Integrated side-scan, sub-bottom profiler and seismic signatures of methane seepage from Omakere Ridge on New Zealand’s Hikurangi margin
Omakere Ridge is one of a series of prominent northeast-southwest orientated anticlinal ridges associated with major thrust faults on New Zealand’s Hikurangi margin. The Hikurangi margin is an extensive gas hydrate province and recent marine surveys have confirmed that the mid-slope Omakere Ridge is a zone of methane-rich seabed seepage. Acoustic flares initially observed in the area by fishermen, were imaged in the water column at Omakere Ridge during a 2006 RV Tangaroa survey (TAN06-07). Anomalous methane concentrations (up to 165 nM) were detected by a methane sensor (METS) attached to a conductivity-temperature-depth-optical backscatter device (CTD) on TAN06-07 and a 2007 RV Sonne survey (SO-191). Six seep sites have been identified at the southern end of Omakere Ridge, where it bifurcates into two parallel ridgelines. All sites are located towards the crests of the two ridgelines in approximately 1150 m water depth. The seabed seeps were identified acoustically with an EdgeTech Deep-Tow side-scan operating at 75 kHz, and are shown as high backscatter intensity areas on processed side-scan data, which are interpreted to be methane derived authigenic carbonate hardgrounds. Acoustic shadows behind hardgrounds in the side-scan far range suggest the seabed features have moderate relief. Sub-bottom profiles acquired with an EdgeTech Deep-Tow chirper system, operating at 2-10 kHz, identified numerous signatures of shallow gas in the near subsurface. These signatures include zones of acoustic turbidity and gas blanking, interpreted to mark shallow gas fronts. The evidence for shallow gas in the subsurface from the sub-bottom profiler displays a marked spatial correlation with seabed expressions of seepage. The seepage sites also correspond to potential gas indicators in multi-channel seismic data, such as interpreted amplitude anomalies. Enigmatic subsurface features in the subbottom profiler data, such as potential amplitude anomalies and gas blanking, which are below the depression that bifurcates the ridge and are not associated with surface expressions of seepage, may represent lithological and topographic features or may be a component of the gas migration pathway which feeds the seeps on the ridge crest. Underwater video and still camera images show seabed seepage sites of high backscatter intensity represent widespread authigenic carbonate concretions and chemoherms associated with biological assemblages including siboglinid tube worms, vesicomyid clams, bathymodiolin mussels, and bacterial mats. A high backscatter intensity site of similar acoustic character to, and directly adjacent to, seep sites on the southern part of the ridge does not contain seep fauna and is interpreted to be a cold-water reef. While this feature may represent a relict seep, this finding highlights the fact that present day seepage cannot be identified with acoustic techniques alone
European skills framework? - but what are skills? Anglo-Saxon versus German concepts
With the proposed introduction of a common framework for comparing qualifications within the European Union (EU), as a result of the Lisbon agreement of 2000, the question of commonly agreed transnational concepts of skills and qualifications is has become a pressing political and practical issue. The paper argues that there are grounds for doubting that there is a ready translation of the English terms 'skill'and 'qualification' in a way that avoids problems of comparing and calibrating German and English vocational qualifications. Reasons for this difficulty are explored, the most important of which relate to: a) the conceptual structure of skill and its cognates in the two languages; b) the differing socio-political role of qualifications; c) different industrial structures and labour processes; d) differences in institutions regulating vocational education and training (VET). These problems are discussed in relation to examples of similar industries and occupations and apparently similar levels of qualification in England and Germany
Vocational teachers and workplace learning: integrative, complementary and implicit accounts of boundary crossing
Where young people’s upper-secondary education spans work and institutional domains, questions arise about learning across both spheres and its guidance. Theoretical accounts of ‘boundary crossing’ have explored how vocational teachers can integrate learning across domains by drawing on extended concepts and theoretical knowledge to solve workplace problems; whilst empirical accounts have validated the role of vocational educators by describing the workplace and schools as equally valid, complementary spheres. Different understandings, described here as ‘integrative’, ‘complementary’ and ‘implicit’, appear to reflect different national patterns of vocational education. The paper reports a qualitative study conducted around two case studies, located in Germany and England, of the way vocational teachers’ understandings of facilitating learning across domains are constructed. Vocational teachers working in Germany’s ‘dual training’ claimed to provide advanced knowledge that they compared to practical work skills, reflecting ‘implicit’ or ‘complementary’ approaches to learning across domains. Teachers in England, where workplace learning elements are more unevenly developed and lack institutional foundations, nevertheless described colleges and workplaces as distinctive, little-connected spheres. These differences suggest that teachers’ approaches are less shaped by the potential or necessity for ‘integrative’ approaches than by the way different systems enable or constrain their conceptualisation of ‘possible futures’.N/
