Empirical constraints on vacuum decay in the stringy landscape
It is generally considered as self evident that the lifetime of our vacuum in
the landscape of string theory cannot be much shorter than the current age of
the universe. Here I show why this lower limit is invalid. A certain type of
``parallel universes'' is a necessary consequence of the string-landscape
dynamics and might well allow us to ``survive'' vacuum decay. As a consequence
our stringy vacuum's lifetime is empirically unconstrained and could be very
short. Based on this counter-intuitive insight I propose a novel type of
laboratory experiment that searches for an apparent violation of the
quantum-mechanical Born rule by gravitational effects on vacuum decay. If the
lifetime of our vacuum should turn out to be shorter than 6 x 10^{-13} seconds
such an experiment is sufficiently sensitive to determine its value with

Falsification Of The Atmospheric CO2 Greenhouse Effects Within The Frame Of Physics
The atmospheric greenhouse effect, an idea that many authors trace back to
the traditional works of Fourier (1824), Tyndall (1861), and Arrhenius (1896),
and which is still supported in global climatology, essentially describes a
fictitious mechanism, in which a planetary atmosphere acts as a heat pump
driven by an environment that is radiatively interacting with but radiatively
equilibrated to the atmospheric system. According to the second law of
thermodynamics such a planetary machine can never exist. Nevertheless, in
almost all texts of global climatology and in a widespread secondary literature
it is taken for granted that such mechanism is real and stands on a firm
scientific foundation. In this paper the popular conjecture is analyzed and the
underlying physical principles are clarified. By showing that (a) there are no
common physical laws between the warming phenomenon in glass houses and the
fictitious atmospheric greenhouse effects, (b) there are no calculations to
determine an average surface temperature of a planet, (c) the frequently
mentioned difference of 33 degrees Celsius is a meaningless number calculated
wrongly, (d) the formulas of cavity radiation are used inappropriately, (e) the
assumption of a radiative balance is unphysical, (f) thermal conductivity and
friction must not be set to zero, the atmospheric greenhouse conjecture is

Computational and Biological Analogies for Understanding Fine-Tuned Parameters in Physics
In this philosophical paper, we explore computational and biological
analogies to address the fine-tuning problem in cosmology. We first clarify
what it means for physical constants or initial conditions to be fine-tuned. We
review important distinctions such as the dimensionless and dimensional
physical constants, and the classification of constants proposed by
Levy-Leblond. Then we explore how two great analogies, computational and
biological, can give new insights into our problem. This paper includes a
preliminary study to examine the two analogies. Importantly, analogies are both
useful and fundamental cognitive tools, but can also be misused or
misinterpreted. The idea that our universe might be modelled as a computational
entity is analysed, and we discuss the distinction between physical laws and
initial conditions using algorithmic information theory. Smolin introduced the
theory of "Cosmological Natural Selection" with a biological analogy in mind.
We examine an extension of this analogy involving intelligent life. We discuss
if and how this extension could be legitimated.

Accelerated expansion from structure formation
We discuss the physics of backreaction-driven accelerated expansion. Using
the exact equations for the behaviour of averages in dust universes, we explain
how large-scale smoothness does not imply that the effect of inhomogeneity and
anisotropy on the expansion rate is small. We demonstrate with an analytical
toy model how gravitational collapse can lead to acceleration. We find that the
conjecture of the accelerated expansion being due to structure formation is in
agreement with the general observational picture of structures in the universe,

