    NASA future mission needs and benefits of controls-structures interaction technology

    Two questions are addressed: (1) which future missions need Controls-Structures Interaction (CSI) technology for implementing large spacecraft in orbit; and (2) what specific benefits are to be derived if the technology is available? The answers to these questions were used to help formulate and direct the CSI technology development program. Many future NASA missions have common CSI technology needs which can best be developed in a broad-based, but focused, technology program to provide the greatest benefit to the largest number of users. Three CSI benefit studies were completed to date as part of ongoing assessment process: (1) missions requiring large antennas; (2) missions requiring large optical systems; and (3) missions requiring the use of closed-loop controlled, flexible, remote manipulator systems (RMS) for in-space assembly.     A technology development program is described involving Controls Structures Electromagnetics Interaction (CSEI) for large space structures. The CSEI program was developed as part of the continuing effort following the successful kinematic deployment and RF tests of the 15 meter Hoop/Column antenna. One lesson learned was the importance of making reflector surface adjustment after fabrication and deployment.     The 15-meter antenna performance optimization using an interdisciplinary approach

    A 15-meter diameter deployable antenna has been built and is being used as an experimental test system with which to develop interdisciplinary controls, structures, and electromagnetics technology for large space antennas. The program objective is to study interdisciplinary issues important in optimizing large space antenna performance for a variety of potential users. The 15-meter antenna utilizes a hoop column structural concept with a gold-plated molybdenum mesh reflector. One feature of the design is the use of adjustable control cables to improve the paraboloid reflector shape. Manual adjustment of the cords after initial deployment improved surface smoothness relative to the build accuracy from 0.140 in. RMS to 0.070 in. Preliminary structural dynamics tests and near-field electromagnetic tests were made. The antenna is now being modified for further testing. Modifications include addition of a precise motorized control cord adjustment system to make the reflector surface smoother and an adaptive feed for electronic compensation of reflector surface distortions. Although the previous test results show good agreement between calculated and measured values, additional work is needed to study modelling limits for each discipline, evaluate the potential of adaptive feed compensation, and study closed-loop control performance in a dynamic environment

    Techniques to improve maneuver stability characteristics of a nonlinear wide-body transport airplane in cruise flight

    The maneuver control stability characteristics of an aircraft are a flying qualities parameter of critical importance, to ensure structural protection as well as adequate predictability to the pilot. Currently, however, maneuver stability characteristics are not uniquely addressed in the Federal Aviation Regulations (FAR) Part 25, for transport aircraft. In past transport category certification programs, the Federal Aviation Administration (FAA) has used a combination of requirements (longitudinal control, vibration and buffeting, high-speed characteristics, and out-of-trim characteristics) to ensure safe and controllable maneuver stability characteristics over a range of flight conditions and airplane configurations. Controversies exist regarding each of these regulations, however, and considerable expenditures in terms of design studies and testing time have resulted from the requirements. It is also recognized that additional engineering guidance is needed for identifying acceptable nonlinear maneuver stability characteristics, particularly as they relate to relaxed stability, highly augmented transport configurations. The current trend in large aircraft design is toward relaxed, or even negative, static margins for improved fuel efficiency. The advanced flight control systems developed for these aircraft, in many instances, have rendered current aforementioned maneuver stability criteria either too stringent or of little practical use. Current design requirements do not account for these advanced designs. The objective was to evaluate a broad spectrum of linear and nonlinear longitudinal stability characteristics to generate data for defining satisfactory and unacceptable maneuver characteristics, as defined by pilot opinion.     Exercise for people living with frailty and receiving haemodialysis:a mixed-methods randomised controlled feasibility study

    Objectives Frailty is highly prevalent in haemodialysis (HD) patients, leading to poor outcomes. This study aimed to determine whether a randomised controlled trial (RCT) of intradialytic exercise is feasible for frail HD patients, and explore how the intervention may be tailored to their needs.Design Mixed-methods feasibility.Setting and participants Prevalent adult HD patients of the CYCLE-HD trial with a Clinical Frailty Scale Score of 4–7 (vulnerable to severely frail) were eligible for the feasibility study.Interventions Participants in the exercise group undertook 6 months of three times per week, progressive, moderate intensity intradialytic cycling (IDC).Outcomes Primary outcomes were related to feasibility. Secondary outcomes were falls incidence measured from baseline to 1 year following intervention completion, and exercise capacity, physical function, physical activity and patient-reported outcomes measured at baseline and 6 months. Acceptability of trial procedures and the intervention were explored via diaries and interviews with n=25 frail HD patients who both participated in (n=13, 52%), and declined (n=12, 48%), the trial.Results 124 (30%) patients were eligible, and of these 64 (52%) consented with 51 (80%) subsequently completing a baseline assessment. n=24 (71% male; 59±13 years) dialysed during shifts randomly assigned to exercise and n=27 (81% male; 65±11 years) shifts assigned to usual care. n=6 (12%) were lost to follow-up. The exercise group completed 74% of sessions. 27%–89% of secondary outcome data were missing. Frail HD patients outlined several ways to enhance trial procedures. Maintaining ability to undertake activities of daily living and social participation were outcomes of primary importance. Participants desired a varied exercise programme.Conclusions A definitive RCT is feasible, however a comprehensive exercise programme may be more efficacious than IDC in this population.</div