A streamlined pathway for transcatheter aortic valve implantation : the BENCHMARK study
Background and There is significant potential to streamline the clinical pathway for patients undergoing transcatheter aortic valve implant-Aims ation (TAVI). The purpose of this study was to evaluate the effect of implementing BENCHMARK best practices on the efficiency and safety of TAVI in 28 sites in 7 European countries. Methods This was a study of patients with severe symptomatic aortic stenosis (AS) undergoing TAVI with balloon-expandable valves before and after implementation of BENCHMARK best practices. Principal objectives were to reduce hospital length of stay (LoS) and duration of intensive care stay. Secondary objective was to document patient safety. Results Between January 2020 and March 2023, 897 patients were documented prior to and 1491 patients after the implementation of BENCHMARK practices. Patient characteristics were consistent with a known older TAVI population and only minor differences. Mean LoS was reduced from 7.7 ± 7.0 to 5.8 ± 5.6 days (median 6 vs. 4 days; P <.001). Duration of intensive care was reduced from 1.8 to 1.3 days (median 1.1 vs. 0.9 days; P <.001). Adoption of peri-procedure best practices led to increased use of local anaesthesia (96.1% vs. 84.3%; P <.001) and decreased procedure (median 47 vs. 60 min; P <.001) and intervention times (85 vs. 95 min; P <.001). Thirty-day patient safety did not appear to be compromised with no differences in all-cause mortality (0.6% in both groups combined), stroke/transient ischaemic attack (1.4%), life-threatening bleeding (1.3%), stage 2/3 acute kidney injury (0.7%), and valve-related readmission (1.2%). Conclusions Broad implementation of BENCHMARK practices contributes to improving efficiency of TAVI pathway reducing LoS and costs without compromising patient safety

O carcinoma basocelular é a neoplasia maligna mais comum em humanos e sua incidência vem aumentando nas últimas décadas. Sua grande frequência gera significativo ônus ao sistema de saúde, configurando problema de saúde pública. Apesar das baixas taxas de mortalidade e de rara ocorrência de metástases, o tumor pode apresentar comportamento invasivo local e recidivas após o tratamento, provocando importante morbidade. Exposição à radiação ultravioleta representa o principal fator de risco ambiental associado a sua gênese. Entretanto, descrevem-se outros elementos de risco: fotótipos claros, idade avançada, história familiar de carcinomas de pele, olhos e cabelos claros, sardas na infância e imunossupressão, além de aspectos comportamentais, como exercício profissional exposto ao sol, atividade rural e queimaduras solares na juventude. Entre 30% e 75% dos casos esporádicos estão associados à mutação do gene patched hedgehog, mas outras alterações genéticas são ainda descritas. A neoplasia é comumente encontrada concomitantemente com lesões cutâneas relacionadas à exposição solar crônica, tais como: queratoses actínicas, lentigos solares e telangiectasias faciais. A prevenção do carcinoma basocelular se baseia no conhecimento de fatores de risco, no diagnóstico e tratamento precoces e na adoção de medidas específicas, principalmente, nas populações susceptíveis. Os autores apresentam uma revisão da epidemiologia do carcinoma basocelular.Basal cell carcinoma is the most common malignant neoplasm in humans and its incidence has increased over the last decades. Its high frequency significantly burdens the health system, making the disease a public health issue. Despite the low mortality rates and the rare occurrence of metastases, the tumor may be locally invasive and relapse after treatment, causing significant morbidity. Exposure to ultraviolet radiation is the main environmental risk factor associated with its cause. However, other elements of risk are described, such as light skin phototypes, advanced age, family history of skin carcinoma, light eyes and blond hair, freckles in childhood and immunosuppression. Behavioral aspects such as occupational sun exposure, rural labor and sunburns at a young age also play a role. Between 30% and 75% of the sporadic cases are associated with patched hedgehog gene mutation, but other genetic changes are also described. The tumor is commonly found in concomitance with skin lesions related to chronic sun exposure, such as actinic keratoses, solar lentigines and facial telangiectasia. The prevention of basal cell carcinoma is based on the knowledge of risk factors, early diagnosis and treatment, as well as on the adoption of specific measures, particularly in susceptible populations.