Short Telomeres Compromise β-Cell Signaling and Survival
The genetic factors that underlie the increasing incidence of diabetes with age are poorly understood. We examined whether telomere length, which is inherited and known to shorten with age, plays a role in the age-dependent increased incidence of diabetes. We show that in mice with short telomeres, insulin secretion is impaired and leads to glucose intolerance despite the presence of an intact β-cell mass. In ex vivo studies, short telomeres induced cell-autonomous defects in β-cells including reduced mitochondrial membrane hyperpolarization and Ca2+ influx which limited insulin release. To examine the mechanism, we looked for evidence of apoptosis but found no baseline increase in β-cells with short telomeres. However, there was evidence of all the hallmarks of senescence including slower proliferation of β-cells and accumulation of p16INK4a. Specifically, we identified gene expression changes in pathways which are essential for Ca2+-mediated exocytosis. We also show that telomere length is additive to the damaging effect of endoplasmic reticulum stress which occurs in the late stages of type 2 diabetes. This additive effect manifests as more severe hyperglycemia in Akita mice with short telomeres which had a profound loss of β-cell mass and increased β-cell apoptosis. Our data indicate that short telomeres can affect β-cell metabolism even in the presence of intact β-cell number, thus identifying a novel mechanism of telomere-mediated disease. Silk garments plus standard care compared with standard care for treating eczema in children: a randomised controlled, observer blind, pragmatic trial (CLOTHES Trial)
Background
The role of clothing in the management of eczema (syn. atopic dermatitis, atopic eczema) is poorly understood. This trial evaluated the effectiveness and cost-effectiveness of silk garments (in addition to standard care) for the management of eczema in children with moderate to severe disease.
Methods and findings
This was a parallel group randomised controlled, observer-blind trial. Children aged 1 to 15 years with moderate to severe eczema were recruited from secondary care and the community in five UK centres. Participants were allocated using on-line randomisation (1:1) to standard care, or standard care plus silk garments; stratified by age and recruiting centre. Silk garments were worn for 6 months. Primary outcome (eczema severity) was assessed at baseline, 2, 4 and 6 months, by nurses blinded to treatment allocation using the Eczema Area and Severity Index (EASI), which was log-transformed for analysis (intention-to-treat analysis). Safety outcome: number of skin infections. Three hundred children were randomised (26th Nov 2013 to 5th May 2015): 42% girls, 79% white, mean age 5 years. Primary analysis included 282/300 (94%) children (n = 141 in each group). The garments were worn more often at night than in the day (median of 81% of nights (25th to 75th centile 57% to 96%) and 34% of days (25th to 75th centile 10% to 76%)). Geometric mean EASI scores at baseline, 2, 4 and 6 months were 9·2, 6·4, 5·8, 5·4 for silk clothing and 8·4, 6·6, 6·0, 5·4 for standard care. There was no evidence of any difference between the groups in EASI score averaged over all follow up visits adjusted for baseline EASI score, age and centre (adjusted ratio of geometric means: 0·95, 95% CI 0·85 to 1·07). This confidence interval is equivalent to a difference of -1·5 to 0·5 in the original EASI scale units which is not clinically important. Skin infections occurred in 36/142 (25%) and 39/141 (28%) for silk clothing and standard care respectively. Even if the small observed treatment effect was genuine, the incremental cost per QALY was £56,881 in the base case analysis from an NHS perspective, suggesting that silk garments are unlikely to be cost-effective within currently accepted thresholds. Main limitations: whilst minimising detection bias, use of an objective primary outcome may have underestimated treatment effects.
Conclusions
Silk clothing is unlikely to provide additional benefit over standard care in children with moderate to severe eczema
Idiopathic pulmonary fibrosis in BRIC countries: the cases of Brazil, Russia, India, and China
Somatically ill persons’ self-nominated quality of life domains: review of the literature and guidelines for future studies
OBJECTIVE: To review which domains somatically ill persons nominate as constituting their QoL. Specific objective is to examine whether the method of enquiry affect these domains. METHODS: We conducted two literature searches in the databases PubMed/Medline, CINAHL and Psychinfo for qualitative studies examining patients' self-defined QoL domains using (1) SEIQoL and (2) study-specific questions. For each database, two researchers independently assessed the eligibility of the retrieved abstracts and three researchers subsequently classified all QoL domains. RESULTS: Thirty-six eligible papers were identified: 27 studies using the SEIQoL, and nine presenting data derived from study-specific questions. The influence of the method of enquiry on patients' self-nominated QoL domains appears limited: most domains were presented in both types of studies, albeit with different frequencies. CONCLUSIONS: This review provides a comprehensive overview of somatically ill persons' self-nominated QoL domains. However, limitations inherent to reviewing qualitative studies (e.g., the varying level of abstraction of patients' self-defined QoL domains), limitations of the included studies and limitations inherent to the review process, hinder cross-study comparisons. Therefore, we provide guidelines to address shortcomings of qualitative reports amenable to improvement and to stimulate further improvement of conducting and reporting qualitative research aimed at exploring respondents' self-nominated QoL domains
