    Using atmospheric model output to simulate the meteorological tsunami response to Tropical Storm Helene (2000)

    [1] In the fall of both 1999 and 2000, unexpected “rapid tides” occurred along the coast of the Avalon Peninsula of Newfoundland. These rapid tides have been linked to the passing of Tropical Storm Jose (1999) and Tropical Storm Helene (2000) over the Grand Banks. Here we examine the dynamic ocean response to Tropical Storm Helene (2000) using a barotropic shallow water ocean model forced by atmospheric pressure and surface winds derived from a simulation of Helene using a dynamical model of the atmosphere. The ocean model is able to capture the main features of the observed response at the coast of Newfoundland as seen in the available tide gauge data. Results show that the simulated sea level response at the coast is driven by a combination of wind stress and atmospheric pressure forcing, the former generally dominating. An exception is Conception Bay, Newfoundland, where the response is captured mainly by atmospheric pressure forcing. Offshore near the edge of the Grand Banks, atmospheric pressure and wind stress forcing are equally important. The wind-forced response depends on the divergence of the surface wind stress and hence on the structure of the storm in the atmospheric model simulation. Sensitivity studies show the importance of having a small time interval (on the order of minutes) at which the atmospheric forcing is supplied to the ocean model and show the importance of the location of the storm track

    Origin of Variability in Northern Hemisphere Winter Blocking on Interannual to Decadal Time Scales

    Variability of mid-latitude blocking in the boreal winter northern hemisphere is investigated for the period 1960/61 to 2001/02 by means of relaxation experiments with the model of the European Center for Medium-Range Weather Forecasts. It is shown that there is pronounced interannual and decadal variability in blocking, especially over the Eurasian continent, consistent with previous studies. The relaxation experiments show that realistic variability in the tropics can account for a significant part of observed interannual blocking variability, but also that about half of the observed variability can only be explained by extratropical tropospheric variability. On the quasi-decadal time scale, extratropical sea surface temperature and sea-ice, in addition to tropical variability, play a more important role. The stratosphere, which has been shown to influence interannual variability of the North Atlantic Oscillation in previous studies, has no significant influence on blocking according to our analysis

    A study of general practitioners' perspectives on electronic medical records systems in NHS Scotland

    <b>Background</b> Primary care doctors in NHSScotland have been using electronic medical records within their practices routinely for many years. The Scottish Health Executive eHealth strategy (2008-2011) has recently brought radical changes to the primary care computing landscape in Scotland: an information system (GPASS) which was provided free-of-charge by NHSScotland to a majority of GP practices has now been replaced by systems provided by two approved commercial providers. The transition to new electronic medical records had to be completed nationally across all health-boards by March 2012. <p></p><b> Methods</b> We carried out 25 in-depth semi-structured interviews with primary care doctors to elucidate GPs' perspectives on their practice information systems and collect more general information on management processes in the patient surgical pathway in NHSScotland. We undertook a thematic analysis of interviewees' responses, using Normalisation Process Theory as the underpinning conceptual framework. <p></p> <b>Results</b> The majority of GPs' interviewed considered that electronic medical records are an integral and essential element of their work during the consultation, playing a key role in facilitating integrated and continuity of care for patients and making clinical information more accessible. However, GPs expressed a number of reservations about various system functionalities - for example: in relation to usability, system navigation and information visualisation. <b>Conclusion </b>Our study highlights that while electronic information systems are perceived as having important benefits, there remains substantial scope to improve GPs' interaction and overall satisfaction with these systems. Iterative user-centred improvements combined with additional training in the use of technology would promote an increased understanding, familiarity and command of the range of functionalities of electronic medical records among primary care doctors

