths from these diseases in 2010 were attributable to the combined effect of these four metabolic risk factors, compared with 67% (7·1 million deaths, 6·6-7·6) in 1980. The mortality burden of high BMI and glucose nearly doubled from 1980 to 2010. At the country level, age-standardised death rates from these diseases attributable to the combined effects of these four risk factors surpassed 925 deaths per 100 000 for men in Belarus, Kazakhstan, and Mongolia, but were less than 130 deaths per 100 000 for women and less than 200 for men in some high-income countries including Australia, Canada, France, Japan, the Netherlands, Singapore, South Korea, and Spain. Interpretation: The salient features of the cardiometabolic disease and risk factor epidemic at the beginning of the 21st century are high blood pressure and an increasing effect of obesity and diabetes. The mortality burden of cardiometabolic risk factors has shifted from high-income to low-income and middle-income countries. Lowering cardiometabolic risks through dietary, behavioural, and pharmacological interventions should be a part of the global response to non-communicable diseases.     Impact of COVID-19 on cardiovascular testing in the United States versus the rest of the world

    Objectives: This study sought to quantify and compare the decline in volumes of cardiovascular procedures between the United States and non-US institutions during the early phase of the coronavirus disease-2019 (COVID-19) pandemic. Background: The COVID-19 pandemic has disrupted the care of many non-COVID-19 illnesses. Reductions in diagnostic cardiovascular testing around the world have led to concerns over the implications of reduced testing for cardiovascular disease (CVD) morbidity and mortality. Methods: Data were submitted to the INCAPS-COVID (International Atomic Energy Agency Non-Invasive Cardiology Protocols Study of COVID-19), a multinational registry comprising 909 institutions in 108 countries (including 155 facilities in 40 U.S. states), assessing the impact of the COVID-19 pandemic on volumes of diagnostic cardiovascular procedures. Data were obtained for April 2020 and compared with volumes of baseline procedures from March 2019. We compared laboratory characteristics, practices, and procedure volumes between U.S. and non-U.S. facilities and between U.S. geographic regions and identified factors associated with volume reduction in the United States. Results: Reductions in the volumes of procedures in the United States were similar to those in non-U.S. facilities (68% vs. 63%, respectively; p = 0.237), although U.S. facilities reported greater reductions in invasive coronary angiography (69% vs. 53%, respectively; p < 0.001). Significantly more U.S. facilities reported increased use of telehealth and patient screening measures than non-U.S. facilities, such as temperature checks, symptom screenings, and COVID-19 testing. Reductions in volumes of procedures differed between U.S. regions, with larger declines observed in the Northeast (76%) and Midwest (74%) than in the South (62%) and West (44%). Prevalence of COVID-19, staff redeployments, outpatient centers, and urban centers were associated with greater reductions in volume in U.S. facilities in a multivariable analysis. Conclusions: We observed marked reductions in U.S. cardiovascular testing in the early phase of the pandemic and significant variability between U.S. regions. The association between reductions of volumes and COVID-19 prevalence in the United States highlighted the need for proactive efforts to maintain access to cardiovascular testing in areas most affected by outbreaks of COVID-19 infection
