[EN] Codoñer house is a Family House project, the result of a close dialogue and work with the client, a family of four members. The idea is simple, to enclose a courtyard with a stonewall on which it rests, two abstract pieces of rigorous geometry built in concrete. This courtyard is the main element of the house, one more inhabited space, a roofless room in charge of making the formal and functional arrangement of the rest of the spaces. And it is by virtue of natural light, formal expression of time, how these spaces come to life and acquire meaning. The Centre for Disease Control and Prevention from Atlanta estimates that about 1 in 68 children have been identified with autism spectrum disorder (ASD). Children with ASD present problems with social, emotional and communication skills in their daily life. Moreover, they present difficulties since early childhood in key areas for learning and development processes. Due to these characteristics most of children with ASD need specific programmes of educative inclusion; in this sense, early action and care is very important to favor a positive long-term development, as well as their inclusion into society. The project entitled “Equal Opportunities to Social Inclusion for Autistic Children, EOSIAC”, funded under the Grundtvig programme from the European Commission (ref. 2013-1-RO1-GRU06-29490) is mainly aimed to support the integration of children between 5 and 10 years old with autism in public education. Through this transnational project professionals from different fields involved in the care of autism from different countries (Romania, Spain, Italy, Germany, Turkey and Bulgaria) are exchanging knowledge and best practices aimed to create a network of specialized educators and counselors. This point is very relevant as it entails an empowerment of professionals in this field through the acquisition of more knowledge about educative inclusion of ASD based in the different realities from several European contexts. Within the project, Polibienestar Research Institute (www.polibienestar.org) is in charge of the development of a methodology for integrating the phenomenon of autistic children in public education that can be applied successfully in each country participating in the project. These methodological guidelines will be based in references and good practices with scientific evidence, taking into consideration a revision of recent researches, initiatives and legislation frameworks. The methodology will contain the main basis for an effective integration of children and it will be easy-to-adapt to different cultural, educative and legislative contexts. Other relevant products of the project will be the development of guidelines for parents of children with ASD to help them in the care of their children; and guidelines for teachers providing knowledge and skills on integration. So, through the involvement of parents of children with ASD and their teachers it is expected that the project has a real impact on children’s quality of life and wellbein

Objectives: The aim of this study is to assess the efficacy of the use of pre-exposure prophylaxis (PrEP) with hydroxychloroquine against placebo in healthcare workers with high risk of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in reducing their risk of coronavirus disease 2019 (COVID-19) disease during an epidemic period. As secondary objectives, we would like to: i) assess the efficacy of the use of PrEP with hydroxychloroquine against placebo in healthcare workers with high risk of SARS-CoV-2 infection in reducing their risk of exposure to SARS-CoV-2 (defined by seroconversion) during an epidemic period, ii) evaluate the safety of PrEP with hydroxychloroquine in adults, iii) describe the incidence of SARS-CoV-2 infection among healthcare workers at high risk of SARS-CoV-2 infection, iv) identify clinical, analytical and microbiological predictors of COVID-19 among healthcare workers at high risk of SARS-CoV-2 infection, v) set up a repository of serum samples obtained from healthcare workers at high risk of SARS-CoV-2 infection for future research on blood markers to predict SARS-CoV-2 infection. Trial design: Multicentre double-blind parallel design (ratio 1:1) randomized controlled clinical trial. Participants: Approximately 440 healthcare workers of four Spanish hospitals (Hospital Clínic of Barcelona, Hospital de la Santa Creu i Sant Pau of Barcelona, Hospital Plató of Barcelona, Hospital General de Granollers, Barcelona) will be recruited. Participants are considered to be at high-risk of SARS-CoV-2 infection due to their frequent contact with suspected and confirmed cases of COVID-19. For eligibility, healthcare workers with 18 years old or older working at least 3 days a week in a hospital with both negative SARS-CoV-2 polymerase chain reaction (PCR) assays and serological COVID-19 rapid diagnostic tests (RDT) are invited to participate. Participants with any of the following conditions are excluded: pregnancy, breastfeeding, ongoing antiviral, antiretroviral or corticosteroids treatment, chloroquine or hydroxychloroquine uptake the last month or any contraindication to hydroxychloroquine treatment. Intervention and comparator: Eligible participants will be allocated to one of the two study groups: Intervention group (PrEP): participants will receive the standard of care and will take 400mg of hydroxychloroquine (2 tablets of 200 mg per Dolquine® tablet) daily the first four consecutive days, followed by 400 mg weekly for a period of 6 months. Control group: participants will receive placebo tablets with identical physical appearance to hydroxychloroquine 200 mg (Dolquine®) tablets following the same treatment schedule of the intervention group. Both groups will be encouraged to use the personal protection equipment (PPE) for COVID-19 prevention according to current hospital guidelines. Main outcomes: The primary endpoint will be the number of confirmed cases of a COVID-19 (defined by a positive PCR for SARS-CoV-2 or symptoms compatible with COVID-19 with seroconversion) in the PrEP group compared to the placebo group at any time during the 6 months of the follow-up in healthcare workers with negative SARS-CoV-2 PCR and serology at day 0. As secondary endpoints, we will obtain: i) the SARS-CoV-2 seroconversion in the PrEP group compared to placebo during the 6 months of follow-up in healthcare workers with negative serology at day 0; ii) the occurrence of any adverse event related with hydroxychloroquine treatment; iii) the incidence of SARS-CoV-2 infection and COVID-19 among healthcare workers in the non-PrEP group, among the total of healthcare workers included in the non-PrEP group during the study period; iv) the risk ratio for the different clinical, analytical and microbiological conditions to develop COVID-19; v) a repository of serum samples obtained from healthcare workers confirmed COVID-19 cases for future research on blood markers to predict SARS-CoV-2 infection