    Towards a theory of close analysis for dispute mediation discourse

    Mediation is an alternative dispute resolution process that is becoming more and more popular particularly in English-speaking countries. In contrast to traditional litigation it has not benefited from technological advances and little research has been carried out to make this increasingly widespread practice more efficient. The study of argumentation in disputemediation hitherto has largely been concerned with theoretical insights. The development of argumentation theories linked to computational applications opens promising new horizons since computational tools could support mediators, making sessions quicker and more efficient.For this, we need tools for close analysis of mediation discourse in order to explore the argumentative activity in depth, and ultimately get an accurate image of how dialogues unfold in this particular context. This paper therefore aims at laying the foundations of a theory of close analysis for discourse in dispute mediation. Theories provided by the literature serve as a basis for argumentative analyses of transcripts of mediation sessions in order to deliver a clear image of the argumentative structure. Analyses of the argumentative strategies in mediationdiscourse will allow for the development of a dialogue protocol that can be used to develop operational models which can be embodied in software to help make the mediation process easier and more effective

    . [1] In the Gulf Stream region, eddy kinetic energy (EKE) peaks in summer while, as measured by the baroclinic eddy growth time scale, the ocean is most baroclinically unstable in late winter. We argue that the seasonally-varying Ekman pumping is unlikely to be responsible for the seasonal variation in growth time, and that the summer peak in EKE results from a reduction in dissipation in summer compared to winter.     Systematically reviewing and synthesizing evidence from conversation analytic and related discursive research to inform healthcare communication practice and policy: an illustrated guide

    Background Healthcare delivery is largely accomplished in and through conversations between people, and healthcare quality and effectiveness depend enormously upon the communication practices employed within these conversations. An important body of evidence about these practices has been generated by conversation analysis and related discourse analytic approaches, but there has been very little systematic reviewing of this evidence. Methods We developed an approach to reviewing evidence from conversation analytic and related discursive research through the following procedures: • reviewing existing systematic review methods and our own prior experience of applying these • clarifying distinctive features of conversation analytic and related discursive work which must be taken into account when reviewing • holding discussions within a review advisory team that included members with expertise in healthcare research, conversation analytic research, and systematic reviewing • attempting and then refining procedures through conducting an actual review which examined evidence about how people talk about difficult future issues including illness progression and dying Results We produced a step-by-step guide which we describe here in terms of eight stages, and which we illustrate from our ‘Review of Future Talk’. The guide incorporates both established procedures for systematic reviewing, and new techniques designed for working with conversation analytic evidence. Conclusions The guide is designed to inform systematic reviews of conversation analytic and related discursive evidence on specific domains and topics. Whilst we designed it for reviews that aim at informing healthcare practice and policy, it is flexible and could be used for reviews with other aims, for instance those aiming to underpin research programmes and projects. We advocate systematically reviewing conversation analytic and related discursive findings using this approach in order to translate them into a form that is credible and useful to healthcare practitioners, educators and policy-makers

    Dynamic sea level changes following changes in the thermohaline circulation

    Using the coupled climate model CLIMBER-3a, we investigate changes in sea surface elevation due to a weakening of the thermohaline circulation (THC). In addition to a global sea level rise due to a warming of the deep sea, this leads to a regional dynamic sea level change which follows quasi-instantaneously any change in the ocean circulation. We show that the magnitude of this dynamic effect can locally reach up to ~1m, depending on the initial THC strength. In some regions the rate of change can be up to 20-25 mm/yr. The emerging patterns are discussed with respect to the oceanic circulation changes. Most prominent is a south-north gradient reflecting the changes in geostrophic surface currents. Our results suggest that an analysis of observed sea level change patterns could be useful for monitoring the THC strength.Comment: Climate Dynamics (2004), submitted.     Multiple Dimensions of Work Intensity: Ambulance Work as Edgework.

    Working life in public sector professions is undergoing significant change and becoming increasingly demanding. This article explores work intensity in NHS ambulance services in England, describing four distinct but interrelated dimensions of intensity: temporal; physical; emotional; and organizational. We use the concept of edgework to explore the complexities involved in how emergency workers attempt to negotiate the rewards and risks associated with multidimensional work intensity. Although certain parts of ambulance work may be intrinsically intense and can provide an important source of validation, organizational elements have the potential to push work intensity to unnecessary extremes. Ambulance services are ‘professionalizing’, but as work in ambulance trusts continues to intensify, issues over dignity, staff retention and the meaning of work are becoming ever more challenging, just as they are in other public service professions.</p