    South African research in the Southern Ocean: New opportunities but serious challenges

    South Africa has a long track record in Southern Ocean and Antarctic research and has recently invested considerable funds in acquiring new infrastructure for ongoing support of this research. This infrastructure includes a new base at Marion Island and a purpose-built ice capable research vessel, which greatly expand research opportunities. Despite this investment, South Africa's standing as a participant in this critical field is threatened by confusion, lack of funding, lack of consultation and lack of transparency. The research endeavour is presently bedevilled by political manoeuvring among groups with divergent interests that too often have little to do with science, while past and present contributors of research are excluded from discussions that aim to formulate research strategy. This state of affairs is detrimental to the country's aims of developing a leadership role in climate change and Antarctic research and squanders both financial and human capital

    Use of Intravascular Imaging During Chronic Total Occlusion Percutaneous Coronary Intervention: Insights From a Contemporary Multicenter Registry

    Background: Intravascular imaging can facilitate chronic total occlusion (CTO) percutaneous coronary intervention. Methods and Results: We examined the frequency of use and outcomes of intravascular imaging among 619 CTO percutaneous coronary interventions performed between 2012 and 2015 at 7 US centers. Mean age was 65.4±10 years and 85% of the patients were men. Intravascular imaging was used in 38%: intravascular ultrasound in 36%, optical coherence tomography in 3%, and both in 1.45%. Intravascular imaging was used for stent sizing (26.3%), stent optimization (38.0%), and CTO crossing (35.7%, antegrade in 27.9%, and retrograde in 7.8%). Intravascular imaging to facilitate crossing was used more frequently in lesions with proximal cap ambiguity (49% versus 26%, P<0.0001) and with retrograde as compared with antegrade‐only cases (67% versus 31%, P<0.0001). Despite higher complexity (Japanese CTO score: 2.86±1.19 versus 2.43±1.19, P=0.001), cases in which imaging was used for crossing had similar technical and procedural success (92.8% versus 89.6%, P=0.302 and 90.1% versus 88.3%, P=0.588, respectively) and similar incidence of major cardiac adverse events (2.7% versus 3.2%, P=0.772). Use of intravascular imaging was associated with longer procedure (192 minutes [interquartile range 130, 255] versus 131 minutes [90, 192], P<0.0001) and fluoroscopy (71 minutes [44, 93] versus 39 minutes [25, 69], P<0.0001) time. Conclusions: Intravascular imaging is frequently performed during CTO percutaneous coronary intervention both for crossing and for stent selection/optimization. Despite its use in more complex lesion subsets, intravascular imaging was associated with similar rates of technical and procedural success for CTO percutaneous coronary intervention.     Position paper: The potential role of optical biopsy in the study and diagnosis of environmental enteric dysfunction

    Environmental enteric dysfunction (EED) is a disease of the small intestine affecting children and adults in low and middle income countries. Arising as a consequence of repeated infections, gut inflammation results in impaired intestinal absorptive and barrier function, leading to poor nutrient uptake and ultimately to stunting and other developmental limitations. Progress towards new biomarkers and interventions for EED is hampered by the practical and ethical difficulties of cross-validation with the gold standard of biopsy and histology. Optical biopsy techniques — which can provide minimally invasive or noninvasive alternatives to biopsy — could offer other routes to validation and could potentially be used as point-of-care tests among the general population. This Consensus Statement identifies and reviews the most promising candidate optical biopsy technologies for applications in EED, critically assesses them against criteria identified for successful deployment in developing world settings, and proposes further lines of enquiry. Importantly, many of the techniques discussed could also be adapted to monitor the impaired intestinal barrier in other settings such as IBD, autoimmune enteropathies, coeliac disease, graft-versus-host disease, small intestinal transplantation or critical care