Spanish Adaptation of the Organizational Culture Assessment Instrument: Reflection on the Difficulty in Transferring the ad hoc Factor

[EN] The Organizational Culture Assessment Instrument is a common instrument for measuring organizational culture in English-speaking countries based on four factors: Clan, ad hoc, Market and Hierarchy. However, to date, there is no proper translation of the scale into Spanish. In this study, we describe the translation and adaptation of the instrument through Exploratory Factor Analysis with a Spanish sample (n(1) = 246; 69.9% men and 30.1% women) and Confirmatory Factor Analysis with a Peruvian sample (n(2) = 510; 70.4% men and 29.6% women). The result reduces the four-factor internal structure to a three-factor structure that retains the Clan, Market and Hierarchy factors, but completely excludes the ad hoc factor. Confirmatory Factor Analysis shows acceptable indicators, reliabilities are good and indication of validity is also confirmed. In conclusion, this study has given rise to the instrument in Spanish, called OCAI-12, which is suitable for evaluating organizational culture. The authors received financial support for the research and publication of this article from the Universitat Rovira i Virgili, Tarragona, Spain.Assens-Serra, J.; Boada-Cueva, M.; Serrano-Fernández, M.; Villajos-Girona, E.; Boada-Grau, J. Trastorn per dèficit d'atenció i hiperactivitat; Comorbilitat; Trastorn d'insomniTrastorno por déficit de atención e hiperactividad; Comorbilidad; Trastorno de insomnioAttention deficit and hyperactivity disorder; Comorbidity; Insomnia disorderIntroduction: Several investigations have been performed on insomnia symptoms in adult attention-deficit/hyperactivity disorder (ADHD). However, the relationship between insomnia disorder and adult ADHD has been neglected in research. The main objective of the current study is to analyze the differences between adult ADHD patients with and without insomnia disorder, in terms of ADHD clinical severity, medical and psychiatric comorbidity, psychopharmacological treatment, and quality of life. Material and Methods: Two hundred and fifty-two adult patients with ADHD (mean age 37.60 ± 13.22 years; ADHD presentations—combined: 56.7%, inattentive: 39.7%, hyperactive/impulsive: 3.6%) were evaluated with an exhaustive clinical and psychological evaluation protocol including semistructured interviews (for comorbidities and ADHD assessment) and symptom rating scales for ADHD. The diagnosis of ADHD and insomnia disorder was made according to DSM-5 criteria. Furthermore, the Pittsburgh Sleep Quality Index, Insomnia Severity Index, and Epworth Sleepiness Scale were administered. Results: Insomnia disorder was found in 44.4% of adult ADHD patients and was more common in combined presentation (64.3%) and in patients with more ADHD severity. Comorbidities (both medical and psychiatric), especially mood disorders (42%), anxiety disorder (26.8%), personality disorder (39.3%), and any substance use disorder (11.6%), were associated with a higher insomnia disorder prevalence. ADHD stimulant treatment was related to lower insomnia disorder compared to patients without medication, as well as ADHD stable treatment. Additionally, worse health-related quality of life was associated with insomnia disorder. Conclusion: Insomnia disorder is highly prevalent in adult ADHD and is related to higher ADHD severity and more psychiatric and medical comorbidities. Some stimulants and stable pharmacological ADHD treatment are associated with better outcomes of insomnia disorder. The research leading to these results has received funding from the Instituto de Salud Carlos III (PI18/01788) and supported by the EU's Horizon 2020 Programme (Grant No. 667302, CoCA and Grant No. 728018 Eat2beNICE)
